Abstract

This study aims to determine the protective activity of Bidens pilosa L. leaf ethanol extract (EEDK) on the immune system based on the white pulp diameter, and histopathology of the spleen of white rats given alcohol. The study was conducted experimentally using a completely randomized design (CRD), with treatments divided into 5 groups: negative control (K-) without EEDK or alcohol administration, positive control (K+) administered 10 ml/kg body weight of alcohol, treatment 1 (P1) administered 250 mg/kg body weight of EEDK and 10 ml/kg body weight of alcohol, treatment 2 (P2) given 500 mg/kgbb EEDK and 10 ml/kgbb alcohol, and treatment 3 (P3) given 750 mg/kgbb EEDK and 10 ml/kgbb alcohol. EEDK was administered 1 hour after alcohol administration and was given orally every day for 43 days. The parameters observed in this study were diameter of the white pulp, and histopathology of the white rat spleen. The observation data were analyzed using one-way ANOVA and followed by a post hoc test with the DMRT test to see the differences between treatments. The results showed that the ethanol extract Bidens pilosa L. potential as an immunomodulator by increasing the diameter of the white pulp of the spleen, and had a significant effect on spleen histology by showing protective activity with a decrease in the level of spleen tissue damage due to alcohol, particularly tissue fibrosis, lymphocyte apoptosis, and necrosis (p£0.05).

Abstract

Background: Maternal mortality remains a significant public health concern in Indonesia, where 19.3% of deliveries are still conducted outside healthcare facilities or without skilled birth attendants. The working area of Ambukha Public Health Centre (Puskesmas) has the lowest proportion of facility-based deliveries in the West Nias District (67% in 2023), indicating the need to analyse factors associated with delivery place choice. Objective: To identify factors related to the selection of delivery place among mothers in the working area of Ambukha Public Health Centre, Lolofitu Moi Subdistrict, West Nias District. Methods: This study employed an observational analytic design with a cross-sectional approach. The population consisted of all mothers who delivered within the Ambukha Public Health Centre area in 2023 (N=78), and samples were obtained using a total sampling approach. Data were collected using structured questionnaires and analysed using Chi-square tests and logistic regression. Results: Significant associations were found between the choice of delivery place and maternal knowledge (p=0.000; OR=2.267), maternal attitude (p=0.000; OR=4.250), national health insurance (JKN) membership (p=0.000; OR=5.333), access to healthcare facilities (p=0.000; OR=6.038), husband’s support (p=0.000; OR=4.250), perceived need factors (p=0.000; OR=3.333), as well as cultural beliefs (p=0.004; OR=2.473). Multivariate logistic regression identified maternal knowledge as the most influential factor in delivery place selection, as indicated by the most significant coefficient value (B = –4.204) and statistical significance (p=0.005). Conclusion: Maternal knowledge, attitude, JKN membership, access to healthcare facilities, husband’s support, perceived need, and cultural beliefs are significantly associated with the selection of delivery place. Efforts to increase facility-based delivery coverage in this area should prioritise improving maternal knowledge, followed by strengthening other supporting factors.

Abstract

Schizophrenia is a global issue with a significant increase in recurrence, affecting approximately 23 million people or 1 in every 345 individuals worldwide, with recurrence rates increasing from 28% in 2019 to 54% in 2021[2]. This narrative review examined the literature from 2015–2024 from PubMed and Google Scholar with inclusion criteria of pharmacist interventions in outpatient schizophrenia patients, the use of assessment instruments such as MARS, PANSS, and WHOQOL-BREF, and the reporting of quantitative outcomes related to adherence and quality of life. Seven studies that met the criteria showed that pharmacist counseling can significantly improve medication adherence. In the Si-Care model, adherence rates increased from 77.38% to 97.57% (an increase of 20.19%; p = 0.000). Higher effectiveness was seen in longer-duration interventions, interactive methods such as home visits, and implementation involving collaboration with psychiatrists, which also supported improvements in quality of life. However, variations in study design resulted in diverse findings, and single-session interventions tended to be less optimal than ongoing support. Without counseling, adherence rates remained low, at around 65%. Overall, the available evidence confirms the benefits of pharmacist counseling, but further research is needed to standardize intervention models and evaluate the long-term impact on outpatients with schizophrenia.

