Abstract

Tamarind leaves (Tamarindus indica L.) are widely cultivated in tropical countries, including Indonesia. Empirically, tamarind leaves are known to have anti-diabetic properties. In this study, an in-silico method was used to test the active compounds found in tamarind leaves (Tamarindus indica L.) as anti-diabetic agents through docking score analysis. The target protein used was Protein Tyrosine Phosphatase (5JNT) which is related to insulin resistance. Glimepiride and native ligand 5JNT were used as reference ligands, while the active compounds of tamarind leaves (Tamarindus indica L.) were used as the test ligands. There were 12 active compounds tested for drug-like properties based on the Lipinski Rule of Five, and 12 compounds met the criteria which were then tested through molecular docking using the PLANTS software. Based on the docking results, there were four compounds that had potential as anti-diabetic agents, namely linalool anthranilate, hexadecanol, pentadecanol, and benzyl benzoate, with docking scores of -85.889, -84.23032, -83.12325, and -81.54821 respectively. Although the docking scores of these compounds exceeded the score of the native ligand 5JNT, they did not exceed the score of the reference ligand glimepiride.

Abstract

Dyslipidemia is an abnormality that occurs in the lipid profile. Obesity can affect lipid profile levels such as increased levels of triglycerides (TG), total cholesterol (KT), Low-Density Lipoprotein (LDL), and decreased High-Density Lipoprotein (HDL), known as dyslipidemia. This study aimed to determine the antidyslipidemic activity of the ethanol extract of Centella asiatica herb in rats induced by a high fat and carbohydrate diet. This research is an experimental prevention method using 6 groups of test animals that were induced for 60 days (except the normal group) accompanied by the administration of the test substance 2 hours before the induction was carried out. Group 1 as a normal control was only given Na-CMC, group 2 as an induced control was only given Na-CMC, group 3 was given fenofibrate suspension 18 mg/Kg BW, and groups 4-6 were given ethanol extract of Centella asiatica herb at a dose of 50 mg/Kg, 100 mg/Kg, and 200 mg/Kg BW. Parameters for examination of lipid profile levels including levels of TG, KT, HDL, and LDL were carried out on day 0 and day 60. The results showed that the administration of the ethanolic extract of Centella asiatica herb at a dose of 200 mg/Kg was effective in providing activity in maintaining normal levels of triglycerides, total cholesterol, and LDL. It can be concluded that the ethanol extract of Centella asiatica has antidyslipidemic activity in a rat model induced by a high fat and carbohydrate diet.

Abstract

       Metformin is the first-line therapy for Diabetes Mellitus (DM), which has not been able to control blood glucose in type 2 DM. One of the Dipeptidyl Peptidase IV Inhibitor (DPP4-I) groups, namely vildagliptin, has been included in the National Formulary which is used as a combination for the therapy of DM patients Type 2. Research objectives to know the effectiveness and safety of the combination of metformin-vildagliptin in type 2 DM at RSPAL Dr. Ramelan Surabaya. This research method uses a quantitative method with an observational study design, with retrospective data collection based on the Medical Records of Type 2 DM patients who first received metformin-vildagliptin combination therapy (n = 30) carried out observations, and recorded the results of Fasting Blood Glucose (GDP) examinations, Postprandial 2-hour blood glucose (GD2JPP), and HbA1C at baseline and after 3 months of therapy, and observed the safety of the combination. Results The metformin-vildagliptin combination reduced GDP 33.13 ± 40.73 mg/dL, GD2JPP 34.30 ± 48.81 mg/dL, and HbA1C 0.89 ± 0.86%. Did not show any side effects of hypoglycemia, but 3.33% experienced nausea/vomiting and dyspepsia. It was concluded that the combination of metformin-vildagliptin was able to reduce GDP, GD2JPP, and HbA1C,

