Abstract

Background: Gastrointestinal stromal tumors (GISTs) are the most frequently seen mesenchymal neoplasm in the digestive system. GISTs was originally described as smooth muscle tumor in the 1980s but advances in molecular diagnostic methods and immunohistochemistry moved GISTs into a different category from smooth muscle tumors. The study aims to report on epidemiological, clinical, immunochemical, and therapeutic characteristics of GISTs. Methods: We performed a retrospective descriptive study of 28 cases of GIST in the gastroenterology and general surgery departments of M. Djamil General Hospital Padang, Indonesia was conducted from January 2019 to December 2021. Data collection using medical records of patients in M. Djamil General Hospital Padang, Indonesia. We reported different data: Age, sex, symptoms, site, and immunohistochemistry of the tumor.  Results: Our study included 28 patients 18 males (64,3%) and 10 females (35.7%), with a median age range of 51-60 years. The presenting symptoms were abdominal mass (53.6%), abdominal pain (14.3%), bloody or dark-colored stools (14.3%), constipation (7.1%), fatigue (7.1%), and nausea and vomiting in 1 case (3.6%). Sixteen patients (57.1%) had a primary tumor and twelve patients (42.9%) had further metastatic lesions. The tumors were found in the stomach (21.4%), small intestine (14.3%), rectum (14.3%), and in other sites such as the retroperitoneal, liver, and distal pancreas. The immunohistochemical study was performed in seven cases (expression of  CD117 and DOG1), in four cases  CD117 was positive, while in three cases CD117 was negative. In two cases, the expression of DOG1 was positive, while in one case, it was negative. Eighteen patients with GIST (64.3%) underwent surgical procedures. A combination of surgical and chemotherapy was prescribed in 9 patients (32.1%). The chemotherapy regimen prescribed is imatinib. One patient is inoperable and has received symptomatic treatment. Conclusion : In conclusion, our result showed that GISTs are highest in the male population with an age range of 51-60 years. The most common symptom of GISTs is abdominal mass. Patients who have been diagnosed with GIST by histopathological examination do not always show positive expression in the immunohistochemical study. Surgical resection was indicated in the majority of patients, and a combination of surgery and chemotherapy is also prescribed as a treatment for patients with GIST.

Abstract

Background: Nodular hidradenoma is a rare benign skin adnexal tumor originating from sweat glands. It can recurrence and possibly future malignant transformation. An intraoperative frozen section guides the operation and provides margin control. Case report: We reported  a case of a 54 year old woman with a lump under the right eyelid who was early diagnosed with suspected basalioma. The lump was initially small as the size of a green bean seed, but grew in these five months to the size of a corn kernel, itchy and sometimes bleeds. A dermoscopic examination showed a nodule with yellowish globules and arborizing vessels. An intraoperative frozen section procedure was performed with a diagnosis of benign lesion and tumor-free incision margins. The patient's final diagnosis of formalin-fixed paraffin-embedded (FFPE) tissue was nodular hidradenoma. Conclusion: Frozen section intraoperative is very useful to prevent recurrence and malignant transformation of nodular hidradenoma. Eyelid tumors, in particular, have shown benefit from frozen section margin assessment for cosmetic or functional purposes.

Abstract

Background: Intraoperative diagnosis by cytological and frozen section procedures is needed in some cases including peripheral nerve specimens. The results of the cytological evaluation and frozen sections can be relied upon to guide the surgeon in making intraoperative decisions. The pathologist provides information on gross and microscopic findings that correlate with the clinical, and radiological. Although the cytology and frozen section diagnosis approach has been carried out in a multidisciplinary manner, there are still limitations in the intraoperative diagnosis of peripheral nerve specimens. Limitations in this procedure can be overcome by understanding and analyzing possible errors and pitfalls during the procedure thereby increasing the accuracy of the diagnosis. Case report: Schwannoma is a nerve sheath tumor consisting of neoplastic cells with Schwann cell differentiation that generally originates from peripheral nerves in the skin and subcutaneous tissue of the head and neck or along the flexor surfaces of the extremities. More than 90% of schwannomas are solitary and sporadic lesions that can affect any age but with a peak incidence in the fourth to sixth decades of life and there is no racial or gender predisposition. Conclusion: Schwannomas often appear as asymptomatic masses or incidental findings. Intraoperative frozen section pathological diagnosis can be used in determining subsequent surgical management.

