Abstract

The implementation of Natural Language Processing (NLP) is crucial for enhancing the quality of medical records. This study aimed to develop an NLP model to improve the accuracy of documenting disease anamnesis for typhoid fever. The problem addressed by this research is the difficulty in analyzing and classifying patient complaints recorded in electronic medical records, which can affect the accuracy of diagnosis and treatment. The urgency of this study lies in ensuring that documented medical information is used accurately to support diagnosis and patient management. A quantitative approach was used, focusing on electronic medical records of patients who underwent anti-salmonella IgM tests in 2023, involving 424 individuals. The study assessed the performance of three models: Support Vector Machines (SVM), Naive Bayes Bernoulli, and Logistic Regression. The SVM model achieved the highest accuracy at 81.4%, compared to 76.7% for Naive Bayes Bernoulli and 79.1% for Logistic Regression. Additionally, four topic models were identified, highlighting common complaint words and their impacts. The most frequently occurring symptoms in the anamnesis of typhoid fever were "defecation," "nausea," "vomiting," "fever," "diarrhea," "heartburn," "weakness," "loss of appetite," "abdominal pain," "cough," and "cold." This study demonstrates that the SVM model provides superior accuracy in analyzing medical records compared to other models.

Abstract

WHO declared COVID-19 as a pandemic on March 11, 2020. Obstacles found in ANC services during the COVID-19 pandemic were pregnant women afraid of contracting COVID-19 if they had to carry out ANC examinations at health services. ANC examinations play an important role in preventing and detecting pregnancy complications. Predisposing factors are one of the factors that influence the behavior of pregnant women in making ANC visits. Some of which are included in the predisposing factors are age and parity. This study aims to determine the effect of age and maternal parity on the number of ANC visits during the COVID-19 pandemic at RSIA AMC Metro. The sample consisted of 100 pregnant women who visited ANC between January and October 2022, and data analysis with multiple logistic regression. The majority of pregnant women were aged 20-35 years (71%), had given birth 2-4 times (36%), and made 1-5 ANC visits (82%). However, the study found that neither age nor maternal parity significantly affected the number of ANC visits during the pandemic at RSIA AMC Metro.

Abstract

Jakarta Pusat mencatat kasus diare tertinggi di Kecamatan Kemayoran, dengan total 7.010 kasus pada tahun 2022. Diare merupakan kondisi yang ditandai dengan frekuensi buang air besar sebanyak tiga kali atau lebih dalam 24 jam, dengan feses yang lebih encer dari biasanya dan disebabkan berbagai faktor. Penelitian ini bertujuan untuk menganalisis hubungan spasial antara kasus diare dan beberapa faktor risikonya. Penelitian dilakukan pada Desember 2023 hingga Januari 2024, dengan sampel mencakup jumlah kasus diare di semua kelompok usia, jumlah sarana air minum, serta jumlah jamban sehat permanen pada tahun 2022 di wilayah Jakarta Pusat yang meliputi Kecamatan Sawah Besar, Gambir, Senen, Johar Baru, Kemayoran, Menteng, Cempaka Putih, Tanah Abang. Analisis data menggunakan metode Bivariate Moran’s I melalui aplikasi GeoDa dengan input data peta dasar kota madya Jakarta Pusat. Hasil penelitian ini menunjukkan adanya korelasi spasial antara kasus diare dan faktor risikonya di berbagai wilayah di Jakarta Pusat. Uji signifikansi mengungkapkan bahwa terdapat satu wilayah, yaitu Kecamatan Tanah Abang, memiliki nilai signifikansi (p<0,05) yang menunjukkan adanya autokorelasi dengan wilayah sekitarnya. Pola penyebaran kasus diare di daerah ini bersifat acak dan termasuk dalam kategori High-Low. Penelitian ini dapat menjadi acuan dalam perencanaan strategi kesehatan masyarakat untuk mengurangi kasus diare di wilayah Jakarta Pusat.

