Abstract

The purpose of this study was to determine the potential of guava and melastoma malabathricum linn leaves mixed gel activity as anti-microbial and wound healing in experimental rats (Sprague dawley). The thick extracts of melastoma malabathricum linn and guava leaves were made in a gel dosage form with a concentration of 4% and 6% and clindamycin as a positive control. This study used experimental laboratory methods, and the population in this study were white male rats and was carried out in the Pharmacology and Pharmaceutical Laboratory, National Institute of Science and Technology. The pharmacological activity was tested in the form of a gel dosage formula, and the rats were made diabetic through an inducing agent, namely alloxan, then the potential of the extract was seen in repairing diabetic wounds in male rats. Melastoma malabathricum Linn and guava Leaves extract gels have an effect in healing diabetic-induced rat wounds using alloxan. This can be seen from the change in the diameter of wound healing. Secondary metabolites of both plant materials are thought to provide anti-microbial activity and stimulate wound healing which helps epithelial cells and tissues so that they regenerate. The chemical content in the extracts of the test preparation has different mechanisms in helping the healing process.

Abstract

Primary Health Care (FKTP) as the gate keeper of health service providers must have a strategy in improving service quality and patient satisfaction. This study aims to evaluate the implementation of regulations regarding commitment-based capitation policies and determine the mechanism of change and contextual factors that affect the implementation of regulations. The evaluation was carried out using a realist evaluation approach and analyzed in terms of the configuration of forms, mechanisms, and results. The research was carried out in Bengkulu Province in 2018-2019 with the research subjects consisting of the health office, the head of the Puskesmas and the JKN monev team, Private Practice Doctors, Pratama Clinics. The sample selection used purposive sampling and data collection was carried out through in-depth interviews. Commitment-Based Capitation (KBK) is running in Bengkulu province. The KBK program shows that in general the non-specialist referral indicator target in Bengkulu province is achieved, namely 5%, but there are also city districts that have not been achieved, this is achieved because the Puskesmas has doctors in Bengkulu province, adequate doctor competence, adequate facilities and infrastructure, staff receive capitation funds incentives, there is a JKN Monev team that is always active in conducting socialization, monitoring and evaluation in all Puskesmas, especially Puskesmas that do not achieve non-specialist referral indicators. This study provides an overview of the context, mechanism and outcome of health service quality policy regulations in the JKN era, especially the Commitment-Based Capitation program.

Abstract

Oil palm shell is one of the palm oil processing wastes, which is quite large, reaching 60% of the production of kernel oil. The plantation waste can be processed into an artificial solid fuel which is more widely used as an alternative fuel called bio-briquettes. The researchers aim to utilize palm kernel shell waste (Palm Kernel Shell) into briquettes. This type of research is Quasi Experiment Design with the "Posttest Only Design." In this design, the treatment or intervention has been carried out (X), then measurement (observation) or posttest (O2) is carried out. As long as there is no control group, O2 results are impossible to compare with others. This design is often called "The One Shot Case Study." The results of this observation (O2) only provide descriptive information. The briquettes that have the most prolonged burning power until they turn to ashes, are briquettes with a variation of the adhesive composition of 300 grams, briquettes with good hardiness, namely those using a variation of the adhesive composition of 100 grams, and the results of heating 1 liter of water to a boil, variations of the adhesive composition of 100 grams are used with a faster time than the variation of the adhesive composition of 200 and 300 grams. Communities can use palm shell briquettes as an alternative fuel that can be used for cooking and burning purposes which are environmentally friendly and economical.
