Abstract

Upper gastrointestinal hemorrhage (PSCA) is bleeding from the lumen of the proximal gastrointestinal tract above the treitz ligament. The causes of PSCA can be classified into variceal bleeding and non-variceal bleeding. The most common causes of upper gastrointestinal bleeding are peptic ulcers, esophageal-gastric variceal veins, ulcerative esophagitis, Mallory-Weiss tears, and bleeding from gastrointestinal vascular lesions. Peptic ulcers are the most common cause of PSCA, accounting for about 50% of cases; the increased incidence is due to nonsteroidal anti-inflammatory drugs (NSAIDs). The Blatchford Score and Clinical Rockall Score are used in patients with non-variceal PSCA to identify the risk of mortality and rebleeding, as well as to triage patients to higher levels of hospital care or immediate endoscopy. Endoscopic examination of PSCA is the chosen way to establish the diagnosis of the cause of bleeding and at the same time useful for performing hemostasis. The top priority in dealing with PSCA hemorrhage cases is the determination of hemodynamic status and resuscitation efforts before establishing a diagnosis or administering other therapies. PSCA therapy is carried out based on the underlying etiology. Patients with variceal bleeding have a worse prognosis than patients with other PSCA etiologies.

Abstract

Frozen shoulder is clinically described as persistent shoulder pain and advanced exacerbation of the shoulder joint leading to upper extremity activity, significant disability, and functional limitations. The intervention that can be given to frozen shoulder is the administration of infrared which aims to stimulate heat and nociceptive receptors to feel warmth thereby increasing blood flow to the muscles and improving muscle stiffness and pain. The aim of this research is to determine the effectiveness of the infared modality to reduce pain in frozen shoulder patients. The research method used is a literature review approach, by searching for references using the PICO method. Results: From the 4 journals studied, infrared had an effect on reducing pain in patients suffering from frozen shoulder, with a therapy duration of 15-30 minutes and a p-value of p<0.05 showing a significant effect. Conclusion: infrared has the effect of reducing pain in patients with frozen shoulder. From the journals quoted, it is proven that there is a degree of reduction in pain when measured using the Visual Analogue Scale (VAS).

Abstract

Diabetic ketoacidosis (DKA) is a serious, acute complication of diabetes mellitus and is an emergency that must be treated immediately. DKA requires fast and precise management, considering the high mortality rate. DKA is a state of metabolic decompensation characterized by the triad of hyperglycemia, metabolic acidosis, and ketosis and is an emergency in the field of endocrinology that is most frequently encountered in daily practice. Data in America is that more than 100,000 patients are treated per year for DKA (3). . Although there is no community data in Indonesia, it seems that the incidence of DKA in Indonesia is not as high as in western countries, considering the low prevalence of type 1 DM. Reports of the incidence of DKA in Indonesia generally come from hospital data and especially in type II DM patients (4). Successful management of DKA requires correction of dehydration, hyperglycemia, acidosis and electrolyte abnormalities, identification of comorbid precipitating factors, and most importantly continuous patient monitoring.
