Abstract

Background: Metabolic syndrome (MetS) is a cluster of chronic conditions, including central obesity, hypertension, impaired glucose metabolism, and dyslipidemia (low HDL, high LDL, and triglycerides). A diagnosis of MetS is made when three or more of these symptoms are present. If left unmanaged, MetS can lead to serious health complications such as cardiovascular disease and type 2 diabetes. Over time, individuals with MetS may experience a decline in their health-related quality of life (HRQoL), especially due to its chronic nature. Objective: This study aimed to evaluate the effects of changes in MetS status on HRQoL. Methods: This study employed a cross-sectional design. Secondary data from the cohort study of Non-Communicable Disease (NCD) risk factors, conducted by the Health Research and Development Agency of the Ministry of Health of the Republic of Indonesia in Bogor City, was used. Data from four follow-up periods (2011/2012 to 2017/2018) were analyzed. A total of 874 respondents were selected via total sampling based on inclusion and exclusion criteria. Data were collected in 2021, which included measures of knowledge, health check-ups, and HRQoL using the SF-36 questionnaire. Statistical analyses, including chi-square tests, t-tests, and multiple regression analyses, were conducted to examine the associations between MetS status and HRQoL. Results: Descriptive analysis revealed that 19% (171 participants) had worsened MetS status, while 80.4% (703 participants) showed improvement. Chi-Square analysis found that respondents with worsening MetS status were 1.6 times more likely to experience poor HRQoL in the physical dimension (95% CI = 1.1-2.3), but no significant effect was found for the mental dimension (PR = 1.1, 95% CI = 0.8-1.6). Multiple logistic regression analysis revealed that comorbidities interacted with worsening MetS status to significantly affect HRQoL in the physical dimension. The adjusted prevalence ratios (PR) were 27.5 (95% CI = 10.3-73.2) for those with comorbidities and 9.2 (95% CI = 5.7-15.0) for those without comorbidities, after controlling for age, mental health, BMI changes, routine health checks, and knowledge. Conclusion: Changes in MetS status towards worsening have a significant negative effect on HRQoL, particularly in the physical dimension. The presence of comorbidities in individuals with worsening MetS status greatly increases the risk of poor HRQoL. Healthcare professionals and nurses should consider the interaction between MetS and comorbidities in patient management. Nurses are encouraged to monitor HRQoL in patients with MetS, promote education on managing comorbidities, and collaborate across disciplines to enhance patient care and intervention programs aimed at improving HRQoL.

Abstract

The performance of hospital blood transfusion services is crucial for patient safety and overall operational effectiveness. Blood management, as a vital and limited resource, plays a significant role. Traditional centralized blood management models often face challenges regarding response speed, potential waste, and quality control. As an alternative, Unit Pengelola Darah Rumah Sakit (UPDRS) offers the potential for performance enhancement. This narrative literature review is based on 16 relevant scientific articles identified through a database search (Scopus, Proquest, Wiley, Google Scholar) and a selection process visualized with a PRISMA diagram. Key findings identified significant impacts on cost efficiency, time efficiency (faster Turnaround Time), blood utilization (waste reduction, improved C:T ratio), and service quality and safety. Overall, the role of UPDRS tends to be positive for performance enhancement, particularly in speed and utilization, but its impact is contextual and requires strong internal management and adequate investment. Implementation challenges such as initial costs and regulations need to be considered.

Abstract

Patient safety incident reporting is a critical component of hospital quality systems. This study evaluated the implementation of a Quality Management Information System (QMIS) for incident reporting at Hospital X using the Structure–Process–Outcome (SPO) framework integrated with the HOT-Fit model. A mixed-methods case study was conducted, combining analysis of 774 incident reports submitted between March 2023 and December 2024 with in-depth interviews involving clinical staff, unit heads, and the Quality and Risk (QR) Manager. Findings revealed that while structural elements such as computing infrastructure and leadership support were in place, gaps remained in staff training, procedural awareness, and system usability. QMIS was actively used for reporting, but advanced features like root cause analysis (RCA) and dashboards were underutilized. Reporting performance was moderate, with 62.8% of reports submitted within 24 hours and a reporting rate of 22.03 per 1,000 patient days, below international benchmarks. Although some process improvements were implemented, feedback loops to frontline staff were limited. To enhance system effectiveness, technical improvements are recommended, including interface simplification, mobile access, data validation, and integration with hospital systems. Strengthening local support and fostering a learning-oriented safety culture is also essential to sustain engagement and improve patient safety outcomes.