Abstract

Background: a natural source that has great potential as a source of antioxidants is Jatropha curcas (L.). This plant has attracted widespread attention due to its significant content of antioxidant compounds in various parts of the plant, including leaves, seeds, and roots. In addition to health benefits, Jatropha curcas also has applications in various fields, including body metabolism. Methods: This research aims to gain a comprehensive insight into research trends, research collaborations, as well as researchers' contributions in the field of Jatropha's nutritional potential for body metabolism. The research method used is qualitative research with a literature review. Results: The results of overlay visualization in 2021 showed that there were many studies conducted on Jatropha curcas. The level of density in the image is indicated by the number of keywords that often appear and can be marked with yellow Jatropha curcas has not been studied much. This indicates a research gap that exists in current onomastic research so the opportunity to research this topic is still very broad. Conclusion: A contribution to healthcare research can be made by involving Jatropha curcas as a potential source of antioxidants. The health benefits derived from antioxidant compounds found in various parts of this plant, including leaves, seeds, and roots, offer a positive contribution to health research. Additionally, insights into the application of Jatropha curcas in enhancing body metabolism open opportunities for the development of therapies or supplements that can support human health and well-being.

Abstract

Background: Head and neck squamous cell carcinoma (HNSCC) is a malignancy that occur in head and neck region and originates from squamous epithelial cell in the upper respiratory tract mucosa. This malignancy has a high heteregenicity. Carcinogenesis due to HPV infection has a different moleculer pathway from HNSCC without HPV infection. Methods: We searched Pubmed collection for open access and English language articles publish access from years 2016-2022 with keyword HPV, HNSCC and carcinogenesis. Results: We had 16 articles that matched from the keywords. The mechanism of HPV leading carcinogenesis in HNSCC because the HPV has a specific protein that can cause malignant transformation in squamous epithelial cells. The E6 protein inactivates the p53 tumor suppressor gene by activating the ubiquitin ligase E6AP causing degradation of p53. The E7 protein inactivates the pRb (protein retinoblastoma) tumpr suppressor protein by blocking the interaction between pRb and E2F, causing E2F depression. Conclusion: Carcinogenesis and malignant transformation in HNSCC correlated with HPV infection (high risk type) via the E6 and E7 proteins. This combination can causes abnormal cell cycle and leads cells to escape cell cycle control and get a malignant transformation.

Abstract

Background: Childhood leprosy is an important marker of the status of the ongoing leprosy control program, as it is an indicator of active disease transmission in the community. Children are believed to be the most vulnerable group to infection with M. leprae due to their immature or nascent immunity and exposure to intrafamilial contacts. Case report: A 8 years ole male patient presented with hypopigmented patch and erythematous patch with not itchy, not pain, and loss of sensation which increasing more since 1 week ago. Initially 2 months ago, white patch that felt numbness appeared on the back, then white patches wider and spread to the chest, both arms and thighs. Dermatologic finding showed erythematous plaques, hypopigmentation macule with infiltration on cheek, back, both of arms, xerosis cutis. Decreased sensory function was found in the median, ulnar and posteriol tibial nerves. Acid-fast bacilli (AFB) staining revealed bacterial index (BI) +5 and morphological index (MI) 80%. Histopathological examination showed atrophic stratified squamous epithelium, grenz zone, many groups of foamy macrophages reached the fat layer. The patient was diagnosed Morbus Hansen borderline lepromatous leprosy type with mild reversal reaction and treated with MDT-MB drug package, vitamin B complex tablet, zinc tablet, paracetamol tablet, and 10% urea cream on dry skin 2 times a day on dry skin after bath during 2 month and the lesion was reduced. Conclusion: The rate of childhood leprosy continues to be a significant problem. There is a clear need to strengthen early detection, treatment, and regular follow up of these cases in both high and low endemic setting is essential in the prevention of deformities.

Abstract

Background: The relationship between stress and blood sugar levels in individuals with diabetes mellitus is complex and can be influenced by various factors. When a person experiences stress, whether physical or emotional, the body responds by releasing stress hormones, can contribute to insulin resistance, a condition where the body's cells become less responsive to the effects of insulin. Furthermore, stress may affect a person's ability to manage diabetes effectively. Chronic stress, if not managed, can have long-term effects on the body, potentially contributing to the development of complications associated with diabetes. It's important to recognize that the relationship between stress and blood sugar levels varies among individuals Methods: We used descriptive analytic with cross sectional design. The population of this study were all patients with Diabetes Mellitus who were treated at the Internal Medicine Clcinic in Dr. Muhammad Zein general hospital Painan. The sample was taken as many as 79 respondents with accidental sampling technique. Data collection tool with documentation and questionnaire studies. Data analysis using chi square test. Results: The result showed that more than half of 49 respondents (62.0%) have an abnormal blood sugar levels, more than half of 54 respondents (68.4%) with moderate stress levels and there is a relationship between moderate stress levels and blood sugar levels with a value of α 0.000 (p value <0.05). Conclusion: The moderate stress levels are associated with increased blood sugar levels. The impacts of this study for nurses, that we must be able to review the psychosocial aspects of patients with diabetes mellitus and provide knowledge about diabetes mellitus either through direct counseling, dialogue, or through other information media such as posters, leafleats and social media.

