Abstract

Ventilasi mekanik jangka panjang (Long-Term Mechanical Ventilation/LTMV) merupakan salah satu bentuk terapi penunjang kehidupan yang diberikan kepada pasien dengan gangguan pernapasan kronis. Seiring meningkatnya penerapan Home Mechanical Ventilation (HMV), berbagai manfaat telah dilaporkan, seperti peningkatan kualitas hidup pasien serta berkurangnya frekuensi dan durasi perawatan di rumah sakit. Namun demikian, penggunaan ventilasi mekanik dalam jangka panjang juga menghadirkan tantangan yang berkaitan dengan keselamatan pasien dan mutu pelayanan kesehatan. Dalam kondisi ini, perawat memegang peranan penting melalui pemantauan kondisi pasien, pencegahan komplikasi, pemberian edukasi kepada keluarga, serta koordinasi pelayanan secara berkesinambungan. Kajian ini bertujuan untuk mengidentifikasi, menelaah, dan mensintesis bukti ilmiah mengenai manajemen keperawatan pada pasien pengguna ventilasi mekanik jangka panjang dengan penekanan pada aspek keselamatan pasien dan kualitas pelayanan. Metode yang digunakan adalah Systematic Literature Review (SLR) mengacu pada pedoman PRISMA. Penelusuran literatur dilakukan pada basis data PubMed, ScienceDirect, ProQuest, dan Google Scholar terhadap artikel yang diterbitkan pada tahun 2021–2024. Proses seleksi berdasarkan kriteria inklusi dan eksklusi menghasilkan sepuluh artikel yang memenuhi syarat untuk dianalisis. Hasil sintesis menunjukkan empat tema utama, yaitu keselamatan pasien pada pengguna LTMV, pemantauan dan pencegahan komplikasi, mutu pelayanan dan keberlanjutan perawatan, serta kontribusi perawat dalam pelaksanaan HMV. Berbagai strategi seperti pemantauan berkelanjutan, pemanfaatan telemonitoring, penggunaan indikator mutu, edukasi bagi caregiver, dan kerja sama multidisiplin terbukti mendukung peningkatan keselamatan pasien serta mutu pelayanan. Dengan demikian, manajemen keperawatan memiliki kontribusi yang signifikan dalam mengoptimalkan luaran pasien melalui peningkatan kompetensi perawat, sistem pemantauan yang terstandar, dan keterlibatan aktif keluarga dalam proses perawatan.

Abstract

Patients with social isolation are often found in psychiatric nursing practice, especially those with mental disorders such as schizophrenia, depression, or social anxiety disorder. Social isolation is someone who avoids social interaction and has difficulty communicating or building significant relationships with others. This condition greatly affects the patient's quality of life and worsens the symptoms of mental disorders experienced. The purpose of this concept analysis is to gain a more detailed and in-depth understanding of Social Skill Training in Socially Isolated Patients.This concept analysis uses the Walker and Avant approach with analysis steps including: utilization of attributes in case models, borderline cases, related cases, and opposite cases, explaining antecedents and consequences. Relevant literature findings related to the concept were obtained through concept databases such as Google Scholar, PubMed, Scopus, Science Direct, NCBI, Researchgate, Publish and Perish and ProQuest. Keywords used in the literature search include "Social Skill Training", "Social Isolation", and "Scizophrenia". The inclusion criteria for articles were articles written in English and Indonesian, full text, and articles published in the last 5 years from 2019 - 2024 and open access.The results Social Skill Training(SST) is effective in improving social skills, reducing symptoms of social isolation, and improving the quality of life of patients with mental health disorders. In the context of social isolation, SST provides a structured approach to help patients develop communication skills, manage emotions, and increase their confidence in social interactions..It is expected Nurses should use SST as a standard intervention in treating patients with social isolation, especially those caused by mental disorders such as schizophrenia, depression, and social anxiety.

Abstract

Indonesia faces a significant challenge with a high population growth rate that affects development quality and family welfare. One solution is to increase male participation in the family planning program through contraceptive methods such as vasectomy. However, the acceptance rate of vasectomy is still low due to various factors. This study analyzes the factors influencing vasectomy acceptance by identifying its attributes, antecedents, and consequences. This study used Walker and Avant's concept analysis approach. The literature analyzed came from various databases, such as PubMed and ProQuest, with publications from 2019 to 2024. A total of 129 articles were found, and 24 articles met the inclusion criteria for further review. The analysis identified four main attributes that influenced the decision to choose vasectomy: knowledge, wife support, economic factors, and planning for the number of children. Antecedents such as age, education, and occupation shaped the level of knowledge, which, together with wife support and economic factors, influenced the family's decision to choose vasectomy. Consequently, vasectomy is accepted as an effective, safe, and cost-effective family planning method. This analysis highlights the need for comprehensive information and support to enhance men's participation in family planning programs through vasectomy.

