Abstract

Sore nipples are one of the problems in breastfeeding, in which the nipples are injured due to inappropriate breastfeeding techniques or trauma to the nipples, resulting in cracks and the formation of fissures. Sore nipples will hinder the mother from breastfeeding because of pain and soreness. One thing that can prevent sore nipples is knowing and doing appropriate breastfeeding techniques. This study aims to determine the relationship between knowledge about correct breastfeeding techniques and the incidence of sore nipples in postpartum mothers in BPM Bd. R in Cibanteng Village, Bogor Regency in 2023. This research method is an analytical survey using a cross-sectional approach. The research instrument is a questionnaire given to 30 respondents. It was taken using a total sampling technique with sample criteria: mothers who breastfeed their babies. The statistical test used is Chi-square. From the results of this study, it was obtained that from the respondents in the category of good knowledge, the incidence of nipple abrasions was as many as five mothers (35.7%), while from the respondents in the category of poor knowledge was 15 mothers (93.8%) experience sore nipples. The results of the chi-square statistical test showed that the ρ value was 0.001<0.05. There is a relationship between knowledge and the incidence of sore nipples. The odds ratio result showed that mothers who are well-informed about the correct breastfeeding technique have a smaller risk of experiencing 0.037 times the incidence of blistering milk compared to those who are less knowledgeable. There are still many breastfeeding mothers who do not know the correct breastfeeding technique, which results in sore nipples.

Abstract

The incidence of non-communicable diseases, especially diabetes mellitus (DM), continues to increase. This condition will have a negative impact on people's health status. Diabetes mellitus that persists for a long time will cause complications that can increase medical costs and reduce the quality of life of diabetes sufferers. There are risk factors that can be known to avoid DM. Health workers at Community Health Centers are expected to be role models in early detection of DM. The research was carried out from September to November 2023 to identify risk factors in health workers at the North Lampung Regional Health Center. The research design was descriptive, with 301 respondents consisting of 16 doctors, 109 nurses, and 176 midwives at 16 community health centers. The measuring tool uses the CANRISK questionnaire. Respondents were interviewed and measured/performed physical examinations related to risk factors for DM, including measuring height and weight, abdominal circumference, and instant blood glucose levels. The measurement results were processed in univariate analysis with the following results: The level of risk factors among health workers in the North Lampung Community Health Center who had high risk was 7 people (2.33%), moderate risk was 40 people (13.29%) and low risk was 254 people (84.39%). For this reason, health workers must change risk factors that can be modified, including BMI, waist circumference, physical activity, consumption of vegetables and fruit, blood pressure, and blood glucose.

Abstract

The SDIDTK program is a comprehensive and quality program for child growth and development through early stimulation, detection and intervention of developmental deviations in the first five years of life. Cadres have a very crucial role in the implementation of Integrated Healthcare Post (Posyandu) and act as one of the keys to success in SDIDTK.This study aims to analyze the roles of cadres in the implementation of SDIDTK services in the work area of Banguntapan 2 Community Health Center of Bantul. Qualitative study design by conducting in-depth interviews with related parties. The respondents were assigned into three groups including 4 cadres in 4 villages in the work area of Banguntapan 2 CHC (Community Health Center), 2 mothers of under-five children and 1 SDIDTK coordinator in Banguntapan 2 CHC. It was revealed that SDIDTK service at Banguntapan 2 CHC was implemented every 2 months by CHC staffs. As an implementer in SDIDTK services, cadres had the role in monitoring growth and finding deviations in child development; as a communicator, they had the role in providing information to mothers about the implementation of SDIDTK in Posyandu; as a motivator, they had the role in evaluating mothers who had not implemented SDIDTK; and as an organizer, they had the role in organizing SDIDTK services in Posyandu. SDIDTK services had not been implemented optimally due to inhibiting factor, that is not all Posyandu cadres had received SDIDTK training. In addition, the supporting facilities for SDIDTK services at Posyandu were considered inadequate. Cadres had several roles in SDIDTK services at posyandu involving the roles as implementer, communicator, motivator, and organizer. Relative parties are expected to facilitate Posyandu cadres in improving SDIDTK service competency by providing SDIDTK training to all cadres, as well as giving awards to active and outstanding cadres.

Abstract

Post-section caesarea pain under spinal anesthesia can inhibit the patient's activities and is one of the reasons the patient does not want to move. One of the non-pharmacological therapies used to reduce pain management is early mobilization. The purpose of this study was to determine the effect of early mobilization on reducing pain in post-section caesarea patients with spinal anesthesia at Padang Panjang Hospital in 2022. Data collection was carried out at Padang Panjang Hospital in January-February 2023. This type of research is quantitative research using the pre-experimental method with the design of a one-group pre-test and post-test. The sample is 35 respondents, and the sampling technique is purposive sampling. The research instrument uses the NRS Scale.  The results showed that most of the respondents were aged 25-35 years, namely 24 people (68.6%), and mostly were high school and bachelor graduates, as many as 15 people each (42.9%). Before early mobilization, most patients experienced moderate pain, namely 29 people (82.9%), and after mobilization, most patients experienced mild pain, namely 29 people (82.9%) at Padang Panjang Hospital. There was a reduction in pain after early mobilization at Padang Panjang Hospital (p=0,000). The study concludes that early mobilization has an effect on reducing pain in post-sectio caesarea patients with spinal anesthesia at Padang Panjang Hospital in 2022. It is recommended for health workers to socialize or implement early mobilization in post-sectio caesarea patients to reduce pain levels in patients.

Abstract

reast cancer is one of the most prevalent cancers in society, especially among women. Tamoxifen is often used as a first-line therapy for breast cancer and reduces the risk of recurrence in ER+ breast cancer. However, the response to treatment varies due to genetic variability resulting from polymorphisms in the DNA encoding drug-metabolism enzymes, including the CYP2D6 gene. This literature review aimed to evaluate the impact of CYP2D6 gene variants on the effectiveness of tamoxifen in breast cancer treatment. A literature search was conducted through PubMed, Google Scholar, and BMC Cancer using relevant keywords. The selected articles were no more than ten years old (2014-2024), with criteria for inclusion and exclusion. The review of articles from various research journals found that in the Asian region, the CYP2D6 gene variant allele *10 was most frequently seen, resulting in Intermediate Metabolism (IM) in tamoxifen therapy. Patients with IM metabolism tend to have suboptimal therapeutic responses and require tamoxifen dose adjustments.Conversely, patients with Normal Metabolism (NM/EM) tend to have better therapeutic responses. Genetic variability in the CYP2D6 gene affects the activity of the CYP2D6 enzyme, which functions to convert tamoxifen into its active form, Endoxifen. Some individuals may have low (Poor Metabolizer), Intermediate (Intermediate Metabolizer), Normal (Extensive Metabolizer), or even high (Ultra-rapid Metabolizer) enzyme activity. Therefore, adjusting tamoxifen therapy based on patient genetic information is essential for optimal treatment, especially in Asia, where the CYP2D6 gene variant allele *10 is most commonly found, resulting in IM metabolism
