Abstract

Introduction: House dust mite (HDM) allergy has been reported as an actual cause of asthma in children. Subcutaneous immunotherapy (SCIT) is a recommended treatment for HDM allergy patients. However, there were limited data about the characteristic of pediatric patients with HDM-SCIT, particularly in Indonesia. This study was aimed to evaluate the characteristic of pediatric patients with allergic asthma. Methods: Study participants were pediatric patients confirmed with HDM allergy from Skin Prick Test (SPT) in the pediatric allergy-immunology outpatient clinic in Saiful Anwar Hospital Malang, Indonesia. Patients who were treated with HDM-SCIT in the early build-up phase of treatment were included in this study. Demographic and clinical characteristics of the patients were recorded. Peripheral blood samples were drawn to evaluate the total eosinophil count (TEC), total basophil count (TBC), neutrophil-lymphocyte ratio (NLR), specific IgE (sIgE) and total IgE (t-IgE) level. Clinical diagnosis of asthma was classified according to the Global Initiative for Asthma (GINA) criteria. The evaluation of asthma control was assessed by the Asthma Control Test (ACT) score. Results: Thirty-two patients were enrolled in this study, including 14 male and 18 female. The mean age of children was 6.92 ± 2.60 years old. There were 21 subjects with uncontrolled asthma and 11 subjects with partially controlled asthma. Demographic characteristics including age, sex, nutritional status and family history were not significantly different between uncontrolled and partially controlled asthma groups (p> 0.05). TEC, TBC, NLR, and tIgE were not different significantly among groups (p> 0.05). This study showed that the mean of sIgE serum level was higher in uncontrolled asthma compared to partially controlled asthma group (p= 0.022). Moreover, it was a negative significant correlation between sIgE serum level and ACT score (p= 0.002, r= -0.532). Conclusion: Higher sIgE levels were correlated with poor asthma control in HDM-SCIT patients.

Abstract

Introduction: The prospective studies have estimated that the DRESS syndrome incidence rate is about 1 per 1000 to 10,000 with the mortality rate is to be around 10% to 20%. Case: We have been reported a case of DRESS in an eleven-year-old boy who came to the emergency room with the chief complaint of maculopapular rash in the body that had been happened for three days. The rash was first appeared in the arms area and thus spread to the trunk, abdomen, and lower extremities. The erythema was well-demarcated, purplish in color. There was a history of fever a week before admission which reached 40 0 Celcius. There are multiple histories of drug administration, either orally or intravenously to this patient, and made the diagnosis of drug reaction is highly possible. The management of these patients consists of suspending suspected drugs and implementing supportive measures. Conclusion: Regarding pharmacological treatment, the use of systemic corticosteroids for a prolonged period with a gradual decrease is suggested, to avoid relapses.

Abstract

Impaired growth is still a problem that needs attention in Indonesia. There are still many incidents of children with short stature due to lack of nutrition, undernourished toddlers, and even malnutrition. Therefore, the detection of child growth still needs to be improved. The Covid-19 pandemic has made parents of patients afraid and anxious to take their children to health facilities, so that monitoring of their growth is neglected, especially in pediatric patients with chronic diseases. This community service activity is intended to provide training to parents of patients on how to measure anthropometric status in pediatric patients with chronic diseases so that parents are expected to be able to screen or detect growth disorders early and independently. Therefore, education and training are held with the aim of increasing the knowledge and skills of parents in examining the anthropometric status of children. Evaluation is done by asking questions and practicing anthropometric status checks at the beginning and end of the activity and good numbers of material uptake by participants are obtained so that participants can independently assess the child's anthropometric status and get an increase in skills. The knowledge of the patient's parents about awareness of growth and development screening is still lacking so that it can affect the detection of developmental disorders in children. With this activity, it can provide the ability of the patient's parents to equip themselves to monitor the growth of their child.

