Abstract

Stunting is a serious health problem that has long-term impacts on children's physical and cognitive growth and national progress. This study aims to explore micro-level social determinants, such as maternal knowledge, caregiving practices, household food access, and health service utilization, that influence stunting in Kediri City, and to formulate context-based strategies to address these issues. The design of this study uses a qualitative approach with a case study method; Data were collected through in-depth interviews and participatory observations of mothers who have toddlers or school-age children. Based on research, the incidence of stunting in Kediri City is influenced by various factors, including: lack of understanding of balanced nutrition, inadequate nutritional intake, malnutrition from an early age, lack of monitoring during pregnancy, Low Birth Weight (LBW), unhealthy eating habits, to recurring child health problems, lack of food variety and limited time and knowledge of mothers also exacerbate the situation. These findings emphasize the importance of nutrition education from pregnancy, improving primary health services such as routine visits to health centers, and continuous monitoring of child growth and development. This study recommends a multisectoral and participatory approach to accelerate the reduction of stunting at the community level.

Abstract

Murraya koenigii (curry leaf) is a tropical plant known for its diverse phytochemical content and potent antioxidant activity. However, limited studies have provided comprehensive quantitative data on its bioactive compounds, especially from Indonesian varieties. This study aimed to quantitatively analyze flavonoid, saponin, and tannin content in young and mature curry leaves and evaluate their in vitro antioxidant activity using the DPPH assay. An experimental design was conducted from January to March 2025. Ethanolic extracts of young and mature M. koenigii leaves from Aceh were prepared via maceration. Phytochemical quantification was performed using spectrophotometric methods, and antioxidant capacity was assessed using the DPPH radical scavenging assay with IC₅₀ determination. Young leaves showed higher levels of total flavonoids (0.19%), saponins (3.34%), and tannins (5.77%) compared to mature leaves (0.13%, 2.53%, and 4.58%, respectively). However, mature leaf extract exhibited stronger antioxidant activity with an IC₅₀ value of 46.77 μg/mL (very strong category), while young leaves showed 53.14 μg/mL (strong category). This suggests that antioxidant efficacy is influenced more by compound maturity and synergistic effects than total concentration. Mature M. koenigii leaves have superior antioxidant potential despite lower phytochemical concentrations, highlighting the importance of compound quality and interactions between compounds.

Abstract

Fluid restriction compliance refers to a patient's adherence to the prescribed limit of fluid intake to maintain optimal health. This compliance is influenced by factors such as age, education level, family support, and knowledge. The purpose of this study was to explore how these factors impact fluid restriction adherence among hemodialysis patients. This correlational research used a cross-sectional design with a sample of 155 hemodialysis patients from Jampangkulon Regional Hospital, selected through proportional random sampling. Data were collected via a questionnaire and analyzed using chi-square and logistic regression. The study found that a majority of patients were compliant with fluid restrictions (71.0%), and most possessed good knowledge on the topic (80.6%). Bivariate analysis identified significant effects of age (p=0.030), education (p=0.041), family support (p=0.015), and knowledge (p=0.001) on compliance. After controlling for all factors in a logistic regression model, knowledge emerged as the sole statistically significant predictor of compliance (p=0.028). In conclusion, while age, education, family support, and knowledge were all correlated with adherence, knowledge was the most significant factor in predicting compliance. It is recommended that Jampangkulon Regional Hospital enhance educational programs and involve families in patient care to improve compliance.

Abstract

Perceived Behavioral Control (PBC) is a key construct in the Theory of Planned Behavior (TPB) that plays an important role in predicting adherence to lifestyle changes among patients with coronary artery disease (CAD). Although the role of PBC has been widely discussed in health behavior research, no instrument has been specifically developed and validated to measure PBC in patients with cardiovascular disease (CVD), particularly CAD. This study used the Rasch analysis approach to evaluate the construct validity and reliability of the Perceived Behavioral Control Scale Related to Cardiovascular Disease (PBCCVD). This was a cross-sectional study involving 92 post-hospitalization CAD patients selected through convenience sampling. The PBCCVD instrument consists of 22 items using a 4-point Likert scale, developed based on TPB theory and previous studies and validated by experts. Data analysis was conducted using Winsteps software, covering reliability, separation, fit statistics, and unidimensionality. The instrument demonstrated high reliability (Cronbach's alpha = 0.86; item reliability = 0.94; person reliability = 0.86). The item and person separation indices were 4.18 and 2.74, respectively. All items met the fit criteria. The explained variance was 34.9%, and the eigenvalue of the first contrast was 2.6054, indicating a possible additional subdimension. The PBCCVD is a reliable and reasonably valid instrument for measuring perceived behavioral control in CAD patients. Although there is an indication of another underlying dimension, the instrument can still be effectively used in both clinical and research settings.

