Abstract

The birth of a fetus through an incision in the uterine wall through the front wall of the abdomen is known as sectio cesarean delivery. The SC process requires anaesthetic management consisting of general and regional anaesthesia. Female celebrities and general citizens are currently having SC deliveries using the Enhanced Recovery After Cesarean Surgery (ERACS) method. Spinal anaesthesia provides several advantages, including keeping the mother awake, reducing aspiration likelihood, and avoiding neonatal depression. It also provides unexpected complications, including haemodynamic changes and the incidence of urinary retention. This study aims to determine the features of intra-anesthetic haemodynamics and postoperative urinary retention in sectio caesarea patients with the Enhanced Recovery After Cesarean Surgery (ERACS) method. This study used a quantitative descriptive approach with a cross-sectional approach. The sampling technique was a purposive sampling of as many as 67 respondents. Data were collected by observing the first 15 minutes after spinal anaesthesia and interviewing the incidence of postoperative urinary retention. The results showed that there were haemodynamic changes and the incidence of postoperative urinary retention.

Abstract

Sectio Caesarea  merupakan operasi yang dapat menurunkan angka kematian ibu saat persalinan pervaginam. Teknik anestesi spinal mempunyai kelebihan karena kesederhanaan tekniknya dan efektivitasnya dalam memberikan blok sensorik dan motorik, terutama pada operasi ekstremitas bawah . Nyeri akibat spinal anestesi di pengaruhi oleh ukuran jarum spinal yang digunkanan. Kompres dingin dengan es dapat memperlambat konduksi saraf perifer, mengurangi pelepasan mediator inflamasi, dan menimbulkan efek anestesi kulit cepat. Penelitian ini bertujuan untuk mengetahui pengaruh kompres es sebelum injeksi spinal terhadap nyeri tusukan jarum pada pasien SC. Metode yang digunakan adalah eksperimen kuantitatif dengan desain post-test only with control group, melibatkan data primer melalui wawancara dengan pasien SC pasca spinal. Pengukuran nyeri dilakukan pada dua kelompok: kelompok perlakuan (kompres es) dan kelompok kontrol. Analisis menggunakan uji Mann Whitney-U. Hasil Uji statistik menunjukkan nilai p value 0,0001 (<0,05), yang berarti kompres es sebelum injeksi spinal signifikan mengurangi nyeri tusukan jarum spinal pada pasien SC.

Abstract

Shivering adalah keadaan dimana ditandai dengan adanya peningkatan aktivitas muskular yang seringkali terjadi sesudah tindakan anastesi, khususnya anastesi spinal pada pasien yang menjalani operasi. Penelitian ini bertujuan untuk menganalisis hubungan indeks massa tubuh (IMT) dengan kejadian shivering pada pasien spinal anestesi di RS Khusus Bedah Jatiwinangun Purwokerto. Penelitian ini menggunakan jenis deskriptif korelasi dengan rancangan crosssectional. Teknik pengambilan sampel dalam penelitian ini menggunakan teknik accidental sampling sebanyak 43 orang. Analisa data dilakukan secara univariat dan bivariat menggunakan uji chi square. Hasil penelitian menunjukkan bahwa dari 43 responden sebagian besar memiliki IMT dalam kategori normal sebanyak 27 responden (62,8%) dan sebagian besar tidak mengalami shivering sebanyak 24 responden (55,8%). Ada hubungan Indeks Massa Tubuh (IMT) dengan kejadian shivering pada pasien spinal anestesi dengan p-value 0,001 (P<0,05). Diharapkan kepada RS Khusus Bedah Jatiwinangun Purwokerto khususnya bagi penata anestesi agar lebih memperhatikan pasien sesuai karakteristiknya dengan resiko shivering yang lebih tinggi seperti indeks massa tubuh (IMT) dengan kategori kurus yang sangat rentan mengalami kejadian menggigil intra operatif.