Abstract

Vitamin D (25-Hydroxyvitamin D3 [25(OH)D3]) has an important role in the immune system. This study aimed to assess the relationship between 25-Hydroxyvitamin D3 and disease activity in Indonesian children with Juvenile Idiopathic Arthritis (JIA) during pandemic Covid-19. A 12-week randomized control trial was undertaken at Saiful Anwar Hospital between June and September 2021. JIA patients are divided into two groups. The control group (CG), without supplementation, and the treatment group (TG) got a high dose of oral cholecalciferol 2000 IU/day. Serum levels of 25(OH)D3 were measured using an enzyme-linked immunosorbent assay (ELISA). Comparison between serum 25(OH)D3 levels and JIA subtypes, peripheral blood Creactive protein (CRP), erythrocyte sedimentation rate (ESR), and Juvenile Arthritis Disease Activity Score (JADAS-27 Score) were analyzed using SPSS. There was a significant increase in serum 25(OH)D3 levels in treatment group (mean: 27.38 ± 6.39 ng/ml vs 42.26 ± 10.95 ng/ml; P=0.000). The JADAS-27 score significantly decrease in treatment group (14.60 ± 4.04 vs 6.24 ± 1.96; p= 0.000). Serum 25(OH)D3 showed a significant correlation with the JADAS-27 score. The levels of 25(OH)D3 are significantly decreased in children with JIA. Decreased 25(OH)D3 levels may be associated with the etiopathogenesis of JIA. Vitamin D levels have correlations with disease activity. Adjunctive treatment of cholecalciferol improves the disease activity in JIA patients.   Keywords: Disease activity, JIA, Pandemic Covid-19, Vitamin D

Abstract

Mental health in children and adolescents with chronic illnesses is still a problem that is rarely a concern in Indonesia. There are still many incidents of children and adolescents with depression due to the length of treatment they are undergoing, disease activity, psychological conditions, and support from the environment and family. Therefore, the detection of depressive disorders in children and adolescents still needs to be improved. The Covid-19 pandemic has made parents of patients afraid and anxious to bring their children to health facilities, so monitoring of mental health has been neglected, especially in pediatric and adolescent patients with chronic illnesses. This community service activity is aimed at providing counseling regarding the signs and symptoms of depression detection. Evaluation was carried out by filling out a questionnaire using the Children Depression Inventory (CDI). The conclusion from this activity is that 20% of children and adolescents with chronic illnesses experience mild depression, so further evaluation and intervention is needed.

Abstract

The survival of cardiac arrest patients highly depends on early recognition and quality resuscitation response. Fresh nursing graduates are often the first responder of cardiac arrest patients. Self-efficacy level not only reduces the anxiety of fresh nursing graduates but also improves their possibility of performing CPR. The study was an observational analysis with a cross-sectional approach. Sampling was conducted using the purposive sampling technique with inclusion criteria and analysis using the phi coefficient test and logistic regression. Data were collected using a questionnaire link of Google Form. One hundred and twenty-three people of 154 fresh nursing graduates were obtained, with 123 people filling the questionnaire, 12 people did not fill the questionnaire, and 19 people were excluded. There was a positive relationship between mastery experience, vicarious experience, and verbal persuasion with the self-efficacy of fresh nursing graduates in performing CPR. Meanwhile, the physical and emotional state had a negative relationship with self-efficacy. The most dominant factor affecting self-efficacy was verbal persuasion. There is a significant relationship between mastery experience, vicarious experience, verbal persuasion, physical state, and emotional state with self-efficacy. The most dominant factor affecting self-efficacy is verbal persuasion. It indicates that persuasion or suggestions from surrounding people that encourage one to solve their problems and direct them to reach the goal and success significantly affect self-efficacy.

Abstract

Lupus patients and their families require adequate support to manage the disease's progression. This community service aims to help the Kupu Parahita Indonesia Foundation provide strengthened moral and material support for lupus patients and their families at the Kupu Parahita Indonesia Foundation. Results: The cinematherapy planned by the PKNM and Community Service Team received much positive feedback from patients and families. The sharing session expressed that patients and their families are grateful for the opportunity to enjoy free recreation together. Enthusiasm comes with enthusiasm to meet people with fellow lupus. . In conclusion, cinematherapy can help the Kupu Parahita Indonesia Foundation provide lupus patients and their families with more substantial moral and material support.

