Abstract

Sulam usus is a distinctive fashion of Lampung, made by sateen, which manufactured manually by hands. This process requiring meticulous detail, that lead to neck flexion and trunk bend posture, which can cause musculoskeletal disorder symptoms. One of the conservative therapy of musculoskeletal disorder is stretching exercise. This study aims to determine the effect of stretching exercise on musculoskeletal disorder symptoms among sulam usus worker in margodadi subdistrict jati agung district lampung selatan regency. This research used quasi experimental method with pre-posttest one group design, involving 28 respondents. Respondents will be given treatment of stretching exercise as much as 2 times daily for four weeks. The first and second score of musculoskeletal symptoms measured using a nordic body maps questionnaire. Data were analyzed by paired- samples t test with α = 0.05. In this study there was a significant difference of musculoskeletal disorder score mean different between the first score dan the second score (p=0,000). There is an effect of stretching exercise on musculoskeletal disorder symptoms among sulam usus worker in Margodadi Subdistrict Jati Agung District Lampung Selatan Regency.

Abstract

Measles is a highly contagious disease. Measles is an important cause of child death worldwide. Groups of children of pre-school age and elementary school age are vulnerable to contracting measles because they have weak immune systems. Measles can cause immune amnesia which will affect the immune system, making sufferers susceptible to other diseases or complications. This indicates that holistic and comprehensive patient care is an important matter through the role of the family doctor. The purpose of writing this article is to identify internal, external risk factors and clinical problems found in patients, apply a holistic and comprehensive family doctor approach according to the problems found in patients, and carry out evidence-based medicine management that is family-approach, patient-centered and community oriented. The study is a case report. Primary data were obtained through history taking, physical examination and home visits. The assessment is based on a holistic diagnosis from the beginning, process and end by means of in-depth interviews, observations and patient filling sheets. Patient An. H, 3 years old, has had a continuous fever since 6 days ago, accompanied by a cold cough. On the fourth day of fever, reddish spots appear, starting behind the ears and then spreading to the face, neck, chest, stomach and arms, accompanied by red and watery conjunctiva of the eyes. Neighbor's children experienced the same complaint. The patient has not received measles immunization.

Abstract

Type 1 diabetes mellitus (DMT1) is a lifelong chronic disorder of glucose homeostasis characterized by autoimmune destruction of insulin-producing pancreatic beta cells, which progressively leads to insulin deficiency and results in hyperglycemia. This disease causes chronic complications that require ongoing treatment management and education for patients and their families. Uncontrolled disease will cause various metabolic complications, macrovascular and microvascular disorders that cause a decrease in the quality and life expectancy of the patient. The purpose of writing this article is to applying the principles of a holistic and comprehensive family doctor approach in detecting internal and external risk factors and solving EBM (Evidence Based Medicine) problems that are family approach and patient centered. This study is a descriptive study of case reports. Primary data were obtained through autoanamnesis and alloanamnesis, physical examination and home visits. Secondary data obtained from the patient's medical record. Patient An.J, aged 17 years, complained of feeling weak and tingling in his legs. The patient has a history of type 1 diabetes on insulin treatment. Patient complaints do not recur and can return to normal activities. Clinically the patient was diagnosed with type 1 diabetes mellitus and diabetic neuropathy. Internal risks are family history of DM, lack of knowledge about type 1 diabetes, high calorie and sugar diet behavior, smoking habits, and alcohol consumption and rarely exercise. External risk with a lack of family knowledge about diabetes mellitus, its triggers and complications faced by the patient. Furthermore, holistic management is carried out, namely non-medical management and mentose medicine with intervention using poster media. In the evaluation, the results obtained in the form of a better understanding of the disease and changes in behavior that have an impact on improving the quality of life. Holistic management with a family medicine approach is needed for patients with type 1 diabetes mellitus with complications of DM neuropathy to identify problems in various aspects in order to support the success of therapy and change patient behavior so as to prevent aggravation of complications.

