Abstract

Background: Scurvy is a potentially fatal disease caused by a severe deficiency of vitamin C, which can manifest as a musculoskeletal problem. Early diagnosis and treatment of vitamin C deficiency are crucial to prevent severe symptoms. Unfortunately, vitamin C level evaluations in Indonesia are very limited due to the lack of laboratory facilities. As a result, patients with scurvy are often underdiagnosed and do not receive adequate management. Case Presentation: 5-year-old boy with musculoskeletal pain and inability to walk. Radiographic findings showed a pathognomonic line of Fränkel, Trümmerfeld zone, and Wimberger ring sign, with laboratory evaluation showing an anemia and vitamin D deficiency. We did not perform a laboratory examination for vitamin C due to a lack of test facilities but the patient was given oral 250 mg of vitamin C daily instead. The patient showed significant improvement in symptoms and laboratory results after three months of vitamin C supplementation. To summarize, that vitamin C deficiency should be considered among patients with related musculoskeletal manifestations supported by imaging results.

Abstract

Background: Medical students face examinations more frequently compared to students from other faculties. Examinations are one of the causes of anxiety commonly experienced by medical students, leading to both physical and psychological responses. Effective coping mechanisms in response to stressors are necessary for medical students as a defense mechanism. Objective: This research aims to explore the causes of exam anxiety, the contributing factors to anxiety and the coping mechanism used by medical students. Methods: The research adopts a qualitative descriptive design, the sample consist of active medical students from Faculty of Medicine, Ahmad Dahlan University who have faced block examinations. Sample selection was done using a test- anxiety inventory, with participants chosen from each academic year and divided into focus group discussion (FGD) sessions. The FGD results were subsequently analyzed using content analysis in the form of research problem categories. Results: The discovered data consists of the main research issues, which are student’s anxiety when facing block exams, the factors causing anxiety, and student’s coping mechanisms. Conclusion: The high anxiety group portrays block exams as a subjective matter and tends to lead to anxiety, students in low anxiety group predominantly utilize problem-focused coping mechanism.

Abstract

Background: Laryngeal carcinoma is a carcinoma that forms in the tissue of the larynx. The cause of laryngeal carcinoma is not known with certainty, but many risk factors can cause laryngeal carcinomas, such as a history of smoking, alcohol consumption, gender, age, and exposure environment. The quality of life of patients with laryngeal carcinoma after chemoradiation can be influenced by several things, including several risk factors for laryngeal carcinoma. Objective: The purpose of this study was to assess the association of risk factors with the quality of life of laryngeal carcinoma patients after chemoradiation at Kariadi General Hospital. Methods: This study is a cross-sectional study with samples of laryngeal carcinoma patients who have received chemoradiation therapy at Kariadi General Hospital. Sampling was carried out using a consecutive sampling technique using a Univesity of Washington Quality of Life (UW-QoL) questionnaire which was translated into Bahasa. The data from the sampling was then processed using the IBM SPSS Statistics 25.0 application. Results: Overall, the quality of life of laryngeal carcinoma patients after chemoradiation was categorized as “good” quality of life with a mean score of 62. There was no significant relationship (p<0.05) between the quality of life scores and each risk factor for laryngeal carcinoma. Conclusion: There is no significant relationship between risk factors on the quality of life of laryngeal carcinoma patients after chemoradiation at Kariadi General Hospital.