Abstract

Coping mechanisms greatly help schizophrenia patients deal with psychotic symptoms such as hallucinations, delusions, and disorganized thoughts, as well as manage the emotional and social effects of their illness. The purpose of this concept analysis is to gain a more detailed and in-depth understanding of the use of coping mechanisms as part of schizophrenia therapy. This concept analysis uses the Walker and Avant approach with relevant literature findings related to the concept were obtained through concept databases such as Google Scholar, PubMed, Scopus, etc. Keywords used in the literature search include "Coping mechanisms", and "Scizophrenia". The inclusion criteria for articles were articles written in English and Indonesian, full text, and articles published in the last 5 years from 2019-2024 and open access as many as 7 articles. The results Adaptive coping mechanisms, such as problem solving and seeking social support, can help patients deal with stress, reduce symptoms, and improve their quality of life. Conversely, maladaptive coping strategies, such as avoidance or denial, actually risk worsening the patient's condition, increasing social isolation, and increasing the likelihood of symptom recurrence. It is hoped that more in-depth education about effective coping mechanisms needs to be included in nurse training and patient counseling.

Abstract

Pulmonary Tuberculosis (TB) is an infectious disease caused by Mycobacterium Tuberculosis, this disease becomes a public health problem, especially if there is a break in treatment which has an impact on TB drug resistance. The purpose of this study is to see the correlation or relationship of determinant factors that influence the success of pulmonary tuberculosis treatment. The type of research is Observational Analytical with a Cross Sectional approach. Data collection uses questionnaires, observations and interviews. The population is 158 and the sample is 91 respondents with cluster sampling. The study was conducted in Parigi Moutong Regency during February-June 2025. Of the 91 respondents who were 94.5% compliant with taking medication, 75.8% had good Family Support, 75.8% had Drug Supervisor (PMO) Support and 83.5% had Health Worker Support, the results of the chi square test showed a relationship between medication adherence and the success of Pulmonary TB Treatment p = 0.000 <0.05, there was a relationship between family support and the success of Pulmonary TB Treatment p = 0.001 <0.05, there was a relationship between support (PMO) and the success of Pulmonary TB Treatment p = 0.001 <0.05 and there was a relationship between Health Worker support and the success of Pulmonary TB Treatment p = 0.000 <0.005. Conclusion There is a relationship between behavior (medication adherence, family support, PMO support and health worker support) and the success of pulmonary TB treatment.

Abstract

Kurang tidur merupakan masalah kesehatan masyarakat global yang berdampak signifikan terhadap fungsi kardiovaskular dan neurologis. Durasi tidur yang tidak adekuat (<7 jam/malam) terbukti meningkatkan risiko hipertensi dan microsleep, dua kondisi yang berimplikasi serius terhadap keselamatan dan kesejahteraan. Pemahaman konseptual yang komprehensif diperlukan untuk mengklarifikasi atribut, anteseden, serta konsekuensi kurang tidur dalam konteks praktik keperawatan. Penelitian ini menggunakan pendekatan concept analysis berdasarkan kerangka Walker dan Avant. Pencarian literatur dilakukan secara sistematis melalui tiga basis data utama—PubMed, ScienceDirect, dan Google Scholar—pada periode September hingga November 2024, dengan kombinasi kata kunci “sleep deprivation”, “short sleep duration”, “hypertension”, dan “microsleep”. Dari 287 artikel yang ditemukan (95 PubMed, 112 ScienceDirect, 80 Google Scholar), sebanyak 45 artikel memenuhi kriteria inklusi setelah proses skrining bertahap (title/abstract, full-text), eliminasi duplikasi, dan penilaian kualitas menggunakan Newcastle–Ottawa Scale serta AMSTAR 2. Analisis mengidentifikasi empat atribut utama kurang tidur, yaitu durasi tidur inadekuat, ketidaksesuaian fisiologis, pola temporal akut atau kronis, dan akumulasi sleep debt. Anteseden utama mencakup faktor okupasional, perilaku, dan lingkungan. Kurang tidur berkontribusi terhadap peningkatan risiko hipertensi sebesar 20–32% melalui aktivasi simpatis dan disregulasi aksis HPA, serta meningkatkan frekuensi microsleep setelah 17–19 jam terjaga. Kurang tidur merupakan faktor risiko yang dapat dimodifikasi dengan implikasi klinis signifikan pada kesehatan kardiovaskular dan keselamatan. Temuan ini menegaskan pentingnya peran perawat dalam deteksi dini, edukasi higiene tidur, dan pengembangan intervensi multi-level berbasis bukti untuk pencegahan dan pengelolaan risiko.