Abstract

Introduction: Hypertension in the elderly is very important to know because the pathogenesis, course of the disease and its management are not entirely the same as hypertension in adults of easy age. Pain is subjective, while the discomfort experienced by a person can be known from autoanamnesis. Distraction technique is a way to relieve pain by diverting the patient's attention to other things so that the patient will forget about the pain he feels. Objective: This service aims to help increase the knowledge of hypertensive elderly people in managing pain against hypertension at the RW/5 Ledug elderly Posyandu. Method: The method of community service activities is to conduct Hypertension Counseling and Pain Management with Distraction Techniques Using Audiovisual Media to Increase Knowledge of Hypertension Patients. This community service activity begins with giving a post-test in the form of a questionnaire to measure the level of knowledge before being given education, then participants are given education, and after participants are given education, a questionnaire is given again to measure the level of knowledge after being given education. Result: The results of community education activities showed that the knowledge value of participants before education was in the good category as many as 23 respondents (76.7%), the moderate category was 7 respondents (23.3%). participants after education in the good category were 30 respondents (100%), the moderate category was 0 respondents (0%). Conclusion: From the results of the service it can be concluded that there is an increase in knowledge in people with hypertension.

Abstract

Background: One of the factors that influences the wound healing process due to appendectomy is early mobilization. Early mobilization has the effect of improving and facilitating blood circulation so that the supply of nutrients to the wound tissue can be adequate and the healing process will be faster. Objective: This study aims to determine the relationship between early mobilization and post-appendectomy wound healing at Purwokerto Islamic Hospital. Method: This research uses a non-experimental quantitative method with a descriptive correlation design. The approach used is cross sectional with non-probability sampling techniques. The data analysis test used in this research is the Spearman rank rho correlation test. Results: The results of this study indicate that early mobilization and wound healing in post-appendectomy patients at the RSU at Purwokerto Islamic Hospital, most of the early mobilization was adequate, 10 respondents (50%) with post-appendectomy patients' wound healing good, 12 respondents (60%). The results of the Spearman Rank test show that the p-value (0.000) < α 0.05, which means that H0 is rejected and Ha is accepted. The correlation coefficient test value = 0.845, so it was found that there was a relationship between early mobilization and post-appendectomy wound healing at the Purwokerto Islamic Hospital. Conclusion: There is a relationship between early mobilization and post-appendectomy wound healing at Purwokerto Islamic Hospital with a p-value of 0.000 and correlation coefficient = 0.845.

Abstract

The purpose of this study was to evaluate the hemodynamic picture of patients receiving spinal anesthesia at RSUD Dr. Soedirman Kebumen, focusing on changes in blood pressure, pulse, and oxygen saturation before and after anesthetic injection. This study used quantitative method with descriptive design and cross-sectional approach. The results showed that before injection, 81.0% of patients had normal blood pressure, but after injection, there was an increase in hypotension cases to 17.0% at the 5th minute. By the 20th minute, 93.2% of patients had normal blood pressure again. For pulse, 10.9% of patients had tachycardia before injection, which increased to 6.8% at the 20th minute after injection. Oxygen saturation remained stable, with 100% of patients achieving normal saturation at the 5th and 20th minutes. The conclusion of this study shows that spinal anesthesia can cause significant hemodynamic changes, especially a decrease in blood pressure and an increase in tachycardia frequency, although oxygen saturation remained stable.

Abstract

Background: Spinal anesthesia is known to affect blood pressure by raising or lowering blood pressure levels, including MAP and pulse rate. While colloidal fluid therapy is indicated for the management of such variations, it remains longer in the intravascular space and supports blood pressure stabilization.  Purpose: This study attempts to analyze the time and amount of Ringer Lactate fluid administration to stabilize blood pressure back to normal levels after spinal anesthesia. Methods: Quantitative research was adopted with an analytical, cross-sectional observation design. Data collection was facilitated using sheets for observing blood pressure and measuring blood pressure using a Tension meter. The study was performed over one month, from July to August, at Awal Bros Batam Hospital. A total of 40 respondents were taken for this study using total sampling. The data collected were analyzed using univariate analytical methods. Results: From the analysis, the mean average time taken for blood pressure changes after spinal anesthesia is 12.15 minutes standard deviation of 3.363. The time taken varied from a minimum of 5 minutes to a maximum of 20 minutes. As for the volume of Ringer Lactate fluid, on average, the volume given was 20 ccc/KGBB. Blood pressure change after spinal anesthesia had a mean of 99.65 standard deviations of 30.783, with the minimum measurement at 22 and the maximum at 167. Conclusion: This study shows the importance of giving Ringer Lactate fluid to stabilize blood pressure following spinal anesthesia, where both time and volume correlate positively with changes in blood pressure

