Abstract

Background: Controlling hypertension through pharmacological treatment is effective but can potentially lead to long-term side effects. Thus, more patients with hypertension have turned to herbal plants for treatment. The objective of this study was to examine the effectiveness of soursop leaf tea in reducing blood pressure in patients with hypertension. Methods: This study employed a quasi-experimental approach with a non-randomized pretest-posttest control group design, involving 30 participants in the intervention group and 30 participants in the control group. Data was analyzed using the Wilcoxon test to assess the disparity in mean blood pressure levels before and after consumption of soursop leaf tea. The Mann-Whitney test was used to examine the distinction in mean blood pressure between the intervention group and the control group. Results: In the intervention and control groups, most respondents were aged 56–65 years (60% and 66.7%), male (70% and 56.7%) and had suffered from hypertension for more than 5 years (70% and 56.7%). There was a significant difference in systolic blood pressure (p<0.001) and diastolic blood pressure (p<0.001) before and after the administration of soursop leaf tea. The mean systolic and diastolic blood pressure in the intervention group decreased significantly compared to the control group (p<0.001).Conclusions: Consumption of soursop leaf tea can reduce systolic and diastolic blood pressure, suggesting that this local herb has a potential effect on individuals diagnosed with hypertension.

Abstract

Diabetes Mellitus (DM) is a chronic metabolic disease characterized by hyperglycemia. This study aimed to analyze the effect of physical exercise on blood glucose levels in patients with Type 2 DM at the Medan Johor Public Health Center. This analytical observational study utilized a case-control design and involved 34 respondents, divided into a case group (with uncontrolled blood glucose levels) and a control group (with controlled blood glucose levels). Data on physical activity, including frequency, duration, and intensity of exercise, were obtained through interviews using the GPAQ questionnaire, while blood glucose level data were sourced from medical records. The results showed a significant relationship between physical activity (p=0.028), frequency of physical exercise (p=0.006), duration of physical exercise (p=0.015), and intensity of physical exercise (p=0.034) with blood glucose levels. Physical exercise performed at least three times a week, for a minimum duration of 30 minutes per session, and at an appropriate intensity proved effective in controlling blood glucose levels in patients with Type 2 DM. This study emphasizes the importance of incorporating physical exercise into the management plan for Type 2 DM.

Abstract

This study aimed to evaluate the impact of healthcare service quality on patient satisfaction among hypertensive outpatients at Muyang Kute Regional General Hospital. Employing a quantitative, cross-sectional design, data were collected from 79 patients using a questionnaire assessing five service quality dimensions (tangibles, reliability, responsiveness, empathy, and assurance) and overall patient satisfaction. Bivariate analysis, utilizing the chi-square test, was conducted to examine relationships between service quality dimensions and patient satisfaction. Results indicated generally positive perceptions of service quality dimensions, with tangibles, reliability, and assurance rated highly. However, empathy and responsiveness showed higher dissatisfaction rates. Notably, only assurance demonstrated a statistically significant association with patient satisfaction (p=0.002), where patients perceiving "Good" assurance were more likely to report satisfaction. Overall patient satisfaction was mixed, with 54.43% reporting satisfaction and 45.57% reporting dissatisfaction. The study highlights the critical role of staff competence and patient confidence (assurance) in driving satisfaction. Recommendations include targeted interventions to improve empathy and responsiveness, continuous monitoring of all service quality dimensions, and further investigation into potential underlying factors affecting overall satisfaction.

Abstract

Humanity and other living organisms coexist within the environment. Rivers, as essential components of the environment, provide critical benefits for human survival, including supplying water for daily use, supporting fisheries, and facilitating water transportation. However, human activities frequently exert negative impacts on river ecosystems, such as the disposal of solid and industrial waste. This study aimed to identify the factors influencing community waste disposal practices in the Deli River, Petisah Village, in 2023. Employing a quantitative, descriptive-analytical cross-sectional survey design, the study population comprised 250 residents residing along the Deli River, from which a sample of 154 participants was selected through simple random sampling. Bivariate analyses were conducted using the Chi-square test. The findings demonstrate significant associations between waste disposal behavior and knowledge (p=0.004), attitude (p=0.004), practices (p<0.001), and infrastructure (p=0.008). Furthermore, logistic regression analysis identified infrastructure as the strongest predictor of waste disposal behavior, indicating that inadequate infrastructure increases the likelihood of respondents disposing of waste into the river.

Abstract

Background: Diabetes mellitus affects over 415 million people globally and is projected to reach 642 million by 2040, with diabetic retinopathy (DR) becoming a major cause of preventable blindness. Specific background: In Indonesia, DR prevalence remains high, yet early detection is often delayed, particularly in community and private clinic settings. Knowledge gap: Most studies on DR risk factors have been conducted in tertiary hospitals or general populations, while evidence from community-based ophthalmology clinics is limited. Aim: This study analyzed factors associated with DR complications among type 2 diabetes mellitus (T2DM) patients at Klinik Mata Vision Max. Results: Using a case–control design with 60 participants (30 cases, 30 controls), multivariate logistic regression revealed that uncontrolled blood glucose (OR = 7.5, p = 0.007), diabetes duration >5 years (OR = 6.4, p = 0.016), hypertension (OR = 4.5, p = 0.043), and male sex (OR = 3.97, p = 0.054) significantly increased the risk of DR, while diet showed no direct effect. Novelty: This is the first systematic case–control study in a community-based eye clinic in Indonesia addressing DR risk factors. Implications: Findings highlight the urgency of integrating DR screening into primary care, emphasizing glycemic control, hypertension management, and targeted patient education to reduce preventable blindness.Highlight : Uncontrolled blood glucose is the dominant factor. Age >45 years and DM duration >5 years increase risk. Hypertension is significantly associated with diabetic retinopathy.