Abstract

Abstract: Electronic Medical Records (EMR) is a computerized recording of patient illnesses and problems. This is stated in PMK No. 24 of 2022 which states that Medical Records need to be organized electronically. However, in its implementation, various problems were found  such as format incompatibility, network system disruptions, unavailability of features that meet user needs, and displays that are not easy to understand. As well as the unavailability of SOPs that serve as guidelines for users, and lack of training in the Use of EMR. This study aims to see the effect of Technology and Training Aspects on the Use of EMR at Patria IKKT General Hospital. The type of research used is quantitative research and uses a cross-sectional design. The population in this study were Professional Care Providers (PPA) and Medical Supporters in outpatient care and the sample in this study was 53 respondents with a total sampling technique. Data collection used a questionnaire with a Likert scale. The data analysis technique used multiple linear analysis tests. The results of the study explain that there is a significant influence on the System Quality variable on the Use of RME with a p-value of 0.002 <0.05, the Information Quality variable has a significant effect on the Use of RME with a p-value of 0.000 <0.05 and the Training variable has a significant effect on the Use of RME with a p-value of 0.018 <0.05. While the Service Quality variable does not have a significant effect on the Use of RME with a p-value of 0.132> 0.05. Keyword: Electronic Medical Records, HOT-Fit, System Quality, Information Quality, Service Quality, Training, System Users Abstrak: Rekam Medis Elektronik (RME) adalah pencatatan penyakit dan permasalahan pasien yang terkomputerisasi. Hal tersebut tertuang dalam PMK No 24 Tahun 2022 yang menyatakan bahwa Rekam Medis perlu diselenggarakan secara elektronik. Namun dalam pelaksanaannya masih kurang optimal, terdapat berbagai permasalahan yang ditemukan seperti ketidaksesuaian format, gangguan sistem jaringan, ketidaksediaan fitur yang memadai kebutuhan pengguna, dan tampilan tidak mudah dipahami. Serta belum tersedianya SOP yang menjadi panduan bagi pengguna, dan kurangnya pelatihan Penggunaan RME. Penelitian ini bertujuan melihat pengaruh dari Aspek Teknologi dan Pelatihan Terhadap Penggunaan RME di RS Umum Patria IKKT. Jenis Penelitian yang digunakan ialah penelitian kuantitatif dan menggunakan desain cross sectional. Populasi dalam penelitian ini adalah Profesional Pemberi Asuhan (PPA) dan Penunjang Medis di rawat jalan dan sampel dalam penelitian ini sebanyak 53 responden dengan teknik total sampling. Pengumpulan data menggunakan kuesioner dengan skala likert. Teknik analisis data menggunakan uji analisis linier berganda. Hasil penelitian menjelaskan bahwa terdapat pengaruh signifikan pada variabel Kualitas Sistem terhadap Penggunaan RME dengan nilai p–value 0.002 < 0.05, Variabel Kualitas Informasi berpengaruh signifikan terhadap Penggunaan RME dengan nilai p–value 0.000 < 0.05 dan variabel Pelatihan berpengaruh signifikan terhadap Penggunaan RME dengan nilai p –value 0.018 < 0.05. Sedangkan pada variabel Kualitas Layanan tidak berpengaruh signifikan terhadap Penggunaan RME dengan nilai p–value  0.132 > 0.05.Kata kunci: Rekam Medis Elektronik, HOT-Fit, Kualitas Sistem, Kualitas Informasi, Kualitas Layanan, Pelatihan, Pengguna Sistem

Abstract

Cervical cancer is a malignancy with high mortality rates in women, and its incidence continues to rise. The main etiological factor for cervical cancer is infection with Human Papillomavirus (HPV), which disrupts the regulation of apoptosis in cells. Several studies have shown a correlation between TNFα polymorphisms, including the -308 position (TNFα -308 G/A), and the incidence of cervical cancer.This gene have a role in proliferation of cancer cells. This study investigates the impact of TNFα-308 polymorphism on the risk of cervical cancer in Asian female populations. A meta-analysis of five sources was conducted to determine potential associations. Findings reveal that neither allele A (OR 95%CI = 1.20 [0.70-2.03], p = 0.51) nor genotype AA (OR 95%CI = 0.85 [0.37-1.91], p = 0.69) were significantly linked with an elevated risk of cervical cancer in Asian women. The same result was seen for the G allele (OR 95%CI = 0.84 [0.49-1.42], p = 0.51) and GG genotype (OR 95%CI = 0.80 [0.44-1.48], p = 0.48). The study results indicate that the TNFα-308 polymorphism is not associated with cervical cancer in Asian women. Further research is needed to investigate the role of other gene polymorphisms in cervical cancer susceptibility in Asian women.

Abstract

This study aimed to examine the historical and cultural values of dangke as a traditional food, its potential as a source of animal protein, and the methods of its preparation and presentation. This qualitative descriptive study used in-depth interviews with key informants, focus group discussions with primary and supporting informants, and observation to collect data. The findings of this study indicate that dangke, which was originally made from buffalo milk and enriched with enzymes from papaya sap, reflects the culinary heritage of the Massenrempulu tribe in Enrekang Regency, South Sulawesi. However, the limited supply of buffalo milk has led to the modification of dangke using cow's milk in some parts of Enrekang. Historically, dangke has been served with rice or "pulu mandoti" to guests since the colonial era. The origin of the term "dangke" itself has roots in the Dutch language, possibly from the expression "dank je wel" (thank you) used by Dutch soldiers after tasting this delicious dish. In addition, previous studies have shown that dangke has a high protein content, making it a valuable food source to support the diversification of animal protein sources. In conclusion, dangke has a high cultural value for the people of Enrekang and is a traditional food with high nutritional value, especially in terms of protein content.

