Abstract

Abstract Background: The high number of diabetes mellitus patients in Indonesia followed by low levels of knowledge and adherence to treatment causes the number of cases of complications due to high blood sugar levels, especially in Health Center increases. Method:This study uses pre-experimental research methods, with a one-group pretest-posttest research design before and after counseling with a leaflet media by pharmacists at Krembung Health Center of Sidoarjo Regency with a sample number of 34 subjects. The research instruments used sociodemographic questionnaires, Diabetes Knowledge Questionnaire (DKQ-24), and modified Morisky Medication Adherence Scale (MMAS) questionnaires. Results and analysis : Chi-square statistical test results at the level of knowledge and compliance, showed there was an influence on the increase in knowledge with a p-value of 0.001, and increased adherence to drugs with a p-value of 0.006, the results of T-test statistical tests also showed an influence on the decline in the value of FPG pre and post with a p-value of 0.002, and HbA1c pre and post values had a p-value of 0.001. Discussion: Patients experienced an increase in compliance after getting counseling with leaflets from 67.7% to73.5% which was statistically significant in addition to an increase in knowledge that caused patients to be more adherence to take drugs so that there was a significant decrease in the mean value of FPG pre 183.32 to FPG post 148.23 significantly and a significant decrease in the value of HbA1c from 8.45 to 7.03. Conclusion: Counseling education with leaflets by pharmacists is influential in increasing knowledge, adherence with drug use, and lowering blood sugar levels (FPG& HbA1c) of type 2 DM patients in Krembung Health Center of Sidoarjo Regency.

Abstract

Background: Drug related problems (DRP) can cause problems in treatments of patients with ischemic stroke.Objective: This study was to determine the correlation between the characteristics of patients with ischemic stroke and the DRP.Methods: A population in this study was 53 patients at dr Iskak Public Hospital in Tulungagung. Inclusion criteria were patients who were = 60 years old and were diagnosed as acute ischemic stroke patients. The DRP was classified based on Pharmaceutical Care Network Europe version 6.2, especially problem and cause domain. Correlation between age, duration of treatment, number of comorbidities and number of drugs and the DRP was analysed by Spearman Rho or by Pearson analysis. The relationship between gender, guaranteed care and the DRP was analysed by using Eta analysis.Results: This study revealed that patients with 60-69 years were 24 patients (45.28%), men were 30 patients (57%), 1 comorbidity was 26 patients (49.06%), duration of 5 days was 14 patients (26.42%), BPJS patients were 23 patients (35.94%), using 6 types of drug was 11 patients (20.75%). Age, sex and care guarantee did not correlate with the DRP (p-values > 0.05). Duration of treatment, number of comorbidities and number of drugs correlated with the DRP (p-value <0.05) with consecutive correlation strength: weak (R = 0.32), moderate (R = 0.45), strong (R = 0.56).Conclusion: Duration of treatment, number of comorbidities and number of drugs correlated with increasing incidences of the DRP in geriatric ischemic stroke patients.

Abstract

The prevalence of Diabetes Mellitus (DM) in Indonesia has increased 0.5% in the last five years. This study aimed to evaluate information provided and level of knowledge among DM patients on their treatment at the outpatient clinic at the X Public Hospital (Rumah Sakit Umum Daerah, RSUD), Surabaya, Indonesia. This was a cross-sectional study using a checklist developed by The Food And Drug Supervisory Agency which consists of 11 types of information. The data were analysed descriptively; to test the relationship between the amount of information provided and the patient's level of knowledge, Spearman correlation analysis was used. A total of 110 patients were included in this study (response rate 90.91%). The mean of information provided and understood by patients was 6.48/11±0.73 and 7.48/11±0.90, respectively. There was a significant relationship between information provided to and known by patients (p=0.001). Characteristic factors such as age (p=0.001), education level (p=0.001), and sources of information related to DM (p=0.014) were shown to influence DM patients' knowledge of their treatment. This study indicated the importance of providing clear and complete drug information, especially for patients on long-term therapy, to improve their knowledge, thus potentially increasing adherence and optimizing therapy outcomes.