Abstract

Discharge planning merupakan komponen krusial dalam transisi perawatan pasien dari rumah sakit menuju rumah, terutama pada pasien post-Ureterorenoscopy (URS) yang memiliki risiko komplikasi seperti nyeri, hematuria, infeksi, dan keluhan terkait ureteral stent. Namun, berbagai penelitian internasional maupun nasional menunjukkan bahwa implementasi discharge planning masih belum optimal, dipengaruhi oleh rendahnya kepatuhan tenaga kesehatan, kurangnya komunikasi, keterbatasan tenaga, serta minimnya edukasi spesifik prosedur yang diberikan kepada pasien. Penelitian ini bertujuan menganalisis konsep discharge planning pada pasien post-URS. Penelusuran literatur dilakukan pada PubMed, CINAHL, ScienceDirect, Google Scholar, serta pedoman EAU dan AUA periode 2022–2025. Dari 156 artikel yang diidentifikasi, 128 artikel diseleksi berdasarkan judul dan abstrak, dan 49 artikel ditelaah secara penuh. Sebanyak 17 artikel memenuhi kriteria inklusi dan dianalisis secara konseptual menggunakan kerangka Walker & Avant dengan pendekatan analisis tematik untuk mengidentifikasi definisi konsep, atribut karakteristik, antecedents, dan consequences discharge planning pasca-ureteroskopi (URS). Hasil analisis mengidentifikasi definisi konsep, atribut karakteristik, antecedents, dan konsekuensi klinis discharge planning pada konteks URS. Atribut utama meliputi asesmen individual, edukasi terarah sesuai prosedur, koordinasi multidisiplin, perencanaan tindak lanjut, dokumentasi komprehensif, serta keterlibatan pasien dan keluarga. Discharge planning yang efektif terbukti meningkatkan keselamatan pasien, mengurangi risiko readmisi, menurunkan kejadian komplikasi, serta memperkuat kontinuitas asuhan pasca-operasi. Penelitian ini menegaskan bahwa discharge planning pada pasien post-URS harus dilakukan secara terstruktur sejak hari pertama perawatan untuk mendukung pemulihan optimal dan mencegah dampak klinis yang tidak diinginkan.

Abstract

Diabetes Mellitus (DM) merupakan penyakit kronis yang terus meningkat prevalensinya di seluruh dunia. Salah satu hambatan utama dalam pengelolaan DM adalah rendahnya kepatuhan pasien terhadap jadwal pemberian insulin. Penelitian ini bertujuan untuk mengevaluasi efektivitas sistem pengingat berbasis aplikasi ponsel pintar dan alarm pintar dalam meningkatkan kepatuhan terapi insulin pada pasien diabetes mellitus. Penelitian menggunakan metode kuantitatif deskriptif dengan pendekatan cross-sectional. Pengambilan sampel dilakukan dengan teknik purposive sampling, melibatkan 30 pasien diabetes mellitus yang menggunakan insulin. Hasil penelitian menunjukkan bahwa penggunaan alarm pintar dan aplikasi pengingat secara signifikan meningkatkan kepatuhan pemberian insulin, tercermin dari peningkatan nilai time-in-range (TIR) dan penurunan kadar HbA1c secara konsisten pada mayoritas peserta. Tingkat kepatuhan terhadap jadwal yang direkomendasikan sistem mencapai angka yang tinggi tanpa adanya peningkatan kejadian hipoglikemia. Penelitian ini menyimpulkan bahwa integrasi teknologi digital berupa aplikasi dan alarm pintar terbukti efektif sebagai alat bantu manajemen mandiri diabetes, dan berpotensi menjadi komponen penting dalam rencana perawatan holistik pasien diabetes mellitus.

