Abstract

Dyslipidemia is a lipid profile abnormality that can escalate the risk of cardiovascular disease. The rate of cardiovascular events in Indonesia is very high. One of the causes of dyslipidemia is due to polymorphisms in genes associated with lipid metabolism. The APOA-1 gene encodes the APOA-1 protein which functions to regulate HDL protein synthesis. The objective of this study is to determine APOA1 gene polymorphisms in patients with dyslipidemia. Samples of healthy controls and dyslipidemia patients were used in this investigation. The lipid profiles of the patients and normal controls are determined at the onset of the study. Following DNA extraction, the APOA-1 gene was amplified and sequenced using the serum sample. MEGA X and BLAST were used to analyze the sequencing results. The study's findings demonstrated that the APOA-1 gene length in samples of dyslipidemic patients and normal controls was 433 bp. While the normal control samples have the same sequence as the database, the dyslipidemic patient samples have an APOA-1 gene polymorphism in the promoter region. APOA-1 gene polymorphism results in disturbances in lipid profiles, particularly HDL which is at risk of developing dyslipidemia. The APOA-1 gene has the potential to be developed as a biomarker for diagnosing dyslipidemia involving a larger number of samples.

Abstract

Antibiotic resistance is an important health problem and a threat to public health. Dogs as pets can be both spreaders and reservoirs of antibiotic-resistant bacteria. Based on spatial analysis, this study aims to determine the distribution pattern of multi-drug resistance (MDR) Escherichia coli found in pet dogs in Kupang City. The spatial analysis method is based on the convex hull technique, elementary analysis of disease, and the Nearest Neighbour Index (NNI). Information on antibiotic-resistant E. coli has been obtained from the laboratory analysis results, and secondary data was obtained for spatial analysis. The results of this study have shown that the distribution pattern of the incidence of MDR E. coli in the City of Kupang in 2020 is a clustered pattern with the Nearest Neighbor Index (NNI) value of 0.783,  which is higher than the NNI AMR E. coli in the City of Kupang of 0.763. The incidence of MDR E. coli that has occurred in domesticated dogs takes place with a prevalence rate of 35% in an area of 5079 Ha. These results have shown that the incidence of MDR E. coli has spread to several areas in Kupang City and is a threat to public health. It requires appropriate prevention and control measures by implementing good hygiene and sanitation in the relationship between humans and their pets.

Abstract

A variety of ultra-invasive portable blood sugar test kits have been available on the market; these kits require drawing blood from capillaries, which can still be painful and uncomfortable and even enhance the risk of infection. To develop a non-invasive blood sugar meter that is painless and comfortable to use, innovation is required. AKUDia, a mobile application for monitoring blood sugar, was developed as a smart wearable device to help older adults with diabetes stay healthy. This study aimed to measure the usability of the AKUDia application using a usage questionnaire. In terms of a feasibility study, this was quantitative. USE Questionnaire contains a list of questions encompassing 30 questions divided into four dimensions: Usefulness, Ease to use, Ease of learning, and Satisfaction. 50 respondents were recruited for this study, meeting the inclusion criteria of DM patients who had completed a week-long training program on using the AKUDia application. Samples were selected by simple random sampling, and descriptive and univariate data analyses were performed. The AKUDia feasibility test results demonstrated a usability value of 83%, which was classified as very feasible; an ease of use value of 74 percent, which was classified as feasible; an ease of learning value of 83%, which was classified as very feasible; and a satisfaction aspect of 71% at the feasible level. The average usability value of the four categories (usability, ease of use, ease of learning, and satisfaction), which was obtained at 78 percent, indicates that the AKUDia application's usability is at a practicable level. The usability of the AKUDia application is feasible. Governments should support the use and implementation of this innovation in addition to healthcare professionals like doctors and nurses. Information technology systems have the potential to serve as an instrument for illness prevention, treatment, and rehabilitation in addition to promoting health. Future studies should be enhanced to investigate, through experimental research, how the AKUDia application supports nursing care and health management for individuals with diabetes mellitus and the general population.

