Abstract

Introduction: Hypertension is a cardiovascular disease in the heart and blood vessels and a serious health problem worldwide. This condition can be characterized by systolic blood pressure ≥ 140 mmHg and diastolic blood pressure ≥ 90 mmHg. An unhealthy lifestyle and a deficit of information account for most of the elements that cause hypertension, and problems arise with ineffective health management. Objective: This study aims to determine the description of community nursing care with Ineffective Health Management: Hypertension in the community of neighborhood 03 in Jumerto Village. Method: This study uses descriptive analysis with a total sampling of 33 samples. Data were collected using a DASH questionnaire, which was carried out before and after the intervention. Data analysis used the Wilcoxon test, with the research results showing a p-value. Result: The results of the intervention showed that there was a significant difference in systolic and diastolic blood pressure before and after the intervention with a significance level (p-value< 0.05), and there is an increase in health education about the hypertension diet with a p-value of 0.000 (p value< 0.05). Conclusion: Health education interventions and demonstrations of giving bay leaf decoction drinks as non-pharmacological therapy can overcome the problem of ineffective health management nursing.

Abstract

Pandemi COVID-19 menjadi tantangan besar dalam mengatasi permasalahan layanan kesehatan pada ibu, bayi baru lahir, dan anak dibawah lima tahun di Indonesia. Pandemi telah menyebabkan keterbatasan akses ke pelayanan kesehatan dan pergeseran prioritas system kesehatan dalam memberikan layanan standar pada masyarakat. Penelitian ini bertujuan untuk mengidentifikasi cakupan pelayanan kesehatan ibu dan anak sebelum dan sesudah Pandemi COVID-19 di Puskesmas di Kabupaten Jember. Penelitian ini merupakan penelitian kuantitatif deskriptif dengan pendekatan analisis data sekunder. Data register tahun 2019 sampai dengan pertengahan tahun 2021 dianalisis dari Tujuh Puskesmas di Wilayah Kerja Kabupaten Jember. Hasil penelitian ini menunjukkan bahwa sebanyak 82.35% dari keseluruhan indikator cakupan pelayanan kesehatan ibu dan anak di Puskesmas mengalami penurunan dari tahun 2019 ke tahun 2020. Sementara hanya tiga indkator yaitu pelayanan antenatal pertama (K1) pada ibu hamil, bayi lahir hidup, dan bayi paripurna yang mengalami kenaikan sebesar 0.63%, 0.95%, dan 4.67%. Pandemi COVID-19 memberikan dampak baik secara langsung maupun tidak langsung terhadap cakupan pelayanan ibu dan anak di Puskesmas. Oleh karena itu, untuk meningkatkan cakupan pelayanan kesehatan ibu dan anak selama pandemi COVID-19 perlu adanya peningkatan aksesibilitas pelayanan dan identifikasi faktor yang berkaitan dengan pemanfaatan layanan kesehatan ibu dan anak di Puskesmas.

Abstract

Efforts to control Tuberculosis (TB) are carried out in various ways, including activities to increase public knowledge about TB. Health Behavior Promotion Efforts are expected to increase changes in patient or client behavior so that they have the will and ability that are conducive to improving overall health. The aim of this research is a case study of community nursing care by providing intervention and implementation in selected target villages. The intervention provided is Promotion of Health Effort Behavior. The sample size was 87 respondents from selected area using the Cluster Sampling technique. Data collection instruments for taking cases managed in this scientific work use clean and healthy living behavior instruments, knowledge instruments about healthy homes, Knowledge Instruments on the Role of the Family as PMO (Drug Dispensing Supervisor), TB Screening and Scoring Instruments, procedures of Hand Washing. The intervention was carried out with Simple Inhalation Therapy Using Eucalyptus Oil Steam accompanied by a physical examination and vital signs measurements. The results of the research show that there is an effect of education on public knowledge with a p value (p= 0.000), the results of implementing the hand washing demonstration show results (p = 0.000) which means there is an effect of hand washing demonstration on public knowledge, while from the implementation of simple inhalation therapy with eucalyptus oil vapor the results were (p = 0.017) which means there is an effect of simple inhalation therapy with eucalyptus oil vapor on the patient's RR and SPO2. From the existing results, effective nursing implementation is carried out for TB prevention is the identification of health behavior efforts that can be improved, screening and health education, as well as the implementation of simple inhalation of eucalyptus oil vapor in TB patients.