Abstract

The aging populations has increased worldwide, resulting in higher prevalences of non-communicable diseases, including Diabetes Mellitus (DM). Elderly with DM requires long-term prescribing which warrants special considerations. Hence, this narrative review aims to identify quality measurement tools and quality prescribing in elderly patients with DM. This review included literature searching in three databases [i.e., Pubmed (MEDLINE), Science Direct, and CINAHL] with combination of keywords: (Diabetes) AND (Elderly OR older OR geriatric) AND (“Potentially inappropriate medication” OR “potentially inappropriate prescribing” OR “inappropriate prescribing” OR “quality prescription” OR “rational prescribing”). The searching resulted in 1059 articles; and after the selection process, 10 articles were included in this study. The extracted data indicated that tools with explicit criteria (such as Beer's Criteria) were more commonly used to measure quality prescribing in elderly patients with DM compared to those with implicit criteria (i.e., Prescribing Quality Assessment Checklist) (9 articles versus 1 article, respectively). Varying quality prescribing were reported dependent on the tools used. The percentage of Potentially Inappropriate Medication (PIM) measured using explicit tools ranged from 22,7% to 68,1%; glibenclamide or glyburide were among the most common PIM. Whereas, 83.2% of precriptions measured with the implicit criteria in one article indicated poor quality. This finding demonstrated the needs of strategy to improve the quality of prescribing in elderly patients with DM.

Abstract

Headache is a symptom which people often self-medicate and community pharmacies are often places where self-medicating patients with headache obtain the medicines. Therefore, community pharmacists played an important role in providing appropriate advice for these patients. A cross-sectional study design was used. A questionnaire consisting of (1) pharmacy and pharmacists characteristics, and (2) two vignette cases of headache (tension headache and migraine due to an oral contraceptive) was developed. Data was collected using a structured interview with community pharmacists in an East Javan district. Data was analysed using content analysis. Appropriate recommendation was determined based on the literature and expert panel opinion. The total population was 81 community pharmacists. Of these, 47 agreed to participate. Of 47 participating pharmacists, 36 (77%) and 23 (49%) pharmacists provided appropriate advice in the case of tension headache and migraine due to the use of an oral contraceptive respectively. Appropriate recommendation provided by pharmacists varied according to the cases. Optimization of pharmacists’ recommendation in a case of headache requiring medical referral is needed.

Abstract

Appropriate recommendations given by pharmacists play a critical role in determining the efficacy and safety of self-medication practices in pharmacies, including in the case of diarrhea with alarm symptoms. This observational study with a cross-sectional design aims to determine the profile and appropriateness of recommendations given by pharmacists in Surabaya when they faced self-medication requests for diarrhea with alarm symptoms. Data were collected using a questionnaire consisting of two parts, i.e., participants’ characteristics and one case of adult diarrhea with alarm symptoms. The case used was newly developed and the content had been validated. The content validation and the determination of appropriate recommendations given by pharmacists involved experts in clinical pharmacy, community pharmacy, and public health. The final data were analyzed and presented descriptively. A total of 160 pharmacists were involved in this study, the majority of whom were managing pharmacists (82.50%), female (85.50%), and have been working as pharmacists for 1 to 7 years (48.75%). Most pharmacists (56.87%) gave more than one type of recommendation, the majority of whom recommended dispensing of pharmaceutical products accompanied by a referral to physicians (21.88%). Only 42 pharmacists (26.25%) gave appropriate recommendations including referral to physicians (18.13%) or referral accompanied by pharmaceutical product (6.88%) and non-pharmacology recommendations (1.25%). Need assessment is required to identify interventions preferred by pharmacists to optimize their role in promoting responsible self-medication practices.

Abstract

Assessing the rational use of medicines or prescription quality is essential in any health system. However, data on the quality of prescribing in patients with hypertension is currently lacking. Hence, this review aimed to identify prescribing quality indicators in patients with hypertension and understand the results. A narrative review was conducted to include literature searching in three databases, including PubMed (Medline), Science Direct, and CINAHL, using keywords and Boolean Logic as follows: “(hypertension OR antihypertensive drugs OR antihypertensive medication OR antihypertensive agent) AND (quality prescription OR inappropriate prescribing OR potentially inappropriate medication OR rational prescribing)”. As a result, a total of 1889 articles were obtained. The articles were then selected based on inclusion and exclusion criteria in which 13 articles were included in this review. Based on the extracted data, explicit as well as implicit frameworks have been used to assess the quality of prescribing in patients with hypertension. Explicit approaches compared patient prescription versus guidelines (n=7 articles) or lists of potentially inappropriate use of medicines (n=3 articles). The implicit framework assessed patient prescription using two tools, including the prescription Quality Index (PQI) – 22 indicators (n=2 articles), and the prescription quality checklist – four indicators (n=1 article). The results of the quality assessment on the prescriptions showed that prescriptions categorized as good quality were between 40-80%. While varied levels of inappropriate selection of antihypertensive therapy were reported (0-65%). In addition, more than 30% of patients with hypertension experienced Potentially Inappropriate Medication (PIM); thus, prescribing among patients with hypertension needs to be optimized.