Abstract

Background: Therapy-related myelodysplastic syndromes (t-MDS) are grouped with therapy-related acute lymphoblastic leukemia (t-ALL) and therapy-related myelodysplastic/myeloproliferative neoplasms (t-MDS/MPN) under therapy-related myeloid neoplasms (t-MNs). Most myelodysplastic syndrome (MDS) prognostic models have excluded t-MDS patients, leaving their prognostic utility uncertain. Objective: This systematic review aims to synthesize the predictive utility of existing and novel risk stratification tools in assessing the survival and transformation of t-MDS. Methods: Searches were conducted in PubMed and ScienceDirect following PRISMA 2020 guidelines, focusing on overall survival and transforming t-MDS into acute myeloid leukemia (AML). Two reviewers independently screened references, extracted data, and assessed quality using the QUAPAS-2 tool. Results: From 1715 abstracts and 13 papers, 6 studies were included. Three studies on the International Prognostic Scoring System (IPSS) showed significant predictive power for survival and AML transformation. Five studies on the revised IPSS (IPSS-R) and WHO-based Prognostic Scoring System-revised (WPSS-R) also showed significant results. One study highlighted the cytogenetic component of IPSS-R (cIPSS-R) as highly prognostic. Conclusion: Existing and novel risk stratification tools demonstrate significant prognostic power for t- MDS. Further refinement and validation are needed to enhance risk assessment and treatment strategies.

Abstract

Background: Purine metabolism, carried out in the liver, results in the production of uric acid. Hyperuricemia is a condition of elevated level of uric acid in the blood that can lead to gout if untreated. Lemon (Citrus Limon.) contains flavonoids which act as antioxidants and have the potential role as antihyperuricemia. Aim: To determine the effect of lemon peel extract on uric acid levels. Methods: Thirty-six male wistar rats from Farmhouse Ungaran were randomly divided into 6 groups, consists of healthy control group (K0), hyperuricemic control (K1), allopurinol control (K2), and 3 treatment groups (P1, P2, P3). Hyperuricemia induced with administration of block broth and potassium oxonate for 4 weeks. Lemon peel extract doses of 17,5, 35, and 70 mg/kg BW, and allopurinol 90 mg/kg BW was given orally for 2 weeks. Statistical analysis was conducted to evaluate differences among groups before and after the intervention. Results: There was a significant difference (p<0,05) in uric acid levels before and after intervention of lemon peel extract in groups K1, K2, and P3. Meanwhile, in groups K0, P1, and P2, there were no significant differences observed. In the pre-test uric acid levels, there was a significant difference (p<0,05) between the K0 group and K1, K2, P1, P2, and P3. Conclusion: There were no significant differences observed in the uric level acid of the treatment and control group after intervention, but there were significant differences in the effects of graded dosages between the treatment groups.

Abstract

Background: Plant-based nanomedicine is a rapidly growing field of research that utilizes plant-derived nanoparticles for the treatment of cancer. Nanoparticles from plant extracts have a number of advantages over synthetic nanoparticles, including biocompatibility, low toxicity, and ease of synthesis Objective: This article comprehensively explores the synthesis, characterization, and anticancer activities of silver nanoparticles (AgNPs), zinc oxide nanoparticles (ZnO-NPs), and gold nanoparticles (AuNPs) synthesized using various plant extracts Methods: The research method used in this article is Systematic Literature Review (SLR). The research method used in this article is the Systematic Literature Review (SLR). The data are collected using Harzing’s Publish or Perish. The included literature sources discuss the synthesis and characterization of plant-based nanoparticles as anticancer, are published within the last 5 years, and are indexed in Google Scholar Results: Various synthesis methods influence the properties and cytotoxic effects of silver, zinc oxide, and gold nanoparticles derived from plant extracts. Characterization techniques provide insights into their synthesis, morphology, crystalline structure, surface chemistry, and stability. Conclusion: The results of this study reveal that silver nanoparticles (AgNPs), zinc oxide nanoparticles (ZnO-NPs), and gold nanoparticles (AuNPs) synthesized from plants have great potential in the treatment of various types of cancer.