Abstract

To date, tuberculosis remains an infectious disease that can cause health problems in the community. In addition to increasing health risks, TB disease has an impact on the economy of a country and its sufferers. This is certainly hard for a developing country like Indonesia, which is still struggling in many ways besides TB disease. TB disease is an infectious disease caused by Mycobacterium Tuberculosis. TB case finding has the meaning of obtaining an overview through various activities including screening for suspected TB, physical and laboratory monitoring, diagnosing and categorizing the type of patient, so that treatment can be carried out as early as possible so that the patient can recover immediately and not spread to others. There are many factors that can affect the success of the tuberculosis program itself, one of which is the factor in management which includes input, process, output. Objective: This paper examines scientific evidence related to the implementation of the tuberculosis program in case detection. Methods: This article is based on a search of scientific journals conducted through online databases namely Scopus, Semantic, PubMed and Scholar published in 2020-2025. Results: In terms of TB case finding, there are many needs that must be carried out in an effort to get TB cases at health facilities. However, there are obstacles in its implementation, including lack of human resources, ineffective planning, and not optimal recording and reporting systems. Conclusion: efforts in TB case finding are highly dependent on the suitability and integration of available inputs, quality in the implementation process, and clear measurement of outputs. Therefore, efforts to improve program effectiveness require improvements in the management of inputs (such as resources and policies), optimization of processes (including more efficient detection and treatment), and continuous evaluation of outputs in TB case finding. Keywords: Tuberculosis, Case Detection, Indonesia

Abstract

Rumah sakit di negara berkembang seperti Indonesia menghadapi tekanan serius yang dapat mempengaruhi kualitas pelayanan kepada pasien seperti keterbatasan tarif, keterlambatan pembayaran klaim, serta ketergantungan pada program Jaminan Kesehatan Nasional (JKN). Penelitian ini bertujuan untuk mengeksplorasi secara sistematis hubungan antara kesehatan finansial rumah sakit dan mutu pelayanan pasien. Penelitian ini dilakukan dengan pendekatan Systematic Literature Review (SLR) menggunakan kerangka PICO. Artikel ilmiah diseleksi dari database Scopus, PubMed, dan ProQuest yang diterbitkan dalam rentang waktu 2015 hingga 2024. Setelah melalui tahap seleksi diperoleh sepuluh artikel yang dinilai relevan dan dianalisis secara mendalam untuk mengidentifikasi hubungan antara kondisi keuangan dan mutu pelayanan rumah sakit. Hasil kajian menunjukkan bahwa rumah sakit dengan kondisi keuangan yang sehat cenderung lebih mampu berinvestasi dalam pengembangan sumber daya manusia, teknologi informasi kesehatan, dan peningkatan infrastruktur layanan yang berdampak positif terhadap kepuasan dan pengalaman pasien. Studi ini menyimpulkan bahwa kesehatan finansial dan mutu layanan merupakan dua elemen yang saling memperkuat dalam membangun sistem pelayanan kesehatan yang berkelanjutan dan responsif terhadap kebutuhan masyarakat. Kebaruan studi ini adalah pendekatan yang secara eksplisit menelaah hubungan antara indikator finansial dan mutu pelayanan rumah sakit dalam konteks negara berkembang, khususnya Indonesia, yang masih relatif jarang dijadikan fokus utama dalam literatur akademik.