Abstract

Background: Postoperative pain in each patient will experience different pain from one another. Inadequate pain management can have a direct impact on postoperative pain, which is disruption of sleep quality. Purpose: The purpose of this research was to identify any correlation between the severity of postoperative pain and the quality of sleep experienced by patients after spinal anesthesia. Methods: This research employs a cross-sectional design based on quantitative correlation. Using a non-probability purposive sampling approach, 51 participants were surveyed after spinal anesthesia for this investigation. The data was collected in the inpatient room at Wijayakusuma Purwokerto Hospital using questionnaire sheets from the Pittsburgh Sleep Quality Index (PSQI) to assess sleep quality and NRS (Numeric Rating Scale) to quantify postoperative pain severity. The Spearman rank test was used to examine the data. Results: Forty-three people (78.4% of the total) reported minor postoperative discomfort, while forty-three people (84.3% of the total) reported satisfactory sleep quality. A correlation force of 0.756 and a p-value of 0.000 were determined using the Spearman rank test. Conclusions: Patients undergoing spinal anesthesia at Wijayakusuma Purwokerto Hospital reported significantly higher levels of postoperative discomfort and worse sleep quality.

Abstract

Abstract Background: Surgical procedures are inseparable from anesthesia, one of the anesthetic procedures is spinal anesthesia. Spinal anesthesia is the most common regional technique performed by most anesthesiologists worldwide. Spinal anesthesia is injecting local analgesic drugs into the subarachnoid space in the area between the lumbar vertebrae L2-L3 or L3 L4 or L4-L5. This study aims to determine the relationship between the duration of surgery and the occurrence of Post Anesthetic Shivering and urinary retention in patients after spinal anesthesia in the recovery room of RSU Aisyiyah Padang. Method: This research method is descriptive quantitative with a cross-sectional approach. The sampling technique used no random sampling with a total sampling of 30 respondents. Data was taken using demographic data for gender and age data. Results: Respondents with the most dominant age category were adults aged 26-45 years, namely 17 respondents (56.7%). Respondents with a more dominant Gender category, namely Women, were 18 respondents (60.0%). In the category of length of surgery, namely more than >1-2 hours, namely 20 respondents (66.7). Based on the category of shivering frequency, the most dominant one is not shivering, as many as 16 people (53.3). The incidence of urine retention, most respondents did not experience urine retention, as many as 27 respondents or (90.0). Conclusion: There is no relationship between the length of surgery and the incidence of post-anesthetic shivering (pas) and urine retention and the direction of the relationship is positive.

Abstract

Background. Basic life support is a basic aspect of rescue action in relation to cardiac arrest. To support the success and quality of life of patients, important aspects include prevention of cardiac arrest, early cardiopulmonary resuscitation (CPR)/cardiopulmonary resuscitation (CPR), activation of the emergency response system, effective advanced life support, and integrated post-cardiac arrest management. The purpose of this community service activity is to provide basic life support training to the general public so that they can provide assistance when an emergency arises. This PKM method is carried out with the practice of Basic Life Support Training. The results of this community service obtained data on the characteristics of participants dominated by men, namely 18 participants (60%), while for participants with female gender there were 12 respondents (40%). The characteristics of education obtained data that the high school education history was 16 participants (53%), for the characteristics of employment status, data obtained that it was dominated by farmers as many as 16 participants (54%). The level of BHD implementation with a good category is 20 respondents (67%), while for the less category level is 10 respondents (33%). The output produced is in the form of leaflets and scientific publications. This PKM activity is expected to fulfill the author’s final assignment in taking the Anesthesiology Nursing Study Program at Harapan Bangsa University, Purwokerto.
