Abstract

Insomnia is increasingly recognized as a systemic disorder involving persistent hyperarousal with broad psychiatric and cardiovascular consequences. This case report describes a 49-year-old woman who developed functional dyspepsia after taking an analgesic, which led to chronic insomnia and, subsequently, essential hypertension (blood pressure reaching 173/93 mmHg) accompanied by generalized somatic distress. Her sleep pattern became non-restorative, with reduced sleep drive and dependence on hypnotics. Psychiatric evaluation revealed Nonorganic Insomnia, a Moderate Depressive Episode, and Somatization Disorder, with an Insomnia Severity Index score of 16. Her clinical course illustrates a psychophysiological cascade. Gastrointestinal distress disrupted sleep architecture, sustained sympathetic nervous system activation, and dysregulated the hypothalamic–pituitary–adrenal axis, thereby contributing to hypertension and somatic amplification. This case highlights the need to view insomnia as a systemic condition rather than an isolated symptom. Early recognition and non-pharmacological interventions—particularly Cognitive Behavioral Therapy for Insomnia (CBT-I)—may prevent long-term cardiovascular and psychiatric complications.

Abstract

Childhood depression often stems from early-life stressors such as peer bullying and negative parenting. Early- onset depression can have long-term impacts on a child’s emotional and cognitive development. Nevertheless, parental awareness of their crucial role in supporting children’s mental health remains limited. This case report describes the clinical manifestation of depression in a 10-year-old boy, triggered by prolonged bullying and insufficient family support, and explores how parental psychoeducation can facilitate recovery. This research employs a descriptive qualitative case study method, in which data were collected through in-depth clinical interviews with the patient and his parents, as well as direct observation. The initial screening used the Strengths and Difficulties Questionnaire (SDQ) to identify emotional and behavioral concerns. Additionally, semi structured interviews were conducted to explore psychosocial history, school experiences, and parenting style over a four week outpatient period. The child presented with persistent sadness, social withdrawal, hallucinations, and academic pressure, particularly from his grandmother, with clear behavioral changes following continuous bullying at school. Through targeted psychoeducation, the mother adopted a more responsive and non-judgmental parenting approach, leading to gradual emotional improvement in the child, as seen by reduced social isolation and improved emotional expression. This case highlights the critical role of family support and parental psychoeducation as non-pharmacological interventions in early- onset childhood depression, emphasizing the importance of early detection of depressive symptoms and involving families in emotional support strategies to strengthen children’s mental resilience.

Abstract

Self-harm represents a serious psychiatric emergency frequently observed in individuals with schizophrenia, especially the paranoid subtype, where hallucinations, delusions, and comorbid depression play crucial roles. Although antipsychotic medications remain the cornerstone of treatment for symptom stabilization, they are often insufficient to address underlying psychosocial vulnerabilities that heighten self-harm risk. Supportive psychotherapy (SPT) has emerged as a cost-effective adjunctive intervention that can be integrated within primary health care to complement pharmacological treatment. This literature review synthesizes studies published between 2011 and 2025, retrieved from PubMed, ScienceDirect, and Google Scholar using the keywords paranoid schizophrenia, supportive psychotherapy, and self-harm. Findings reveal that SPT, when implemented after acute psychotic symptoms have subsided, enables patients to express emotions, receive reassurance, and modify maladaptive thoughts through supportive persuasion. Evidence indicates that SPT effectively reduces suicidal ideation and self-harm severity, commonly assessed using the Suicidal Intention Rating Scale (SIRS). Additionally, involving families and providing psychoeducation improve treatment adherence, reduce relapse rates, and create a more supportive home environment. Compared to structured psychotherapies such as cognitive behavioral therapy (CBT) or family-focused therapy (FFT), SPT is simpler, more flexible, and feasible in low-resource settings since it can be delivered by general practitioners with basic training. Overall, this review concludes that SPT offers significant potential to mitigate psychiatric emergencies and enhance the quality of life in paranoid schizophrenia patients at risk of self-harm. It recommends training primary care providers in SPT, integrating family psychoeducation, and implementing a stepped-care model that positions SPT as an initial stabilization phase before advancing to structured therapies.

