Abstract

The atmosphere of treatment in the isolation room is scary for Covid-19 patients, especially at the beginning of the pandemic, plus there is "stigma" in the community and the patient must be alone without any family who dares to accompany during hospitalization. All of these conditions make a heavy burden on the patient's psyche. The community service activities are provided mainly using WhatsApp media, as a telehealth service of communication information and education (namely T-health-KIE). The service was conducted from May to September in the year 2021, aimed to provide psychological support and offer correct information about the patient's illness, as well as other relevant information. Patients are also given a package containing snacks and toiletries/body care products as patients’ needs during treatment in the isolation room. The package given is received directly by the patient or the patient's family representative at the hospital. The T-health-KIE has conducted among 59 patients in an isolation room at a private hospital in Bali, consisting of 33 male and 26 female patients, with an age range of 19 to 78 years old. Fortunately, is that there has been a decreased anxiety of patients who threatened in isolation rooms in the 2nd year of the pandemic compared to the previous year (at the beginning of the Covid-19 pandemic). The T-health-KIE service is an innovative and safe health service for health workers during the Covid-19 pandemic. In addition to being useful for patients, it also helps to ease the burden of doctors with a heavy workload in handling Covid-19 patients. Abstract

Background and purpose: Drug dispensing errors have a major impact on the incidence of medication errors, and can be very dangerous to patients and affect the quality of care. Root cause analysis (RCA) is an approach to prevent dispensing errors by identifying the root of the problem, make efforts to improve comprehensively across departments so that incidence of dispensing errors are not repeated. The purpose of this study is to determine the differences in the incidence of dispensing errors before and after the RCA. Methods: This is a descriptive study by calculating the incidence of dispensing errors before and after the RCA, conducted in four inpatient wards at Sanglah Hospital Denpasar. Dispensing errors were categorized into ten types namely errors in drug’s identity, name, dosage, number of drugs, absence of high alert labels, incorrect drug preparations, incorrect strength, expired drugs, incorrect place of delivery and incorrect instructions. In February and April 2018 there were 80 and 40 dispensing errors which met the study criteria, respectively. Results: Crude incidence of dispensing errors before and after RCA were 2.54 per 1000 (95%CI: 1.98-3.10) and 1.26 per 1000 (95%CI: 0.87-1.65), respectively. The decrease in dispensing error after RCA was 50.39%. The ratio of dispensing error incidence rates after and before the RCA is 0.49. Significant decrease in the incidence of dispensing errors was only found in drug identity errors of 1.27 per 1000 (95%CI: 0.88-1.66) before an RCA and 0.06 per 1000 (95%CI: -0.03-0.15) after RCA, or decreased by 95.27% and there was no significant decrease in other dispensing errors. Conclusion: The decrease of dispensing errors after the RCA was only found in the medication identity error and not found in the other dispensing errors. Direct observation and intensive supervision are needed in order to accurately calculate and reduce the incidence of dispensing errors.

Abstract

Stigma toward mental illness is a complex construct incorporating personal, social, and structural. One of the social components that might significantly comprise stigma is the cultural aspect. Asian culture, generally, and Balinese culture, especially, is heavily embedded with the practice of paternalism which is a part of sexism. The practice of sexism as one form of discrimination might imply a high tendency to stigmatize people with mental illness. Therefore, it would be relevant to discuss the possible correlation between stigma and mental illness within the context of the predominant sexism-loaded paternalistic culture in Bali. This literature review covers several subtopics, including stigma and discrimination, sexism and its correlation with mental illness, and cultural aspects that might influence stigma concluded with recommendations for culturally sensitive mental health practice.

Abstract

Anxiety in health care workers can be detrimental to health care. Support from family, loved ones, workplaces and the environment is important for nurses because people under stress need advice, solutions and care to ease their worries about the COVID-19 situation. This study aims to determine the effects of social support and non-physical work environment on the anxiety of nurses during the COVID-19 pandemic at Tabanan Hospital. This study used a cross-sectional method of observational study design. This study will be conducted at Tabanan hospital from May to June 2022 with 82 nurses selected from the entire sample. Different topics include social support and non-physical work and anxiety for nurses during the COVID-19 pandemic. Data were collected from questionnaires and analyzed using logistic regression analysis with 95% confidence intervals. RESULTS: In this study, nurses in physical workplaces were found to experience stress 21 times (OR=21.482, p-value=<0.001) compared to nurses in physical workplaces. Nurses with positive social support experienced 7 times less anxiety than nurses with positive social support (OR=6.823, p value=<0.010).The prevalence of nursing anxiety during the COVID-19 outbreak at Tabanan Hospital is 64.6%. Social support and work environment were not physical variables related to anxiety (p<0.05). The prevalence of anxiety among nurses during the COVID-19 pandemic in Tabanan Hospital is 64.6%, which is 58.5% mild anxiety, 6.1% moderate anxiety. Social support variables and non-physical work environment variables influence anxiety (p < 0.05). no less than 92.9% of nurses with good social support had no fear. no less than 89.3% of nurses with a good non-physical work environment had no fear. Nurses with a sufficiently demanding physical work environment experienced anxiety 21 times (OR = 21.482, p value = <0.001) compared to nurses with a good physical work environment. Nurses with adequate social support were less depressed and experienced anxiety 7 times (OR = 6.823, p value = <0.010) compared to nurses with good social support

