Abstract

Dengue Hemorrhagic Fever (DHF) is an infectious disease transmitted through the bite of the Aedes aegypti mosquito and is still a public health problem. Dengue cases in the work area of the Peusangan Health Center continue to increase every year, so it becomes a threat to family health. This study aims to find out the factors that affect dengue prevention efforts in the working area of the Peusangan Health Center, Peusangan District, Bireuen Regency. The study uses a quantitative design with an analytical survey through a cross sectional study approach. The population is the entire community in nine villages with a total of 19,271 families. The sample was determined by stratified random sampling of 99 families. Data analysis was carried out in univariate, bivariate, and multivariate manners. The results of the study show that education, knowledge, attitudes, the role of health workers, water reservoirs, and the existence of waste have an effect on dengue prevention efforts. The most dominant factor was education with a value of p = 0.000 < 0.025; CI 95% = 5,389–316,539; and OR = 41.154. It was concluded that the educational variable has an important role in the prevention of dengue. The Peusangan Health Center is advised to conduct regular larval checks every three months, carry out fogging, and improve health promotion through the application of the 3M Plus principle to reduce the incidence of dengue incidence.

Abstract

The coverage of Hepatitis B immunization in toddlers in the Gambir Baru Health Center Working Area in 2021 only reached 40.56%, far below the national target. This study aims to analyze the factors that affect the low immunization coverage. The method used is a mix method with an explanatory research design, to be carried out in July-August 2025. The research sample amounted to 66 mothers who had toddlers, obtained through purposive sampling, and 11 informants consisting of key, main, and supporting informants. Quantitative data were analyzed by univariate, bivariate (Chi-square), and multivariate (logistic regression) tests, while qualitative data used interactive analysis. The results showed that the majority of respondents were 36–45 years old (56.1%), had a high school education (53.0%), and worked as self-employed (37.9%). Factors of knowledge, attitude, trust, facilities, husband support, and health worker support affect immunization coverage. Multivariate analysis shows trust is the most dominant factor. Qualitatively, it was found that there is a public belief that the raw materials for Hepatitis B immunization are not halal, so many children are not immunized. It is suggested that the results of this study be a reference and evidence-based for students and health workers related to increasing immunization coverage.

Abstract

Doctors have a central role in improving the quality of hospital services, especially through medical decision-making, explanation of diagnosis, and medical measures that protect patients' rights. However, the performance of inpatient doctors at Haji Medan Hospital in North Sumatra Province is still low, with an achievement of only 47% of the expected target. This condition has the potential to reduce the quality of service if not handled immediately. This study aims to analyze the factors that affect the performance of inpatients at Haji Medan Hospital. The study used a quantitative design with a cross sectional approach. The sample consisted of 74 doctors with a total sampling technique. Data analysis was carried out through univariate, bivariate, and multivariate tests using logistic regression. The results showed that workload (p=0.015), work stress (p=0.008), work environment (p=0.001), incentives (p=0.003), and compensation (p<0.001) had a significant effect on physician performance. Thus, the main factors that affect the performance of doctors are aspects of workload, psychological condition, work environment support, and reward and compensation systems. These findings are expected to be input for hospital management to improve policies and create more supportive working conditions, as well as the basis for further research with broader variables.

Abstract

The use of elderly posyandu plays an important role in improving the quality of life, but the level of use is still low. This study aims to analyze the factors that affect the use of elderly posyandu in the Working Area of the Blangkejeren City Care Health Center. The research method used a cross sectional design with a sample of 91 respondents selected through stratified random sampling from a population of 973 elderly people. Data were collected using a questionnaire in March–May 2025 and analyzed by chi-square test and logistic regression. The results showed that education (p<0.001), attitude (p=0.001), knowledge (p<0.001), family support (p=0.016), health worker support (p=0.016), and health service facilities (p<0.001) were related to the use of elderly posyandu, while age (p=0.010) and distance (p=0.139) were not related. The dominant variable was attitude (p=0.001; Exp(B)=7.276). It was concluded that increasing the use of elderly posyandu needs to pay attention to education, attitudes, and family support and health workers.

