Abstract

Pendahuluan: Kanker prostat merupakan penyakit yang menyerang pria dengan prevalensi tertinggi ke-2 serta penyebab kematian ke-5 dengan perkiraan mencapai 1,4 juta kasus dan mortalitas berjumlah 375.000 di seluruh dunia. Terdapat 112 dari 185 negara melaporkan kejadian kanker prostat. Di Indonesia jumlah kasus kanker prostat mencapai 13.563 jiwa. Data menunjukkan bahwa 10-20% pasien dengan metastatik kanker prostat mengembangkan metastatic castration-resistant prostate cancer (mCRPC) selama 5 tahun serta memiliki keberlangsungan hidup sekitar 14 bulan. Namun belum terdapat terapi yang efektif dalam kasus mCRPC. Dengan adanya potensi yang dimiliki oleh Myeloid Leukemia 1 (MLC-1) inhibitor yang menargetkan langsung pada jalur intrinsik apoptosis sehingga bisa menekan anti-apoptosis sel dapat dijadikan inovasi terapi pada mCRPC. Metode: Kajian literatur yang bersumber dari Google Scholar, Pub Med, Science Direct. Tahapan pemilihan artikel yaitu identifikasi, skrining serta kesesuaian terhadap variabel inklusi dan eksklusi sehingga didapatkan 11 artikel. Pembahasan: Pemanfaatan potensi MCL-1 sebagai terapi mCRPC tentunya membutuhkan senyawa yang akan bekerja sebagai inhibitor protein anti-apoptosis MCL-1. Senyawa-senyawa tersebut memiliki jalur inhibisi yang berbeda-beda untuk dapat menurunkan MCL-1. Berbagai penelitian in vitro dan in vivo menjelaskan mekanisme senyawa yang berpotensi menjadi MCL-1 inhibitor melalui berbagai metode untuk membuktikan bahwasannya senyawa tersebut dapat menurunkan protein anti-apoptosis MCL-1 sehingga menghasilkan efek terapeutik terhadap mCRPC. Simpulan: Dengan demikian, MLC-1 inhibitor diharapkan dapat menjadi sebuah inovasi dalam pengobatan terapi bagi pasien mCRPC dengan hasil yang lebih baik dan memuaskan.

Abstract

Background: Patellofemoral Pain Syndrome (PFPS) is a common cause of knee pain, often exacerbated by specific activities. Muscle imbalances, particularly in the thighs and hips, contribute to its development. Diagnosis relies on subjective and objective assessments, including specific tests. Case Illustration: We present a case of a 20-year-old female basketball player with PFPS symptoms, characterized by postural abnormalities, limited range of motion, and positive physical examination findings. Discussion: The patient's symptoms match PFPS. Postural assessment shows left posterior chain muscle stiffness and asymmetrical shoulder and hip alignment. Palpation reveals tenderness and tightness in the left quadriceps and hamstring. Limited active range of motion and limited ankle ROM support PFPS diagnosis. Basketball and gym exercises could worsen lower limb muscle overuse. Squat analysis indicates lumbopelvic instability with increased knee anteriorization. Greater hip adduction during lunges suggests weak gluteus medius. Asymmetrical lumbopelvic motion during single leg stance may indicate compensatory mechanisms. Navicular drop test shows more significant changes on the left side. Conclusion: Lumbopelvic imbalance contributes to PFPS in this patient, supported by symptoms and physical examination findings. Gender, thigh and hip muscle imbalances are contributing factors to PFPS.