Abstract

Childhood undernutrition include stunting remains a global public health problem that affects many middle-income countries. Nutrition education generally seeks to increase nutritional knowledge, thereby influencing attitude and practices towards good nutrition. This study aimed to analyze knowledge of stunting prevention before and after stunting prevention promotion in Purwakarta Village, Banten Indonesia. This study was quasi-experimental design with one group pre-posttest in 2023. The population in this study were mothers who had children under five years old in Purwakarta Village, Banten Indonesia. A total of 78 mothers who participated in stunting prevention promotion were selected as a respondent. Data was collected by questionnaire and analyzed using paired-t test. The mean score of stunting prevention knowledge was higher after intervention (Mean= 10,45; SD=0,83) than before intervention (Mean= 6,12; SD= 1,84) and it was statistically significant (p < 0,05). Appropriate nutrition promotion plays a role to inform and remind of stunting prevention behavior.

Abstract

Kelengkapan pengisian rekam medis yang rendah akan mempengaruhi informasi perawatan pasien, pembayaran pelayanan kesehatan yang diberikan dan tidak bisa dijadikan bukti untuk perlindungan hukum apabila dibutuhkan. Rendahnya persentase rekam medis yang lengkap merupakan indikator rendahnya kinerja dokter dalam pengisian rekam medis di rumah sakit. Hal ini diduga sebagai akibat rendahnya motivasi dokter dalam melaksanakan pekerjaannya, baik motivasi instrinsik maupun motivasi ekstrinsik. Penelitian ini bertujuan untuk mengetahui hubungan motivasi kerja dengan kelengkapan rekam medis rawat inap di Rumah Sakit X. Penelitian ini merupakan penelitian kuantitatif dengan desain penelitian cross-sectional. Pengumpulan data dilakukan dengan menggunakan kuesioner dan lembar checklist. Teknik analisis data menggunakan analisis univariat dan bivariat dengan menggunakan uji korelasi Spearman. Responden dalam penelitian in adalah dokter spesialis sebanyak 35 orang dengan 140 rekam medis yang di teliti. Masing-masing dokter mengisi 4 rekam medis. Berdasarkan hasil uji korelasi pada variable motivasi kerja dan variabel kelengkapan rekam medis didapatkan nilai p-value 0,539>0,05. Berdasarkan hasil penelitian, variabel motivasi kerja tidak memiliki hubungan yang signifikan dengan kelengkapan rekam medis rawat inap. Namun, pihak rumah sakit perlu mengevaluasi  kelengkapan  pengisian rekam medis sehingga dapat sesuai dengan standar pelayan minimal

Abstract

Improving patient quality and safety (PMKP) is a key component in ensuring hospital service quality. One measurement tool used is the quality indicator, which includes the National Quality Indicator (INM), Priority Unit Quality Indicator (IMPU), and Hospital Priority Quality Indicator (IMPRS). The Registration Unit at the Medical Records Installation of Bethesda Serukam General Hospital plays a strategic role in supporting PMKP implementation. However, the unit still requires service improvements to promote systematic quality enhancement. This study aims to design a suitable priority quality indicator for the Registration Unit. A descriptive qualitative method was used, with data collected through interviews, observation, scoring, and Focus Group Discussions (FGD). The study was conducted from November to December 2024 at Bethesda Serukam General Hospital, located at Jalan Raya Singkawang–Bengkayang KM.49, Pasti Jaya Samalantan, Desa Suka Maju, Kecamatan Sungai Betung, Kabupaten Bengkayang, Kalimantan Barat. The results identified the completeness of patient identity data in the Electronic Medical Record (EMR) as the priority quality indicator. This was selected because it met the most criteria, although the current completeness rate is still low (50%). Establishing this indicator is expected to improve service quality, patient safety, and foster a culture of quality at Bethesda Serukam General Hospital.

Abstract

Hospitals face efficiency and quality challenges within the Case-Based Groups (CBG's) financing system, where a patient’s Length of Stay (LOS) is critical. Accurate LOS data is crucial for strategic decisions, cost management, and quality care. A study at a Type B Hospital in Bekasi City found significant variation and outliers in LOS, indicating a non-normal distribution. This observational analytic study, involving 3,151 inpatient claims from January 2024, analyzed LOS data and its impact on clinical documentation and coding quality. The analysis compared the Arithmetic Mean Length of Stay (AMLOS) and the Geometric Mean Length of Stay (GMLOS) to identify outliers, followed by a Wilcoxon test. Results showed LOS varied from 1 to 48 days, with an AMLOS of 7.13 and a GMLOS of 6.76 days, indicating positive skewness from outliers. AMLOS was consistently higher than GMLOS in the top 10 CBG's, especially for moderate and severe cases. The Wilcoxon test (p<0.05) confirmed a significant statistical difference, showing GMLOS more accurately represents the appropriate LOS. The presence of outliers (e.g., >30 or 44 days) suggests potential issues with documentation or coding. Therefore, using the more robust GMLOS is crucial for hospitals to optimize management, improve care, and maintain the quality of clinical documentation and coding.