Abstract

Stroke is the third leading cause of death in the world after coronary heart disease and cancer, both in developed and developing countries. Hyperglycemia can exacerbate neurological deficits and even increase mortality in ischemic stroke patients, but this is still controversial. This study aims to determine the effect of hyperglycemia at the beginning of hospital admission to the incidence of mortality in ischemic stroke patients at the Abdul Moeloek Regional General Hospital in 2017-2020. This type of research is analytic observational with a retrospective cohort study design. Data were taken from medical records with a total of 62 samples selected using a consecutive sampling technique. The results of the univariate analysis test showed that 53% of subjects experienced death (33 people). The results of the bivariate analysis test obtained p value <0.05 (p = 0.000), relative risk of 3.7, and a threshold value of 222.5 mg/dL. There is an effect of hyperglycemia at the beginning of hospital admission to the incidence of mortality in ischemic stroke patients at Abdul Moeloek Regional General Hospital.

Abstract

Non-communicable diseases (NCDs) and other degenerative diseases are still a major national health problem in Indonesia. One of the NCDs that continues to increase in Indonesia is Diabetes Mellitus cases. Morbidity and mortality rates due to type 2 diabetes and tuberkulosis (TB) are increasing among people with type 2 diabetes. According to WHO data, people with type 2 diabetes have a three times higher chance of contracting tuberkulosis (TB) than people without the disease. The aim of the family approach in the treatment of pulmonary tuberkulosis in diabetes mellitus sufferers is to determine the clinical, psychological and psychosocial determinants that influence the family. By implementing comprehensive family doctor services that are based on evidence-based medicine and a patient-centered approach. A 53 year old patient, who is the head of a family with teenage children and a school member whose only income is from the wife of a trader at school, has complaints of a cough that has not healed since the last month accompanied by intermittent fever, itchy throat, decreased appetite, body weight, and tingling in the hands and feet with a history of suffering from diabetes mellitus for 15 years. The patient was found to be suffering from diabetes and a new case of pulmonary tuberkulosis. The patient has difficulty carrying out activities and needs the role of family such as the patient's wife and children to help heal the patient's condition. After the intervention, the patient's fasting blood sugar decreased, clinical symptoms improved, and the level of knowledge increased. The patient's diagnosis of a new case of pulmonary tuberkulosis with type 2 diabetes was made according to recommendations. After implementing the patient-centered approach, family approach intervention, the patient's level of knowledge changed. As a result of implementing a comprehensive management strategy led by a family doctor, patient and family understanding of type 2 diabetes mellitus and pulmonary TB has increased. So that from the results of this research and intervention, it is hoped that patients and families will receive comprehensive services both in terms of physical, psychological health and level of knowledge with changes in behavior in managing the patient's illness.

Abstract

Dermatophytosis is a superficial infection caused by dermatophyte fungi. This infection is also often called tinea. Classification is based on the part of the body affected, namely tinea capitis (head), tinea faciei (face), tinea barbae (beard), tinea corporis (body), tinea manuum (hands), tinea cruris (groin), tinea pedis (legs), tinea unguinum (nails). The prevalence of dermatophytosis in Asia is 35.6%, while in Indonesia it is 52%. Tinea capitis is the first and most common infection that occurs in pre-school and school-age children when compared to other types of tinea with risk factors originating from exposure to the school environment and hygiene applied at home. The aim of implementing family doctor services based on evidence-based medicine for patients is to identify risk factors, clinical problems, and patient management based on a framework for solving patient problems using a patient centered and family approach. The data obtained were primary data obtained through autoanamnesis, alloanamnesis, physical examination, and home visits and secondary data obtained from the patient's medical records. Patient An. B aged 6 years as the age with the highest prevalence has internal risks, namely lack of knowledge, poor personal hygiene, and external factors found in the patient's environment. Management of patients holistically and comprehensively, patient-centered, family approach, and community-based in literature based on EBM. Interventions are provided medically and non-medically. Based on the evaluation results, it was found that there was an increase in knowledge, changes in clean and healthy lifestyles and understanding of disease.