Abstract

Background: Tuberculosis (TB) is a deadly tropical infectious disease caused by Mycobacterium tuberculosis (MTB). The combination of antibiotics as the current TB management raises new problems such as high rates of drug resistance and serious adverse effects on the liver. Alternative therapies that are effective, safe, and affordable in treating TB are needed. Discussion: One of the therapeutic approaches that can be used to avoid bacterial resistance is Host-Directed Therapy (HDT) because it utilizes the host's capabilities. Curcumin is a yellow-colored polyphenol pigment that has a variety of beneficial pharmacological effects including anti-tuberculosis effects through its modulation of the human immune system so that it can be one of the HDTs. However, curcumin still has the major disadvantage of decreased pharmacokinetics because it is easily degraded and its oral bioavailability is poor. To improve its performance, curcumin can be combined with nanoencapsulation agent, chitosan. Curcumin is proven to have the ability to work as Host-Directed Therapy from various studies. Curcumin can inhibit the growth of MTB intracellularly while inducing apoptosis in MTB-infected cells. Nanoencapsulation using chitosan is proven in various trials to improve the pharmacokinetic profile and bioavailability of curcumin so that its performance becomes better. The use of curcumin with chitosan nanoencapsulation in Indonesia is very possible because both agents are very easy to produce domestically and have a relatively affordable price. Conclusion: Nanoencapsulation of herbal curcumin by chitosan has the potential to be an effective, safe, and affordable cutting-edge therapeutic modality in treating tuberculosis in Indonesia. Abstract

Background: Respiratory diseases are a significant global health burden, affecting the quality of life of millions of people worldwide. Genetic therapy using CRISPR-Cas9 technology has become a paradigm shift in the treatment of respiratory diseases. This article presents a comprehensive literature review on the use of CRISPR-Cas9 in addressing respiratory diseases, including cystic fibrosis, chronic obstructive pulmonary disease (COPD), and SARS-CoV. This review includes an introduction that provides background on the importance of respiratory disease therapy and the potential of CRISPR-Cas9 in overcoming these challenges. Methods: The literature search method was conducted through scientific databases such as PubMed, Google Scholar, and Scopus. The results reveal that CRISPR-Cas9 can be used to edit genes related to respiratory diseases, reduce inflammation, excessive mucus production, and lung tissue damage, as well as detect the presence of these diseases. Discussion: The discussion highlights the effectiveness of CRISPR-Cas9 based on experimental studies on animal models and in vitro experiments. Challenges that need to be addressed include the efficiency and precision of the technology, long-term safety, and effective delivery. Conclusion: The potential of CRISPR-Cas9 as a therapy for respiratory diseases emphasizes the need for further research and technological development to optimize the effectiveness and safety of this therapy. Thus, breakthroughs in respiratory disease therapy with CRISPR-Cas9 have the potential to improve the health and well-being of the global population. Abstract

Introduction: Urinary tract infections (UTIs) are prevalent among women and are commonly caused by Escherichia coli. Although antibiotics are effective, their misuse contributes to rising antimicrobial resistance. Psidium guajava L., a traditional Indonesian medicinal plant, has shown promise as an alternative antibacterial agent. This study aimed to evaluate the inhibitory effect of P. guajava leaf extract against E. coli. Methods: Antibacterial activity was assessed using the agar diffusion method with E. coli ATCC 25922. Ethanolic extracts were obtained by maceration. Six treatment groups, including various concentrations of levofloxacin and dimethyl sulfoxide (DMSO), were tested to compare antibacterial effects. Results: Phytochemical screening revealed the presence of flavonoids, alkaloids, and tannins—compounds known for their antimicrobial activity. Inhibition zone diameters were measured and analysed using the Kruskal–Wallis one-way ANOVA. Discussion: The guava leaf extract demonstrated inhibitory activity against E. coli, likely due to its bioactive compounds. These constituents may exert antibacterial effects through disruption of nucleic acid synthesis, interference with membrane integrity, inhibition of energy metabolism, and prevention of biofilm formation. Conclusion: The ethanolic extract of Psidium guajava leaves exhibits significant antibacterial activity against Escherichia coli.