Abstract

Complete basic immunization coverage is an important indicator in improving public health status. The government through the Health Operational Assistance (BOK) policy seeks to strengthen access and quality of immunization services, but in some areas coverage is still low. This study aims to analyze the implementation of BOK policies in increasing basic immunization coverage at the Blangkejeren City Treatment Health Center, Gayo Lues Regency, Aceh Province. The research method used a qualitative approach with in-depth interviews. The main informants are the head of the health center, the core informant in charge of the BOK program, and the supporting informant of the three midwives implementing immunization. Triangulation was carried out with five mothers under five. Data analysis uses thematic analysis techniques. The results of the study show that the implementation of BOK supports the implementation of basic immunization, but is not optimal. Supporting factors include routine fund allocation, coordination, and a positive attitude of officers. The main obstacles are limited health workers, ineffective planning and reporting, and low public participation due to lack of information and myths about vaccine side effects. In conclusion, BOK policies have helped the implementation of basic immunization, but there is still a need to strengthen coordination, program planning, and public education so that immunization coverage can be increased overall.

Abstract

Pelayanan kesehatan merupakan suatu produk, berupa jasa kepada pasien, aktifitas ini tidak bisa dimiliki namun bisa dirasakan oleh pasien. Indikator yang ingin dicapai pada produk jasa ini adalah kepuasan pasien terhadap pelayanan kesehatan. Untuk mencapai produk jasa yang sesuai dengan standar maka diperlukan kinerja maksimal dari pelaku jasa yaitu perawat. Kinerja perawat merupakan ujung tombak dari keberhasilan jasa pelayanan kesehatan yang jika perawat mengurangi perannya maka kepuasan pasien tidak akan tercapai. Tujuan penelitian untuk menganalisis faktor yang mempengaruhi kinerja perawat di UPTD RSUD Datu Beru Takengon Aceh Tengah. Metode penelitian yang digunakan adalah survey analitik dengan rancangan cross sectional. Uji statistik yang digunakan adalah uji chi square dan regresi logistik. Populasi adalah seluruh perawat sebanyak 236 orang. Tehnik pengambilan sampel menggunakan accidental sampling yang diperkuat dengan rumus slovin didapatkan sampel 148 orang. Hasil penelitian menunjukan bahwa variabel kepemimpinan (P-value 0,001), motivasi (P-value 0,002), Kedisiplinan (P-value 0,001) dan lingkungan kerja (P-value 0,001). Hasil uji regresi logistik berganda menunjukkan bahwa variabel kepemimpinan (OR=1.718), kedisiplinan (OR=3.684) dan lingkungan kerja (OR=1.104) artinya bahwa variabel kedisiplinan mempunyai pengaruh 3 kali lipat mempengaruhi kinerja perawat. Kesimpulan didapatkan bahwa kepemimpinan, motivasi, kedisiplinan dan lingkungan kerja mempengaruhi kinerja perawat di UPTD RSUD Datu Beru, Takengon, Aceh Tengah.

Abstract

According to WHO data (2020), prenatal classes have been shown to reduce maternal mortality, as they provide information on pregnancy outcomes and prevent pregnancy-related complications. The WHO explains that approximately 287,000 women experience high-risk pregnancies. Data from the Sarulla Community Health Center (Puskesmas) indicates that 67.4% of those attending prenatal classes are far from the 100% target. The purpose of this study was to determine factors influencing pregnant women's adherence to prenatal classes in the Sarulla Community Health Center's work area in North Tapanuli Regency. The research design was analytical research using a cross-sectional approach. The population was all 67 pregnant women. The sample size was 67 pregnant women in their third trimester. Primary and secondary data were used. Data analysis used univariate, bivariate, and multivariate analysis using binary regression tests.The results of the study, obtained using a chi-square test, showed an effect of education on the participation of pregnant women in pregnancy classes, knowledge on the participation of pregnant women, distance to health facilities, the role of health workers, and family support. The most dominant factor was knowledge, which significantly influenced pregnant women's adherence to pregnancy classes in the Sarulla Community Health Center, North Tapanuli Regency. Conclusion: Education, knowledge, health workers, and family support influence the participation of pregnant women in pregnancy classes in the Sarulla Community Health Center. Health workers are expected to be more active in promoting the importance of pregnancy classes through various media such as integrated health posts (Posyandu), community health centers, social media, and other community activities. Keywords: Education, Knowledge, Distance to Facilities, Role of Health Workers, Family Support, Pregnancy Classes