Abstract

The development of natural-based antibacterial agents is essential to reduce dependency on antibiotics and the risk of resistance. Sweet orange peel and fig fruit contain bioactive compounds with potential antibacterial effects. This study aimed to develop tea bag formulations made from sweet orange peel with the addition of fig fruit as an additive and to evaluate their in vitro antibacterial activity against Staphylococcus aureus. This experimental study employed two approaches, including disc diffusion and microdilution on several combination formulations of sweet orange peel and fig tea bags. The formulations consisted of F1 (1 g sweet orange peel and 1 g fig), F2 (1.5 g sweet orange peel and 0.5 g fig), and F3 (1.75 g sweet orange peel and 0.25 g fig). The disc diffusion assay showed that all formulations exhibited weak antibacterial activity against Staphylococcus aureus, producing inhibition zones of approximately 6–7 mm, except for F3 infusion, which demonstrated the widest inhibition zone with a mean diameter of about 8.21 mm (P-Value = 0.688). In the microdilution test, the MIC of F3 infusion was found to be 40 mg/mL, while the MBC was not detected at the tested concentrations. Overall, it can be concluded that the infusion of the tea bag formulation containing 1.75 g sweet orange peel and 0.25 g fig was the most promising combination; however, its antibacterial activity remained weak and was bacteriostatic rather than bactericidal.

malnutrition so that the child is too short for his age. Stunting has an impact on children's cognitive decline and productivity in the future. So far, addressing malnutrition has focused on babies from birth to five years old. In fact, the golden period is when the child is still in the womb until the age of two years. Therefore, a paradigm shift in handling malnutrition is needed. Therefore, the government is implementing the Acceleration of Nutrition Improvement Program with the First 1000 Days of Life Movement. SKI data for 2023, Dairi district is in second place with a figure of 32.6% of stunted toddlers out of 439 toddlers and based on the entry of toddler data on the e-PPGBM application in December 2024, the prevalence of stunted toddlers in the Sumbul Community Health Center, Sumbul District is around 13.98% or 258 toddlers. The incidence of stunting is influenced by several factors, namely maternal height, maternal education, compliance with taking blood supplement tablets and smoking. The aim of this research is to determine the determinants of stunting incidents at the Sumbul Community Health Center UPT, Sumbul District, Dairi Regency in 2024. This type of research is cross sectional with research methods. The population in this study were all mothers with toddlers. The number of samples in this study was 70 mothers.  Based on the research results, it was found that there was a relationship between height and stunting (p-value=0.004), there was a relationship between maternal education and stunting (p-value=0.004), there was a relationship between adherence to taking blood supplement tablets and stunting (p-value=0.000) and there was a relationship between smoking and stunting (p-value=0.000). Increasing health promotion to the community about the importance of parenting patterns and fulfilling nutrition in the First 1000 Days of Life, starting from pregnant women until children are 2 years old, as well as outreach about the health of young women and prospective brides.

Abstract

Background: Wasting, a form of acute malnutrition characterized by low weight-for-height, remains a critical public health problem in Indonesia. This condition increases the risk of child mortality and reflects short-term nutritional deficiencies. The prevalence of wasting in Indonesia continues to exceed the World Health Organization (WHO) threshold, underscoring the need for targeted interventions. Objective: This study aimed to analyze factors associated with wasting among children aged 6–24 months. Methods: A cross-sectional study was conducted at the UPT Puskesmas Bestari, Medan. The sample comprised 35 children aged 6–24 months selected using total sampling. Data were collected through interviews, nutritional status assessment (weight-for-height), and questionnaires. Data analysis included univariate analysis, bivariate analysis (Chi-square test), and multivariate analysis (logistic regression). Results: The prevalence of wasting was 71.4% (25 of 35 children). Bivariate analysis showed significant associations between wasting and inadequate dietary intake (p = 0.004), low maternal knowledge (p = 0.000), and low household income (p = 0.004). No significant associations were found between wasting and child sex (p = 0.421) or family size (p = 0.134). Multivariate analysis identified low maternal knowledge as the most dominant factor associated with wasting. Conclusion: Wasting among children aged 6–24 months is significantly influenced by inadequate dietary intake, low maternal knowledge, and low household income. Comprehensive interventions focusing on maternal nutrition education, strengthening household food security, and improving family economic capacity are essential to prevent wasting in this vulnerable age group.

Abstract

This study aimed to evaluate the effectiveness of a lotion enriched with butterfly pea flower extract (Clitoria ternatea) in improving skin hydration in mice (Mus musculus) exposed to ultraviolet (UV) light. Butterfly pea flower extract is known to contain anthocyanins and phenolic compounds, which act as antioxidants and are thought to maintain skin moisture. The study design used an experimental method. Mice were randomly allocated into five groups (n = 6 per group), consisting of a negative control group, a positive control group, and three treatment groups receiving butterfly pea flower extract lotion at concentrations of 0.5%, 1%, and 2%.  Skin hydration levels were measured using a Scalar Moisture Checker after five minutes of UV exposure. The results showed a significant difference in hydration levels between groups. The 1% butterfly pea flower extract lotion showed a significant increase in skin hydration compared to the negative control (p < 0.05), indicating its optimal moisturizing efficacy among the tested formulations. These findings indicate that butterfly pea flower extract has potential as an active ingredient in lotion formulations to improve hydration in skin damaged by UV light exposure. This study provides a scientific basis for the use of butterfly pea flower extract as a natural moisturizing agent.

