Abstract

Hypertension is one of the non-communicable diseases that are experienced by many elderly and can cause various health problems, one of which is sleep pattern disorders. Untreated sleep pattern disorders can reduce the quality of life of the elderly. One of the non-pharmacological therapies that can be used to overcome this problem is music therapy. The purpose of this case study is to describe the management of sleep pattern disorders in the elderly with hypertension through music therapy at the East Kalongan Ungaran Health Center. The method used is a descriptive case study with a nursing process approach that includes assessment, nursing diagnosis, intervention, implementation, and evaluation. The study subjects were elderly people aged 60–70 years who had hypertension and sleep pattern disorders. The intervention was in the form of Benson music therapy for 20 minutes every day for 3 consecutive days accompanied by monitoring sleep patterns and blood pressure. The results of the study showed that the patient had difficulty sleeping, often woke up at night, felt dissatisfied with his sleep, and had a blood pressure of 160/110 mmHg. An established nursing diagnosis is that sleep pattern disorders are associated with lack of sleep control. After 3 days of music therapy, patients reported better sleep, less frequent waking up, increased sleep time, and felt more rest. Blood pressure also decreased to 145/90 mmHg. Evaluation shows that the problem of sleep pattern disorders is partially resolved and music therapy can help improve sleep quality and lower blood pressure in the elderly with hypertension.

Abstract

Type 2 Diabetes Mellitus (DM) is a chronic metabolic disease characterized by hyperglycemia due to impaired insulin secretion or action, with one of its complications being skin integrity disorders such as diabetic ulcers, which are difficult to heal because of peripheral  neuropathy and circulatory impairment. Wound care using the moist wound healing method is a modern approach that maintains wound moisture, thereby accelerating the healing process and reducing the risk of infection. This study aims to describe the management of skin integrity disorders in patients with type 2 Diabetes Mellitus through moist wound healing care at Pandan Arang Hospital, Boyolali. The method used is descriptive with a nursing case study approach on one patient with type 2 Diabetes Mellitus and diabetic ulcer treated in the Akar Wangi Ward, with data collected through interviews, observation, physical examination, and medical record documentation over three days. The results showed that the patient had a stage III wound measuring 8×6 cm with a random blood glucose level of 778 mg/dl, and after three days of moist wound healing care, improvements were observed in the form of granulation tissue formation, reduced swelling, decreased tissue damage, and reduced pus, although the wound size remained unchanged. The conclusion of this study is that moist wound healing is able to improve wound conditions and enhance the tissue healing process, although skin integrity problems have not been fully resolved, thus requiring continued care.

Abstract

Chronic Obstructive Pulmonary Disease (COPD) is the third leading cause of death worldwide, characterized by airflow obstruction in the respiratory tract. The primary problem that often arises is ineffective airway clearance due to excess mucus production and increased thick secretions. At Dr. Ario Wirawan Lung Hospital in Salatiga, 482 cases of COPD were recorded in the past year (2024-2025), of which 61.2% required nebulizer therapy intervention. This scientific paper aims to describe the management of ineffective airway clearance through nebulizer therapy in COPD patients in the Emergency Department (ER) of Dr. Ario Wirawan Lung Hospital in Salatiga. The author used a descriptive case study method to address this nursing problem. The study subject was an elderly patient (77 years old) who complained of shortness of breath for 3 days and additional breath sounds, rhonchi. The primary intervention was nebulizer therapy using bronchodilator medication to widen the airways and thin and facilitate the expectoration of secretions. Implementation was carried out for 1 x 3 hours in the emergency room, monitoring breathing patterns and oxygen saturation. The nursing care results showed that after administering nebulizer therapy and supportive measures such as suctioning and oxygen administration, the problem of ineffective airway clearance was resolved. The patient reported feeling lighter in breathing, reduced rhonchi, improved respiratory rate, and increased oxygen saturation (SpO2) from 89% to 93%. It is recommended that healthcare workers in the emergency room consistently implement nebulizer therapy according to standard operating procedures to improve the quality of respiratory care and prevent further complications in COPD patients.

Abstract

The integration of interactive learning media in higher education requires lecturers to possess adequate technological, pedagogical, digital, and motivational readiness. This study aimed to assess the overall readiness of lecturers to utilize interactive learning media in Indonesian language teaching at Universitas Islam Labuhan Batu and to identify the strongest and weakest dimensions of their readiness. A quantitative approach with a descriptive survey design was employed. The participants consisted of five lecturers in the Indonesian Language Education Study Program, selected using a total sampling technique. Data were collected through a 20-item questionnaire covering four dimensions: technological readiness, pedagogical readiness, digital competence, and motivational readiness. The data were analyzed using descriptive statistics, including mean scores, percentages, and standard deviations. The findings revealed that the lecturers demonstrated a high overall level of readiness, with a mean score of 3.89 (77.8%). Technological readiness scored 3.92 (78.4%), digital competence 3.76 (75.2%), and pedagogical readiness 3.64 (72.8%), all categorized as high readiness. Motivational readiness emerged as the strongest dimension, with a mean of 4.24 (84.8%), categorized as very high, while pedagogical readiness was the weakest dimension. These findings indicate that the lecturers have a strong foundation for implementing interactive learning media, particularly in terms of motivation and technological capability. However, further professional development is needed to strengthen pedagogical integration and ensure that interactive media are used strategically to support meaningful Indonesian language learning.
