Abstract

Background: Tuberculosis (TB) is one of the most severe global health problems. The World Health Organization recommends the application of and compliance with infection control measures, one of which is cough etiquette to prevent transmission of pathogenic droplets. Purpose: The aim of the current research was to determine the effectiveness of cough etiquette counseling on changes in the knowledge, behavior, and awareness of patients with confirmed and suspected TB at Public Health Center (PHC) of C in K city. Methods: This research was designed as a pre-experimental study and was conducted in July 2018. The target population of the research comprised TB patients who were being treated in PHC of C in K city. The inclusion criteria were patients at PHC of C in K city with confirmed TB recorded between January and June 2018 and those with suspected TB reported between May and June 2018. This research was analyze based on univariable and bivariable analysis using T-testing and Wilcoxon testing. Results: There were significant differences in knowledge changes before and after cough etiquette counseling (mean difference = 3.72; p < 0.00). There was a significant difference in behavior before and after counseling (mean difference = 1.12; p = 0.04). There was also a significant difference in awareness before and after counseling (mean difference = 5.89; p < 0.00). Conclusion: Changes in knowledge, behavior, and awareness were observed in confirmed and suspected TB patients after cough etiquette counseling.

Abstract

Kolaborasi interprofesional terbukti meningkatkan kualitas pelayanan kesehatan. Pendidikan interprofesional (IPE) di tahap pendidikan akademik dan klinik penting untuk menanamkan kemampuan kolaborasi. Penelitian ini bertujuan untuk mengetahui persepsi dan kompetensi IPE mahasiswa sebelum dan sesudah IPE rotasi klinik. Penelitian ini merupakan penelitian potong lintang yang dilakukan pada bulan November 2020 hingga Januari 2021 di Fakultas Kedokteran dan Fakultas Ilmu Kesehatan Universitas Muhammadiyah Malang (FK-FIKES UMM). Pengambilan sampel dilakukan dengan consequtive sampling. Penelitian ini melibatkan 278 mahasiswa dari tiga program studi yaitu, 152 mahasiswa kedokteran (54,7%), 80 farmasi (28,8%) dan 46 keperawatan (16,5%). IPE di rotasi klinik berlangsung selama dua minggu menggunakan metode pembelajaran aktif. Persepsi IPE diukur menggunakan Interdisciplinary Education Perception Scale (IEPS) sedangkan Interprofessional Collaborative Competency Attainment Survey (ICASS) untuk menilai kompetensi IPE sebelum dan sesudah IPE. Hasil menunjukkan bahwa jenis kelamin (p=0,033) dan persepsi (p=0,000) mempengaruhi kompetensi IPE sesudah IPE. Tidak didapatkan perbedaan persepsi dan kompetensi IPE antar profesi sebelum maupun sesudah IPE, namun semua mahasiswa mengalami peningkatan persepsi dan kompetensi secara bermakna. Di awal pembelajaran, persepsi (61,29±6,537) dan kompetensi IPE (121,92±14,039) mahasiswa kedokteran berada di urutan kedua setelah keperawatan. Setelah IPE, persepsi mahasiswa kedokteran menempati urutan tertinggi (65,22±8,63) sedangkan kompetensi IPE mahasiswa kedokteran berada di urutan terendah (126,08±4,345) dibandingkan dua kelompok lain. Jenis kelamin dan persepsi berperan penting untuk mempengaruhi kompetensi IPE. Upaya berkelanjutan diperlukan untuk membangun persepsi IPE agar dapat meningkatkan kompetensi IPE sehingga menghasilkan kolaborasi yang baik.

