Abstract

The Mobile JKN application, a groundbreaking innovation by BPJS Kesehatan, empowers national health insurance participants to conduct various service transactions online, including registering for outpatient treatment at health facilities. Many patients have not taken advantage of the ease of online registration through Mobile JKN because they do not understand how to use it and lack education related to Mobile JKN online registration, so they choose the manual method. This study aimed to evaluate the effectiveness of the Mobile JKN application in boosting online outpatient registration using descriptive qualitative research methods. Through interviews, analysis of primary data on outpatient visits, and a psychosocial educational approach, I act as an education provider to provide knowledge and understanding of using theĀ  JKN mobile application. The evaluation revealed that 5% of the socialization of the Mobile JKN application proved successful and effective in increasing online outpatient registration in May 2024. The percentage increase is insignificant because the researcher only conducted socialization in April 2024.

Abstract

Diabetic ketoacidosis (DKA) is a complication of diabetes mellitus characterized by uncontrolled of hyperglycemia, metabolic acidosis and increased ketone concentrations in the body. Thyroid crisis is one of the acute and life-threatening complications of hyperthyroidism where the symptoms involve multi-organ systems. The coincidence of DKA with Thyroid Crisis is rare and the pathophysiology of this coincidence is not well known and is still widely debated. Early recognizing and managing both emergencies will improve the success of patient management. In these case report there is a coincident of DKA and thyroid crisis at Klungkung Regional General Hospital. Case Report: KS, female, 57 years old came to the emergency room of RSUD Klungkung with the main complaint of nausea and vomiting accompanied by shortness of breath, cough with yellow phlegm and fever since 5 days ago. Other complaints were weakness, dizziness, and palpitations. The patient has a history of DM since 3 years ago and is routinely treated using basal and prandial insulin. Thyroid disease was known since 6 months ago and routinely took Tiamazol, but since 5 days ago the patient stopped the medication because he felt weak and ate little because of complaints of nausea and vomiting with concerns that blood sugar was falling. On examination of vital signs the patient appeared very ill, consciousness E3V4M6 appeared agitated. Blood pressure 130/80 mmHg, pulse 102x/min. Respiratory rate 28x/min seemed fast and deep breaths, axillary temperature 390C with oxygen saturation 89% room temperature and 98% with oxygen 4 liters per minute nasal cannul. On auscultation of the lungs, coarse rhonki sound in the right and left paracardial confirms the clinical pneumonia. On laboratory examination, hyperglycemia was found, with blood glucose (BS) 456mg/dL, urinalysis results obtained ketones +3 and glucose +3. The results of blood gas and electrolyte analysis showed metabolic acidosis at pH 7.17, PCO2 19.0mmHg, HCO3- 7.0 mmol/L, BE (B) -22mmol/L and Potassium 5.4mmol/L. FT4 was 29.87 pmol/L and TSH <0.10 uIU/mL with a Burch Wartofsky score of 55 supporting the diagnosis of thyroid crisis. The patient was admitted to the intensive care unit with the management of hydration to overcome fluid and electrolyte balance disorders, blood glucose regulation with rapid insulin drip, administration of thyroid hormone activity antagonists and management of pneumonia with adequate antibiotics. Discussion: DKA and thyroid crisis are two separate events and their co-occurrence is rare. Both are life-threatening conditions that if not treated promptly will lead to death. The pathophysiology of the coincidence of DKA and thyroid crisis is unknown and debated. One theory states that thyrotoxicosis will change carbohydrate metabolism and increase insulin resistance by increasing glycogen breakdown in the liver, while uncontrolled glucose production will increase metabolic damage. Management of this coincidence requires tight regulation of the patient's glucose levels as the administration of corticosteroids in the management of thyroid crisis runs the risk of increasing glucose levels and thus aggravating the patient's DKA condition. Conclusion: A case of DKA coincidence with thyroid crisis triggered by pneumonia in a 57-year-old woman at RSUD Klungkung has been reported. By correcting fluid and electrolyte balance disturbances, tight blood glucose regulation, administration of antithyroid and corticosteroids at optimal doses and handling pneumonia with adequate antibiotics gave good results to the patient.

