Abstract

Overweight has become a serious public health issue, as proven by the increasing morbidity and mortality rates in this population. Therefore, physical exercise interventions such as high-intensity interval training (HIIT) and moderate-intensity continuous training (MICT) are needed as training programs that can improve cardiorespiratory fitness (CRF) and reduce cardiometabolic risk. The aim of this study is to determine the differences in the effects of HIIT and MICT on CRF and the waist-to-hip ratio (WHR). This study used the randomized control group pretest-posttest design research method. The research sample included of 20 adolescents divided into two groups. Group 1 received the HIIT intervention, and Group 2 received the MICT intervention. CRF was assessed using the 20-meter shuttle run test (20m-SRT) also WHR. The mean test on CRF show that in group 1 (p<0.05) and group 2 (p<0.05) there are significant differences pre and post-test training. The Mann-Whitney U-test results show significant difference (p<0.05). Meanwhile, the WHR mean test results show that in group 1 (p<0.05) and group 2 (p<0.05) also has significant differences pre and post-test training. The Independent sample t-test results show no significant difference (p>0.05). This study concluded that HIIT is more effective than MICT in improving cardiorespiratory fitness. Thus, HIIT is as effective as MICT in reducing the waist-to-hip ratio of overweight adolescents.

Abstract

Sport is a structured and planned physical activity aimed at improving fitness and overall health. One rapidly growing discipline is rock climbing, particularly in the speed category. This high-intensity anaerobic activity often causes muscle fatigue, elevated blood lactate, and increased recovery heart rate. Excessive lactate accumulation disrupts recovery and may impair performance, making effective recovery methods essential. Two commonly used techniques are Proprioceptive Neuromuscular Facilitation (PNF) stretching and cold water immersion, or ice bath. This study compared the effectiveness of PNF stretching and ice bath in reducing blood lactate and recovery heart rate among rock climbing athletes in Denpasar City. A quasi-experimental pre-test–post-test design was conducted on three groups: PNF stretching (n=6), ice bath (n=6), and control (n=6). Interventions were performed twice weekly for two weeks. Blood lactate levels were measured with the Lactate Scout, while recovery heart rate was recorded manually at one and five minutes post-exercise. Results indicated that both PNF stretching and ice bath significantly reduced blood lactate (p<0.05) compared to control, but the ice bath produced greater and more consistent reductions. For recovery heart rate, no significant differences were found between groups (p>0.05). However, the ice bath group displayed a more favorable downward trend, suggesting potential clinical relevance. PNF stretching did not significantly affect heart rate recovery compared to ice bath or control but still demonstrated positive recovery effects. In conclusion, the ice bath method proved more effective than PNF stretching in reducing blood lactate, making it a recommended recovery strategy for speed climbing athletes. PNF stretching remains valuable for flexibility and muscular relaxation and may be best applied alongside other recovery methods. Future studies should investigate long-term outcomes, varied immersion protocols, and larger athlete populations to enhance generalizability

Abstract

Background: Diabetic retinopathy (DR) represents a leading cause of preventable blindness in Southeast Asia, where diabetes prevalence continues rising dramatically. Deep learning models show promising diagnostic accuracy for DR detection, yet validation in Southeast Asian populations remains limited. Objective: To evaluate the accuracy and clinical applicability of a deep learning model for early DR detection through comprehensive qualitative analysis in Indonesian healthcare settings. Methods: A mixed-methods diagnostic validation study was conducted across three Indonesian provinces from January 2023 to December 2024. The study employed a convolutional neural network-based deep learning model trained on 15,000 retinal images for DR classification. Qualitative data collection included semi-structured interviews with 30 ophthalmologists, 20 primary care physicians, 15 healthcare administrators, and 40 patients. Thematic analysis explored stakeholder perspectives on diagnostic accuracy, implementation barriers, and clinical integration potential. Results: The deep learning model demonstrated 89.3% accuracy (95% CI: 86.7-92.1%), 91.7% sensitivity, and 87.1% specificity for detecting referable DR. Qualitative analysis revealed high stakeholder acceptance (87.5% patient trust, 90.0% physician interest) despite implementation concerns. Key themes included diagnostic accuracy validation needs, workflow integration challenges, infrastructure requirements, and cost-effectiveness potential. Primary barriers included image quality standardization, internet connectivity limitations, and regulatory approval processes. Conclusion: Deep learning models demonstrate promising diagnostic performance for DR screening in Southeast Asian populations, with strong stakeholder support for implementation. However, successful deployment requires addressing infrastructure limitations, regulatory frameworks, and clinician training needs. These findings support the potential for AI-enhanced DR screening to improve early detection outcomes in resource-constrained healthcare systems across Southeast Asia.