However, the analysis of narrative-based reports is often time-consuming and resource-intensive, thereby hindering the effectiveness of incident reporting and learning systems. Although artificial intelligence (AI) has been widely explored in healthcare, its application in patient safety incident reporting remains limited and under-researched. This study aims to evaluate the use of AI in patient safety incident reporting systems using a literature review method.   A total of 179 articles were identified from the ProQuest database through structured searching, and 9 articles were selected for in-depth analysis. The findings indicate that AI, particularly through machine learning and natural language processing (NLP), has been applied to classify incident types, detect risk patterns, and predict events from electronic medical records. Furthermore, AI simplifies the reporting process through free-text narratives, reducing administrative burden and increasing reporter participation. Key challenges in implementation include infrastructure readiness, system integration, and data protection. Abstract

Purpose: To identify and analyze the factors influencing complete basic immunization (CBI) coverage at two community health centers (CHCs) with contrasting coverage levels. Methods: This study employed a qualitative approach with a case study design. Informants were purposively selected and included health workers, community health volunteers, and parents. We collected data through in-depth interviews and document review, then analyzed it using a qualitative thematic approach. Results: Supply-side factors included the number and competence of health workers, availability of facilities and infrastructure, and funding support. Demand-side factors comprised community knowledge and awareness, sociodemographic characteristics, and trust in vaccines. Ciapus faced shortages of health personnel and community resistance to immunization, while Bojong Nangka demonstrated active collaboration between health workers and the community. Conclusion: The disparity in CBI coverage between Bojong Nangka and Ciapus CHC is caused by an imbalance between supply and demand factors. In Ciapus, low coverage was driven by limited human resources, inadequate infrastructure, and insufficient funding (supply-side), as well as poor public knowledge, awareness, and trust in vaccines, and the minimal involvement of local leaders (demand-side). In contrast, Bojong Nangka succeeded by strengthening its supply and demand components. Comprehensive interventions are necessary to enhance immunization services, increase public awareness and participation, and thereby achieve equitable CBI coverage.

Abstract

This study investigated the relationship between National Health Insurance (NHI) patient revenue and profit-private hospital financial performance in Indonesia. As the NHI provider, Badan Penyelenggara Jaminan Sosial (BPJS) Healthcare Security’s influence on hospital revenues has raised concerns about its impact on financial sustainability. This study used financial performance indicators to develop a financial performance index—Return on Assets (ROA), Return on Equity (ROE), Earnings Before Interest, Taxes, Depreciation, and Amortization (EBITDA) Margin, Current Ratio, and Net Profit Margin (NPM)—to assess the financial impact of BPJS Healthcare Security revenue. A partial model approach of multiple linear regression was employed using secondary data from seven private-profit hospitals listed on the Indonesian Stock Exchange from 2017 to 2022. The findings indicated a negative correlation between BPJS Healthcare Security patient revenue and the hospital financial performance index. Specifically, higher patient revenue correlated with lower performance across key financial indicators, including ROA, ROE, EBITDA Margin, Current Ratio, and NPM. It was essential for hospitals but not necessarily to improve their financial health performance. Hospitals need to optimize their revenue mix and explore alternative financial strategies to enhance performance.

Abstract

Life expectancy is widely recognized as a fundamental indicator of population health, and its improvement is often linked to government health spending, particularly in decentralized health systems. This study aimed to investigate the relationship between local government health expenditures and life expectancy in decentralized Indonesia. Data from 492 cities and districts, spanning from 2015 to 2019, were analyzed using a two-way fixed effects regression model to control for time-invariant district characteristics and common temporal shocks. The findings indicated that a 10% increase per capita in government health expenditure was associated with a 0.01% increase in life expectancy. This association was established after adjusting for key health system and contextual variables, including physician density, sanitation coverage, poverty rates, and proportion of fiscal transfers from the central government. The association remained strong and statistically significant across all model parameters. The results demonstrated that increased local health expenditure improves population health outcomes.

Abstract

Hospitals know smart users better through digital marketing than traditional marketing. The goal of hospital marketing is to assess and understand potential patients' needs and wants so they can meet those needs at the highest level. The Internet, email, and social media are cheaper than direct marketing and can create opportunities to market services to virtual customers, bridge the gap and increase their awareness of the services offered everywhere. This paper aims to determine the effect of digital marketing in hospitals. This scoping review followed a systematic review's study selection process. Relevant studies were collected from Open Knowledge Maps and Google Scholar. The results of this study indicate that some information related to the implementation of digital marketing has challenges. Therefore, there is a need for innovation in implementing digital marketing. In addition, the use of hospital applications can be used in online registration media and media marketing. During the pandemic, hospitals need innovative marketing strategies to attract new patients, expand the business, increase customer trust, and strengthen loyalty. Digital marketing media significantly impact sales promotion, business expansion, and service quality.