Abstract

Stunting remains a significant nutritional problem in Indonesia, requiring practical and acceptable nutritional interventions. Moringa oleifera L. leaves are known to contain essential minerals such as zinc, iron, and iodine, which play an important role in supporting optimal growth. This study aimed to formulate effervescent tablets of Moringa leaf extract and evaluate their physical quality as an alternative nutritional supplement for stunting prevention. Moringa leaf extract was obtained through maceration and formulated into four effervescent tablet formulas with varying extract concentrations. The evaluations included granule flow properties, angle of repose, pH, size uniformity, weight uniformity, dispersion time, FTIR analysis, and hedonic testing. The results showed that all formulas met the physical quality requirements of effervescent tablets. The granules exhibited good flow characteristics with an angle of repose below 40°, while the tablet pH values ranged from 4,4 -5,8, indicating suitability for oral administration. All formulas complied with the standards for size and weight uniformity, and the dispersion time ranged from 1 to 2 minutes. FTIR analysis confirmed the presence of hydroxyl, carbonyl, nitroso, and benzene functional groups associated with flavonoids, saponins, and tannins, which contribute beneficial mineral content. Based on physical and organoleptic evaluations, Formula 2 showed the best combination of extract content and granule flow properties, while Formula 1 was the most preferred in terms of taste.

Abstract

Tuberculosis remains a major global health challenge, particularly due to the increasing emergence of drug-resistant strains, highlighting the need for new antimycobacterial agents from natural sources. Clerodendrum minahassae, a medicinal plant traditionally used in North Sulawesi, has demonstrated antibacterial activity; however, its antimycobacterial potential has not been previously explored. This study aimed to evaluate the antimycobacterial activity of the ethanolic leaf extract of C. minahassae against Mycobacterium smegmatis as a surrogate model and to estimate its minimum inhibitory concentration (MIC) and minimum bactericidal concentration (MBC). The extract was tested at concentrations of 5, 7.5, and 10 µg/disc using the disc diffusion method. Inhibition zone diameters increased in a concentration-dependent manner, ranging from 9.40 ± 0.52 mm to 10.60 ± 0.92 mm. Quantitative analysis using Bloomfield-based linear regression revealed a strong dose–response relationship (R² = 0.9926), from which MIC and MBC values of 0.63 µg/disc and 2.52 µg/disc were estimated, respectively. These findings indicate that the ethanolic extract of C. minahassae exhibits measurable inhibitory and bactericidal activity against M. smegmatis at relatively low concentrations. While this study represents an exploratory screening, the results support the potential of C. minahassae as a source of bioactive compounds with antimycobacterial relevance. Further investigations involving fractionation, compound identification, toxicity assessment, and validation against pathogenic Mycobacterium tuberculosis strains are warranted.

Abstract

Endometriosis is a chronic gynecological disease characterized by the growth of endometrial-like tissue outside the uterus, which triggers local inflammation and lesion progression. The pathological process involves increased proinflammatory cytokines, including TNF-α, and activation of proliferative and angiogenic pathways. Nanocurcumin, with higher bioavailability compared to conventional curcumin, is known to have the potential to modulate inflammation and inhibit ectopic tissue growth. This study aimed to analyze the effect of nanocurcumin at a dose of 10 mg/kgBW on TNF-α expression and endometriosis lesion diameter in endometriosis-induced mice. The laboratory experimental study used a post-test only control group design with three treatment groups (control, nanocurcumin after induction, and nanocurcumin before induction), each consisting of 10 mice. TNF-α expression was measured using the ELISA method, while lesion diameter was assessed macroscopically. Data analysis was performed using the Shapiro–Wilk normality test, Levene's homogeneity test, ANOVA, and Tukey's extended test. The results showed no significant difference in TNF-α levels between groups (p = 0.933). However, there was a significant difference in endometriosis lesion diameter (p < 0.001), with the nanocurcumin group showing the greatest lesion shrinkage compared to the control and other treatment groups. Nanocurcumin significantly reduced the diameter of endometriosis lesions in mice, although it did not significantly reduce serum TNF-α levels. These findings suggest that the therapeutic effects of nanocurcumin may be mediated through local mechanisms that are not fully reflected in serum biomarkers.