Abstract

Background: Chronic kidney disease is a disease that occurs as a result of structural and functional kidney damage that lasts for more than three months with a glomerular filtration rate (GFR) or Glomerular Filtration Rate (GFR) of less than 60mL/min/1.73 m2. The prevalence of kidney failure in Indonesia increases every year, to treat it requires kidney replacement therapy, one of which is hemodialysis which must be done for life. Hemodialysis therapy is a substitute for kidney function to remove metabolic waste or certain toxins from human blood circulation which aims to remove fluids, electrolytes and metabolic waste from the body, thereby indirectly extending the patient's life. Objective: To determine the relationship between depression and the quality of life of chronic kidney disease patients undergoing hemodialysis at Drs. H. Amri Tambunan. Population: The population in this study were patients with chronic kidney disease undergoing hemodialysis at the hemodialysis unit of RSUD Drs. H. Amri Tambunan. The number of research samples is 40 people. Method: The type of research used is categorical correlative analytic in RSUD Drs. H. Amri Tambunan. This study used a cross-sectional design. The research instrument used the KDQOL-SF and BDI which were analyzed using the Spearman test. Results: Kidney disease patients undergoing hemodialysis at Drs. H. Amri Tambunan did not experience depression as many as 22 respondents (55.0%) while the quality of life was mostly in the category of moderate quality of life as many as 22 people (55.0%). Conclusion: After the spearment statistical test was carried out, a significant value or sig (2-tailed) of 0.001 was obtained which showed that there was a relationship between depressive symptoms and the quality of life of chronic kidney disease patients undergoing hemodialysis at Drs. H. Amri Tambunan. The correlation coefficient value was 0.524 which indicated that there was a strong relationship between depressive symptoms and the quality of life of chronic kidney disease patients undergoing hemodialysis at Drs. H. Amri Tambunan. The correlation coefficient is negative, namely -0.524, so the relationship between the two variables is not unidirectional.Thus it can be interpreted that the higher the symptoms of depression, the level of quality of life will decrease.

Abstract

Pendahuluan: Munculnya pandemi COVID-19 sejak tahun 2020 di Indonesia telah membawa perubahan yang sangat signifikan di semua sektor termasuk kesehatan pada masyarakat. Meredanya pandemi akibat sudah menurunnya secara signifikan kasus penyebaran COVID-19 membuat banyak orang, kembali beradaptasi dengan aturan baru yang diterbitkan pemerintah. Keadaan adaptasi tersebut akan menimbulkan perubahan pada perilaku kesehatan masyarakat. Hal tersebut akan memicu adanya gaya hidup baru pada manusia secara global. Metode: Penelitian ini menggunakan metode systematic literature review yaitu menyusun literature review yang diawali dengan mengumpulkan artikel-artikel penelitian yang berkaitan dengan topik berdasarkan metode PEO, kemudian dibaca, dan diulas, kemudian dibahas dan didukung dari sumber-sumber relevan lainnya. Pembahasan Dari hasil pencarian artikel jurnal, didapatkan beberapa perubahan gaya hidup sehat oleh masyarakat Indonesia yang terjadi baik disaat maupun setelah COVID-19 seperti kebiasaan mencuci tangan dengan bersih, memakai masker, kebiasaan memasak sendiri, peningkatan makanan bergizi, pengurangan konsumsi makanan cepat saji dan juga ditemukan penurunan kuantitas aktivitas fisik masyarakat. Simpulan: Terdapat perubahan perilaku kesehatan pada masyarakat akibat pandemi yang mayoritas mengarah ke arah yang lebih baik, disamping masih memiliki beberapa perilaku kesehatan yang kurang baik.