Abstract

Background and purpose: Leadership style, work environment and motivation of nurses are related to the implementation of nursing care standards. This study aims to determine factors associated with the implementation of nursing care standards in the hospital’s inpatient services.  Methods: A cross-sectional study was conducted with all of the total 65 nurses working in the inpatient wards of the Udayana University Hospital, Bali in December 2022. Data were collected with a structured questionnaire covering nurses’ characteristics, leadership style, work environment, motivation and implementation of nursing care standards. Data were analyzed descriptively and analytically through path analysis assisted by a statistical software. Results: Of the 65 nurses, 69.2% were aged 26-30 years, 64.5% were female, 75.5% have worked under 3 years, and 75.4% have a bachelor's degree in nursing. There is a relationship between motivation (with a path coefficient of 0.603, p<0.001) and supportive leadership style (with a path coefficient of 0.615, p=0.028) with the implementation of standard nursing care. In addition, supportive leadership style (path coefficient 0.554, p=0.002), participative leadership style (path coefficient -0.353, p=0.061) and achievement-oriented leadership style (path coefficient 0.426, p=0.002) were associated with work motivation. Conclusion: Nurse motivation is directly associated, while supportive leadership style is indirectly related to the implementation of nursing care standards through increasing nurse motivation. Supportive, participatory leadership styles and achievement orientation are also directly related to nursing motivation. The hospital leaders and room manager must maintan and strengthen the participatory leadership style which will improve motivation and nursing care standard.

Abstract

Schizophrenia is a serious concern in psychiatric problem around the world. In 2018, this disorder has affecting 6.7 per 1000 incidence in Indonesia. Pharmacology and non-pharmacology treatment are recommended treatment for schizophrenia, those treatments will run effectively by supports from their family caregiver. Nowadays, family play an important role in caregiving schizophrenia patients. However, the recent status quo stated that not all caregiver understands how to take care the patients in a proper way. So that, caregiver need some education and other information to understand the schizophrenia patient. Based on Ozkan and Khatri research, providing psychoeducation data via technology (telehealth) to the family caregiver shows a positive and significant result. In reducing family caregiver burden of the patients with schizophrenia, Coaching Caregiver is an innovation to train family caregiver in taking care their family. Some aspects in this program based on DSM V and PPDGJ III such as education, consultation and decision support, psychosocial and behaviour therapy, social support, and monitoring that needed by a caregiver. Using a technology-based approach, this application expected to provide and encourage health services to reach a wider community. In addition, it is expected to reduce the burden of family caregiver in taking care their relatives/family.

Abstract

ABSTRACT Gelahang marriage is a unique marital system in Balinese society that allows couples to maintain responsibilities toward both of their families. While this system offers benefits such as cultural heritage preservation and social support, it also presents significant psychological challenges. This study reviews the psychiatric impact of Pada Gelahang marriage, focusing on dual-role stress, identity conflicts, and the risks of anxiety and depression among couples in this system. Additionally, it explores psychodynamic perspectives and psychological coping strategies to mitigate stress in dual-family dynamics. Findings indicate that while Pada Gelahang marriage enhances cultural and social resilience, appropriate psychological interventions are essential to reduce emerging mental health risks. ABSTRAK Pernikahan Pada Gelahang merupakan bentuk perkawinan unik dalam masyarakat Bali yang memungkinkan pasangan untuk tetap memiliki tanggung jawab terhadap keluarga masing-masing. Meskipun sistem ini menawarkan manfaat dalam hal pelestarian warisan budaya dan dukungan sosial, ia juga menimbulkan tantangan psikologis yang signifikan. Studi ini meninjau dampak psikiatri dari pernikahan Pada Gelahang dengan menyoroti tekanan peran ganda, konflik identitas, serta risiko kecemasan dan depresi pada pasangan yang menjalani sistem ini. Selain itu, artikel ini juga mengeksplorasi perspektif psikodinamik dan strategi penanganan psikologis yang dapat diterapkan untuk mengurangi stres dalam dinamika keluarga ganda. Temuan menunjukkan bahwa meskipun pernikahan Pada Gelahang dapat meningkatkan resiliensi budaya dan sosial, intervensi psikologis yang tepat sangat diperlukan untuk mengurangi risiko kesehatan mental yang muncul.
