Abstract

Background: The 2004 Tsunami in Aceh caused infrastructure damage, thousands of deaths, and significant economic and health crises. Health facilities such as Community Health Centers (Puskesmas) which are the main health facilities in saving patients' lives were destroyed. To overcome this impact, health infrastructure must be rebuilt with earthquake-resistant building structures using certain materials and systems to withstand vertical and lateral loads that occur during earthquakes. In addition, nurses as primary health workers must have sufficient knowledge and skills to prevent loss of life if the disaster occurs through disaster mitigation preparedness. Purpose:  To identify the geographical distribution of Puskesmas locations in Banda Aceh City based on the level of tsunami risk and assess the level of knowledge of nurses' preparedness in facing disaster situations. Methods: Descriptive research type with spatial analysis method was used in this study. The data used included topographic maps, tsunami risk maps, and Puskesmas locations, which were analyzed using ArcGIS software. For nurses' knowledge level data, the population used was the entire number of nurses at the Banda Aceh City Puskesmas and used a total sampling technique which resulted in a sample of 87 nurses. Results: The study showed that 7 out of 11 Puskesmas (63.6%) in Banda Aceh were in the moderate to high tsunami risk zone. The level of knowledge of nurses regarding tsunami disaster preparedness was mostly good, namely 65 people (75%). Conclusion: Almost half of the Puskesmas in Banda Aceh are geographically located in the high tsunami risk zone category and the level of knowledge of community health center nurses regarding tsunami disaster preparedness is mostly good.

Abstract

Background: Inguinal hernia occurs when there is a protrusion of the contents of a cavity through a weak part of the inguinal gap in the abdominal wall. The occurrence of inguinal hernia is caused by repeated and continuous increase in intra-abdominal pressure. The increase in inguinal hernia cases has now reached 20 million cases, most of which are suffered by late adults to the elderly with the main problem being acute pain and its management through herniotomy. Slow Deep Breathing Exercise is one of the non-pharmacological therapies that is often used to overcome acute pain felt by pre-operative patients. Purpose:  To provide an overview of the application of slow deep breathing in the management of acute pain in preoperative patients with left lateral inguinal hernia treated at the Zainoel Abidin Regional General Hospital, Banda Aceh. Methods: Descriptive research with case studies through the nursing process approach from assessment, nursing diagnosis, planning, implementation, and evaluation. The subject of the study was 1 Pre-Operative Hernia Inguinalis Lateralis Sinistra patient who was treated in the internal medicine room who was given Slow Deep Breathing Exercise intervention. The research instruments used to collect data were medical surgical nursing care sheets and pain measuring instruments using the Numeric Rating Scale. Results: After implementing Slow Deep Breathing Exercise twice a day (duration 15 minutes during the intervention) for 3 days combined with oxygen therapy via nasal cannula (3 liters/minute), the patient's main problem, namely acute pain, decreased from a scale of 8 to 6 with all patient activities still being assisted. Conclusion: Based on the research results, it can be concluded that the patient's problem, namely acute pain, was partially resolved through the Slow Deep Breathing Exercise intervention because the pain still often occurs with an uncertain duration and interferes with the patient's basic needs.

Abstract

Background: Difficulty in establishing social relationships is a significant issue that can negatively impact the psychological well-being of nursing students. Often feeling isolated, withdrawing from the social environment, and experiencing decreased academic motivation are difficulties that usually arise in students. Various factors contribute to this condition, including pressure to be accepted in certain social groups, such as trying to adjust appearance or behavior to be accepted by the environment, which actually triggers stress and identity crises. Understanding the background and causal factors to the differences in the level of social difficulties in nursing students is very important in efforts to improve psychological well-being and support the academic success of nursing students. Purpose: To determine the differences in social relationship difficulties in students at different levels of the diploma program in nursing. Methods: The study used an analytical survey design with a comparative approach. The research sample consisted of 642 respondents from students of the diploma program in nursing at Poltekkes Kemenkes Aceh and was obtained through a total sampling technique. The instrument used to measure the level of difficulty in social relationships was The Revised Cheek and Buss Shyness Scale (RCBS), which has been proven valid and reliable in measuring the level of individual personality. Data analysis was carried out using the Kruskal-Wallis test to compare differences in the level of difficulty in social relationships based on the group of variables studied. Results: Based on the results of statistical tests using the Kruskall Wallis test, it shows that the p-value is 0.01 (<0.05), which indicates that there is a significant difference in the average difficulty of social relationships in students between levels of the diploma program in nursing at Poltekkes Kemenkes Aceh. Conclusion: There is a significant difference in the level of difficulty of social relations between students based on the level of the diploma program in nursing at Poltekkes Kemenkes Aceh, which shows that the level of education is related to students' social adaptation abilities.

