Abstract

Introduction: Comprehensive midwifery care is care that is provided continuously from pregnancy, childbirth, postpartum and newborns. According to the Aceh Health Service report, in 2023 the number of maternal deaths was 3 people, 1 person was caused by heart disease, and 2 people were caused by bleeding. The number of live births in West Aceh district was 2,643 people and infant deaths were 16 people. Objective: This study aims to provide comprehensive midwifery care from pregnancy, childbirth, newborns and the postpartum period (including family planning counseling for postpartum mothers). Research Method: This study is a case study, carried out with midwifery care documented with SOAP. The subjects of this study were mothers K G1P0A0 aged 30 years starting from 37 weeks 3 days of pregnancy to 9 days postpartum. Data collection techniques were primary data and secondary data. Research results: Based on the results of the research conducted, the results obtained were that the pregnancy process of mother K was normal and no complications were found during the assessment starting from the gestational age of 37 weeks 3 days and the mother was able to adapt to her pregnancy. The labor process took place normally without complications. At the gestational age of 38 weeks 6 days there were no complications or problems in Mother K until 6 hours postpartum. During newborn care, it was found that Mother K's baby was a full-term male and postpartum care was carried out 2 times (including family planning counseling for Mother K) the mother planned to use a 3-month injection of birth control. Conclusion: Comprehensive midwifery care for mother K at TPMB Dewi Sandra Lubis, Meureubo District, West Aceh Regency took place normally. Suggestion: It is expected to improve knowledge and skills by studying cases during practice in the form of SOAP management and implementing care according to the established midwifery service standards in accordance with the authority of midwives that have been given to the midwife profession.

Abstract

  Human Immunodeficiency Virus (HIV) and Acquired Immunodeficiency Syndrome (AIDS) are global health problems that are still a serious challenge, including in Indonesia. One of the main accesses of HIV transmission in children is from mother to child which can occur during pregnancy, childbirth, and breastfeeding. To be able to break this chain of transmission, Voluntary Counselling and Testing (VST) is a special screening program that deals with the incidence of infectious diseases such as HIV/AIDS. However, the utilization of VCT in Indonesia is still very low, especially in remote areas such as large islands outside Java. This study aims to systematically identify the various factors that can influence the utilization of VCT in pregnant women in Indonesia. The method used was systematic review with PRISMA guidelines. Through literature searches on databases including Pubmed, Google Scholar, and ScienceDirect using the keywords “VCT”, “HIV/AIDS”, “Pregnant Women”, and “Indonesia” in Indonesian and English. 2,189 articles were found, which were then screened, and 10 articles were obtained that met the inclusion and exclusion criteria. The results of the review showed that there are many factors that are important determinants in the utilization of VCT by pregnant women such as knowledge and education. Therefore, this study is expected to be the basis for developing appropriate intervention strategies to prevent HIV/AIDS transmission and improve the health status of mothers and children.

Abstract

Lead (Pb) is a toxic metal that is used to trigger oxidative stress. Oxidative stress causes damage to organs, especially the liver increasing aspartate aminotransferase (AST) and alanine aminotransferase (ALT). This study aims to determine the synergistic effect of antioxidant as Java plum leaf extract and vitamin C together on lead-induced levels of AST and ALT in rats.This type of research is true experimental research with a randomized posttest-only-control group design. The study used 20 male white rats which were divided into 4 groups. The treatment was carried out for 30 days. The examination for AST and ALT levels using the Micro Lab 300 with the Sample Star method. Data analysis was performed by using One-way Anova and Post Hoc-Turkey HSD.In this study, The results were obtained in the form of a decrease in AST and ALT levels in rats that had been induced lead acetate 40 mg/KgBW by administration of Java plum leaf extract dose 75 mg/KgBW and vitamin C dose 35 mg/KgBW as well as the administration of Java plum leaf extract at a dose of 150 mg / KgBB and vitamin C dose of 75 mg / KgBB with significant influence at the lower dose (p=0,01).It can be concluded that administrating of Java plum leaf extract dose 75 mg/KgBB and vitamin C dose 35 mg/KgBB was effective to protect the liver based on the decrease of AST and ALT levels in rats that had been induced lead acetate dose 40 mg/KgBB

