Abstract

Immunization is one of the most effective health interventions to reduce morbidity, disability, and mortality caused by vaccine-preventable diseases (VPDs). However, in practice, many children still experience delays in receiving basic immunizations, including those in Samba Danum Village, Central Katingan District. This community service activity aimed to improve parents’ knowledge about basic immunization and to conduct catch-up immunization for children aged ≥ 9 months who had not received immunizations according to schedule. The implementation method consisted of preparation, education through interactive lectures and discussions, catch-up immunization in collaboration with health workers, and evaluation using pre-test and post-test. The results showed a significant increase in parents’ knowledge, with higher average post-test scores compared to pre-test. In addition, catch-up immunization coverage reached more than 80% of the target group. This activity demonstrated that proper education and accessible health services can enhance community awareness and improve basic immunization coverage. In the future, similar activities are expected to be expanded to other areas with similar conditions, accompanied by continuous monitoring to ensure the sustainability of the immunization program.

Abstract

Nutritional problems that have the potential to arise include the incidence of malnutrition in infants and toddlers, conditions in which babies do not get breast milk (ASI) due to separation from their mothers, and limitations in the provision of adequate complementary foods (MP-ASI) for toddlers. Good knowledge and skills of mothers in feeding practices can support growth and development and improvement of nutrition in children. This community service activity was carried out in Palingkau Lama Village, Kapuas Regency, Kalimantan Tengah Province. This community service aims to improve the knowledge and skills of pregnant women and mothers of toddlers in the practice of child feeding. Increasing maternal knowledge is carried out through education about PMBA and improving maternal skills is carried out through complementary food preparation training activities, mainly the introduction of the right food texture according to the child's age. The method used consists of the preparation, implementation, and evaluation stages of activities. The results obtained resulted in an increase in maternal knowledge about feeding babies and children using local ingredients.

Abstract

Non-communicable diseases (NCDs), particularly hypertension and diabetes mellitus, remain major contributors to morbidity and mortality in Indonesia. Suboptimal patient engagement and poor adherence to therapy continue to hinder effective NCD management at the primary care level. This community-based intervention aimed to enhance patients’ knowledge, treatment adherence, and self-management through the use of a print-based educational tool, the SEHATI book. The program was implemented over a one-week period at Tanjung Karang Primary Health Center, Mataram City, involving 17 participants, including healthcare providers, community health cadres, and patients with hypertension and diabetes. The intervention consisted of group education sessions, simulation activities, cadre-led home visits, and pre- and post-intervention assessments. The results demonstrated a substantial improvement in participants’ knowledge, increasing from 52% to 89%, and medication adherence, rising from 61% to 90%. Qualitative findings indicated that the SEHATI book facilitated self-monitoring practices and enhanced patient motivation. Additionally, health cadres reported that the tool strengthened their capacity as community educators. This community-based approach, particularly through the active involvement of health cadres, proved effective in promoting patient engagement, especially in settings with limited access to digital health resources. The intervention is consistent with the Health Belief Model, as it enhances perceived benefits and self-efficacy among patients. Overall, the SEHATI book represents a practical, scalable, and contextually appropriate educational strategy for sustainable NCD management in primary healthcare settings.

Abstract

The Professional Nursing Care Model (PNCM) using the primary method emphasizes patient-centered care by assigning one primary nurse with full responsibility for managing comprehensive nursing services throughout a patient’s hospitalization. However, its implementation in inpatient units has not been fully optimal, as reflected in issues related to continuity of care, unclear role distribution, and the absence of standardized operational guidelines. This community service initiative aimed to optimize the implementation of the PNCM–Primary Method to enhance the quality of nursing services in inpatient settings. The program was conducted through several stages, including situational analysis, development and implementation of standardized operational procedures (SOPs), structured role assignment for primary nurses, and continuous training and mentoring. Evaluation methods included direct observation, feedback from nursing staff, and audits of nursing documentation. The results indicated notable improvements in the continuity and coordination of care, increased accuracy and completeness of nursing documentation, and higher patient satisfaction levels. Nurses also demonstrated improved understanding of their roles, stronger accountability, and more effective communication within interprofessional teams. Additionally, the establishment of clear guidelines supported consistency in care delivery. In conclusion, optimizing the PNCM–Primary Method effectively improved the quality of nursing services, strengthened professional responsibility, and promoted a more patient-centered approach. This model is recommended for sustained implementation across inpatient units as part of ongoing quality improvement and accreditation readiness efforts.

Abstract

Suboptimal discharge planning in hospitals remains a critical issue, contributing to high readmission rates and inadequate patient and family readiness for post-hospital care. This community service activity aimed to improve the effectiveness of discharge planning through the implementation of the MANDIRI-D innovation program, a digital-based approach utilizing QR Codes. The program provided accessible discharge information, including care guidelines, flowcharts, and patient education materials tailored to patient needs. The activity involved 20 patients and family members using a quasi-experimental pre–post design without a control group. Interventions included structured education delivered at least one day prior to discharge, access to QR Code-based educational media, and reinforcement through the teach-back method. The results demonstrated an improvement in patient knowledge and readiness for discharge (Δ total score 1.05), increased nurse compliance with discharge planning standard operating procedures (≥85%), and more efficient delivery of patient education. The MANDIRI-D program effectively enhanced patient and family preparedness, improved the quality of nursing services, and has the potential to reduce readmission rates. This innovation is recommended for broader implementation and integration into hospital electronic medical record systems to support sustainable and standardized discharge planning practices.

Abstract

Micro, Small, and Medium Enterprises (MSMEs) serve as the primary engine of Indonesia's economy, yet they remain hindered by low digital literacy and traditional management practices. This community service program aimed to accelerate the digital transformation of MSMEs through a structured, participatory mentoring framework. The intervention was executed in five strategic phases: socialization, intensive training, technology implementation, monitoring and evaluation, and sustainability planning. Through this approach, MSME partners successfully integrated digital ecosystems, including e-commerce platforms (Shopee and Tokopedia), social media marketing, and digital financial management applications. The results demonstrate a significant shift in business capacity, particularly in precise production cost calculation, professional branding, and digital content creation. Post-program evaluation revealed that over 70% of participants experienced expanded market reach and increased sales volume. Sustainability is secured through the formation of "Digital Champion" groups and the integration of program outcomes into university curricula. This initiative proves that systematic assistance is vital for transitioning traditional MSMEs into the digital landscape. To foster long-term economic resilience, this model recommends cross-institutional collaboration and collective branding strategies as a blueprint for MSME empowerment in other regions.

Abstract

Community empowerment in preventing dental caries is a relevant and strategic approach to improving public oral health. This community service program aimed to enhance community knowledge and promote preventive behaviors through educational interventions and routine dental examinations at a clinic. The implementation method was designed to effectively achieve empowerment and prevention goals by integrating health education with direct health services. Evaluation results demonstrated a significant improvement in participants’ understanding following the intervention. Prior to the educational session, only 40% of participants correctly answered questions and demonstrated adequate knowledge of basic dental caries prevention. This proportion increased to 85% after the intervention, indicating the effectiveness of the educational approach. Clinical examinations of 50 participants revealed that 18 individuals (36%) had dental caries, with varying severity. The program also demonstrated strong community engagement, as evidenced by active participation and positive responses throughout the activities. The findings suggest that community empowerment through education and routine dental checkups is effective in increasing knowledge, fostering behavioral change, and raising awareness of the importance of maintaining sustainable oral health.