Abstract

Background and purpose: Tuberculosis (TB) is an infectiousdisease caused by Mycobacterium tuberculosis (MTB). The high incidence of TB acquires the active role of TB cadres in TB case finding. Motivation is an important factor that support the performance of TB cadres. This study aims to explore factors that affect the motivation of TB cadres case detection rate of TB. Methods: This was an observational analytic study with crosssectional study design, conducted in Kediri City. A total samplingtechnique was applied with a total of 59 TB cadres in the workingarea of Sukorame Public Health Center, in Kediri City. TB cadres filled in a questionnaire containing questions about sociodemographic, attitudes, knowledge, and motivation with a total of 39 questions. Data were analyzed with univariate descriptive statistics, bivariate test by Fisher and Chi Square tests and followed by multivariate analysis using logistic regression. Results: Of the 59 cadres, 37 (67.2%) have a good motivation onTB case findings. The results of multivariate analysis showed that there were four independent variables which have a significant association with the motivation of TB cadres, including age, education, jobs and incentive. Cadres who are 41-60 years have better motivation compared to 25-40 years (OR=31.49; 95%CI:2.373-417.907; p=0.009), cadres who have higher education (university/college) have better motivation (OR=36.957; 95%CI:2.389-571.805; p=0.010). While those who not getting incentives (OR=0.100; 95%CI: 0.013-0.758; p=0.026) and having a job (OR=0.120; 95%CI: 0.021-0.670; p=0.016) were less likely to have a good motivation. Conclusion: Motivation of TB cadres in Kediri City is influenced by intrinsic factors (age, education and employment) and extrinsic factor (incentive). Providing appropriate incentive will significantly boost cadre motivation and relevant stakeholders should consider making this strategy sustainable.

Abstract

Improving the Skills of Posyandu Cadres in Lactation Counseling as an Effort to Promote Exclusive Breastfeeding Background: During the 3 years of the achievement of exclusive breastfeeding in Puskesmas Kendal Kerep Malang has fluctuated, and only 1 out of 17 people related to the counseling of the breastfeeding who has been trained in breastfeeding counseling. Objective: To determine the effect of breastfeeding counseling training to support health workers in breastfeeding in Puskesmas Kendal Kerep Malang Methods: The study was quasi-experimental design with non-equivalent control group Results: There were differences in the support of health workers (pre-test and post-test) in the control and treatment groups (Wilcoxon test = 0.000 < á 0.05. There were differences in the support of health workers in breastfeeding (pre-test and post-test) in the control and treatment groups (Mann-Whitney = 0.000 < á 0.05). average ratings pre-test and post test both the control group and the treatment of the support of health workers had increased quite dramatically (9.167). Conclusions: There was a positive difference to the health workers support mothers in breastfeeding before and after breastfeeding counseling training. Breastfeeding counseling training was very effective to change the treatment of health workers in support of breastfeeding.

Abstract

Background: Supplementation of iron (Fe) tablets are essential to prevent pregnant women from iron deficiency anemia. Several other factors, such as knowledge, attitude, side effects, and personal access to information about iron tablet supplements, reduce the compliance of pregnant women to consume iron tablets. This study uses a combination of interventions, Short Message Service (SMS) reminders, and counseling. Purpose: This research aims to discover the effects of SMS reminders and counseling as well as factors that influence pregnant women's compliance in consuming Fe tablets. Methods: 170 pregnant women in Balowerti Public Health Center were divided into control and intervention groups. Each group received pre and post-test. The intervention group was given the treatment of SMS reminders and counseling for seven days before the post-test. The data analysis was using McNemar's paired test. Results: McNemar test results obtained the effects of the SMS Reminder and counseling on knowledge (p = 0.01), attitude (p = 0.00), and adherence (p = 0.00) of pregnant women in the intervention group as compared to the control group at the time of the post-test. The logistic regression analysis found that the factors that influence the compliance of pregnant women to consume Fe tablets (p <0.05) during the post-test comprise attitude and number of Ante Natal care (ANC). Conclusion: SMS reminders and counseling interventions improve pregnant women's adherence, knowledge, and behavior to consuming iron tablets. This result is important as a strategy to reach women's empowerment in reproductive health.

