Abstract

Acute coronary syndrome (ACS) is one of the leading causes of acute chest pain, requiring emergency care and eventual hospitalization. Wellens’ syndrome is one such example, which in ACS patients is a catastrophic event often accompanied by extensive anterior myocardial infarction and high mortality rates. Wellens’ syndrome is a pattern of T-wave changes seen in the anterior (V2-V3) leads on critical stenosis of the left anterior descending (LAD) coronary artery. We reported a 65-year-old man admitted to our emergency facility because of intermittent chest pain that had been experienced for the past 2 days. An initial ECG performed in the emergency room while pain free showed sinus rhythm with biphasic T waves in V2–4 which is typical of Wellen's Syndrome. Chest x-ray shows cardiomegaly. Cardiac enzyme examination showed an increase in CKMB and Troponin T levels of 18.40 U/L and 560 ng/L respectively. The echocardiography (ECHO) demonstrated a hypokinesis of the anteroseptal with left ventricular ejection fraction (LVEF) of 47 %. Cardiac catheterization showed total occlusion in the mid-LAD and other coronary arteries had non-significant lesions. He then underwent balloon angioplasty and placement of a stent in the proximal and mid LAD with a good result. Wellen’s syndrome often represents a pre-infarction state of myocardial infarction. Early recognition of these ECG features is crucial to identify these high-risk clients, and the definitive evaluation and treatment is cardiac catheterization with intervention to relieve the LAD obstruction.

Abstract

Food stalls play a role in helping fulfil daily needs by offering convenient and fast services, so people like to eat food at food stalls. If contaminated cutlery is used, those who use it may be infected with disease-causing microbes. Objective: The purpose of this study was to determine the presence of Coliform bacteria in water both before and after being used to clean food utensils in food stalls around the Medistra Institute of Health Lubuk Pakam. Methods: The method used is quantitative, as well as experimental to test water quality with applicable quality standards and describe the results of research based on literature review. Results: Of the 4 food stalls around the Medistra Institute of Health, it shows that there are 3 food stalls in the category of not meeting health requirements and 1 other food stall in the category of meeting health requirements based on the maximum number of total coliforms for piped and non-piped water 10/100 and 50/100 ml of water samples respectively in accordance with Permenkes No. 416/Per/Menkes/IX/1990. The quality of washing water (clean water) used in food stalls around the Medistra Institute of Health based on the results of laboratory tests of total Coliform, from four food stalls around the Medistra Institute of Health, 3 food stalls in the category of not meeting health requirements and 1 other food stall in the category of meeting the requirements according to Permenkes No. 416/Per/Menkes/IX/1990, concerning clean water quality for piped and non-piped water with numbers 10/100 ml and 50/100 ml of water samples. Abstract

Disini kami menyajikan seorang pasien dengan keluhan nyeri dada khas infark selama 2 hari dan menjalar ke punggung bagian belakang. Pada pemeriksaan saat ini, EKG awal menunjukkan elevasi segmen ST (>0,5 mm) di hampir semua sadapan. Pasien didiagnosis menderita STEMI anteroseptal akut dan syok kardiogenik. Pasien kemudian dilakukan pemeriksaan angiografi koroner yang dilaporkan hasilnya normal, tanpa lesi yang signifikan dan tidak ada tanda-tanda penyakit arteri koroner obstruktif, diseksi koroner, bridging atau emboli pada arteri koroner. Berdasarkan temuan di atas, kami menganggap pasien ini sebagai infark miokardium dengan arteri koroner non obstruktif (MINOCA). Kondisi pasien di perparah dengan adanya VT disertai dengan ketidakstabilan hemodinamik sehingga pasien harus di kardioversi. Pasien segera dirujuk setelah 2 hari pengobatan. Defibrilasi telah dilakukan dua kali pada pasien ini. MINOCA biasanya terjadi pada pasien yang lebih muda dan wanita, yang mungkin memiliki risiko kejadian kardiovaskular lebih rendah dibandingkan pasien dengan CAD obstruktif. Penyebab paling umum dari MINOCA diwakili oleh penyakit plak koroner, diseksi koroner, emboli pada arteri koroner, spasme mikrovaskuler koroner, kardiomiopati Takotsubo, miokarditis, tromboemboli koroner, bentuk lain dari infark miokard tipe 2 dan MINOCA dengan etiologi yang tidak pasti. Diperlukan pemeriksaan lebih lanjut seperti resonansi magnetik jantung (CMR) untuk menegakkan diagnosis lebih baik. Studi elektrofisiologi juga di pertimbangkan pada pasien ini dikarenakan pasien sudah berulang mengalami VT.