Abstract

Introduction: Hypertension is a type of non-communicable disease that can significantly cause morbidity and mortality of sufferers. Adult age is a risk factor that has a great influence on hypertension. Interventions that can be given to the community are Health System Guidance to identify hypertension problems in the community and develop the ability to overcome existing health problems, namely hypertension problems. Objective: To determine the effectiveness of nursing interventions for health system guidance. Method: Descriptive and analytical research design with a sample of 50 productive age who have hypertension in the RW 08 area of Patrang Village and 5 health cadre. This research carried out implementation in the form of primary prevention (health education), secondary prevention (hypertension screening and cadre empowerment) and tertiary prevention (education and demonstration of giving star fruit juice). Data collection used respondent characteristic instruments, hypertension knowledge questionnaire, hypertension diet questionnaire, standart operating procedure for blood pressure measurement, standart operating procedure for making star fruit juice. Data analysis used paired sample t-test, wilcoxon test, and mann whitney test. Results: There was a significant difference between knowledge before health education and after health education (p = 0.000). In cadre empowerment, there was an increase in the percentage of cadre ability and the mean of cadre knowledge. In tertiary prevention, there was a significant difference between blood pressure after giving star fruit juice therapy between the intervention and control groups (p < 0.05). Conclusion: Health system guidance is effective in improving health management of hypertension.

Abstract

Abstrak Perilaku kesehatan lansia berkaitan dengan kemampuan individu merefleksikan serta mengontrol perilaku mereka. Penelitian ini bertujuan mengidentifikasi hubungan refleksi diri dengan pengendalian diri pada lansia di Panti Sosial Tresna Werdha (PSTW). Penelitian ini menggunakan desain cross-sectional dengan melibatkan 113 lansia yang diseleksi dengan menggunakan Teknik Simple Random Sampling. Brief Self-Control Scale (BSCS) dan The Self-Rection and Insight Scale (SRIS) digunakan untuk mengukur pengendalian diri dan refleksi diri pada lansia. Uji Somers’d dan Spearman-Rank digunakan untuk menganalisis hubungan antara variabel. Hasil penelitian ini menunjukkan sebagian besar lansia memiliki refleksi diri dan pengendalian diri dalam tingkat sedang yaitu 59.3% dan 66.4%. Jenis kelamin berhubungan signifikan dengan self-reflection (p-value 0.033, r=-0.201), dan riwayat penyakit pada lansia berhubungan signifikan dengan pengendalian diri (p-value 0.015, r=-0.211). Uji Spearman-Rank menunjukkan bahwa terdapat hubungan yang kuat dan signifikan antara refleksi diri dengan pengendalian diri pada lansia (p-value 0.000, r=0.687). Semakin tinggi refleksi diri pada lansia semakin tinggi pula pengendalian diri mereka. Refleksi diri dan pengendalian diri berkaitan dengan perilaku lansia termasuk bagaimana lansia menyadari perilaku mereka, belajar dari pengalaman, serta mengatur perilaku agar sesuai dengan tujuan kesehatan yang ingin dicapai. Oleh karena itu, perlu adanya peningkatan refleksi diri dan pengendalian diri pada lansia khususnya di lansia di PSTW.  Kata kunci: refleksi diri, pengendalian diri, lansia, Panti Sosial Tresna Werdha   Abstract Health behavior among elderly is related to an individual's ability to reflect on and control their behavior. This cross-sectional study involved 113 elderly selected using Simple Random Sampling. This study showed that most older people had moderate levels of self-reflection and self-control, 59.3% and 66.4%, respectively. Gender was significantly correlated to self-reflection (p-value 0.033, r=-0.201), and a self-reported past medical history was significantly related to self-control with a weak relationship strength (p-value 0.015, r=-0.211). The Spearman-Rank Analysis indicated a strong and significant correlation between self-reflection and self-control (p-value 0.000, r=0.687). The higher the self-reflection in the elderly, the higher their self-control. Self-reflection and self-control are related to elderly behavior, including how elderly know their behavior, learn from experience, and regulate behavior to suit the health goals they want to achieve. Therefore, there is a need to increase self-reflection and self-control in the elderly, especially in the elderly at nursing homes. Keywords: self-relection, self-control, elderly, nursing homes