Abstract

Schizophrenia and related psychotic disorders pose a substantial global health burden, with treatment often complicated by antipsychotic-induced extrapyramidal side effects. Akathisia, characterized by profound inner restlessness and repetitive movements, is particularly distressing and frequently leads to medication non-adherence and relapse. This case report and comprehensive review details the management of a 30-year-old woman with schizoaffective disorder who developed severe akathisia following three months of risperidone therapy, resulting in treatment discontinuation. Upon readmission, a multimodal therapeutic strategy was implemented: risperidone was switched to aripiprazole, and adjunctive therapy included clobazam, trihexyphenidyl, folic acid (B9), and mecobalamin (B12). The clinical rationale extended beyond conventional management to incorporate targeted nutritional support, based on evidence that vitamin B6 modulates dopaminergic, serotonergic, and GABAergic pathways, while folate and B12 are crucial for one-carbon metabolism, homocysteine regulation, and neuroprotection. Following this integrated intervention, the patient's akathisia resolved, daily functioning stabilized, and treatment adherence was restored. This report provides an in-depth discussion of the synergistic neurobiological mechanisms through which B vitamins may alleviate akathisia, situates the findings within the broader context of nutritional psychiatry, and highlights the socio-economic relevance of this low-cost, low-risk adjunctive strategy for resource-limited settings. While current evidence remains preliminary, this case underscores the importance of early akathisia recognition and supports the integration of nutritional assessment and B-vitamin supplementation into personalized treatment plans for patients intolerant to standard therapies. Further randomized controlled trials are warranted to establish standardized dosing and identify patient subgroups most likely to benefit.

Abstract

Functional dyspepsia is an upper gastrointestinal complaint without organic abnormalities that may interact with sleep through gut–brain axis mechanisms. We report the case of a 49-year-old woman with persistent insomnia occurring concurrently with epigastric pain for approximately one year. The patient relied on hypnotic medication and experienced early-morning awakenings. Psychiatric, somatic, and psychosocial evaluations indicated that the sleep disturbance was related to dyspeptic complaints. Management included psychoeducation, supportive psychotherapy, and Cognitive Behavioral Therapy for Insomnia (CBT-I), along with dyspepsia therapy (sucralfate and omeprazole) and a spiritual approach according to the patient’s preferences. Following integrated therapy, sleep patterns improved and the Insomnia Severity Index score decreased in parallel with the resolution of dyspepsia symptoms. This case demonstrates that dyspepsia can trigger sleep disturbances through gut–brain axis mechanisms and highlights the importance of a multidisciplinary approach in managing insomnia with gastrointestinal comorbidity. The report also emphasizes the clinical relevance of addressing biopsychosocial factors and incorporating culturally sensitive, patient-centered care to enhance therapeutic outcomes and prevent symptom recurrence.

Abstract

ABSTRAK Remaja akan menghadapi berbagai masalah yang kompleks terkait dengan perubahan fisik, kecukupan gizi, perkembangan psikososial, emosi dan kecerdasan yang akhirnya menimbulkan konflik dalam dirinya dan akan mempengaruhi kesehatannya. Kegiatan pengabdian pada Masyarakat ini berupa program “Healthy Youth Corner” yang bertujuan untuk meningkatkan pengetahuan dan keterampilan pada remaja, orang tua, serta kader terhadap kesehatan diri. Metode yang digunakan dalam kegiatan pengabdian masyarakat ini adalah dengan memberikan pelatihan, penyuluhan, dan sosialisasi. Kegiatan ini dilaksanakan selama satu bulan dan diikuti oleh 42 remaja, orang tua, dan kader. Sebelum dilakukan kegiatan pengabdian, Sebagian besar (64%) responden memiliki pengetahuan danketerampilan kurang dan setelah mengikuti kegiatan pengabdian ini sebagian besar (69%) memiliki pengetahuan danketerampilan baik. Program Healthy Youth Corner dapat meningkatkan pengetahuan dan keterampilan remaja terhadap diri. Rencana tindak lanjut yang dapat dilakukan adalah dengan pendampingan serta peningkatan keterampilan berkelanjutan bagi kesehatan remaja. Kata Kunci: Kesehatan, Pemberdayaan Remaja, Program Healthy Youth Corner  ABSTRACT Adolescents will face various complex problems related to physical changes, nutritional adequacy, psychosocial development, emotions, and intelligence which ultimately give rise to conflicts within themselves and will affect their health. This community service activity is the "Healthy Youth Corner" program which aims to increase the knowledge and skills of teenagers, parents, and cadres regarding personal health. The method used in this community service activity is to provide training, counseling, and outreach. This activity was carried out for one month and was attended by 42 teenagers, parents, and cadres. Before carrying out service activities, the majority (64%) of respondents had poor knowledge and skills and after participating in this service activity the majority (69%)had good knowledge and skills. The Healthy Youth Corner program can increase teenagers' knowledge and skills regarding themselves. The follow-up plan that can be carried out is with assistance and continuous improvement ofskills for adolescent health. Keywords: Health, Youth Empowerment, Healthy Youth Corner Program