Abstract

Morbidity surveys conducted by the Sub-directorate of Disease Control Prevention and Environmental Health (P2PL) of the Health Office from 2000 to 2010, there was an increase in the incidence of diarrhea. Many factors contribute to the increasing incidence of diarrhea, especially among children under five years of age, including maternal personal hygiene and environmental sanitation.  In 2014 there were six diarrhea outbreaks that were widely spread in five provinces and six districts or cities, the number of patients increased to 2,549 people with twenty-nine deaths (CFR 1.14%) and North Sumatra was one of the five provinces with the most cases. This study was conducted in Deli Tua Subdistrict, Deli Serdang Regency, North Sumatra, for approximately 8 months from April to December 2016 is an observational analytic study with a cross sectional approach. The sample of this study were 50 mothers who brought their toddlers to the Deli Tua Health Center. The statistical analysis used was chi square test. The results showed there was a relationship between the mother's hand washing behavior (p = 0.05), the use of water sources (p = 0.04) and the age of toddlers (p = 0.00) with the incidence of diarrhea in toddlers in the Deli Tua Health Center working area.

Abstract

Background: The erythrocyte sedimentation rate (LED) examination is a hematological test that describes the rate at which red blood cells settle in plasma, using 3.8% sodium citrate anticoagulant and is measured in mm/hour. Lemon (Citrus limon L.) is a plant with natural properties as a metal binding agent or chelating agent. Lemons contain vitamin C, citric acid, potassium, and limonene. Citric acid compounds are able to form complex bonds with metals because they have COOH and -OH functional groups. Apart from citric acid, lemons contain flavonoids which function as antiviral, anticancer and antiallergenic. Objective: To test whether lemon juice (Citrus limon L.) can be used as an alternative ingredient to replace sodium citrate in LED tests. Method: This research is an experiment using the Westergreen method. Results: LED examination with 3.8% sodium citrate produced a value of 10 mm/hour. Lemon juice with a concentration of 3.8% produces a value of 15 mm/hour, 10% produces a value of 10 mm/hour, 50% produces a value of 18 mm/hour, and 100% produces a value of 0 mm/hour. Conclusion: A lemon juice concentration of 10% showed results closest to the control, with similar LED values. Thus, lemon juice (Citrus limon L.) can serve as a substitute for sodium citrate in the LED test

Abstract

Background: Cervical cancer is one of the second-highest sexually transmitted diseases in the world, accounting for 13% of the total 22% of deaths caused by non-communicable diseases. Today there have been many developments about cervical cancer diagnosis methods especially in early detection aimed at recognizing cervical cancer at an earlier stage, including molecular biology detection using PCR tools and electrophoresis. Objective: To identify the results of DNA electrophoresis in vaginal mucosal swabs with cervical cancer. Method : The research method used is experimental. Results: Based on the results of a study using a documentary gel, 2 samples of HPV DNA tape were detected, it was tested positive because there was 450 bp DNA tape and 4 samples of negative results were obtained. Conclusion: The frequency distribution of respondents from 6 samples by using electrophoresis and focumerary gel as readers detected 2 positive samples. A positive result is expressed if a result in the documentary gel is found to be a DNA band. The size of the DNA ribbon produced with the DNA marker markers is at 450 bp, according to the sample target. This shows that the DNA bands in the sample are about 450 bp in size.

