Abstract

Hyperglycemia conditions will result complications in patients both for cardiovascular and neurological system. The incidence of neuropathy and peripheral arterial disease (PAD) are two things that increase mortality. Loss of protective sensation will reduce the patient's awareness of ulcers so that patients do not receive treatment early. Meanwhile, PAD conditions increase the incidence of ischemia in the distal area, especially in the legs, resulting ulcer. This research aims to look at the factors that contribute to these two conditions. Data collected to 40 respondents with the inclusion criteria who are DM patients which never had an amputation, the relationship and contribution of factors to neuropathy and PAD (were measured by sensation points on sole by monofilament and dorsalis pedis artery palpation) and analyzed with ordinal regression. The results of the study showed that none of the respondents experienced PAD so that bivariate tests could not be carried out. Meanwhile, the results of the bivariate test for neuropathy showed that 3 factors were significantly related to gender (p=0.038), duration of illness from DM (p=0.05), and foot condition (p=0.003). It is found from ordinal regression that foot condition (β= 1.237; p= 0.028; OR= 1.237) has significant effect to neuropathy.

Abstract

Palliative and near-death conditions in critical ill patients may occur, either predictable prognosis or acute conditions. Nurses are often unprepared for these care needs, resulting in suboptimal patient care. This study aims to review empirical evidence over the past 10 years regarding when to start, how to apply, and constraints during applying palliative care and end of life (EoL) care in intensive care. This review was conducted using journal database CINAHL, PubMed, Scopus, and WOS and selected article by PRISMA. The findings show EoL care begins when the medical team has assessed and approved by the patient and family including using the referral checklist format from intensive care to EoL care including the resources needed. In addition, during the upbringing process counselling and consultation with the family for any decision-making medical measures, nursing rounds, emotional and spiritual support and touches and hugs for the patient and family, including support for the entire medical team. However, there are also obstacles in the right communication time to families, discussions about death, and improving the competence of nurses through EoL education and training.

Abstract

Hypertension as a chronic disease with a major sign of increased blood pressure is becoming a problem globally. Increased blood pressure can be the cause of various other chronic diseases such as coronary artery disease and stroke. As an effort to prevent these complications, hypertensive patients need good and sustainable self-care. Self-efficacy is one of the factors that contribute to good self-care. This study aims to determine the effectiveness of self-efficacy on self-management in hypertensive patients. This study was conducted in Palembang with 35 respondents with hypertension aged >17 years. Data was taken through self-efficacy and self-management questionnaires that have been declared valid and reliable. Most 35 respondents have elementary and high-school education backgrounds, high self-efficacy, and moderate self-management. Self-efficacy strongly predicts self-management, although age and educational background do not significantly influence it.

Abstract

Diabetes mellitus (DM) is a chronic disease that can lead hyperglicemia condition. Hyperglicemia can cause macrovasculer, microvascular, and neurophaty diseases which lead to foot disease. To prevent it, patients need knowledge and practice of foot care. This study aimed to find factors that contribute to patient’s knowledge and foot condition. The data were collected by a questionnaire to assess patient’s knowledge and an observation checklist to assess patient’s foot condition. The bivariate analysis used Kendall Tau test and ordinal regression for multivariate. This study had 30 respondents with characteristic of pre-elderly age (56.7%), graduating from high school (76.7%), living with DM <1 year (53.3%), hyperglicemia (53.3%), low in knowledge (73.3%), and having a bad foot condition (73.3%). The results showed that respondents’ characteristics have no significant correlation to knowledge and foot condition. The analysis results of Kendall Tau test showed that hyperglicemia (τ=0.409; p=0.012) and patients’ knowledge (τ=0.480; p=0.015) significantly related to foot condition. Hyperglicemia and patients’ knowledge became predictor of foot condition with determinant coefision of 39.5%. Thus, if patients have a good knowledge and do not have hyperglicemia, they will achive a good foot condition. Therefore, education about foot care and how to stabilize blood sugar to the patients is necessary for further study.

Abstract

Diabetic ulcer wound care has changed from conventional to modern wound care based on the principle of moist balance. Modern dressing can heal diabetic foot ulcers faster and inexpensive. This research conduct effectiveness of modern dressing in diabetic ulcer. This study used dressing that contain hydrogel, chitosan and zinc oxide. Each diabetic patient given three times wound care, once every day for three day and evaluate with Bates-Jansen Wound Assessment before and after intervention. The results is each patient reduces scores in the BWAT assessment from 46 to 37 in first patient, 52 to 48 in second patient and 37 to 30 in third patient. Based on results using modern dressing methods proven reduce ulcer size in each patient, ease pain, less exudate, and reduce inflammation signs. This method can be used for further research by combine more than one type of primary dressing, manage blood glucose by diet and medication program, and involve family in wound care.