Abstract

The main problem in conducting lupus treatment is that most drugs for the therapy provided have not been authorized so that they are still included in the class of off-label drugs for the treatment of lupus. The purpose of this study was only to see the impact of side effects of off-label drugs on health costs. This study was conducted using a cross-sectional method by means of a survey using the Naranjo Algorithm instrument (PESO Book) to determine the level of incidence of side effects that exist in LSE therapy using off-label drugs and calculate the health costs of lupus patients. The results showed that the most respondents were in adulthood (19 respondents  47.5%), then the most respondents were female (37 respondents or 92.5%). Data on the incidence of side effects obtained the most respondents in the Probable category (most likely to occur ROM) of 14 respondents or 35%. In addition, data on health costs incurred by more respondents experienced an increase, namely 23 respondents or 57.5%. The research data shows the calculated r value is 0.3044 and the Sig (2-tailed) value is 0.000 with a confidence level (CI) of 0.05. The results of the analysis clearly show that there is a significant relationship between the side effects of using off-label drugs on the health costs of lupus patients, where if the value of side effects experienced by respondents is high, the value of respondents' health costs is also high.

Abstract

Kepok banana (Musa balbisiana L.) natural starch easily undergoes syneresis due to starch retrogradation which can reduce the stability and viscosity of a suspension preparation, so it is necessary to modify starch to overcome this problem. One of the starch modification technologies is chemical modification with crosslinks. Utilization of kepok banana phosphate pregelatinated starch (Musa balbisiana L.) as a suspending agent in chloramphenicol palmitate suspension has been carried out and evaluated for its physicochemical properties and stability during storage. The method of making the suspension is the dispersion method. F1 used 5% kepok banana phosphate pregelatinated starch, F2 (7.5%) and F3 (10%). The results showed that the best formula was F3 with observations of organoleptic, viscosity and flow properties, degree of sedimentation, redispersibility, density, and pH which met the requirements during 4 weeks of storage.

Abstract

Coagulation is an important physiological mechanism of the body to maintain optimal function of recovery. The success of this mechanism is determined by normal levels of biomarkers are involved, such as platelets. Deficiency of levels of these biomarkers can cause diseases, such as dengue fever, shock and hemophilia. Diet provides biochemical elements that the body can then use to synthesize platelets in the blood. The purpose of study was to compare the levels of biomarkers of coagulation function in white rats (Rattus norvegicus) with a variety of vegetarian diets. The method of study was using in vivo experimental research. The dependent variable is the level of platelets and the independent variable is a variety of vegetarian diets (quasi, lacto-ovo and vegan). The sampling technique used the Federer formula with are samples of 28 white rats. Statistical test using T test and Wilcoxon test. The research data showed that giving a vegetarian diet caused significant changes in platelet levels in the quasi, lacto-ovo and vegan vegetarian diet groups with a significant value for each group, namely p = 0.026, p = 0.042 and p = 0.001. While on the standard diet there was no significant change in platelet levels (p=0.291). On average, the highest platelet levels were found in the quasi-vegetarian diet group and the lowest in the standard diet group. In conclusion, the quasi-vegetarian, lacto-ovo and vegan diets affects the levels of blood coagulation biomarkers (platelets) in white rats

Abstract

Lemon cui also known as fish lemon or kalamansi lemon is known to contain one of them vitamin C which has the potential as an antioxidant so that it can be applied as active ingredient in peel off mask preparations. The aims of this study weretoobtaining the best PVA concentration in the manufacture of peel-off gel masks from lemon cui juice that meet the stability test requirements. physique. The method of making a sample of lemon cui juice is obtained by grinding it using a blender, adding 100 mL of water and then separating the pulp to obtain the juice. The formulation of peel-off gel mask preparations uses variations in the concentration of PVA (Polyvinyl Alcohol), namely F1 (10%), F2 (13%) and F3 (16%). Physical stability tests included organoleptic tests, homogeneity tests, spreadability tests, pH tests, adhesion tests and drying time tests.The results showed that the preparations F1, F2 and F3  showed physical stability  based on the results of organoleptic tests, homogeneity tests, spreadability tests, pH tests, adhesion tests and drying time tests before and after accelerated storage conditions. Based on the test results obtained, it can be concluded that the formulation of the Lemon Cui peel-off gel mask preparation from the three concentrations of PVA met the requirements for physical stability. The best PVA concentration is 10% because it has the best dry time.