Abstract

Introduction. Non-communicable diseases as a cause of death in Indonesia are increasing. Hyperglycemia, hyperlipidemia, and obesity are some of the factors that contribute to an increased risk of non-communicable diseases. Education workers are a group of office workers who are at risk of obesity and non-communicable diseases because they have low physical activity. The study aimed to determine a correlation between body mass index with fasting blood glucose levels and total cholesterol levels in the health screening of educational staff faculty of medicine of Andalas University.  Methods. This research was an analytical study with a cross-sectional design using secondary data for the faculty of medicine of Andalas University health screening that was held on September 08, 2021. The tools used in the study were a calibrated glucometer to measure fasting blood glucose and a calibrated auto-check for total cholesterol examination. 37 data met the inclusion and exclusion criteria. Data were processed and analyzed using univariate and bivariate analysis using the Pearson correlation test.  Results. The result showed that the educational staff of the medical faculty of Andalas University are mostly in the late adult category, most are female and most of them are obese. The mean of body mass index is 25.3 kg/m2, the mean of fasting blood glucose level is 126.57 mg/dL and the mean of total cholesterol level is 205, 14 mg/dL. The bivariate analysis results showed no correlation between body mass index and fasting glucose levels (r = 0.316; p = 0.057) and no correlation between body mass index and total cholesterol levels (r = 0.091; p = 0.591). Conclusions. This study concludes that there is no correlation between body mass index with fasting blood glucose levels and total cholesterol levels in the health screening of educational staff faculty of medicine of Andalas University.

Abstract

Background: Valvular Heart Disease (VHD) occurs when one of the valves in the heart is damaged or dies. The highest prevalence of VHD is in mitral regurgitation. The management action that can be given to patients with mitral valve problems is Mitral Valve Replacement (MVR). Objective: To discuss the indications of Temporary Pacemaker (TPM) insertion in intra-operative MVR and the indications of collaborative drug administration in patients. Methods: Descriptive case-report of a 70-year-old woman with VHD after MVR surgery. Case Description: The patient had a history of Permanent Pacemaker (PPM 2023) ec. Bradycardia. Currently the patient was diagnosed with VHD and MVR surgery was performed. At the time of weaning Cardiopulmonary Bypass (CPB) to off, the patient's heart rhythm was not pacing and blood pressure dropped. So it was decided to go back on CPB and install a TPM. The patient received medical support therapy in postoperative care. Discussion: The indication for TPM insertion is to prevent postoperative heart rhythm disturbances and to temporarily replace the inactive PPM function. In postoperative management, the patient received medical therapy and therapeutic interventions. The results obtained are the patient's hemodynamic status is stable, pain is reduced, fluid balance is more negative, there are no signs of infection in the surgical wound, and minimal bleeding. Conclusion: TPM placement is required in patients with PPM if no pacing rhythm is found. The provision of collaborative medication with appropriate doses and indications is effective in reducing the risk of postoperative complications of MVR and stabilizing the patient's hemodynamics.

Abstract

Low self-esteem is a feeling of shame or inferiority that is usually caused by negative judgments from oneself or others. Complications in patients with low self-esteem include social isolation, even violent behavior. One intervention that can be done for patients with low self-esteem is the application of positive affirmation therapy. Positive affirmations are utterances of sentences that can strengthen positive beliefs in a person. This research method is a descriptive study with a case study type of research. The subjects used were 2 patients with nursing problems of the application of positive affirmation therapy in patients with low self-esteem at Prof. DR. Muhammad Ildrem Mental Hospital Medan. The results of the study from the two respondents during the psychiatric nursing care with the application of positive affirmation therapy in patients with low self-esteem for 3 days with 6 meetings were obtained, both respondents were able to carry out SP 1 to SP 4 to control their lack of confidence. The conclusion of the assessment to evaluation in carrying out nursing care for patients with low self-esteem successfully controlled their lack of confidence with the application of positive affirmation therapy SP 1 to SP 4. The suggestion of this study is expected to be a reference in conducting further research on psychiatric nursing cases with low self-esteem using positive affirmation therapy.