Abstract

Ovarian cysts are highly prevalent among women, particularly those of reproductive age. However, giant ovarian cysts are rare, as most cases are detected early during routine gynecological or ultrasound examinations. A giant ovarian cyst is defined as an ovarian cyst that is more than 10 cm in diameter. Cases with giant size pose significant clinical challenges. We report a case of an 18-year-old female patient with symptoms of an enlarged abdomen for six months causing shortness of breath, difficulty in activities and supine position. Initial examination revealed an abdominal circumference of 120 cm and a large cystic mass that required laparotomy. Challenges in anesthetic management are related to increased abdominal pressure by the mass, including aspiration risk, hemodynamic disorders, supine hypotensive syndrome, and re-expansion pulmonary edema. Consideration of anesthetic agent selection and perioperative monitoring strategies is key to minimizing complications and ensuring patient safety. In this case, anesthetic agents were carefully selected due to the presence of a giant ovarian cyst that significantly increased intra-abdominal pressure. A balanced general anesthesia approach was employed, prioritizing agents with minimal cardiovascular effects to maintain hemodynamic stability. Propofol was chosen for induction because of its rapid onset and ease of titration, while sevoflurane was used for maintenance due to its favorable cardiovascular profile and rapid recovery characteristics. Rocuronium was administered to facilitate muscle relaxation, and fentanyl was selected for analgesia owing to its potency and minimal impact on hemodynamics. To mitigate the risk of aspiration, rapid sequence induction was performed with endotracheal intubation in a semi-recumbent position. This anesthetic strategy was specifically tailored to address the risks associated with large intra-abdominal masses, such as aspiration, cardiovascular instability, and respiratory compromise.

Abstract

West Sulawesi has a high prevalence of underweight among toddlers, recorded at 22.5% in the 2022 Indonesian Nutritional Status Survey (SSGI). Penja fish, a local food with higher protein content than anchovies, offers potential as a supplementary food (PMT) for addressing child undernutrition. However, its traditional use remains limited to salting and direct consumption. Processing penja fish into flour for cookies provides a sustainable, innovative strategy to diversify intake and mitigate seasonal supply challenges. This experimental study, employing a post-test only control group design, aimed to assess the nutrient composition and acceptability of cookies with varying levels (0%, 25%, 50%, 75%, and 100%) of penja fish flour substitution. Research was conducted between September and November 2024 across laboratories at Universitas Hasanuddin and Universitas Muslim Indonesia. Nutritional analyses included proximate composition, and acceptability was measured through hedonic testing. Significant differences were found across formulations in color, aroma, texture, and taste (p < 0.001). Cookies containing 25% penja fish flour (Formula B) were the most preferred, with a proximate composition of 9.83% protein, 29.39% fat, 54.45% carbohydrates, 2.04% ash, and 5.30% moisture. Penja fish flour substitution enhanced the nutritional profile and acceptability of cookies. Formula B meets the nutritional requirements for supplementary feeding programs targeting underweight toddlers.

Abstract

Mental health is a critical component of overall well-being, especially for nursing students who encounter distinct academic and clinical stressors throughout their education. This study seeks to gain an in-depth understanding of nursing students' experiences during clinical practice, with a particular focus on their mental health. This study used a qualitative phenomenological approach to explore in-depth the mental health experiences of nursing students during clinical education. The study involved 15 final year nursing students who met the inclusion criteria such as being or having undergone clinical practice and being able to articulate their experiences. Data were collected through in-depth semi-structured interviews using open-ended questions and supported by observations of non-verbal responses. Data were analyzed using identification of meaningful statements, thematic grouping, and descriptive construction, while applying bracketing to minimize researcher bias. The findings identified five key themes affecting nursing students’ psychological well-being during clinical practice: psychological stress, lack of confidence and fear of mistakes, social support, coping strategies, and personal meaning of the experience. Despite facing various pressures, students were able to build mental resilience through social support and coping strategies and found positive meaning from the practice experience. Nursing educational institutions are advised to provide psychological support and stress management training to help students cope with stress during clinical practice.

Abstract

One of the factor’s causing stunting is infection related to poor household food safety practices. This study aims to analyze the correlation between household food safety practices and the incidence of stunting in Sibakur Village, Sijunjung District, Indonesia. The research used a case-control design with a sample size of 60 toddlers (30 cases and 30 controls) selected by simple random sampling. Data were collected through interviews using a questionnaire and analyzed by chi-square test, calculation of odds ratio (OR) with Confidence Interval (CI). The results showed a significant relationship between food safety practices and the incidence of stunting (p = 0.000; OR = 8.5; 95% CI =2.3-30.4), which means that toddlers from households with poor food safety practices have an 8.5 times higher risk of being stunting. Food safety factors closely related to stunting include sanitation, personal hygiene, raw materials, pest control, and cross-contamination. In conclusion, poor household food safety practices significantly increase the risk of stunting. Therefore, educative interventions of food safety need to be strengthened as a strategy to prevent stunting at the household level.

Abstract

Nurses play a pivotal role in determining the quality of healthcare services. Job satisfaction among nurses not only influences their professional performance but also directly affects the quality of patient care. Dissatisfaction may lead to burnout, errors, and decreased patient safety. This study aimed to identify and analyze the factors related to nurses’ job satisfaction and to explore how these factors impact the quality of patient services. A scoping review was conducted using the PRISMA-ScR guidelines. Literature searches were performed through PubMed, CINAHL, and Google Scholar based on the PCC framework. Articles were selected through a systematic screening process, and data were extracted and analyzed thematically. The review identified 25 key factors associated with nurses’ job satisfaction, categorized into four main domains: (1) organizational and leadership factors, (2) individual nurse characteristics, (3) work environment, and (4) professional development and practice. Supportive leadership, organizational commitment, and recognition were strongly associated with higher satisfaction. Improving nurses’ job satisfaction through targeted strategies is a critical component for ensuring high-quality patient care and favorable outcomes. Strategies such as enhancing work conditions, fostering leadership support, and providing development opportunities are vital for strengthening both nurses' well-being and patient outcomes.