Abstract

Medication error adalah setiap kejadian yang menyebabkan pelayanan obat tidak tepat atau membahayakan pasien yang dapat dihindari. Tujuan penelitian untuk mengetahui prevalensi terjadinya medication error pada tahap prescribing dan transcribing pasien Rawat Jalan poli Interna, Saraf dan Anak. Penelitian dilakukan secara cross-sectional, pengambilan data dilakukan secara retrospektif dari data sekunder dengan melihat resep yang masuk ke Instalasi Farmasi Rawat Jalan Rumah Sakit “X” Kabupaten Malang sejumlah 375 sampel resep pada periode bulan Juni – Agustus 2023. Dari penelitian ini didapatkan analisis kelengkapan resep yang berpotensi menjadi penyebab medication error pada tahap prescribing penulisan resep tidak jelas sebesar 7,47%, diikuti kelengkapan identitas pasien yang tidak tepat sebesar 3,74%. Sedangkan tahap transcribing adanya duplikasi obat pada penulisan resep sebesar 5,07%, diikuti adanya interaksi obat dalam penulisan resep sebesar 29,33%. Kategori Interaksi obat berdasarkan hasil keparahan interaksi obat dari 375 resep terdapat 110 resep yang memiliki interaksi obat dengan persentase sebesar 29,33%. Dari 110 resep yang teridentifikasi interaksi obat, didapatkan interaksi mayor pada 10 resep dengan persentase 9,09%, interaksi moderate pada 96 resep dengan persentase 87,27% dan interaksi minor pada 4 resep dengan persentase 3,64%. Berdasarkan data disimpulkan bahwa masih terjadi medication error pada tahap prescribing dan transcribing di Rumah Sakit “X” Kabupaten Malang

Abstract

A cross-sectional study was performed to describe self-medication practices of customers at kiosks and minimarkets in District X, Indonesia, during COVID-19 pandemic in 2021. A sample of 98 customers from 67 kiosks, and 100 customers from 52 minimarkets were interviewed using a validated questionnaire. Descriptive statistics were used to summarise the findings. Most drugs purchased at kiosks and minimarkets were general sales drugs, i.e., analgesic-antipyretic (64.3% versus 46.0%); however, prescription drug sales were reported (3.1% vs 3.0%, respectively). Kiosk and minimarket customers purchased medications mainly based on the recommendations of non-health workers (69.4% vs. 34.0%, respectively), or electronic media (36.7% vs. 43.0%, respectively). The reasons to choose kiosks or minimarkets included accessibility, personal preferences, and service quality. Most kiosk/minimarket customers had knowledge on the drug brand names and indications, but only some knew the composition (8.16% vs. 29.0%, respectively) or side effects (9.18% vs. 35.0%, respectively). These findings indicated limited quality assurance on self-medication practices at non-pharmacy outlets.

Abstract

Anemia is a major health problem that threatens all ages, especially adolescent. Adolescent girls are more at risk of experiencing anemia, due to the loss of nutrition (iron) during menstruation. Providing iron supplement tablets is one of the efforts to prevent anemia and increase the loss of nutrients during menstruation. The purpose of this study was to determine the level of knowledge of adolescent girls in the Denpasar area regarding anemia and iron supplement tablets. This research is an observational study with a survey method. The sampling technique uses total sampling. This research involved 94 adolescent girls in a high school in the Denpasar area. The results showed that 30.8% of the respondents had a low level of knowledge, 56.4% had a sufficient level of knowledge and about 12.8% had a good level of knowledge. Overall, only a small proportion of respondents know the causes of anemia and the side effects of iron supplement.   ABSTRAK Anemia merupakan salah satu masalah kesehatan utama yang mengancam seluruh usia terutama usia remaja. Remaja putri lebih berisiko mengalami anemia, dikarenakan mengalami kehilangan gizi (zat besi) selama menstruasi. Pemberian suplemen tablet tambah darah salah satu upaya dalam mencegah anemia serta meningkatkan nutrisi yang hilang selama menstruasi. Tujuan penelitian ini mengetahui tingkat pengetahuan remaja putri di wilayah Denpasar terkait anemia dan tablet tambah darah. Penelitian ini merupakan penelitian observasional dengan metode survei. Teknik pengambilan sampel menggunakan total sampling. Penelitian ini melibatkan 94 remaja putri di salah satu SMA di wilayah Denpasar. Hasil penelitian menunjukkan bahwa sebanyak 30,8% responden memiliki tingkat pengetahuan rendah, 56,4% memiliki tingkat pengetahuan cukup dan sekitar 12,8% memiliki tingkat pengetahuan baik. Secara keseluruhan hanya sebagian kecil responden yang mengetahui penyebab anemia dan efek samping tablet tambah darah.