Abstract

Background: Dengue hemorrhagic fever (DHF) is a viral infection transmitted by mosquitoes and is one of the health problems in community. DHF is currently still a health problem in Indonesia because the incidence of DHF cannot be reduced. The DHF morbidity in Indonesia in 2015 reached 50.75 per 100,000 populations, and the Incidence Rate (IR) in 2016 reached 78.85 per 100,000 populations, exceeding the national IR target (49 per 100,000 populations). The presence of mosquito larvae is an indicator of mosquitoes in an area. As a matter of fact, there are many factors that affect the presence of mosquito larvae. Objective: To determine the house environment factors related with the presence of mosquito larvae Method: Observational analytic with cross-sectional design. The sampling used cases and controls of 285 houses. The sample filled out the informed consent agreement, then the researchers gave the questionnaire, and made observations, next the researchers filled out the checklist according to the observation. The data were analyzed in bivariate technique with chi-square test, followed by multivariate test using logistic regression test. Results: The results of the multivariate test showed 7 factors that influenced the presence of larvae, consisting of the variables of PSN (Mosquito Breeding Ground Eradication) action (p = 0.012; OR = 0.022; CI = 0.001-0.435), PSN attitude (p = 0.005; OR = 1658247.9; OR: CI = 0.000-0.014), Number of people in the house (p = 0.013; OR = 0.071; CI = 1.731-113.550), Frequency of cleaning containers (p = 0.006; OR = 1139.1; CI = 0.000-0.080), Mosquito breeding sites (p = 0.006; OR = 0.006; CI = 0.424-6148.76), Fish in containers (p = 0.003; OR = 434.272; CI = 0.000-0.134), container location (p = 0.007; OR = 0.006 ; CI = 4.047- 4653.77). Conclusion: Many factors affect the presence of mosquito larvae, both in the home environment and containers owned by residents of the house. The presence of fish in containers is the most influential factor. Breeding fish as mosquito larva predators is included as parts of 3M Plus which is quite easy to do by the community. Thus, it requires maximum effort to increase knowledge of the community about these steps.

Abstract

Non-Communicable Diseases (NCD), also known as chronic diseases such as hypertension, diabetes, and obesity, are from combined genetic, physiological, environmental, and behavioral factors. The number of NCD screening visits at Ngletih Public Health Center (PHC) Kediri decreased in 2018 was 29% and became 24% in 2019, with a target of 10,718 people. This study aimed to know the factors affecting community awareness for screening NCD. The respondents are people aged 15-59 years who live in the working area of Ngletih PHC and have never been diagnosed with hypertension, diabetes, and obesity. This research used an analytical observational study with a cross-sectional design. The number of samples taken is 1,000 with purposive sampling; then, respondents filled out 27 questionnaires self-administered. The data analysis was implemented univariate, bivariate using chi-square, and multivariate using multiple logistic regression. The chi-square test showed eight variables that increase awareness for screening NCD: age, gender, education level, marital status, family history, source of health information, knowledge, and attitude. The multiple logistic regression tests showed five factors that increase awareness for screening NCD: age, gender, marital status, sources of health information, and knowledge. The dominant factor affecting community awareness for screening NCD is a source of health information (p=0.023; OR=4.353; CI=1.220-15.537).

Abstract

Indonesia now ranks fourth in the world in terms of the largest number of unimmunized children. SMS (Short Message Service) reminders and counseling have been shown to increase outreach, but no research has combined these two interventions. This study aimed to determine the effectiveness of SMS Reminder and Counseling on the knowledge, attitude, and willingness of mothers under five and to find out the factors that influence this willingness. This study used the one-group method, pretest, and posttest design. The samples were 186 children under five (18-24 months) who felt incomplete and mothers who had children under five (control and intervention groups). The Chi-Square test shows significant results during the post-test (p 0.05). The Mc-Nemar test shows the influence of counseling and SMS Reminders on increasing knowledge, attitudes, and willingness (p = 0,000). The logistic regression test shows the factors that affect the advanced immunization of under two years child, that was sufficient knowledge (ρ = 0,000; OR = 13,384; CI = 5,986-29,926). SMS Reminders and counseling affect parents’ knowledge, attitudes, and willingness. Factors affecting parental willingness to continue immunization are adequate knowledge. Collaboration between officials and health agencies, the community, and cellular operators is needed to implement the training of health workers.