Abstract

Post-Traumatic Stress Disorder (PTSD) has traditionally been viewed as a psychiatric condition; however, emerging evidence from psychoneuroimmunology (PNI) demonstrates that PTSD involves systemic physiological dysregulation extending across neural, endocrine, cardiovascular, gastrointestinal, and immune domains. This systematic review synthesizes recent findings from studies published between 2004 and 2025 to conceptualize PTSD as a psychoneuroimmunological cascade. Literature retrieved from PubMed, Scopus, and Web of Science was analyzed thematically to identify converging mechanisms of neuroendocrine, immune, and metabolic alterations associated with PTSD. The review reveals consistent patterns of  hypothalamic pituitary adrenal (HPA) axis dysfunction, autonomic overactivation, chronic inflammation, gut–microbiota imbalance, and endocrine–immune cross-talk. These interconnected processes form self-perpetuating feedback loops that translate psychological trauma into systemic disease. PTSD should therefore be reframed as a multisystem disorder sustained by psychoneuroimmunological dysregulation rather than as a purely psychological condition. Integrative, multidisciplinary interventions—combining pharmacotherapy, mind–body therapies, and lifestyle modulation—are essential to restore systemic homeostasis and improve long-term outcomes. This reconceptualization expands theoretical understanding of PTSD, bridges psychiatry with internal medicine, and informs trauma-informed clinical and policy practices.

Abstract

Hebephrenic schizophrenia, or disorganized-type schizophrenia, represents one of the most disabling and treatment-resistant subtypes of psychotic disorders, marked by disorganized thinking, affect, and behavior. These symptoms often persist despite standard pharmacological treatment and are associated with poor cognitive and social functioning. This case report explores the effectiveness of an integrated multimodal therapeutic approach that combines pharmacological, cognitive-behavioral, and metabolic strategies in managing a young woman with chronic hebephrenic schizophrenia. The patient, a 23-year-old female with a six-year illness duration and history of medication nonadherence, was treated with aripiprazole 15 mg/day (dopamine D2 partial agonist), trifluoperazine 5 mg/day (full D2 antagonist), folic acid 1 mg/day, and vitamin B6 100 mg/day. Cognitive Behavioral Therapy (CBT) and family psychoeducation were conducted twice weekly for three weeks. After treatment, she demonstrated measurable improvement, including a 1-point reduction in conceptual disorganization and a 2-point improvement in blunted affect (PANSS-R), alongside qualitative gains in coping with auditory hallucinations and affective responsiveness. These results suggest that synergistic modulation of dopaminergic circuits, cognitive restructuring, and metabolic support may yield short-term benefits for patients with treatment-resistant disorganization symptoms. Further longitudinal studies are warranted to evaluate long-term outcomes and neurobiological mechanisms.

Abstract

Ovarian cancer is a malignant tumor originating from the ovaries, affecting various age groups, with multiple histological types. Depression is a common mental disorder characterized by emotional and physical symptoms. Cancer diagnosis and treatment are widely recognized as major psychological stressors that can precipitate or exacerbate mental health disturbances, particularly depressive and anxiety disorders, due to the profound emotional, social, and physiological challenges experienced by patients. This case report highlights the relationship between ovarian cancer and depressive symptoms. This case report examines the relationship between ovarian cancer and depressive symptoms, with a focus on the psychological and inflammatory pathways influencing mental health. A 44-year-old woman presented with a loss of interest in activities, beginning three weeks after her ovarian cancer diagnosis. Research indicates that factors such as advanced cancer stage, poor physical functioning, and the absence or discontinuation of chemotherapy due to disease progression or treatment intolerance may increase the susceptibility to mental health disorders among ovarian cancer patients. Cancer patients, including those diagnosed with ovarian cancer, experience heightened stress and depression compared to those with non-neoplastic diseases. Depression can arise due to both psychological stress and inflammatory processes triggered by cancer treatments.

Abstract

Depression is a psychological condition characterized by persistent sadness, hopelessness, and loss of interest that affects emotional, cognitive, and behavioral functioning. Spirituality is believed to play a significant role in reducing depressive symptoms by fostering inner harmony, emotional resilience, and a sense of meaning in life. This case report explores the relationship between spirituality and depression in a 44-year-old woman diagnosed with major depressive disorder with psychotic features. The patient continues to experience negative emotions such as pessimism, lack of self-confidence, continuous crying, and feelings of inferiority that began after being dismissed from her job, a traumatic event that deeply affected her psychological state. She also experienced a decline in spiritual beliefs and practices, which was attributed to her father’s overly strict and inconsistent religious upbringing. This case highlights the close relationship between spirituality and mental health, demonstrating that spiritual well-being can serve as a protective factor against depression by enhancing emotional stability, personal resilience, and positive psychological adjustment. The integration of spiritual care into mental health interventions is therefore essential, as it may improve recovery outcomes and enhance the overall quality of life for individuals experiencing depression.
