Abstract

Snakebite remains a prior Neglected Tropical Disease in the World, especially in South-East Asia. There were many diversities in the management of snake bites based on the different levels of clinician experience. Anti-snake venom remains the mainstay treatment in venomous snakebites, meanwhile, the distribution of anti-snake venom is very sparse. Snakebite is time-limited and could be a life-threatening event. This case report described a delayed anti-snake venom administration in snakebite grade III following a viper snake bite. Tight observation including clinical manifestation and laboratory tests is the foremost thing to do in managing snakebite, due to its unpredictable clinical manifestation. Anti-snake venom dosage must be decided based on the progress of clinical manifestation and laboratory results.

Abstract

Background: In 2020, lectures were passed online due to the pandemic Covid 19, but now it is offline as before. This method has increased hand duration for writing. Using a hand for long and repetitive duration can affect pathological abnormalities on the nerves that lead to complaints such as soreness, pain, numbness, and weakness. Of these symptoms, 90% refer to a pathological disorder named Carpal Tunnel Syndrome (CTS).Objective: To determine the relationship between writing activities and subjective complaints of CTS using research subjects of students of the Department of Medical Science, Faculty of Medicine, Universitas Diponegoro Semarang Indonesia. Methods: This study is analytical with a crosssectional method. CTS complaints were measured Boston Carpal Tunnel Syndrome Questionnaire(BCTS-Q). Data analysis was performed using univariate analysis and Spearman correlation bivariate analysis. Results: 99 respondents fit the criteria that experience subjective complaints of CTS based on the BCTS-Q; 69.7% mild, 4% moderate, and 1% very severe. There was no correlation between duration (p= 0.852), frequency (p = 0.824), and rest time of writing (p = 0.285) with subjective complaints of CTS. It could be the cause of high subjectivity in interpreting the severity of CTS without a physical examination and difficulty in recalling the time they needed to write in one week. Conclusion: There is no relationship between the duration, frequency, and length of rest time of writing and subjective complaints of CTS.

Abstract

Background: Renal atheroembolism is an under-recognized cause of renal failure. Atheroembolism is caused by cholesterol crystals from ulcerated atherosclerotic plaques and is influenced by inflammation and endothelial dysfunction. The formation of complicated atherosclerotic lesions is a prerequisite for the development of cholesterol crystal emboli. The exact incidence of atheroembolism remains unclear due to frequent underdiagnosis. In a previous report, the rate of embolism in the kidney among patients dying after surgery for acute atherosclerosis of the aorta was 77%. Non-pharmacological therapeutic management (such as diet and physical exercise) is an important factor in preventing and reducing the risk of atherogenesis. Prior research suggests that controlling body weight, primarily through physical exercise, is a critical factor in preventing and reducing the risk of atherogenesis, in addition to medication adherence and other therapeutic management alternatives. Another study found that mice with metabolic syndrome given Garcinia mangostana pericarp can reduce inflammatory cell infiltration, thereby reducing physiological symptoms, metabolic syndrome, liver disorders, and cardiovascular symptoms. The combination of physical exercise and metabolic supplements in the treatment was even able to significantly reduce atheroma plaque area as well as the incidence of plaque rupture, and prolong survival time. Objective: To investigate the effect of Garcinia Mangostana Linn Pericarp administration along with Physical Exercise on the histopathologic features of renal atheroembolism in Wistar rats with metabolic syndrome. Methods: This study is a true experimental study with research subjects in the form of male Rattus norvegicus Wistar rats which were randomly divided into 3 groups. All groups were induced metabolic syndrome through the administration of a diet similar to the "Western" purified atherogenic diet patent Envigo® formulated as a high fat diet (20 - 23% BW; 40 - 45% kcal from fat), saturated fatty acids (SFA >60% of total fatty acids), and drinking tap water ad libitum. However, in group K (no additional treatment), P1 (garcinia pericarp extract 800 mg / kg / day with physical exercise), and P2 (nanoemulsion garcinia pericarp extract 50 mg / kg / time of administration with physical exercise). Results: In the group treated with garcinia pericarp extract 800 mg/kg bw/day with physical exercise and the group with nanoemulsion 50 mg/kg bw/day with physical exercise, there were no fatty streaks, inflammation, and impaired myocyte coherence. There was 0-5% fibrous connective tissue, one layer of foam cell layer and myocyte-lipid, no cholesterol crystal embolism was found. Conclusion: The administration of Garcinia Mangostana Linn pericarp extract and/or nanoemulsion with physical exercise can have an effect on the histopathological picture of the kidneys of Wistar rats with metabolic syndrome, although there was no difference in effect between the two treatments.

Abstract

Background: Inflation-deflation technique had been a conventional method for delineating the intersegmental plane during lung segmentectomy. Over the last decade, the use of icg has shown a significant increase as an alternative method. According to the European Society of Thoracic Surgeons' (ESTS) newly released expert consensus recommendations, Systemic ICG is the preferred method for performing ISP delineation. Objective: This study aimed to determine the safety of intravenous ICG in lung segmentectomy compared to inflation-deflation method. Methods: PubMed, Science direct, and Scilit were systematically reviewed. Studies comparing ICG with inflation-deflation method in lung segmentectomy were included. The main outcome included operation time while blood loss, length of hospital stay, and air leakage event became secondary. Odd Ratio (OR) and Mean Difference (MD) with 95% of Confidence Interval (CI) were applied for dicotomous and continous variable, respectively. Heterogeneity was assessed using Cochrane Q and I statistics, as reviewers also manually tested for heterogeneity with sensitivity analysis. Results: Six studies with a total of 839 patients were retrieved. All of them were retrospective comparative studies, mainly with a diagnosis of pulmonary nodules. Most studies utilized peripheral vein injections of 2.5 mg/mL ICG solution, which had a dosage range of 3–10 mL. Intravenous ICG administration was associated with a noticable operation time [MD = -19.30, 95% CI -28.29 to -10.31, p < 0.00001], length of hospital stay [MD = -0.61; 95% CI -1.16 to -0.06, p = 0.03], as well as a significant OR observed in the number air leakage [OR = 0.39; 95% CI 0.20 to 0.75, p = 0.005]. Meanwhile, there was no significant difference in the amount of bleeding between the inflation-deflation group and the ICG group [MD = -5.18, 95% CI -12.08 to 1.72, p = 0.14]. Conclusion: This meta-analysis has demonstrated statistically that the duration of surgery, length of hospital stay, and the probability of postoperative air leak are significantly lower with the application of ICG in lung segmentectomy.

Abstract

Background: Ulcers are one form of chronic wounds as one form of complication experienced by leprosy sufferers. In the process of healing wounds such as ulcers, TGF-β has a broad role. Several studies have stated that in chronic wound conditions there is a decrease in TGF-β levels so that the wound is increasingly difficult to heal. This study aims to determine the relationship between TGF-β serum levels and the degree of ulcer wound healing. Methods: This study is an observational analysis study with a cross-sectional design and involved 33 leprosy ulcer patients undergoing treatment at the dr. Rehatta Jepara Hospital in the period March-April 2024. Serum TGF-β levels were measured using the Enzyme-linked immunosorbent assay (ELISA) method while the degree of wound healing was assessed by scoring the Pressure Ulcer Scale for Healing (PUSH). Bivariate analysis using the Spearman Correlation Rank Test. Results: The correlation test between serum TGF-β levels and the degree of wound healing (p = 0.807) and duration of leprosy as covariate (p = 0.96) showed insignificant results. Conclusion: There is no relationship between serum TGF-β levels and the degree of wound healing and duration of leprosy.
