Abstract

Chronic Obstructive Pulmonary Disease (COPD) is a progressive respiratory disorder frequently associated with pulmonary tuberculosis (TB), particularly in individuals with risk factors such as smoking. We report a 65-year-old male with COPD group E who presented with dyspnea and chronic productive cough. The patient was diagnosed with newly detected pulmonary TB and was undergoing the intensive phase of anti-tuberculosis drug (OAT) therapy. Physical examination revealed bilateral wheezing with an oxygen saturation of 90%, which increased to 93% after administration of 2 L/min of supplemental oxygen; cardiac function was within normal limits. The patient received inhaled bronchodilators, intravenous methylprednisolone, levofloxacin, and OAT. After 10 days of hospitalization, dyspnea improved along with an increase in daily peak expiratory flow (PEF) values. This case highlights the importance of a multidisciplinary approach involving infection control and pharmacotherapy adjustment to achieve optimal clinical improvement in COPD patients with coexisting tuberculosis.

Abstract

Pendahuluan: Hiperplasia endometrium merupakan kondisi proliferatif yang berpotensi menjadi lesi prakanker. Ketidakseimbangan hormonal, terutama dominasi estrogen tanpa antagonisme progesteron, diyakini menjadi faktor penting dalam perkembangan gangguan ini. Penelitian ini dilakukan untuk menganalisis hubungan antara usia dan gambaran histopatologi hiperplasia endometrium di RSUD Dr. H. Abdul Moeloek Provinsi Lampung. Metode: Penelitian analitik observasional dengan desain cross-sectional ini menggunakan data rekam medis pasien hiperplasia endometrium periode 2021–2024 di RSUD Dr. H. Abdul Moeloek. Total sampling digunakan untuk mengumpulkan seluruh pasien yang dilakukan kuretase dengan hasil histopatologi hiperplasia endometrium. Analisis hubungan antarvariabel dilakukan menggunakan uji Fisher exact. Hasil: Dari 31 sampel yang memenuhi kriteria, sebagian besar pasien berusia ?45 tahun dan terbanyak memiliki gambaran hiperplasia non-atipik. Analisis statistik menunjukkan hubungan bermakna antara usia dan tipe histopatologi, dengan kecenderungan peningkatan hiperplasia atipik pada kelompok usia lebih tua. Pembahasan: Usia berperan sebagai faktor penting terhadap gambaran histopatologi hiperplasia endometrium. Perubahan hormonal pada fase perimenopause dan pascamenopause mendukung peningkatan risiko lesi atipik seiring bertambahnya usia. Simpulan: Terdapat hubungan signifikan antara usia dan gambaran histopatologi hiperplasia endometrium dengan kelompok usia yang lebih tua menunjukkan proporsi hiperplasia atipik yang lebih tinggi.

Abstract

Background: Osteoarthritis (OA) is a chronic degenerative joint disease that causes pain, limited mobility, and reduced ability to perform daily activities. The severity of OA can be assessed radiologically using the Kellgren–Lawrence classification. One of the inflammatory markers used to evaluate inflammatory activity in OA is the Neutrophil Lymphocyte Ratio (NLR), which reflects the balance between neutrophils and lymphocytes. This study aimed to analyze the relationship between NLR and the radiological severity of knee osteoarthritis in female patients at Dr. H. Abdul Moeloek Hospital. Method: This study used a cross-sectional approach. The samples consisted of medical records of female patients diagnosed with knee osteoarthritis during the 2022–2025 period. Data were analyzed using univariate and bivariate analyses with the Spearman rank correlation test. Results: The Spearman test showed no significant relationship between NLR and the radiological severity of osteoarthritis (p=0.789; r=-0.041). Discussion: These findings indicate that NLR is not a dominant factor in determining the radiological severity of knee OA. This may be influenced by other factors, such as the involvement of various inflammatory mediators besides NLR and the locally limited nature of inflammation in OA. Conclusion: There was no significant relationship between NLR and the Kellgren–Lawrence radiological severity in female patients with knee osteoarthritis at Dr. H. Abdul Moeloek Hospital