Abstract

Background: After surgery with subarachnoid block anesthesia, patients need to be closely observed, one of which is by monitoring the motor function of the lower extremities using the Bromage Score parameter. The length of motor recovery time is an important indicator because recovery that is too long can increase the risk of complications, such as paresthesia, which ultimately leads to an extension of the patient's treatment period. The time to achieve a normal Bromage Score is influenced by various factors, one of which is the patient's physical status which is classified through the American Society of Anesthesiologists (ASA) assessment. Accurate ASA physical status assessment is very important because it can help predict the risk of anesthesia, estimate the duration of recovery, and minimize the occurrence of serious complications that have the potential to threaten patient safety. Purpose: To determine the correlation between ASA physical status and the achievement of Bromage score ≤ 2 in post-subarachnoid block anesthesia patients at PKU Muhammadiyah Hospital, Yogyakarta. Methods: This study is a quantitative study with a correlation analytical approach. The population taken were patients who would undergo surgery with subarachnoid block anesthesia using a saturated sampling technique with a sample size of 76 respondents. The research instrument used a physical status observation sheet according to ASA standards and an observation sheet for achieving a Bromage score ≤ 2 and a watch to calculate the achievement time. Results: Based on statistical analysis using the Spearman's Rho test, a 2-tailed sig value of 0.000 < 0.05 was obtained, which means that there is a correlation between ASA physical status and the achievement of a Bromage score ≤ 2 in patients after subarachnoid block anesthesia. Conclusion: There is a correlation between ASA physical status and the achievement of Bromage score ≤ 2 in patients after subarachnoid block anesthesia at PKU Muhammadiyah Hospital Yogyakarta with a very strong level of relationship and a positive direction of correlation.

Abstract

Background: Delivery through cesarean section (C-Section) is a way of giving birth to a fetus through an incision in the abdominal wall and uterine wall by laparotomy. One of the problems that occurs in post C-Section patients is a nutritional deficit. Adequate nutrition can help in the recovery of health, because nutrition is also a source of energy needed by various organs in the body as well as a source of building and regulating substances in the body, especially post C-Section mothers. Unfulfilled nutrition in post C-Section mothers can have a negative impact on the wound healing process, breast milk production, and general health. Purpose: To find out the implementation of nursing care for post C-Section patients, especially in fulfilling nutritional needs. Methods: Descriptive research type with case study design that describes the implementation of nursing care in post C-Section patients in fulfilling nutritional needs. The research respondents were 2 post C-Section patients, with the criteria of first day post C-Section, good general health condition, and willing to be respondents. Data were taken using the postpartum assessment format, while case study data were presented in the form of nursing care narratives. Results: Based on the results of the implementation of nursing care, it was found that the patient complained of pain in the area of ​​the former operation, no appetite, nausea and vomiting, there was a scar on the stomach, an IV and catheter were installed. From these data, the following problems were obtained: acute pain, nutritional deficits and risk of infection. This case study focused on the problem of nutritional deficits. After nursing actions were carried out for 3 days with the main intervention of nutritional management and supporting interventions of nutritional monitoring, the nutrition of both research subjects could be met according to the set time target, with the results of the evaluation of the problem being resolved and the intervention being continued at home by the family. Conclusion: After nursing care was carried out for 3 days, the problem of nutritional deficit in 2 research respondents, especially on the third day, the problem was resolved with clinical indicators of increased food intake, increased desire to eat, increased frequency of eating and increased appetite.

Abstract

Background: Alexithymia is a psychological condition characterized by difficulty in identifying, understanding, and expressing emotions. Individuals with alexithymia often experience difficulties in managing emotions effectively, so this condition is often associated with various psychological problems (anxiety, depression, to addictive behavior). The prevalence of alexithymia is quite high in various populations including adolescents and this shows the importance of a deep understanding of the factors and impacts that contribute to the emergence of this condition. The current phenomenon of adolescents who are always associated with antisocial life and are more focused on social media to other virtual worlds may be the impact of alexithymia. Purpose: To test the relationship between alexithymia and social media addiction in students of SMPN 1 Sumbang Banyumas.. Methods: This study is a quantitative study with a correlation analytical approach. The population taken were patients who would undergo surgery with subarachnoid block anesthesia using a saturated sampling technique with a sample size of 76 respondents. The research instrument used a physical status observation sheet according to ASA standards and an observation sheet for achieving a Bromage score ≤ 2 and a watch to calculate the achievement time. Results: Based on statistical analysis, it shows a very significant positive relationship between alexithymia scores and social media addiction scores with a p-value of 0.000 and a correlation coefficient value of 0.631. Conclusion: There is a relationship between alexithymia and social media addiction in students at SMPN Sumbang Banyumas (the relationship between the two variables is strong). This can happen because the higher the level of alexithymia of a student, the higher the level of social media addiction.