Abstract

Background: Carbohydrates in kluwih seeds have sufficient nutrients, such as potatoes and tubers, making it possible to be used as a medium for mushroom growth. The nutritional composition of breadfruit contains 25% carbohydrates, 1.5% protein, and 0.3% fat. Aspergillus niger fungi require nutrients such as carbohydrates and protein to grow. This study aims to determine whether kluwih seed starch can be used as an alternative medium for the growth of Aspergillus niger fungi. Method: This study used a pre-experimental design with laboratory observation. The study was conducted by utilizing kluwih seed starch as a medium, compared to Potato Dextrose Agar (PDA) and Sabouraud Dextrose Agar (SDA). Results: The results showed that kluwih seed starch can be used as an alternative medium for the growth of Aspergillus niger, with fungal colonies that grow macroscopically and microscopically almost the same as PDA and SDA media. Conclusion: Kluwih seed starch can be an alternative substitute for PDA media for the growth of Aspergillus niger.

Abstract

Background: Microbiology is the study of microorganisms, which are microscopic organisms that can only be observed through a microscope. These microorganisms have an important role in various aspects of life, including in the health sector. Objective: This study aims to evaluate whether alternative media, such as potato and corn, can be used as a substitute for Nutrient Agar in supporting the growth of Staphylococcus aureus bacteria. Methods: This research is experimental, by comparing the effectiveness of potato and corn media as an alternative to Nutrient Agar for the growth of Staphylococcus aureus. Results: The results of the research in the One-way Anova test obtained a significant value (0.00) which means (p <0.05), so it can be concluded that there is a significant difference between the ability of Nutrient Agar media, potato media and corn media in growing Staphylococcus aureus bacteria. To find out each group that has a significant difference, the Post-hoc follow-up test is carried out. The results of the post-hoc follow-up test shown in Figure 4.2.2 obtained a significant value between Nutrient Agar media and potato media (0.00), Nutrient Agar media with corn media (0.558), and between potato and corn media (0.00). Conclusion: The results showed that potato media was significantly different in its ability to grow Staphylococcus aureus compared to Nutrient Agar and corn. Potato media shows potential as an alternative to Nutrient Agar, despite its different effectiveness.

Abstract

Introduction : Butterfly pea flower (Clitoria ternatea L.) is one of the most commonly found plants.  Butterfly pea flowers contain anthocyanins which are abundant in the flowers and play a role in giving red, violet-blue color to the flowers. Anthocyanins are the main molecules of plant colorants. The potential of anthocyanins in butterfly pea flowers can be used as a natural coloring reagent. Objective : The purpose of this study was to determine whether telang flower extract can be used as a substitute for gentian violet in Gram staining. Method : The research method used is experimental. Result : The results of Gram staining research using original pH butterfly pea flower extract at concentrations of 1:10, 2:10, 3:10, 4:10, and pH 5 butterfly pea flower extract at concentrations of 1:10, 2:10, 3:10, 4:10, 5:10 on Staphylococcus aureus bacteria showed poor bacterial staining results because the bacteria were not purple while the original pH butterfly pea flower extract at a concentration of 5:10 showed good staining results because the bacteria were purple. While in Escherichia coli bacteria, butterfly pea flower extract shows good staining results because the bacteria are red. Conclusion : Data analysis using the Kruskal Wallis test obtained Asymp. Sig (0.000) <0.05, it can be concluded that there is a significant difference in Gram staining using gentian violet and telang flower extract with variations in pH and composition as a substitute for gentian violet.

Abstract

Hypertension, a medical condition characterized by chronically elevated blood pressure, is one of the most prevalent cardiovascular diseases. Although asymptomatic in its early stages, hypertension can lead to serious complications in the cardiovascular system and other vital organs. Cupping therapy is a complementary treatment modality that has been used as an alternative approach to manage hypertension. This study aims to evaluate the effectiveness of wet cupping therapy in lowering blood pressure. The method employed involves the application of wet cupping therapy on patients with hypertension. The study results indicate that cupping therapy significantly lowers blood pressure in hypertensive patients, as evidenced by the Wilcoxon test showing a significant difference before and after the intervention. Based on the effectiveness of wet cupping therapy in lowering blood pressure, it shows potential for development as an alternative treatment for hypertension.
