Abstract

Health service administration is an important foundation in supporting the quality of services in primary health facilities such as community health centers. This study was conducted to evaluate the effectiveness of the administration system at the Bontonompo 1 Community Health Center in Bontoramba Village, South Bontonompo District, Gowa Regency, which includes aspects of efficiency, completeness of documents, utilization of information technology, staff competence, and patient satisfaction. This study aims to evaluate the health service administration system at the Bontonompo 1 Community Health Center in Gowa Regency. The evaluation was conducted based on several main indicators such as service efficiency, completeness of documents, utilization of information technology, staff competence, and patient satisfaction. The research method used was a quantitative descriptive approach with a survey approach to patients and community health center employees. The results showed that most indicators of administrative efficiency were classified as good, such as completeness of documents and the conformity of medical record data. However, problems arose in the long waiting time and the still low utilization of the digital patient information system. In addition, although the level of patient satisfaction with the friendliness and speed of service was quite high, the clarity of information and service flow still needed to be improved. Staff competence also indicated the need for further training, especially in the use of information technology and understanding of administrative regulations. It is recommended that improvements be made to the queue management system, strengthening of administrative HR training, and comprehensive implementation of information technology to improve the quality and efficiency of services.

Abstract

Preoperative anxiety is commonly experienced by patients before undergoing general anesthesia and can affect surgical outcomes. Approximately 80% of patients experience anxiety. Education using e-booklet media is expected to help reduce anxiety by providing clear information about the anesthesia procedure. To determine the effect of education using e-booklet media on the anxiety level of patients undergoing general anesthesia in the preoperative room at Queen Latifa General Hospital, Yogyakarta. This study used a quantitative approach with a quasi-experimental design, employing a one-group pre-test and post-test design. Respondents were patients undergoing surgery with general anesthesia for the first time. Data were collected using the Amsterdam Preoperative Anxiety and Information Scale (APAIS) questionnaire, completed before and after education via the e-booklet. Data analysis was performed using the Wilcoxon test. Before the intervention, most respondents experienced moderate to severe anxiety levels. After the intervention, the majority were at a low anxiety level. The Wilcoxon test showed a p-value of 0.001 (<0.005), indicating a significant difference in anxiety levels before and after the education. Education using e-booklet media has an effect on reducing the anxiety level of patients undergoing general anesthesia in the preoperative room. Hospitals are encouraged to implement the use of e-booklets as educational media for patients undergoing anesthesia procedures. Further research is recommended to explore the long-term effects and use more varied educational methods to enhance patient understanding.

Abstract

This study aimed to evaluate the effectiveness of irrigation solution application time duration in cleaning dentinal tubuli in the middle third of the root canal through Passive Ultrasonic Irrigation (PUI) and manual agitation irrigation techniques. This study was a laboratory experiment with posttest-only control group, involving 16 extracted mandibular premolars and divided into four treatment groups according to the duration of irrigation application (15 and 30 seconds) and the irrigation technique used. Evaluation of detine tubuli clearance was performed by Scanning Electron Microscope (SEM), and statistical analysis was performed by Kruskal-Wallis test. The results showed no significant difference in dentinal tubule clearance between treatment groups (p = 0.120; p > 0.05). Thus, 15 and 30 seconds of irrigation solution application time duration in the mechanical agitation technique had no significant effect on the cleanliness of dentinal tubules in the middle third of the root canal.

Abstract

Hypertension is a major risk factor for cardiovascular diseases, including atherosclerosis, heart failure, kidney failure, and stroke. The long-term use of chemical medications poses the risk of side effects that may damage certain organ functions. Thus, alternative therapies are needed to reduce dependence on medication and help maintain the quality of life of hypertensive patients. One alternative treatment that can be considered for lowering blood pressure is herbal therapy. The herbal plants examined in this literature study are Orthosiphon aristatus and celery. Orthosiphon aristatus contains apigenin compounds that act as diuretics, while celery contains flavonoids that work by dilating blood vessels, allowing blood to flow more freely and thereby reducing blood pressure. Research objective to identify herbal plants and their active compounds used as hypertension therapy based on prescriptions at UPTD WKJ Tegal. The research method used is a literature study developed based on prescriptions for hypertension patients at UPTD WKJ Tegal. According to the journal findings on celery leaves, the difference in blood pressure before and after administration of celery leaf decoction was measured. The average difference (mean) in diastolic blood pressure was 11.33 mmHg, and the difference in systolic blood pressure was 8.67 mmHg. This indicates a significant reduction in blood pressure after administering celery leaf decoction for five consecutive days. In the study on Orthosiphon aristatus leaves, the frequency of urination in elderly patients before administration of Orthosiphon aristatus infusion was 3.00 times (range 2–6). After administering the infusion once daily for seven consecutive days, the frequency increased to 6.00 times (range 3–9), with an average difference of 2.2 times in urination frequency. This demonstrates that the infusion of Orthosiphon aristatus leaves has a diuretic effect on elderly hypertensive patients. Celery (Apium graveolens) and Orthosiphon aristatus herbal plants have been shown to effectively reduce blood pressure in patients with hypertension