Abstract

The increasingly advanced world will improve human lifestyles, especially their physical, one part of body measurement is body mass index which can be used to measure a person's physical abilities, this is related to human hemodynamics, namely blood pressure, so there needs to be a clear problem solving so that the risk of disease due to blood pressure that we can know earlier by measuring body mass index, The purpose of the study is to determine the relationship between body mass index and the physiological mechanism of blood pressure in vegetarians and non-vegetarians. This type of research is a quantitative type with a survey design with a cross-sectional approach. Samples aged 18-49 years amounted to 25 vegetarians and 25 non-vegetarians, data collection of body mass index with anthropometric tools (scales and meters) while to measure blood pressure with an automatic sphygmomanometer. The results of the study showed that there was a strong positive significant relationship between body mass index and blood pressure with a p-value of 0.000, Thus, along with increasing BMI also increases blood pressure in vegetarians and non-vegetarians.

Abstract

Hypertension (HT) still become silent killer disease leading to mortality. Although many efforts and programs of HT increase blood pressure control, but internal factors such as age, gender, duration of illness (DOI), attitude, medication adherence (MA), routine check-up (RCU), and blood pressure (BP) still play important role to enhancing blood pressure control and even more so the quality of life (QoL) hypertensive patients. This research aims to testing a hypothetic modelling of previous research to find an internal factors model of QoL in hypertensive patients. Cross-sectional study used to 89 samples randomize due to online questionnaire collecting data. Correlation used to eliminate variables which did not significant adding to the model. Then model testing by partial-least square analysis. The result shows that attitude directly affect the MA and RCU significantly, also MA to BP and BP to QoL, but interestingly did not significant in RCU to BP. As addition, it has significant effect as attitude to BP through MA, MA to QoL through BP, and so simultaneously attitude to QoL with MA and BP as intervening. So, it becomes very important as nurses to empower patient’s attitude to manage and maintain their lifestyle with their hypertensive condition to enhance their adherence in medication so they blood pressure in controlled and will have a good quality of life.

Abstract

Kasus COVID-19, semakin hari terus bertambah, berdasarkan data dari Gugus Tugas Percepatan Penanganan COVID-19 Republik Indonesia, jumlah kasus terkonfirmasi positif hingga 30 November 2020 adalah 534.266 orang dengan jumlah kematian 16.815 orang. Melihat data tersebut masih sangat mengkhawatirkan artinya masyarakat belum memahami tentang pencegahan penyebaran COVID-19. Resiko penyebaran ini dikarenakan tidak mematuhi protokol kesehatan seperti; menggunakan masker, mencuci tangan, dan menjaga jarak, maka diperlukan pendampingan untuk meningkatkan derajat kesehatan dan Aman dari Covid 19. Kegiatan pendampingan dalam bentuk penyuluhan kesehatan, untuk meningkatkan pemahaman masyarakat dalam mencegah dan meningkatkan derajat kesehatan dan aman dari COVID-19. Diharapkan kegiatan pendampingan ini memberikan dampak positif, bagi siswa/siswi untuk mematuhi protokol kesehatan.

Abstract

Abstrak: Kusta merupakan penyakit menular kronis yang masih menjadi masalah kesehatan masyarakat di Indonesia. Stigma negatif terhadap penderita kusta menyebabkan mereka mengalami diskriminasi dan kesulitan reintegrasi sosial. Program pengabdian masyarakat ini bertujuan memberdayakan penderita kusta melalui penguatan UMKM dan peningkatan akses pelayanan kesehatan di Panti Rehabilitasi Kusta. Metode kegiatan meliputi penyediaan alat pelindung diri untuk pertukangan, pengadaan obat dan alat medis dasar, serta pembentukan sistem pelayanan kesehatan dasar. Evaluasi dilakukan melalui wawancara kepada sebagai pengrajin mebelair yang dirawat luka secara rutin. Hasil kegiatan menunjukkan peningkatan keselamatan kerja pengrajin mebelair, terfasilitasinya ruang pengobatan yang lengkap, dan terbangunnya sistem pelayanan kesehatan rutin. Program ini berhasil meningkatkan kualitas hidup 44 penghuni panti yang terdiri dari 19 kepala keluarga. Keberlanjutan kegiatan PKM dilakukan secara rutin oleh mitra kerjasama lintas institusi. Hal ini terbukti efektif dalam mengatasi masalah kompleks penderita kusta dan diharapkan dapat menjadi model pemberdayaan penderita kusta yang berkelanjutan.Abstract: Leprosy is a chronic infectious disease that remains a public health problem in Indonesia. The negative stigma against leprosy sufferers causes them to experience discrimination and difficulty in social reintegration. This community service program aims to empower leprosy sufferers by strengthening UMKM and improving access to health services at Leprosy Rehabilitation Center. The activity methods include providing personal protective equipment for carpentry, procuring basic medicines and medical equipment, and establishing a basic health care system. Evaluation was conducted through interviews with furniture craftsmen who regularly treat wounds. The results of the activity showed an increase in work safety for furniture craftsmen, the provision of a complete treatment room, and the establishment of a routine health care system. This program successfully improved the quality of life of 44 residents of the center, consisting of 19 heads of families. The continuation of PKM activities is carried out routinely by institutional collaboration partner. This has proven effective in addressing the complex problems of leprosy sufferers and is expected to become a model for sustainable empowerment of leprosy sufferers.