Abstract

The problem of malnutrition is still the main problem faced in human resource development. The problem of growth and development of Baduta is a serious problem because at this age the process of physical and intellectual growth and development is critical. Malnutrition problems mostly occur in poor families but are not caused by poor families who have well-nourished children. The Belawan Community Health Center working area is one of the community health centers with the highest number of cases of malnutrition in Medan City. To overcome this, a program is needed that is believed to be effective and sustainable. One of the existing programs to overcome the problem of malnutrition and malnutrition is positive deviation. This research aims to identify positive behavior in feeding (type of food, processing of food ingredients, mealtimes and method of feeding) of toddlers who are well-nourished in poor fishing families in the Belawan Community Health Center Working Area in 2022. This research method is qualitative research with a narrative approach. Researchers studied individual lives by asking one or several individuals to tell stories about their lives related to positive behavior in feeding well-nourished animals. This information is then retold by the researcher in a narrative chronology. The findings of this research indicate that addressing nutritional challenges in children can be effectively achieved by leveraging the potential and resources within the community. This is done through the identification of positive deviant behaviors, particularly those related to beneficial feeding practices. These practices encompass aspects such as the types of food provided, the methods of food preparation, the timing of meals, and the approach to food administration. Keep up the great work! Abstract

Positive deviance in low-income families is a mechanism by which individuals or groups continue to thrive or meet standards despite facing challenging conditions. One of the goals of positive deviance is to prevent child malnutrition by changing community habits about feeding, caring, hygiene, and health seeking practices. Malnutrition early in life will affect the growth and development of children and the quality of life thereafter. The study aims to compare the positive deviances between children with normal nutrition and undernutrition. This cross-sectional study was conducted in 39 public health centres in Medan, West Indonesia, where a total of 80 children (40 with normal nutrition and 40 with undernutrition) were recruited using purposive sampling. Nutritional status (normal or undernutrition) was determined using body mass index-for-age (BMI-for-age) z-scores, according to WHO reference values. Positive deviance behaviours were assessed using a validated questionnaire, and anthropometric parameters such as weight, height, BMI, and triceps skinfold were measured. Haemoglobin levels were taken as a biological measurement of the condition. The Mann-Whitney test demonstrated that the positive deviance practices of feeding, caring, hygiene, and health-seeking behaviours were significantly better among normal nourished children than undernourished children (p<0.05). Children from families with positive deviance practices have better nutritional status despite living in economically limited conditions. This indicates that family behaviours and practices can play a crucial role in enhancing children's nutritional and health status, regardless of financial or environmental constraints. Integrating positive deviance approaches alongside additional interventions could be effective in improving children's nutritional status.

Abstract

Tuberkulosis (TB) paru merupakan penyakit menular langsung dan merupakan ancaman besar bagi pembangunan sumber daya manusia. Adapun tujuan dalam penelitian ini adalah untuk menganalisis faktor faktor yang memengaruhi kepatuhan penderita tb paru bta (+) dalam menelan obat di Puskesmas Tapaktuan. Jenis penelitian ini merupakan survei analitik dengan rancangan restropektif. Penelitian dilakukan di Puskesmas Tapaktruan Kecamatan Tapaktuan Kabupaten Aceh Selatan Provinsis Aceh Tahun 2023. dengan menggunakan teknik purposive sampling yaitu sebanyak 60 responden. Untuk menganalisis data tersebut digunakan uji regresi logistik. Berdasarkan hasil penelitian diketahui bahwa ada hubungan kepatuhan menelan obat dengan variabel yaitu pengetahuan (p=0,000), sikap (P=0,000), petugas kesehatan (p=0,021) dukungan keluarga (p=0,001) dan budaya (p=0,000) namun tidak terdapat pengaruh ketersediaan OAT dengan kepatuhan menelan obat karena P Value (p=0,510) Di Puskesmas Tapaktuan Kecamatan Tapaktuan. Berdasarkan hasil penelitian, dapat disimpulkan bahwa faktor yang pengaruh terhadap kepatuhan menelan obat di Puskesmas Tapaktuan Kecamatan Tapaktuan Kabupaten Aceh Selatan adalah pengetahuan, sikap, petugas kesehatan,dukungan keluarga dan budaya. Faktor yang paling dominan mempengaruhi kepatuhan menelan obat di Puskesmas Tapaktuan Kabupaten Aceh Selatan Provinsi aceh yaitu pengetahuan yang memiliki nilai Exp(B) atau Odd Ratio 37.200. Saran membuat program penyuluhan tentang faktor memengaruhi kepatuhan penderita dalam menelan obat baik kepada keluarga penderita dan penderita TB paru untuk ,meningkatkan pengetahuan masyarakat tentang cara dan pola pengobatan TB yang benar agar dapat memaksimalkan hasil penyembuhan.