Abstract

Background: Effective documentation can help prevent various issues experienced by patients in hospitals. By maintaining thorough, systematic, and accurate records, nurses can mitigate the risk of errors and ensure the continuity of care. This research seeks to identify the factors influencing the quality of nursing care documentation in accordance with the Indonesian Nursing Diagnostic Standards (SDKI), Indonesian Nursing Outcome Standards (SLKI), and Indonesian Nursing Intervention Standards (SIKI) at Dr. M Djamil General Hospital in Padang in 2023. Methods: This research employed a cross-sectional study design. The study population comprised all nurses working in the inpatient unit of Dr. M. Djamil General Hospital Padang. The sampling technique used in this research was proportional random sampling, and the sample size consisted of 238 respondents. Results: The research results indicated that 61.3% of the documentation quality was deemed inadequate, with 63% lacking in knowledge, 61.8% exhibiting poor attitudes, 57.6% demonstrating low motivation, 68.9% experiencing insufficient supervision, and 57.1% showing inadequate socialization. The study found significant relationships between knowledge, attitude, supervision, and socialization with the quality of nursing care documentation, with a p-value of <0.005. The factor most strongly associated with the quality of nursing care was socialization (OR = 2.019, 95% CI 1.045-3.901). Conclusion: This research identified that the quality of nursing documentation was still suboptimal, highlighting the ongoing need for continuous and intensive socialization regarding the documentation of nursing care.

Abstract

Background : Regenerative medicine stands at the forefront of transformative healthcare, offering innovative approaches to address tissue damage and degenerative conditions. Central to this paradigm is the profound impact of stem cell therapy, a dynamic and promising avenue within regenerative medicine. As we navigate the complexities of this evolving field, our systematic literature review seeks to unravel the diverse applications, breakthroughs, and challenges inherent in utilizing stem cells for regenerative purposes Methods : We searched in Science direct, Pubmed, NIH and another e-library with keyword stem cell, application and regenerative medicine. Our review aims to contribute to the ongoing narrative of stem cell therapy in regenerative medicine. Through this exploration, we aspire not only to enhance our understanding but also to guide future research endeavors and clinical applications in this dynamic and promising field. Result : We reviewed the article  with the theme article is stem cell and regenerative medicine. Stem cell therapy consistently demonstrated remarkable efficacy in promoting tissue regeneration. Across applications, stem cells showcased a remarkable versatility in enhancing tissue-specific cell behavior and activating regenerative pathways. his versatility positions stem cells as a promising tool with broad applications in regenerative medicine. Conclusion : This review emphasize the versatility and efficacy of stem cells in diverse regenerative scenarios. Evaluating methodological quality and comparing studies reveal nuanced insights, guiding future research directions. This review provides a comprehensive understanding of the current landscape, paving the way for innovative approaches in regenerative medicine.

Abstract

Background: This study aims to determine the appropriate preparation of simple PRP protocol to yield the maximum growth factor concentration, specifically for PDGF and TGF-β1. Methods: Blood samples were collected from 5 healthy volunteers who signed informed consent for participation in the study. The samples then processed by single centrifugation at 4 four different speed (600, 800, 1000, and 1200 rpm) for 4 different centrifugation times ( 8, 10, 12, 14 minutes). The Platelet Derived Growth Factor (PDGF) and Transforming Growth Factor β1 (TGF- β1) concentrations were determined by enzyme-linked immunosorbent assay (ELISA). Blood routine test analysis were measured by hematology analyzer. Results: The 8 min with 600 rpm centrifugation protocol resulted in a slightly greater release of TGF-β1 (7,127 ng/mL) and PDGF (473,909 pg/mL). While the other samples resulted from 50,296 pg/mL (14 min with 1200 rpm) until 406.883 pg/mL (8 min with 800 rpm) of PDGF. The other results of TGF-β1 vary from 0,558 ng/mL (14 min with 1200 rpm) to 6,322 ng/mL (10 min with 600 rpm). Conclusion: The highest concentration of PDGF and TGF-β1 were obtained from centrifugation process at 600 rpm for 8 minutes. Meanwhile, the lowest concentration of PDGF and TGF-β1 were obtained from centrifugation process at 14 min with 1200 rpm.