Abstract

Introduction: In developing nations, like Indonesia, urinary tract infection, or UTI, is a frequent infection in women that results from the growth of microorganisms in the urinary tract. Escherichia coli is the most common cause. Antibiotics are the major form of treatment; however, misuse of these drugs has led to resistance. Innovation in potential medical plants is crucial. The curry leaf plant (Murraya koenigii) is one of them. Methods: The agar diffusion method was used to assess antibacterial activity. Murraya koenigii leaves were gathered from a garden in Tanjong Morawa. Escherichia coli ATCC 25922 is the type of bacteria that is used. Using 96% ethanol as the solvent, Murraya koenigii leaves were extracted using the maceration method. There were six treatments with concentrations of 6.25%, 12.5%, 25%, and 50%, positive control levofloxacin, and negative control DMSO. Results: The results of the phytochemical screening of the extracts showed that metabolites like flavonoids, tannins, and triterpenes had antibacterial effects. Data on the diameter of the inhibition zone were examined using the Kruskal-Wallis statistical test. The findings demonstrated that all treatments had statistically significant differences (p<0.05). Discussions: The presence of compounds such as flavonoids, tannins, and triterpenes in curry leaf extract has an antibacterial effect on Escherichia coli. This compound can prevent the creation of nucleic acids, inhibit cell membrane function, damage the permeability of bacterial cells, and deactivate bacterial enzymes. Conclusion: The ethanol extract of curry leaves has an inhibitory effect on the growth of Escherichia coli bacteria.

Abstract

Background: Type 2 diabetes mellitus (T2DM) is a glucose homeostasis disorder characterized by chronic hyperglycemia caused by insulin resistance. It is believed that this condition is associated with intestinal bacteria dysbiosis that potentially worsens T2DM progressivity. A number of experimental studies have recommended synbiotic supplementation, based on the principle of intestinal microbe regulation, as nutrition therapy for T2DM. This systematic review evaluates the effect of synbiotic supplement on T2DM patients’ glycemic and lipid profile. Methods: Articles published from 2014 to 2021 were searched with a number of search engines: PubMed, ResearchGate, Google Scholar, and Elsevier. This systematic review aims to evaluate the effect of synbiotic supplement on T2DM patients’ glycemic and lipid profile. Discussion: Eighty randomized control trials (RCTs) met the inclusion criteria to be analyzed qualitatively in this systematic review. The quality of each included study was evaluated with Cochrane’s Risk of Bias (ROB-2). Overall, in comparison to the control (placebo) group, the intervention group that received synbiotic supplementation showed a more significant decrease in fasting glycemic and HbA1c level. T2DM patients also appeared to experience improvement in lipid profile (decrease in cholesterol, triglyceride, and low density lipoprotein cholesterol (LDL-C), along with increase in high density lipoprotein cholesterol (HDL-C)) after receiving synbiotic supplementation. In addition, synbiotic supplementation is evaluated as safe to be consumed by T2DM patients with minimal side effects. Conclusion: The available evidence demonstrates the potential use of synbiotic supplementation as a nutrition therapy for T2DM treatment. Abstract

Background: The COVID-19 pandemic has affected children's daily activities, including school activities that are forced to be conducted online and thus increasing their total screen time. Along with this increase, there is likewise an increase in the prevalence of childhood obesity as children become less active. However, it is still unclear how prolonged screen time becomes a risk factor for overweight/obesity in children. This paper aims to determine the association between prolonged screen time and overweight/obesity in children. Methods: The articles in this study were selected from four databases (Pubmed, Embase, Cochrane Library, and ScienceDirect) with language limitations (published in English). Each of the included studies was evaluated using Oxford Critical Appraisal Tools. Discussion: There are three articles that are included in this study, two of which are meta-analysis and systematic review, and the other one is a cohort study. The included studies have positive similarities with clinical questions and showed positive associations between prolonged screen time and overweight/obesity in children. Two meta-analysis studies showed positive correlation with each odds ratio are 1,67 (95% CI(1.48,1.88)) and 1,262 (95% CI(1.155,1,379)). Another article showed large increases in TSB (time spent in sedentary behavior) (? 81.3) among overweight students and a modest increase (? 27.4) among obese students compared to normal weight students. Conclusion: Prolonged screen time (>2 hours/day) has a significant relationship with increased BMI leading to overweight or obesity.