Abstract

Limited hamstring flexibility and decreased knee extension range of motion (ROM) are risk factors for musculoskeletal injuries in recreational female runners. This study aimed to compare the effectiveness of proprioceptive neuromuscular facilitation (PNF) with nerve glide in improving hamstring flexibility and knee extension ROM. This quasi-experimental pre-test–post-test study involved 20 female runners from the Solo Runners community, who were randomly assigned to two groups (PNF and nerve glide, n=10 each). Both groups underwent 12 intervention sessions over 4 weeks (3 times/week). Flexibility was measured using the Sit and Reach Test (SRT), while knee extension ROM was measured using the Active Knee Extension (AKE) test. Data were analyzed using the Wilcoxon test, paired t-test, and Mann–Whitney U test. Both interventions significantly improved hamstring flexibility (PNF Δ=+7.1 cm, p=0.004; nerve glide Δ=+5.4 cm, p=0.004) and knee extension ROM (PNF Δ=−13.2°, p<0.001; nerve glide Δ=−10.1°, p=0.005). The improvement in flexibility was greater in the PNF group than in the nerve glide group (p=0.001), whereas the between-group difference in Δ ROM was not statistically significant (p=0.057). PNF is more effective than nerve glide in improving hamstring flexibility, whereas both techniques yield similar results in improving knee extension ROM. The selection of technique should be tailored to the primary source of movement limitation, whether muscle-dominant or nerve-related.

Abstract

The worldwide population of older adults is growing, which may yield a demographic bonus; yet, this growth simultaneously heightens frailty risk among the elderly. The dynamic and reversible nature of frailty provides opportunities and challenges in overcoming the aging process. Multicomponent exercise and concurrent exercise are two prominent forms of exercise in elderly exercise programs to reduce frailty risk. This research was to prove the difference in the effect of multicomponent exercise and concurrent exercise on reducing frailty risk in the elderly in Karangasem. Pre and post-test two group methods were used in this study’s true experimental design. Twenty-two participants were chosen by a simple random sample technique and subsequently divided into two groups: the multicomponent exercise group (n = 11) and the concurrent exercise group (n = 11). The exercises were given 3 times for 8 weeks. Frailty risk was assessed based on the Fried's Frailty Phenotype criteria. The Paired Sample T-test results indicated a significant difference in frailty risk reduction before and after exercise in both the multicomponent exercise group (p = 0.001; p < 0.05) and the concurrent exercise group (p = 0.001; p < 0.05). The Independent Samples T-test results indicated no significant difference in frailty risk reduction between the multicomponent exercise group and the concurrent exercise group (p = 0.318; p > 0.05). Based on the study’s findings, multicomponent exercise and concurrent exercise are equally good in reducing frailty risk in the elderly in Karangasem.

Abstract

The COVID-19 pandemic has significantly increased anxiety and depression prevalence among young adults globally, with Indonesia experiencing substantial mental health challenges. Mobile application-based cognitive behavioral therapy (mCBT) interventions have emerged as promising solutions to address these widespread mental health issues. Objective: This randomized controlled trial aimed to evaluate the efficacy of a mobile CBT application in reducing anxiety and depression symptoms among Indonesian young adults in the post-COVID-19 pandemic period through comprehensive qualitative analysis. Methods: A mixed-methods randomized controlled trial was conducted with 25 young adults aged 18-25 years. Participants were randomly assigned to receive either the mCBT intervention or waitlist control. Qualitative data were collected through in-depth semi-structured interviews, focus group discussions, and thematic analysis following a phenomenological approach. Results: Thematic analysis revealed six major themes: Digital Therapeutic Alliance (89 mentions), Behavioral Change Process (92 mentions), Empowerment Through Technology (76 mentions), Resilience Building (67 mentions), Social Connection in Digital Space (54 mentions), and Challenges and Barriers (43 mentions). Participants demonstrated significant improvements in coping mechanisms, stress management, and emotional regulation through the mCBT intervention. Conclusions: Mobile CBT applications show promising efficacy in addressing post-pandemic anxiety and depression among Indonesian young adults. The intervention facilitated meaningful behavioral changes, enhanced self-efficacy, and provided accessible mental health support during challenging recovery periods