Abstract

Background: Refractive errors result in difficulty in focusing on an object clearly, leading to blurred vision. A recent study by the WHO has reported that refractive errors have become the leading cause of visual impairment. The development of refractive errors has been a significant concern over the last two decades due to its high prevalence worldwide. One possible condition that may occur in individuals with refractive errors is a change in axial length. Axial length is defined as a measurement from the corneal surface to the retinal surface. Aims: To analyze the correlation between axial length and the occurrence of refractive errors in myopia and hypermetropia patients at the Ophthalmology Department of Diponegoro National Hospital. Methods: This observational analytical study with a cross-sectional design was conducted on 36 eyes from 18 patients; each suffering from myopia and hypermetropia at the Ophthalmology Department of Diponegoro National Hospital. Patients provided informed consent, and then the axial length was measured using ultrasound biometry. Data analysis was performed using univariate analysis and Spearman correlation bivariate analysis. Results: The results of the Spearman statistical test showed a significant correlation between the axial length and the occurrence of myopia (p=0.003) with a moderate correlation strength and the occurrence of hypermetropia (p=0.000001) with a strong correlation strength. Conclusion: The axial length has a significant correlation with the occurrence of refractive errors, especially in myopia and hypermetropia patients.

Abstract

Background: Leprosy is an infectious disease caused by Mycobacterium leprae bacteria which is still the focus of a global problem and causes disability in patients. The degree of disability in leprosy is assessed as mild and severe as a result of the inflammatory process. This study aims to determine the relationship between the degree of disability in leprosy and IL-6 serum levels as an inflammatory biomarker in leprosy patients. Methods: A Cross-sectional observational analytical study on leprosy patients aged 20-60 years, not pregnant, and undergoing treatment at Donorojo Jepara Hospital from March to April 2024. Serum IL-6 levels were analyzed using the ELISA method and tested for correlation with the degree of disability leprosy. Results: Statistical analysis of Spearman's correlation rank test shows a p value = 0.279 and the results of the line pattern on the Scatter dot graph are not linear. Conclusion: There is no correlation between serum IL-6 levels and the degree of disability in leprosy patients at Donorojo Jepara Hospital.Keywords: Degree of Disability in Leprosy, Leprosy, Serum IL-6

Abstract

Focal brain lesions can induce abnormal involuntary movement disorders, including hemichorea, hemiballismus, parkinsonism, myoclonus, dystonia, tremor, and asterixis. The most common cause of focal brain lesions is stroke, followed by trauma, neoplasm, anoxia, vascular malformations, metabolic disease, and multiple sclerosis. Hemichorea-hemiballismus as an initial presentation of acute ischemic stroke is very rare, with an incidence rate of 0.54%. Case Presentation: A 65-year-old male with a past medical history of hypertension and diabetes experienced sudden, involuntary, non-rhythmic, and uncontrollable movements of the left extremity one day before being admitted to the hospital. A non-contrast CT scan of the head revealed acute cerebral infarction on the right basal ganglia. The symptoms of hemichorea improved with the initiation of haloperidol after three days of treatment. Regarding this case, acute ischemic stroke should be considered suppose a patient without hyperglycemia and with sudden onset of hemichorea visits to emergency room.

Abstract

Background: Elderly are susceptible to health problems due to decreased organ function such as increased blood sugar levels that are at risk for diabetes mellitus, so proper management is needed with physical activity. Foot exercise and hydrotherapy can be an alternative to physical activity for elderly to lower blood sugar levels.Objective: To determine the effectiveness of the combination of foot exercise with and without hydrotherapy on blood sugar levels in the elderly.Methods: Experimental study with parallel 3 groups pre and post-test design. Research subjects were 21 elderly, divided into control group (n=7), combination of foot exercises and hydrotherapy intervention group (n=7), and foot exercise intervention group (n=7). The intervention was done 3 times a week for 5 weeks. Blood sugar levels were measured using POCT.Results: There is no significant decrease in blood sugar levels from control group (p>0.05). There is significant decrease in blood sugar levels from foot exercise and hydrotherapy intervention group (p<0.05) and foot exercise intervention group (p<0.05). The difference in blood sugar levels showed significant results between foot exercise and hydrotherapy intervention group and control group (p=0.000) and between foot exercise intervention group and control group (p=0.004). There is no significant differences in blood sugar levels between foot exercise and hydrotherapy intervention group and foot exercise intervention group (p=0.112).Conclusion: Both foot exercise and hydrotherapy are effective in lowering blood sugar levels in the elderly.