Abstract

Ada sekitar 64 juta remaja berusia antara 10 hingga 24 tahun, atau 28,64% dari total populasi Indonesia. Pendidikan kesehatan reproduksi bagi remaja merupakan upaya penting untuk menghindari pergaulan bebas, penyebaran penyakit menular seksual dan kehamilan yang tidak diinginkan. Tujuan dari pengabdian masyarakat ini adalah untuk memberikan penyuluhan tentang kesehatan reproduksi kepada remaja guna mewujudkan remaja yang sehat dan berprestasi. Metode pelaksanaan promosi kesehatan yang digunakan adalah Participatory Action Research (PAR) dengan pelatihan tentang masalah kesehatan reproduksi dan pencegahan penyakit menular seksual yang diikuti oleh siswa-siswi Madrasah Aliyah (MA) Dinniyah Putri, Kabupaten Pesawaran, Lampung. Hasil pengabdian ini menunjukkan terdapat perbedaan yang signifikan antara nilai rerata pretest (63,96) dan nilai posttest (92,2) tentang kesehatan reproduksi pada remaja di MA Dinniyah Putri, Pesawaran, Lampung. Kegiatan pengabdian ini terbukti dapat meningkatkan pengetahuan dan pemahaman remaja tentang kesehatan reproduksi sehingga dapat menjadi salah satu upaya dalam mencegah terjadinya penyakit.

Abstract

Political parties play an important role in aggregating people's interests, including in the formulation of the New Energy and Renewable Energy Bill (RUU EBET) or the EBET Bill, both at the central and regional levels. This study aims to look at the dynamics between the three faces of a political party, namely the party in the public office, central office, and at the grassroots level (Katz & Mair 1993), during the formulation of RUU EBET or the EBET Bill. Using the Prosperous Justice Party (PKS) as a case study, this research finds that parties in the public and central office actually depoliticize the roles of parties at the grassroots level. This depoliticization manifests itself in three ways: (1) assumptions about the inability of party structures at the regional level to discuss the EBET issues, (2) not including the interests of developing EBET at the regional level as part of the EBET Bill, and (3) narrow definitions and restrictions of the EBET Bill as a central and elite issue.

Abstract

Dementia is the 4th cause of death after heart disease, cancer and stroke. To prevent dementia, 6 pillars of dementia prevention are needed. This service aims to screen for dementia and health education about preventing dementia. The benefit of this activity is to improve the quality of life for the elderly. Participatory Impact Monitoring (PIM) activity methods include involvement, active participation and community empowerment. The instrument uses the Mini Mental Status Examination (MMSE) questionnaire, sphygmomanometer, scales and microtoise. Indicators of achievement are dementia screening, health services and increasing knowledge for the elderly. The partners' targets are 34 elderly people at the Tresna Nursing Home Bakti Yuswa Natar. South Lampung. The results of the activity showed that blood pressure was hypertension stage 1 (35.29%), hypertension stage 2 (8.82%), hypertensive crisis (2.94%). Examination of nutritional status: overweight (26.47%), obesity I (8.82%), obesity II (11.76%). Dementia screening showed probable dementia (44.12%), definitive dementia (23.53%). The results of health education showed that there was a difference in the mean knowledge about dementia before and after being given health education (p=0.02). and the solutions offered are health screening, health education and the SMART Elderly program. The results disseminated are the SMART Elderly Program and health education. It requires collaboration and joint commitment from all parties to prevent dementia. The continuation of this activity is in the form of the SMART (Healthy, Independent and Active) Elderly program in collaboration with the Natar Community Health Center, South Lampung.

Abstract

Working in a hot environment can disrupt the balance of body temperature so that the body tries hardly to control the heat which can cause a lack of fluids and is called dehydration. Dehydration can affect cognitive performance, causing decreased productivity and increased risk of work accidents, and in the long term can trigger some non-communicable diseases such as hypertension, urinary tract disease, obesity and others. Pak Rama Education (Active Workers Diligently Drink Water) is an educational and empowerment activity for the worker community, especially those who work in hot environments, in an effort to prevent dehydration and non-communicable diseases. The aim of this activity was to increase workers' knowledge about healthy hydration and increase workers' ability to assess hydration status independently. This activity was carried out using the education method about healthy hydration and a demonstration of how to assess hydration status using a urine color chart. Pak Rama's activity was held on 17th November 2022 at PTPN VII Unit Way Berulu South Lampung. The participants were 50 workers consisting of management officers, factory and plantation workers. From the evaluation results, it was found that the average pre-test score of participants was 66.7 and there was an increase of participants' knowledge about healthy hydration which was seen from the increase in the average score on the post-test, namely 86.6. Based on analysis using the Wilcoxon test, it was found that there was a statistically significant difference between average pretest and posttest scores. The results of the assessment of participants' ability to assess hydration status using a urine color chart were also found increase. Pak Rama's education has proven effective in increasing workers' knowledge about healthy hydration and their ability to assess hydration status independently.