Abstract

Educational administration and supervision are two important aspects of the education system that have a strategic role in improving the quality of education. They function to manage, supervise, and develop the entire learning process at the Faculty of Medicine, Islamic University of North Sumatra. Educational administration focuses on the management of human resources, facilities, and educational policies so that learning objectives can be achieved effectively and efficiently. Meanwhile, educational supervision functions as professional development for lecturers to improve the quality of the learning process. This study aims to describe the relationship and role of educational administration and supervision in improving the quality of education at the Faculty of Medicine, Islamic University of North Sumatra. The method used is a literature study (library research) with a descriptive qualitative approach, using various sources of books, scientific journals, and laws and regulations. The results of the study indicate that integration between administration and supervision is the key to success in improving the quality of educational institutions. Effective administration supports the implementation of systematic supervision, while quality supervision strengthens the implementation of administrative functions. The collaboration of the two produces educational management that is directed, transparent, and oriented towards improving the professionalism of lecturers at the Faculty of Medicine, Islamic University of North Sumatra

Abstract

The Integrated Health Service Post (Pos Pelayanan Terpadu), better known as POSYANDU, serves as a means for community health empowerment because its implementation model is activated and initiated by the community, aligned with their respective needs. The role of cadres (health volunteers) in organizing Posyandu is crucial, as they are not only providers of health information to the community but also community mobilizers, encouraging people to visit the Posyandu and adopt clean and healthy living behaviors. The objective of this study was to determine the role of cadres in the implementation of Posyandu activities in Pudun Jae Lk I Village, within the working area of the Batunadua Health Center, Padangsidimpuan City, in 2025. This study used a descriptive qualitative method. There were 10 informants in this study, consisting of 4 mothers with toddlers, 4 Posyandu cadres, 1 health worker, and 1 Posyandu leader. Data analysis was conducted using qualitative descriptive methods, and data validity was ensured through data triangulation. The study results indicated that cadres had not yet fulfilled their roles in managing malnutrition among toddlers. Obstacles faced by the cadres included a lack of knowledge about Posyandu, particularly concerning malnutrition, due to a lack of training. The conclusion of this study is that the role of cadres in implementing Posyandu activities in Pudun Jae Lk I Village, Batunadua Health Center working area, Padangsidimpuan City, has not been carried out optimally. It is recommended that Posyandu cadres and officers enhance the knowledge and skills of cadres, particularly in toddler nutrition, and improve counseling programs for mothers regarding Posyandu and its benefits for mothers and toddlers

Abstract

The infant mortality rate in Indonesia based on the 2012 Indonesian Demographic and Health Survey (SDKI) was 32 per 1,000 live births and the Neonatal Mortality Rate (AKN) in 2012 was 19 per 1,000 live births.This research is a quantitative analytical study with a case-control design using secondary data from medical record. The population size in this study was 104 mothers who gave birth with low birth weight (LBW).Padangsidimpuan City Hospital in 2025 period from January to September 2025. Researchers can draw the following conclusions 51% of mothers who gave birth to LBW were at risk, 79.8% had low education, 78.8% had high risk parity, 41.3% were working mothers and 65.4% experienced complications during pregnancy. Factors related to the incidence of LBW are maternal age (p value 0.000), education (p value 0.002), parity (p value 0.002) and pregnancy complications (p value 0.000). The dominant variable is maternal age with an OR value of 5.042 (95% CI 2.782-9.132) which means that mothers who are at risk (<20 and >35 years) have a 5.04 times higher chance of giving birth to LBW babies compared to mothers who are not at risk (20-35 years)

Abstract

Background: Many women experience physical discomfort during menstrual periods with symptoms of menstrual disorders, such as abdominal pain and emotional instability, including mood swings and depression. PMS decreases adolescent creativity and productivity. Vitamin B6 induces the production of serotonin, which is assumed to reduce PMS symptoms. Objective: To find out the effectiveness of vitamin B6 supplementation in reducing PMS symptoms and which symptoms are most responsive to Vitamin B6. Methods: We conducted quasi-experimental, randomised, blinded, and controlled trials. The intervention and control groups each consisted of 20 adolescent girls aged 15-17 years. Adolescent girls who experienced PMS were randomly divided into two groups: group A was given vitamin B6 supplementation, and group B was given a placebo. where the respondent does not know which group he belongs to. The modified Premenstrual Syndrome Questionnaire and Premenstrual Symptom Questionnaire were used to determine the severity of PMS before and after the intervention. The data was analysed with a T-test. Initially, 154 girls were screened using an initial questionnaire, of which 142 were known to have PMS. 40 adolescent girls were randomly assigned to either a vitamin B6 group or a placebo. Results: There was a significant difference in the severity of PMS before and after in the group given vitamin B6 supplementation intervention with a P value of ≤ 0.05. The change in the severity score was the most in type D PMS. Conclusion: Vitamin B6 supplementation has an effect in reducing PMS. The largest score decline was in type D (depression), followed by type A (anxiety). Therefore, vitamin B6 may be recommended as a supplement for adolescent girls who experience PMS.