Abstract

Background:. Pulmonary tuberculosis remains a major health problem and requires a simple marker to assess disease severity. The platelet-lymphocyte ratio (PLR) has been widely studied as an indicator of inflammation, but its relationship with lung lesion extent remains inconsistent. This study aimed to assess the relationship between PLR and lung lesion extent in pulmonary tuberculosis patients at Abdul Moloek Regional Hospital. Methods: This study used a cross-sectional design with a total sampling technique. A total of 52 patients who met the inclusion and exclusion criteria were analyzed. PLR data were obtained from complete blood counts, and lesion extent was assessed using chest radiography. Spearman correlation analysis was used to test the relationship between the two variables. Results: The distribution of lesion extent showed the largest proportion in the extent category. PLR values ??varied significantly between subjects. The Spearman test showed a correlation coefficient of 0.084 with a p-value of 0.553, indicating no significant relationship between PLR and lung lesion extent. Conclusion: This study concluded that PLR is not associated with lung lesion extent in pulmonary tuberculosis patients. PLR cannot be used as a radiological marker to assess the severity of lung damage, so additional parameters or a multimodal approach are needed in clinical evaluation

Abstract

Pendahuluan:Tinea versicolor (panu) memiliki prevalensi tinggi di pondok pesantren (51,6%-71,1%), yang erat kaitannya dengan higenitas perorangan dan rendahnya pengetahuan. Dibutuhkan metode promosi kesehatan yang efektif. Penelitian ini bertujuan menganalisis efektivitas metode Focus Group Discussion (FGD) dalam meningkatkan pengetahuan santri tentang pencegahan tinea versicolor. Metode: Penelitian ini menggunakan desain quasi experimental (satu kelompok pre-test post-test). Sampel adalah 26 santri jenjang SMA di Pondok Pesantren X, Bandar Lampung, yang dipilih berdasarkan kriteria inklusi. Instrumen kuesioner teruji validitas (Pearson 0,382-0,767) dan reliabilitasnya (Cronbach's Alpha 0,817). Data dianalisis univariat dan bivariat menggunakan Uji Wilcoxon. Hasil: Terdapat peningkatan pengetahuan yang signifikan. Sebelum intervensi, 80% (n=21) santri memiliki pengetahuan 'Cukup Baik'. Setelah intervensi FGD, 100% (n=26) santri mencapai kategori pengetahuan 'Baik'. Hasil Uji Wilcoxon menunjukkan perbedaan yang sangat signifikan secara statistik (p < 0,001). Pembahasan: Efektivitas ini menunjukkan FGD adalah metode yang tepat untuk meningkatkan pengetahuan santri terkait Tinea versicolor . Sifat interaktif dan partisipatif dari metode ini memungkinkan peserta untuk bertukar pengalaman dan menganalisis informasi secara kritis, yang berkontribusi pada pemahaman yang lebih komprehensif. Simpulan: Metode Focus group discussion (FGD) secara signifikan efektif dalam meningkatkan pengetahuan santri tentang pencegahan tinea versicolor.

