Abstract

Adverse drug reactions are undesirable effects that can be harmful or detrimental after medication administration when dosage and way of administration are in accordance with the purpose of treatment. Identification of the most common type of adverse drug reaction in Rumah Sakit Jantung dan Pembuluh Darah Harapan Kita (RSJPDHK) was executed through Adverse Drug Reactions Monitoring from January to June 2021. The aim of this study is to obtain real data on adverse drug reactions for the purpose of educating medical colleagues in order to reduce similar adverse drug reactions. The designs of this study were qualitative descriptive research (using Naranjo Algorithm) and quantitative. The number of incoming reports were 102. The reports came from inpatient and outpatient wards. The most common form of adverse drug reactions occurred during this period were gastrointestinal bleeding (31.37%) and prolonged INR (International Normalized Ratio) without sign of bleeding (25.49%). Utilisation of blood-thinning medications that cause bleeding, specifically were warfarin, aspirin, and clopidogrel. The results of the qualitative analysis for the highest incidence of adverse drug reactions in the study period were gastrointestinal bleeding with a Naranjo score of 5-8 and the number was 69.77%. The incidence of adverse drug reactions caused by anticoagulants and antiplatelets are quite high even though these two types of medications are the most widely used in RSJPDHK. Socialization of the results of these reports on a regular basis to medical colleagues is necessary in order to reduce the incidence of adverse drug reactions in RSJPDHK.

Abstract

Antiplatelets can inhibit platelets aggregation and have been used in prevention and treatment the trombotic desease. Binahong leaves, ginger dan turmeric have antiplatelets activity. The mixture of these plants are expected to increase the antiplatelets potency. This study aimed to determine the optimum dose of ethanol extracts of binahong leaves, ginger dan turmeric as antiplatelets agent. The antiplatelets testing were carried out by in vivo method using parameter of bleeding and coagulation time. The testing solutions were administered orally once a day for 8 days. There were 7 treatment groups consist of positive control (Acetosal 0.29mg/20gBW), negative control (CMC-Na 0.5%), binahong 10mg/20gBW, mixture 1 of binahong:ginger:turmeric (5:0.5:1mg/20gBW), mixture 2 (10:0.5:1mg/20gBW), mixture 3 (5:1:1mg/20gBW), and mixture 4 (5:0.5:2mg/20gBW). The bleeding and coagulation time were determined at 0, 3rd, 6th and 9th day. The bleeding time was determined by injuring mice tail, the blood that comes out were absorbed with absorption paper. From the first bleeding time until stopped was calculated as bleeding time. The coagulation time was determined by observing the formation of fibrin thread in the broken capillary pipe. The blood from the end tail were absorbed with capillary pipe, and then was broken every 15 seconds. The antiplatelets activity in mice with bleeding time parameter showed that mixture 3 was more effective at 3rd day with an increased of 16.60%, than the positive control group (3.92%). The coagulation time parameter showed that the binahong group was more effective at 9th day with an increased of 203.55%, than the positive control group (89.71%). The mixture of binahong, ginger, and turmeric were the most effective in increasing bleeding time, whereas binahong was the most effective in increasing coagulation time.

Abstract

An in vivo test study to prove its effectiveness as an antihyperuricemia requires an animal model with a hyperuricemic condition. Foods that contain high purine bases have the potential to increase uric acid levels in the blood, including chicken liver and melinjo seeds. This study aims to determine the effectiveness of the combination of chicken liver juice and melinjo seed powder in producing hyperuricemia conditions with the most optimal onset and duration in rats in vivo. Six groups of white Sprague Dawley rats (n=5) consisted of a normal group, a positive control group, a chicken liver juice group, a melinjo seed powder group and two groups that were given a combination of chicken liver juice and melinjo seed powder with different compositions. The treatment was given every day for 15 days orally. The parameters observed were uric acid levels which were measured every 3 days and, body weight. The results showed that the combination of chicken liver juice and melinjo powder with a half dose of a single dose each had a better onset and duration. The conclusion of this study is that the combination of chicken liver juice and melinjo seed powder is potent as an inducer of hyperuricemia in rats.