Abstract

Excess weight is a major public health problem with high prevalence rates increasing every year. Excess weight in adults has increased 10-fold over the past four years. Decreased sleep time is a factor that plays a role in increasing weight/obesity. This study aims to determine the quality of sleep in students with normal body mass index and excess weight. This study is a analytical study of medical faculty students of General Achmad Yani University class of 2020 with consecutive sampling method. Measurement of sleep quality was carried out through filling out the Pittsburgh sleep quality index (PSQI) questionnaire and measurement of body mass index was carried out by measuring body weight and height analyzed by the Chi Square test. Of the 88 research subjects showed that in the group of students with normal body mass index there were 37 students with poor sleep quality (78.7%) and 10 students with good sleep quality (21.2%). In the group of students with excess body mass index there were 30 students with poor sleep quality (73.1%) and 11 students with good sleep quality (26.8%). From the results of the analysis, it was found that there was no relationship between the quality of sleep of students and body mass index (p = 0.542). Decreased sleep time can lead to poor sleep quality which can cause imbalances in the hormones ghrelin and leptin. From the results of this study, other factors that can cause disturbed sleep are health problems and psychological disorders.

Abstract

Post-mastectomy pain in breast cancer patients is a clinical problem that often has long-term effects, including the development of post-mastectomy pain syndrome (PMPS). The use of single opioids has limitations due to the risk of side effects and dependence, so intravenous multimodal analgesia approaches with various combinations of analgesic agents are increasingly being studied. This study aims to evaluate the effectiveness of multimodal intravenous analgesic combinations in controlling postoperative pain in breast cancer patients undergoing mastectomy with general anesthesia. A systematic review was conducted according to the PRISMA 2020 guidelines through searches in PubMed, Cochrane Library, ScienceDirect, and EBSCO for publications from 2005 to 2025. Articles that met the inclusion criteria were randomized controlled trials (RCTs) and cohort studies that assessed the effectiveness of multimodal intravenous analgesic combinations in mastectomy patients. The methodological quality was assessed using the Joanna Briggs Institute (JBI) instrument. A total of 11 studies involving more than 800 mastectomy patients were analyzed. The combination of pregabalin and esketamine was shown to significantly reduce the incidence of chronic pain up to 6 months post-surgery (14.3% and 7.1% vs. 46.3% and 31.7%; p<0.01), as well as reduce opioid consumption. Remifentanil–dexmedetomidine provided better analgesia while improving the patient's immunological profile. Ketamine-based anesthesia was more effective than opioids in reducing acute pain, while the combination of ketamine–magnesium sulfate was proven to be safe and opioid-sparing. The use of diclofenac reduced opioid consumption but increased the risk of bleeding, while intravenous lidocaine showed inconsistent results. The combination of multimodal intravenous analgesics is effective in optimizing pain control after mastectomy, reducing opioid consumption, and potentially preventing chronic pain. Adjusting the multimodal protocol according to the patient's profile is necessary to maximize benefits while minimizing the risk of side effects.

Abstract

An uncontrolled or unplanned event caused by human, situational, or environmental factors that disrupts the work process with or without resulting in injury, illness, death, or damage to work property. This study aims to determine the effect of work shifts and work fatigue on work accidents with unsafe actions as an intervening variable in engineering workers at PT. X. This study is an analytical observational study using a cross-sectional research design. The population in this study is the total number of engineering workers. The sample in this study amounted to 65 workers determined using the total sampling method where all members of the population were used as samples. The collected data were analyzed using the chi-squared test for bivariate and path analysis for multivariate. The results of the path analysis showed that the work shift variable was not affected by work accidents (p = 0.333), work fatigue had an effect on work accidents (p = 0.029), work shift had an effect on unsafe action (p = 0.002), there was an effect of work fatigue on unsafe action (p = 0.000), there was an effect of unsafe action on work accidents (p = 0.000). Based on the intervening test of work shifts on work accidents with unsafe action, significant results were obtained (p = 0.011) and work fatigue on work accidents with unsafe action obtained significant results (p = 0.006). It can be concluded that the unsafe action variable is an intervening variable in the influence of work shifts, work fatigue, on work accidents. It is hoped that future researchers will expand the population and number of samples to obtain better research results.

Abstract

Dyslipidemia is a major risk factor for cardiovascular diseases such as coronary heart disease and stroke. Leaflets are a suitable information delivery medium for providing education to patients. This study used a quasi-experimental design with a one-group pre-test and post-test design. Respondents were given a knowledge questionnaire before the intervention (pre-test), then received education using a digital leaflet, and then were re-measured (post-test) to see any changes in their level of knowledge. Population: Kediri City residents who are at risk or have predisposing factors for dyslipidemia. The results showed a significant increase in knowledge among respondents after receiving education using a digital leaflet. In the pre-test, the average percentage of correct answers was 79.5%. This measurable increase in knowledge proves that digital leaflets are an effective medium for conveying health information. Education using digital leaflets is effective in increasing public knowledge about dyslipidemia and its treatment in Kediri.