Abstract

Pendahuluan: Binahong (Anredera cordifolia (Ten.) Steenis) adalah salah satu jenis tanaman yang telah dipercaya memiliki manfaat dalam dunia kesehatan dengan berbagai kandungan senyawa kimia dalam ekstrak kental daunnya, meliputi flavonoid, fenol, tanin, saponin, alkaloid, terpenoid, dan steroid dengan efek farmakologi sebagai analgetik, antiinflamasi, antidiabetes melitus, antihiperkolesterolemia, antimikroba, dan sitotoksik. Untuk mengidentifikasi kandungan senyawa-senyawa tersebut yang terekstraksi dalam pelarut etanol 70%, diperlukan skrinning fitokimia. Metode: Penelitian ini dilakukan dengan pendekatan kualitatif menggunakan ekstrak daun binahong yang diperoleh melalui proses maserasi dengan pelarut etanol 70%. Ekstrak yang dihasilkan kemudian dianalisis secara fitokimia menggunakan pereaksi spesifik untuk mengidentifikasi kelompok senyawa, antara lain alkaloid (pereaksi Mayer, Dragendorf, dan Bouchardat), flavonoid (uji Mg-HCl), tanin (FeCl?), saponin (uji busa), serta steroid dan terpenoid (uji Liebermann–Burchard). Hasil: Ekstrak daun binahong (A. cordifolia) dengan pelarut 70% etanol memiliki kandungan flavonoid, fenol, tanin, saponin, alkaloid, terpenoid, dan steroid ditunjukkan dari hasil masing-masing uji positif. Pembahasan: Senyawa-senyawa yang terkandung tersebut membuktikan bahwa daun binahong memiliki potensi efek farmakologi dan penggunaan pelarut etanol 70% efektif mampu mengekstraksi senyawa aktif tersebut karena bersifat semi-polar. Simpulan: Skrinning fitokimia yang dilakukan dengan uji kualitatif membuktikan bahwa daun binahong dari kabupaten Way Kanan yang sudah diekstraksi dengan pelarut etanol 70% mengandung berbagai senyawa aktif yang mampu memberikan efek farmakologi bagi makhluk hidup.

Abstract

Background: Chronic kidney disease (CKD) stage 5 is the final phase that requires patients to undergo hemodialysis as the main treatment. Monitoring blood urea levels before hemodialysis is an important aspect because these values can reflect the risk of death in patients. Based on this, the study aims to examine the relationship between pre-hemodialysis urea levels and mortality rates in CKD stage 5 patients at Dr. H. Abdul Moeloek Regional General Hospital in Lampung Province. Method: This study applied a quantitative observational design with a retrospective cohort approach. The data analyzed were obtained from secondary sources, namely, medical records of patients diagnosed with stage 5 CKD. The sample included 150 patients who received hemodialysis in 2023–2024, selected through consecutive sampling. Data were analyzed univariately; normality was assessed using the Shapiro-Wilk test, and bivariate analysis was conducted using the Mann-Whitney Test. Results: The mean pre-hemodialysis urea level in deceased patients was higher than in discharged patients. The Mann-Whitney test showed a significant difference in urea levels between the two groups (p=0.0001), indicating a relationship between pre-hemodialysis urea levels and patient mortality. Discussion: The findings suggest that pre-hemodialysis urea levels can serve as an indicator of mortality risk in patients with stage 5 CKD. Patients with higher urea levels tend to have a greater risk of death. Conclusion: There is a significant correlation between pre-hemodialysis urea levels and mortality in patients with stage 5 CKD. Patients with higher urea levels have a higher risk of death.

Abstract

Introduction: There is no consensus on the optimal vaccine platform for preventing respiratory syncytial virus (RSV) infection. Recent advances in RSV vaccine development aim to improve efficacy and safety across various platforms. This study aimed to compare the efficacy and safety of available RSV vaccines using a network meta-analytic approach to identify the most effective strategy for RSV prevention.Methods: A systematic search of  PubMed, ScienceDirect, Cochrane, and Scopus (up to November 2025) identified randomized controlled trials (RCTs) of RSV vaccines in healthy populations. Evaluated interventions included Ad26.RSV.preF–RSV preF,  Ad26.RSV.preF, Bivalent RSVpreF, RSVPreF3OA, and placebo controls. Network meta-analyses were performed using the netmeta package in R. Risk of bias and certainty of evidence were appraised using RoB 2.0, CINeMA, and GRADE frameworks, respectively.Result: Ten RCTs were analyzed. Ad26.RSV.preF–RSV preF Protein Vaccine possesses superior efficacy in reducing RSV-related respiratory illness compared to placebo (RR 0.25 [95% CI: 0.15-0.45]).  Ranking analysis corroborated this finding, identifying it as the most effective intervention (P-score = 0.827). Safety profiles were comparable across all interventions; however, the precision of these estimates was limited by wide confidence intervals and substantial heterogeneity. Conclusion: Ad26.RSV.preF–RSV preF demonstrated the highest efficacy for RSV prevention. However, their safety profile has not yet been clearly defined, and further research is needed to assess long-term effectiveness and monitor potential late adverse effects.