Abstract

Optimal health services are considerably increasing patient satisfaction. Optimization of health services could be implemented using the Modular Professional Nursing Care Method (MAKP Modular). MAKP Modular provides a clear division of tasks and responsibilities for nurses in accordance with the distribution of nursing staff. This literature review aimed to identify the effect of MAKP Modular in increasing patient satisfaction through the optimization of health services. The research design is a literature review. The journal article was conducted on the Google Scholar journal database website and 10 journal articles were selected according to inclusion criteria. The results of the review show that the application of MAKP Modular affects the quality of implementation of nursing care standards. The performance of nurses manifested in the implementation of MAKP Modular also influences patient satisfaction. However, there are still some deficiencies in the application of modular MAKP in the health service setting. These deficiencies are influenced by factors of knowledge, attitudes, and motivation of nurses. The application of MAKP Modular to the healthcare system is conducted by combining the team method and the primary method. The combination of these two methods allows for a more effective and efficient process of providing nursing care. Evaluation of the implementation of MAKP Modular in clinical and hospital settings needs to be engaged on an ongoing basis to determine the effectiveness of implementation.

Abstract

Chronic hyperglycemia in patients with diabetes mellitus (DM) can be worsened by prolonged physical and emotional stress, which increases stress hormones like cortisol and raises blood glucose levels. Effective stress management is therefore essential to prevent glucose fluctuations and reduce the risk of diabetes-related complications. Techniques such as deep breathing exercises, progressive muscle relaxation, and hypnosis have been shown to lower stress hormones, enhance endorphin release, and improve glycemic control. This study aimed to measure patients' blood sugar levels before and after administering stress management interventions, including deep breathing relaxation techniques and five-finger hypnosis. The study method used a quasi-experimental design with a pre- and post-test approach, without a control group, was used. A total of 61 DM patients from the Diabetes Center in Ternate City were recruited through simple random sampling. The intervention consisted of deep breathing exercises, progressive muscle relaxation, and five-finger hypnosis. Blood glucose levels were measured before and after the intervention. Data analysis involved paired t-tests, ANOVA, and correlation tests. The results showed that the mean blood glucose level decreased from 215.75 mg/dL to 209.62 mg/dL following the intervention, with a mean difference of -6.13 mg/dL. However, this reduction was not statistically significant (p = 0.0593; α = 0.05). It was concluded that there was a decrease in blood glucose levels after the intervention, although the difference was not statistically significant.  The application of this study suggests that stress management techniques such as deep breathing, relaxation, and five-finger hypnosis may provide benefits in managing stress. Further studies using controlled designs and larger sample sizes are recommended.

Abstract

The need for the consumption of immunomodulators is very important to support the body’s immunity of health workers during the Covid-19 pandemic to remain prime in providing services. Because the helath of healthcare workers during the pandemic determines the service that will be provided to patients. This study aims to analyze the influence of external factors on the practice of consuming immunomodulators in health workers at the Denpasar City Health Center. This research is non-experimental research using a cross-sectional method. The population of the research is 550 people and the sample size used in this study amounted to 232 people who have met the inclusion criteria. The results obtained for R square were 0.605 which indicated that the support of the head of the Health Center, family support, co-worker support, and advertising promotion had a large role, namely as much as 60.5% in influencing the practice of consuming immunomodulators in health workers. The R-value obtained in this study was 0.778; F of 86,852; and p of 0.000 (p<0.05) where the hypothesis is accepted, which means that there is a significant influence of external factors on the practice of consuming immunomodulators. There is a significant relationship between the support of the head of the Health Center, family support, co-worker support, and advertising promotion on the practice of consuming immunomodulators. External factors greatly influence healthcare workers' attitudes toward maintaining their health, one of which is their responsiveness in consuming immunomodulators during the pandemic.

Abstract

Childhood routine immunization is a critical stage to ensure the health of infants and protect against serious diseases. Therefore, adequate strategies are urgently needed to increase childhood immunization coverage to prevent global disease and death. This review has identified the effect of interventions to increase immunization coverage among children in developing countries. A review included published studies from 2013 to 2023 on randomized controlled trials (RCT) and pre-post intervention that met eligible criteria. All included studies had been conducted in English-published articles on Pub Med and Google Scholar, without being limited to geographical sites. A total of 1107 published articles were accessed and 12 final eligible articles were reviewed. 66.67% of the included studies were conducted in Africa, 16.67% in South East Asia countries, and others were conducted in East Asia and America. These studies demonstrated that different interventions (SMS and call reminders, sticker reminders, immunization education, home-based records, and community-centered) had significant increases in immunization coverage for childhood compared to the control group with standard care or without any interventions. The present findings suggest that interventions including implementing SMS and call reminders, sticker reminders, education both from health workers and local leaders, and home-based records can potentially reduce immunization dropout. However, strategies to improve coverage for immunization uptake should also be considered preferred community-based to extend the marginal groups.