Abstract

Background: The pregnancy process is something that is awaited by every couple but is not free from various psychological challenges, one of which is anxiety in pregnant women. Husband's support is one of the important factors that can determine the level of anxiety of pregnant women, especially in the third trimester. Emotional, instrumental, and informational support from the husband can provide a sense of security and comfort to pregnant women. Purpose: To determine the relationship between husband's support and anxiety in pregnant women in the third trimester. Methods: This study uses quantitative research with correlational methods through a cross-sectional approach. The research location is at Posyandu, Samudra Kulon Village, Gumelar District, Banyumas Regency with a sample of 62 parents and toddlers aged 3-5 years taken using Purposive Sampling. Data collection was carried out using the Developmental Pre-Screening Questionnaire (KPSP) consisting of 9-10 questions and the Parenting Styles and Dimensions Questionnaire - Short Version (PSDQ) consisting of 32 questions. Data analysis used the Spearman rank test. Results: There is a significant and strong relationship between husband support and anxiety in third trimester pregnant mothers with a significance value of p value < 0.05 with a correlation coefficient value of r = -691. Conclusion: There is a positive correlation between parenting patterns and the development of toddlers aged 3-5 years. The better the parenting patterns applied by parents, the more optimal the development of the child.

Abstract

Background: Hospital-acquired nosocomial infections, which cause more than 100,000 patient deaths each year, are often associated with poor nurse compliance with prevention guidelines, including the use of Personal Protective Equipment (PPE). Compliance with PPE effectively reduces the risk of infection, but several studies have shown low levels of nurse compliance, associated with unfavorable work environments, high workloads, and lack of knowledge and competence. Following the Covid-19 pandemic, compliance with PPE has become increasingly important in preventing the spread of infectious diseases. Purpose: To determine the factors related to the level of nurse compliance in using PPE. Methods: This study uses a literature study method to explore factors related to the level of nurses' compliance in using PPE. Data were taken from articles published from January 2023 to January 2024 through the Pubmed National Library of Medicine database. Article searches were limited by the criteria of articles having full text, in English, and keywords used in the search including compliance, nurses, personal protective equipment (PPE), and nosocomial infections. Results: Research shows that nurses' knowledge, attitudes, behaviors, education, training, and motivation play a significant role in increasing awareness of PPE use. In addition, adequate provision of PPE, supporting facilities, safety culture, hospital organizational commitment, and government regulations also determine compliance. Conclusion: Although the availability of PPE facilities is a major contribution to prevention, nurse preparedness is needed to reduce disease transmission and ensure that the health care system continues to function during times of crisis, especially during the spread of infectious diseases.

Abstract

Background: Cardiac catheterization is an invasive procedure that can cause various emotional reactions in patients, one of which is anxiety. This anxiety can arise due to pain and fear of the catheterization procedure involving a vital organ of the body, namely the heart. High levels of anxiety can have an impact on increasing blood pressure to decreasing oxygen saturation, so that it can affect the smoothness of the cardiac catheterization process itself. To overcome this, one of the non-pharmacological interventions that can be applied is relaxation therapy through mindfulness meditation. Purpose: To find out the effect of mindfulness on the anxiety level of pre-cardiac catheterization patients. Methods: Quantitative research type with quasi-experimental design with one group pretest-posttest design. The sampling technique used was accidental sampling. The sample in this study was 34 respondents with inclusion criteria, namely pre-cardiac catheterization patients with full consciousness and cooperation. The study was conducted from March to May 2024 by measuring anxiety levels using the Visual Analog Scale (VAS-A). Researchers measured the anxiety of pre-catheterization patients and provided mindfulness therapy intervention for 1 hour before cardiac catheterization, and took post measurements afterwards. Results: Based on the results of the Wilcoxon test, a p-value of 0.000 (<0.05) was obtained, which means that there is an influence of mindfulness therapy on the level of anxiety of pre-cardiac catheterization patients. Conclusion: After the intervention, there were 25 respondents who experienced a decrease in anxiety levels, 1 respondent experienced an increase, and 8 respondents with the same level of anxiety. The majority of anxiety levels after the intervention were at a moderate level.

Abstract

Background: The development of children aged 3-5 years is a golden age that is greatly influenced by parenting patterns. Through the right parenting patterns can support the development of motoric, cognitive, social, and language of children. However, if not appropriate, it can inhibit child development and increase the risk of developmental delays. Purpose: To determine the relationship between parenting patterns and the development of toddlers aged 3-5 years in Samudra Kulon Village, Gumelar District, Banyumas Regency Methods: This study uses quantitative research with correlational methods through a cross-sectional approach. The research location is at Posyandu, Samudra Kulon Village, Gumelar District, Banyumas Regency with a sample of 62 parents and toddlers aged 3-5 years taken using Purposive Sampling. Data collection was carried out using the Developmental Pre-Screening Questionnaire (KPSP) consisting of 9-10 questions and the Parenting Styles and Dimensions Questionnaire - Short Version (PSDQ) consisting of 32 questions. Results: Spearman rank correlation analysis shows a p-value ≤ 0.05, which means that there is a relationship between parenting patterns and the development of toddlers aged 3-5 years with a correlation coefficient value of 0.877 (the relationship between the two variables is very strong). Conclusion: There is a positive correlation between parenting patterns and the development of toddlers aged 3-5 years. The better the parenting patterns applied by parents, the more optimal the development of the child.