Abstract

Introduction: Antimicrobial resistance (AMR) remains a critical global issue. By 2050, it is projected to cause around 10 million deaths if current trends persist. Traditional antimicrobial discovery struggles to keep up with rapidly evolving resistance  due to its lengthy process, high cost, and high failure rate. Developing a single drug can take over a decade of research and cost millions of dollars. These challenges demand more efficient approaches, with artificial intelligence (AI) offering a promising path to accelerate and improve antibiotic development. Methods: GoogleScholar, PubMed, ScienceDirect, and Scopus were systematically searched following the PRISMA 2020, yielding 13 eligible studies. All included in vitro validation, and four extended to in vivo investigations. Risk of bias was evaluated using the QUIN (in vitro) and the SYRCLE (in vivo) tools. Discussion: Across studies, AI supported multiple stages of antibiotic discovery, including target identification, lead compound optimization, also enhancement of pre-clinical testing. In target identification, two studies revealed novel antibacterial targets distinct from classical pathways. During lead optimization, applied in most studies, AI-generated compounds demonstrated strong antimicrobial activity and low MIC values against broad-spectrum and multi-drug resistant bacteria. Four in vivo studies further showed that these de novo antibiotics exhibited superior antimicrobial efficacy to current standard therapies. Finally, in preclinical testing, AI models accurately predicted cytotoxicity and hemolysis, later confirmed experimentally. Conclusion: AI has markedly improved efficiency and accuracy in antibiotic development. While continued model refinement, validation, and ethical oversight remain crucial, AI-integrated pharmaceutical research indicates growing maturity and transformative potential.

Abstract

Introduction: Chronic inflammation and oxidative stress are major mechanisms in degenerative diseases, including cancer, diabetes, and cardiovascular disorders. The NF-kB and Nrf2 pathways play a role in maintaining redox balance and inflammatory response. Curcumin, the main bioactive compound of Curcuma longa L., can simultaneously modulate both pathways through pharmacogenomic mechanisms influenced by individual genetic variations. Method: This study used the Systematic Literature Review method in accordance with the PRISMA 2020 guidelines. Searches were conducted in PubMed, Scopus, ScienceDirect, and Google Scholar until October 2025 using the keywords “Curcumin,” “NF-kB,” “Nrf2,” “Oxidative Stress,” and “Pharmacogenomics.” Studies assessing the molecular modulation of NF-kB/Nrf2 by curcumin and gene-dependent effects were included. Results and Discussion: A total of 31 studies met the inclusion criteria, including in vitro, in vivo, in silico, and clinical studies. Curcumin suppressed NF-kB activation and activated Nrf2/HO-1, thereby reducing ROS and proinflammatory cytokines. Variations in the ERCC5 rs751402 gene, as well as the expression of SLC7A11 and ATAD3A/B, influenced the cellular response to curcumin. In silico and network pharmacology analyses revealed multigenic targets related to inflammation and oxidative stress. Nanoformulations enhance bioavailability and clinical immune response. Conclusion: Curcumin acts as a dual-regulator pharmacogenomic agent that balances the NF-kB and Nrf2 pathways, reducing inflammation and oxidative stress in a gene-dependent manner. These findings support its potential as a natural biomolecule for the development of precision therapies for chronic diseases involving inflammation and oxidative stress.   Keywords: Curcumin, NF-kB, Nrf2, Oxidative Stress, Pharmacogenomics