Abstract

Spiritual Wellbeing (SWB) is essential in palliative care for end-of-life (EoL) patients. However, its implementation in emergency settings is often overlooked despite the significant spiritual needs of patients at the end of life. This study aims to describe and explain the concept of SWB in EoL patients in the Emergency Setting. The study used a qualitative approach based on concept analysis of the literature. Data were collected from 18 relevant articles purposively selected from international databases (2020-2024). The analysis focused on the attributes, antecedents, and consequences of SWB through thematic evaluation and cross-referencing. The results showed that four main attributes of SWB were identified: relationship with higher power, inner peace, meaning of life, and active role of family and health workers. Antecedents of SWB included clergy and family support, while consequences included improved patient quality of life and reduced anxiety. These findings demonstrate the importance of integrating spiritual care into emergency setting practice. Holistic collaboration between patients, families and healthcare professionals is key to improving the spiritual wellbeing of EoL patients.

Abstract

ABSTRACT Ventilator weaning is a critical stage in the management of patients receiving mechanical ventilation, contributing to the reduction of complications, length of hospital stay, healthcare costs, and mortality rates. The growing implementation of nurse-led ventilator weaning has increased the volume of scientific publications, highlighting the need for a comprehensive mapping of research developments in this field. This study aimed to analyze global research trends, scientific collaboration patterns, and thematic developments related to nurse-led ventilator weaning among patients with chronic diseases between 2000 and 2026. A bibliometric approach was employed using publications retrieved from the Dimensions database. The identification and selection process followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) framework. A total of 19,589 publications were initially identified, and after the screening process, 7,988 publications were included for analysis. Bibliometric analyses were conducted using VOSviewer version 1.6.20, including publication trend analysis, research field distribution, keyword co-occurrence analysis, network visualization, overlay visualization, and density visualization. The results demonstrated a dynamic development of publications related to nurse-led ventilator weaning throughout the study period. The dominant research fields were Biomedical and Clinical Sciences (7,722 publications), Health Sciences (5,466 publications), Clinical Sciences (2,324 publications), and Nursing (930 publications). Keyword co-occurrence analysis identified three major research clusters focusing on mechanical ventilation and the weaning process, patient safety and clinical outcomes, and evidence-based practice alongside clinical guideline development. Overlay visualization revealed a shift in research focus from the technical aspects of mechanical ventilation toward evidence-based practice implementation, patient safety, and quality improvement in critical care nursing. This study highlights the growing importance of nurse-led ventilator weaning as a multidisciplinary research area with substantial contributions to evidence-based critical care nursing practice. Sebanyak 19.589 publikasi teridentifikasi dan setelah proses penyaringan diperoleh 7.988 publikasi yang dianalisis menggunakan perangkat lunak VOSviewer versi 1.6.20. Analisis meliputi tren publikasi, distribusi bidang penelitian, keyword co-occurrence, network visualization, overlay visualization, dan density visualization. Hasil penelitian menunjukkan bahwa publikasi mengenai nurse-led ventilator weaning mengalami perkembangan yang dinamis selama periode pengamatan. Bidang penelitian didominasi oleh Biomedical and Clinical Sciences (7.722 publikasi), Health Sciences(5.466 publikasi), Clinical Sciences (2.324 publikasi), dan Nursing (930 publikasi). Analisis kata kunci mengidentifikasi tiga klaster utama penelitian yang berfokus pada ventilasi mekanik dan proses weaning, keselamatan pasien serta luaran klinis, dan praktik berbasis bukti beserta pengembangan pedoman klinis. Overlay visualization menunjukkan pergeseran fokus penelitian dari aspek teknis ventilasi mekanik menuju implementasi evidence-based practice, keselamatan pasien, dan peningkatan mutu pelayanan keperawatan kritis. Penelitian ini menunjukkan bahwa nurse-led ventilator weaning berkembang sebagai area penelitian multidisiplin dengan kontribusi yang semakin besar terhadap pengembangan praktik keperawatan kritis berbasis bukti. Temuan ini dapat menjadi dasar untuk pengembangan penelitian lanjutan terkait penyakit kronis spesifik, integrasi teknologi digital, dan pelayanan ventilasi berbasis komunitas. Kata Kunci: Bibliometric Analysis, Weaning ventilator, Mechanical Ventilation, Critical Care Nursing, Nurse-Led Weaning.