Abstract

Childhood stunting, or low height for age, continues to be a concern for world health since it raises the possibility of both mortality and growth and development disorders. West Kalimantan province ranks seventh with the highest prevalence of 29.8% by 2021. But little is known about Pontianak's children's stunting risk factors, especially in the UPT Puskesmas Pal Lima (one of the community health centers with a high prevalence of stunting). The study aimed to determine risk factors associated with stunting in the UPT Puskesmas Pal Lima, such as high-risk pregnant women, pregnant women with chronic energy deficiency, communication of educational information about stunting, occupancy density, and age of pregnant women in the UPT Puskesmas Pal Lima. Cross-sectional study design was used. The research was conducted in February-March 2023. The population in this study were all stunted toddlers in the UPT work area of the Pal Lima Health Center. Sampling used quota sampling with all 75 stunted toddlers. Data analysis used univariate and bivariate analysis (chi square test with 95% CI). This research found that 61,3% with high risk pregnant women, 21,3% with chronic energy deficiency, 52%  not exposed to information communication and education about stunting, 32% occupancy density were not qualified, 48% risk at age of pregnant mothers, 32% severe stunting and 68% stunting. There was a significant relationship between high-risk pregnant women (p-value = 0,015), pregnant women with chronic energy deficiency (p-value = 0,001), the information communication and education about stunting (p-value = 0,046), occupancy density (p-value = 0,043), and the age of pregnant women (p-value = 0,049), and the incidence of stunting in toddlers in the working area of UPT Puskesmas Pal Lima, West Pontianak District. Based on the results of this study can be used as a basis for the primary prevention of stunting in pregnant women.

Abstract

The growth of E. coli is influenced by several factors, including environmental pH. Environmental pH unsuitable for bacterial growth conditions will interfere with the enzyme activity and influence bacterial growth. This study aimed to determine the effect of pH variations (5.3, 5.8, 6.3, 7.3, 7.8, 8.3) in EMBA medium on E. coli growth. The research method used was true experiment post-test-only control design. The samples in this study were suspensions with dilutions of 10-4, 10-5, 10-6, and 10-7, inoculated using the duplo test to EMBA with various pH conditions, so that the total sample size is 48. The results showed that the mean number of E. coli bacteria on EMBA with pH 5.3 was 9.1 x 106 CFU/mL; with pH 5.8 was 9.6 x 106 CFU/mL; with pH 6.3 was 1.2 x 107 CFU/mL; with pH 7.3 was 1.1 x 107 CFU/mL; with pH 7.8 was 9.7 x 106 CFU/mL; and with pH 8.3 was 7.1 x 106 CFU/mL. Growth in positive control showed the mean number 1.4 x 107 CFU/mL; negative control showed no growth of E. coli or other microorganisms. Based on the One-Way ANOVA statistical test with a 95% confidence level, there was no difference in the mean number of E. coli bacteria in the six pH variations of EMBA medium (p-value > 0.05). E. coli bacteria grew best at neutral pH. Its growth decreases in slightly acidic and slightly alkaline pH, but it can still be observed. This allows E. coli to survive in extreme pH. Pathogenic E. coli have developed the potential to live inside the human body. They will experience temporary stress in unfavourable conditions before finally adapting. The advice for future researchers is to test the effect of pH on E. coli growth by using other E. coli growth media or with a wider range of pH (more acidic and more basic). It is also recommended to conduct further research about the effect of various environmental conditions such as temperature, nutrients, and others on bacterial growth.

Abstract

The meaning of life is considered to affect breast cancer patients positively. Finding meaning in life is key to improving well-being during and after experiencing a traumatic event such as cancer. Previous research was still rare to explore the factors affecting the meaning of life in breast cancer patients. The study aimed to analyze the factors influencing the meaning of life of breast cancer patients.  This study was an analytical observational method with a cross-sectional design. The inclusion criteria of the respondents were breast cancer patients who had obtained at least one cycle of chemotherapy. We used purposive sampling, and the total sample in this study was 135 respondents. Questionnaires were used to collect the data. Data were analysed by multivariate test, namely a linear regression test with a significance level of p ≤ 0,05. The result showed only two variables affect the meaning of life, communication of health workers and spirituality, respectively. The strongest variable was the communication of health workers.  The equation from the analysis showed that 11 % explained the phenomenon. The spirituality and communication of health workers positively affected the meaning of life. Application of therapeutic communication as well as spiritual support are needed in nurses to patients while undergoing therapy. Therefore, nurses must pay attention to spirituality and good therapeutic communication when providing patient services.