Abstract

The incidence of drug-related problems (DRPs) in the dose category on the use of antihypertensive drugs has been associated with the failure of the patient's optimal blood pressure target. The study aims to analyze the relationship between drug-related problems (DRPs) on administering amlodipine and captopril drugs to the blood pressure conditions of elderly hypertensive patients in Puskesmas. This research is observational research with a cross-sectional design of analytical nature—purposive sampling techniques. The total number of samples meeting the inclusion and exclusion criteria was 435 patients. Analysis of DRPs dose category relationship data on administration of amlodipine and captopril against blood pressure conditions using a chi-square test. The results of the study showed that the characteristics of patients visited tended to be aged 65-69 years (53.33%), female (68,51%), stage II hypertensive (62.1%), amlodipine (80.92%). The incidence of the DRP dose category in stage II hypertensive (7.6%). Statistically known DRPs of amlodipine and captopril dosing categories showed different significance in each patient's condition of prehypertension (p=0.260), hypertension stage I (p = <0,001), hypertensive stage II (p<0,001). This study concludes, there is a relationship between the incidence of DRPs in the dose category in patients with stage I and stage II hypertension.

Abstract

Sunscreen is a preparation used on the surface of the skin that works to absorb, scatter or reflectrays ultraviolet. One of the compounds that have benefits as a sunscreen is propolis. Propolis has antioxidant activity and its components can be used as a sunscreen formulation. The flavonoids contained in propolis have potential as sunscreen because of the presence of a chromophore group (conjugated double bond) which can absorb both UV B and UV A rays. To make a good preparation, a good formula is needed, one of the components in making lotions is acid. stearicate used as emulgator. This study aims to formulate a lotion with variations of stearic acid which functions as anas emulgator well as to test the physical and chemical properties of thepreparations lotion. The lotion is made in three variations of the composition of stearic acid, namely 2.5%: 5%; 7.5%. The lotion obtained was evaluated for the physical and chemical properties of thepreparation lotion including organoleptic test, homogeneity, emulsion type, pH, spreadability, adhesion and viscosity as well asmeasurement of the SPF value in vitro. The results of the analysis showed that variations in the concentration of stearic acid had a significant effect on the physical properties of the lotion including the results of pH, spreadability, adhesion, and viscosity. Stearic acid the higher the viscosity, consistency, and increased adhesion but the spreadability decreases. The SPF test results on theformula were lotion 6.13 (F1), 6.11 (F2), and 6.05 (F3) and the SPF value decreased after thetest freeze thaw.

Abstract

Diclofenac Na is an NSAID class of drugs that functions to treat pain and inflammation of rheumatoid arthritis, osteoarthritis or joint disease. The aim of this study is to investigate the effect of carbopol 940, HPMC K15M and propyleneglycol variations on the emulgel physical properties, carbopol concentration, HPMC, propyleneglycol to produce an optimum formula.The method used in this optimization is Simplex Lattice Design. The independent variables which include carbopol 490 , HPMC K15M , propylene glycol were then included in the expert design to obtain 16 runs. Each formula was tested for viscosity, spreadability, adhesion, pH, diffusivity and physical stability. Each parameter has a contour plot and by superimposing it can be determined the shaded area which is the area of ​​the optimum formula. The optimum formula was carried out using a one sample t-test with a 95% confidence level. Based on the results of superimposed superimposed graphic observations for the manufacture of diclofenac narium emulgel with test parameters including spreadability, viscosity, adhesion, PH, diffusion ability and stability test, the composition of Carbobol 940  0.5 - 1.22%, HPMC K-15M 3.24 - 3.77 Propylene glycol 5 - 6.26%. The results of the t-test showed that the Simplex Lattice Design equation in the formula showed results that were not significantly different, this shows the significance value of the results before treatment with treatment results > 0.05. This shows that the equation is valid and the stability test is stable.