Abstract

Chronic kidney failure is a condition of decreased kidney function caused by the inability of the kidneys to remove metabolic waste from the body or perform their regular functions. Intervention in chronic kidney failure is performed with hemodialysis. The hemodialysis process can cause changes in body temperature to hypothermia, so it requires proper treatment. The purpose of this study is to provide an overview of the implementation of hypothermia management in patients with chronic kidney failure after hemodialysis. The research method is descriptive research with a medical-surgical nursing approach consisting of assessment, diagnosis, intervention, implementation, and evaluation. The study was conducted on two patients undergoing hemodialysis. Interventions and implementation of hypothermia management were carried out, such as monitoring body temperature, identifying the cause of hypothermia, monitoring signs and symptoms of hypothermia, providing a warm environment, changing wet clothing and/or linen, covering and applying warm compresses.The study found that body temperature returned to normal in both patients. The study concluded that monitoring body temperature, identifying the cause of hypothermia, monitoring signs and symptoms of hypothermia, providing a warm environment, changing wet clothing and/or linens, covering the patient with blankets, and applying warm compresses can help treat hypothermia in post-hemodialysis patients. This study recommends that hyperthermia management can help patients achieve normal thermoregulation, thus effectively managing hypothermia

Abstract

Dyslipidemia is a health problem that needs attention because the incidence of dyslipidemia in Indonesia continues to increase from year to year. Macronutrient intake in a diet can influence a person's lipid profile. This study aims to determine the correlation between macronutrient consumption in obese health workers at RSUP Dr. M. Djamil Padang in 2022.  This research is an analytical research with a cross sectional study design on secondary data from diet development research on health workers at RSUP Dr. M. Djamil Padang who is obese in 2022 with research subjects of 40 respondents taken using total sampling technique. Data analysis used univariate and bivariate analysis with the Pearson Spearman correlation test. Correlation is declared significant if p<0,05.  The research results showed that the highest mean macronutrient consumption was carbohydrates (219.92 ± 74.21), then fat (64.30 ± 28.35) and the lowest was protein (62.56 ± 18.74). Respondents also had lipid profile values that were in the normal range for total cholesterol (157.75 ± 51.58) and triglycerides (125.33 ± 33.21) and there was an increase in LDL (106.46 ± 40.13) and HDL ( 76.35 ± 27.54).  From this study it was concluded that there was a significant correlation between carbohydrate consumption and total cholesterol (p value = 0.026; r = 0.351) and LDL (p value = 0.008; r = 0.415), but there was no correlation between carbohydrates, protein and fat with the profile other lipids in obese health workers at RSUP Dr. M. Djamil Padang.

Abstract

This study aimed to compare the onset and duration of analgesia in spinal anesthesia for cesarean section using hyperbaric bupivacaine combined with fentanyl or with meperidine. A double-blind randomized controlled trial was conducted involving sixty patients undergoing either elective or emergency cesarean section at Jejaring Hospital and H. Adam Malik General Hospital Medan. Patients were randomly assigned to receive hyperbaric bupivacaine with fentanyl or hyperbaric bupivacaine with meperidine, and the data were analyzed using appropriate statistical methods to examine differences between the groups. The findings revealed that the combination of bupivacaine with fentanyl produced a faster onset of analgesia compared to bupivacaine with meperidine, whereas the combination with meperidine provided a longer duration of analgesia. Both combinations were effective in achieving intraoperative and postoperative pain control, each offering distinct advantages. The study concludes that the choice of spinal anesthesia combination can be tailored to the clinical needs of patients, with fentanyl being preferable for rapid onset and meperidine offering prolonged analgesia.

Abstract

School-aged children are a group that is vulnerable to dental and oral health problems. Dental and oral health is important to maintain from an early age to prevent disorders of the teeth and their supporting tissues, the results of research at SDN 05 Pontianak Utara showed that students' knowledge, attitudes, and behavior were still very low. The initial survey found that only 30% of students knew the right time to brush their teeth, 93% did not know when to replace their toothbrush, and 80% did not know the importance of regular dental check-ups, this study aims to determine whether health promotion using leaflet media can improve students' knowledge about how to maintain dental and oral health. This study used a pre-experimental method with a one group pretest-posttest design. The number of respondents was 90 5th grade students of SDN 05 Pontianak Utara who were selected using a total sampling technique. The research instrument was a negative question questionnaire consisting of 15 multiple-choice questions. The questionnaire was given twice, namely before (pretest) and after (posttest) the health promotion was carried out. The results of this study showed a significant increase in students' knowledge before health promotion with leaflet media, students' knowledge was in the poor knowledge category of 63 students (70%) after health promotion with leaflet media, students' knowledge was in the good category of 73 students (81%). The conclusion of this study is that the level of students' knowledge after being given health promotion became good and leaflet media proved effective in increasing the knowledge of fifth grade students of SDN 05 North Pontianak about how to maintain dental and oral health