Abstract

Effect of Incision Boundary, Mitosis Index, and Tumor Necrosis on the Recurrence Rate of Phyllodes Tumors Background: Phyllodes tumor is one neoplasms fibroepitelial rare, approximately less than 1 % of all breast neoplasms. Determination of prognosis of phyllodes tumors based on histopathologic classification is difficult to implement. To evaluate the phyllodes tumor, an important marker for prognostic factors, mainly based on the criteria of phyllodes tumor itself (such as mitotic index, limits the incision). Objective: To determine the effect limits the incision, mitotic index and tumor necrosis against phyllodes tumor recurrence rate. Methods: An observational analytic study of the period 2009-2011 and followed through 2014. To determine the effect on recurrence rate limit incision Mann Whitney test , to determine the effect on the mitotic index recurrence rate testunpaired t -test , and to determine the effect of the tumor necrosis recurrence rate chi square test. Result: Obtained 16 samples that met the inclusion criteria with 10 samples and 6 samples did not relapse relapse. There were no influences between adjacent incisions (p = 0.05) , mitotic index (p = 0.06) , and tumor necrosis (p = 0.152) for phyllodes tumor recurrence rate Conclusion: There were no influences between adjacent incision, mitotic index , and tumor necrosis against phyllodes tumor recurrence rate

Abstract

Relationship between Sex and Clinical Features of Non-Syndromic Cleft Lip at CLP Center FK UMMBackground: Non-syndromic cleft lip is a type of cleft that is not accompanied by abnormalities in the head and neck. There are sex differences in the timing of palate closure.Objective: To determine the relationship between sex and clinical features of non-syndromic cleft lip (with or without cleft palate) at CLP Center FK UMM (2016-2017).Method: CrossSectional. Data on sex and clinical features of non-syndromic cleft lip were obtained from the CLP Center FK UMM as many as 241 out of 271. Data analysis used the chi-square test with conditionsexpectedcount less than five not more than 20%. If it does not meet the requirements use the non-parametric Mann Whitney test.Results: The results of the chi-square test data analysis did not meet the requirements so that a non-parametric Mann Whitney test was carried out which showed p = 0.152 (p> 0.05), which means that there is no sex relationship and clinical features of cleft lip (with or without cleft palate). ) non-syndromic.Conclusion: There is no sex relationship and the clinical picture of cleft lip (with or without cleft palate) is non-syndromic.Latar Belakang: Celah bibir non sindromik merupakan jenis celah yang tanpa disertai kelainan pada kepala dan leher. Terdapat perbedaan jenis kelamin terhadap waktu penutupan langit-langit.Tujuan: Mengetahui hubungan jenis kelamin dan gambaran klinis celah bibir (dengan atau tanpa celah langit-langit) non sindromik di CLP Center FK UMM (tahun 2016-2017). Metode:  CrossSectional. Data jenis kelamin serta gambaran klinis celah bibir non sindromik yang diperoleh dari CLP Center FK UMM sebanyak 241 dari 271. Analisis data menggunakan uji chi-Squaredengan syaratexpectedcountkurang dari lima tidak lebih dari 20%. Jika tidak memenuhi syarat menggunakan uji non parametrikMann Whitney.Hasil: Hasil analisis data uji chi-Squaretidak memenuhi syarat sehingga dilakukan uji non parametrikMann Whitney yang menunjukkan p=0,152 (p>0,05), yang berarti tidak ada hubungan jenis kelamin dan gambaran klinis celah bibir (dengan atau tanpa celah langit-langit) non sindromik.Kesimpulan: Tidak terdapat hubungan jenis kelamin dan gambaran klinis celah bibir (dengan atau tanpa celah langit-langit) non sindromik.