Abstract

The article on the influence of training, discipline, intrinsic motivation and competence on employee performance with consumer behavior as a mediating variable is a scientific literature article in the scope of human resource management science. The purpose of this study is to build a hypothesis regarding the influence between variables that can later be used for further research in the scope of marketing management. The research method used is descriptive qualitative. The data collection technique uses literature studies. Data were obtained from previous studies that are relevant to this study and sourced from academic online media such as Publish or Perish, Google Scholar, digital reference books and Sinta journals. Abstract

Humans are an important asset in any organization or company. As a valuable resource, humans are assets that need to be managed properly. Therefore, a human capital strategy is needed to improve the quality of human resources. This study uses a systematic literature review approach to analyze human capital strategies for improving the quality of human resources. The data collected through Google Scholar and Mendeley.com was analyzed, summarized, described, and concluded. This research has contributed to providing guidance for organizations or companies to develop a human model strategy. The limitation of this research is that it only uses limited references collected from the search engine used so that researchers in the future can research more deeply about human capital strategies for a certain sector or several sectors.

Abstract

With limited electrical resources and the increasing use of loads on the industry, it is required to optimize the use of electrical power. Thus, it requires continuous monitoring and automatic load control. Overall monitoring of power loads, and electrical control settings in the optimization of production machines that can be monitored remotely and comprehensively. To overcome this problem, the design of the electrical power load control device on production machines with scada based on programmable logic controllers and smartphones is made. This study uses an experimental method to obtain a suitable machine simulator, then the following steps are carried out: collecting data and information, determining the controller and communication system, making hardware design, wiring simulator, software controller design, android smartphone software design and HMI software design. As for the control of the electric power control system in optimizing the operation of production machines with scada and smartphones. From the test results obtained a system that can work well. This is evidenced by looking at the measurement data for testing the input/output signal from the controller by looking directly at the controller, as well as on the HMI monitor by displaying the amount of electrical power used and the amount of available electrical resources, which is done by simulating analog input/output. with a scale of 0-100% (0-10 V), 0-1000 KW (0-10 V).

Abstract

Employee productivity improvement article: analysis of employee engagement in the scope of human resource management science. This article aims to make a hypothesis related to the relationship between factors, which can then be used for further research in the field of human resource management. The approach used in this study is descriptive qualitative. The data used in this study comes from previous studies that are still relevant to the current investigation. Data were collected from leading academic online platforms, including Publish or Perish, Google Scholar, digital reference books, and Sinta journals. Abstract

Lecturers often face unique challenges in maintaining a balance between work and personal life due to the nature of their profession. Therefore, lecturers at IAIN Kerinci require work-life balance to fulfill their duties effectively. This study aims to identify aspects related to work-life balance among lecturers at IAIN Kerinci and strategies that institutional management can implement to sustain this balance. The research employs a Systematic Literature Review (SLR) method using the PRISMA pattern. Findings highlight several aspects linked to lecturer work-life balance, including: 1) time allocation between work and family, 2) workload and commitments, 3) dual roles as lecturers and family members, 4) resilience in fulfilling tridharma responsibilities, and 5) job satisfaction. Human resource management implications include scheduling considerations, adequate rest periods, and access to physical and mental health resources, especially for lecturers' families.

Abstract

The purpose of this study was to determine and analyze: (1) Price; (2) Product quality; (3) Consumer Satisfaction; (4) Price Effect on Consumer Satisfaction; (5) Effect of Product Quality on Consumer Satisfaction; and (6) Effect of Price and Product Quality on Consumer Satisfaction of Goat Milk at Company both simultaneously and partially. The research method used in this research is Descriptive and Verification, the unit of analysis in this study is the consumers of Goat milk Company in Bandung Regency and surrounding areas with a sample of 100 people. The analysis used in this research is Path Analysis with the help of the SPSS for Windows 22.0 program. Based on the results of the study, it was found that: (1) The price at Company is good (2) Product Quality at Company good (3) Consumer Satisfaction at Company is quite good (4) Price affects Consumer Satisfaction at Company (5) Product Quality affects Consumer Satisfaction at Company (6) Price and Product Quality have a joint effect on Consumer Satisfaction at Company. The results showed that price and product quality proved to have an effect on consumer satisfaction of Etawa peranakan goat milk at Company both partially and simultaneously, but when viewed partially, it turns out that product quality is the more dominant influence.
