Abstract

Stroke is a non communicable disease whose prevalence is increasing in Indonesia. The results of the Riset Kesehatan Dasar (Riskesdas) in 2018 showed that an increase in the prevalence of stroke from 7% (in 2013) to 10.9% with the highest proportion was in East Kalimantan (14.7%) and the lowest proportion was in the Papua region (4.1%). The increased prevalence is related to unhealthy lifestyles. Besides that hypercholesterolemia (high cholesterol levels in the blood) is also a risk factor for stroke which if not handled properly will lead to atherosclerotic plaque formation in blood vessels. This plaque can block the flow of blood that carries oxygen to the brain so that it can cause a stroke. One type of plant in Indonesia that has benefits in lowering cholesterol levels is Aloe vera. There are many ingridients of Aloe vera such as amino acids, salicylic acids, anthraquinons, enzymes, magnesiums, zincs, minerals, vitamins (A, C, E), lignins, saponins, phytosterols, monosaccharides, and polysaccharides (glucomannans and acemannans). Based on several studies, Aloe vera can reduce blood cholesterol levels with saponins, phytosterols, and glucomannans compound that have a direct role in lowering cholesterol levels. Saponin will inhibit the absorption of cholesterol in the gastrointestinal tract and increase its excretion, phytosterols will bind cholesterol and prevent cholesterol from being absorbed by the gastrointestinal tract, whereas glucomannan which is fiber will absorb water and inhibit the process of recycling bile salts so that only a few will return to the liver and more excreted through feces. Reduced blood cholesterol levels due to Aloe vera will have a good effect on atherosclerotic plaque so as to reduce the risk of stroke.

Abstract

Dyslipidemia or hyperlipoproteinemia is a quantitative change in the concentration of total cholesterol, its fraction, or just triglycerides in plasma. In addition, there is ample evidence to support that pediatric dyslipidemia is associated with atherosclerosis in adulthood. To reduce cardiovascular risk in adulthood, early identification and treatment is needed to reduce morbidity and mortality. Dyslipidemia itself can occur due to changes in primary lipoprotein metabolism due to genetic (primary) causes or other pathologies such as the result of accumulation of exogenous (secondary) factors. Therefore, mixed dyslipidemia results from genetic and environmental associations with risk factors for cardiovascular disease. However, lipid metabolism screening in pediatrics is not carried out in a targeted manner, causing problems where the outcome is not generally accepted. In addition, no one knows the effect of long-term pharmacological therapy. Therefore, this study aimed to review the various available literature resources on pediatric dyslipidemia to present data on a structured approach to pediatric dyslipidemia that focuses on screening, atherosclerotic risk stratification, and therapeutic approaches.

Abstract

Excessive consumption of monosodium glutamate (MSG) has a neurotoxic effect on the cerebellum, which will lead to a state of oxidative stress characterized by decreased activity of catalase. This study aims to determine the effect of Lampung Robusta Coffee extract (Coffea canephora) againsts to activity of the cerebellar catalase enzyme in male rats (Rattus norvegicus) strain sprague-dawley induced by monosodium glutamate. This research is a type of experimental study with a Posttest Only Control Group Design approach. The subjects were 25 rats which were divided into 5 groups which are K- (aquadest 3,5 ml/day), K+ (MSG 4 g/kgBW/day), P1, P2, P3 (MSG 4 g/kgBW/day and Lampung Robusta Coffee extract 1,5 ml/200gBW/day with a concentration of 0,03 g/ml; 0,06 g/ml; 0,12 g/ml respectively) for 14 consecutive days. The activity of the cerebellar catalase enzyme is calculated with spectrophotometer 210 nm. The average specific activity of the cerebellum catalase enzyme in K +, K-, P1, P2 and P3 were 0.00104 U/mg, 0.00154 U/mg, 0.00139 U/mg, 0.00161 U/mg, and 0.00190 U/mg respectively. One-Way ANOVA test obtained p value = 0.001 (p <0.05). Post Hoc LSD test showed a significant difference (p <0.05) between the K + and K- (p = 0.007), K+ and P2(p = 0.003), K + and P3 (p = 0.000), and P1 and P3 (p = 0.007), which means that there is an effect of Lampung Robusta Coffee extract (Coffea canephora) againsts to activity of the enzyme catalase of cerebellar in male rats (Rattus norvegicus) strain Sprague-dawley induced by MSG.