Abstract

Introduction: Systemic lupus erythematosus (SLE) in children can interfere with their growth and social functioning and the treatment process is often hampered by low compliance and the unmet secondary needs of the patient. The aim of this community service activity is to improve the quality of life of patients by fulfilling their secondary needs and providing free consultation and education. Methods: This community service was carried out by educating and provide free consultation for 38 SLE pediatric patients accompanied by their parents directly at RSSA, monthly for 6 months, and we emphasise topics on management and healthy lifestyle for SLE patients. We also make this opportunity as a means of monitoring patient's condition which includes disease activity through SLEDAI Score, clinical symptoms, and psychosocial support, and we complete it by providing vitamin D supplements, sunblock, and wheelchairs. Results: All the subjects in this community service were 100% girls and 50% of them had active SLE status with various clinical manifestations. 79% of patients never took vitamin D supplements and all patients did not use sunblock. There were 3 patients with neurological disorders who needed a wheelchair. We provide their secondary needs to maximize the standard therapy we have been doing. After 6 months, there was a decrease in the number of patients who missed monthly controls, and a decrease in patients SLEDAI Score indicating that their lupus activity was appropriately controlled. Conclusion: Although rarely considered, the fulfillment of secondary needs in SLE patients such as vitamin D, sunblock, and wheelchairs for patients with mobility problems is very meaningful to them, which can optimize the main therapy given and make patients routinely attend monthly controls, because they find it helpful in terms of mobility. In addition, they do not need to buy vitamin D and sunblock themselves.

Abstract

Systemic lupus erythematosus (SLE) is a chronic autoimmune disease with diverse manifestations, requiring long-term treatment that can have side effects, particularly in pediatric patients. Nigella sativa (NS) has shown potential for improving SLE symptoms due to its anti-inflammatory and immunomodulatory effects. The aim of this study was to investigate the immunomodulatory effect of N. sativa oil (NSO) on disease activity, T lymphocyte activity and inflammatory cytokine profiles in pediatric SLE patients. A randomized, double-blinded, placebo-controlled clinical trial was conducted at Saiful Anwar Hospital in Malang, Indonesia, from January 2022 to January 2023. Pediatric patients with SLE were randomly assigned to receive either one gram of NSO or a placebo containing starch in capsule form as adjunct therapy alongside their SLE primary treatment. Blood samples were collected before treatment and after eight weeks of daily capsules. Disease activity was assessed using the SLE Disease Activity Index 2000 (SLEDAI-2K); flow cytometry was used to identify T helper lymphocytes, and serum cytokine levels were measured using ELISA. The statistical analysis tests were performed to compare the outcomes between groups at baseline or after the treatment, and within-group comparisons before and after the study period, as appropriate. A total of 32 patients were included in the study. A significant decrease in the SLEDAI-2K score was observed at post-treatment in both the NSO and placebo groups (p<0.001 and p=0.025, respectively). The percentage of T helper 17 (Th17) cells was significantly reduced in both the NSO and placebo groups post-treatment compared to pre-treatment (p=0.026 and p=0.034, respectively). Conversely, the post-treatment percentage of regulatory T (Treg) cells increased significantly in both groups. A significant reduction in interleukin (IL)-2 levels was observed in the NSO and placebo groups at post-treatment compared to pre-treatment (p=0.006 and p=0.046, respectively). Additionally, there were increases in IL-4 and IL-6 serum levels in both groups at post-treatment compared to pre-treatment (p<0.05). This study highlights that although disease activity was not significantly different between NSO and placebo groups, NSO could affect the inflammatory cytokine profiles in pediatric SLE patients.

Abstract

Background: Obesity is a global epidemic caused by excessive body fat, which is increasing free fatty acids in the liver, causing oxidative stress and liver cell damage. Green tea yogurt (GTY) with encapsulated Lacticaseibacillus paracasei E1 (GTY-LpE1) might have a beneficial effect in reducing liver cell damage. This study was conducted determine GTY-LpE1 effect on superoxide dismutase (SOD) expression, malondialdehyde (MDA) expression and liver histopathology in high-fat high-fructose diet (HFFD) mice. Material and Methods: A completely randomized design (CRD) with 7 groups, including normal diet (ND) group, HFFD group, 1.3 mg/kg BW simvastatin (SIM)-administered HFFD group, 5 g/kg BW probiotic yoghurt (PY)-administered HFFD (PY), 2.5 g/kg BW GTY-administered HFFD (2.5 GTY), 5 g/kg BW GTY-administered HFFD (5 GTY), and 10 g/kg BW GTY-administered HFFD (10 GTY). The diet was given for 16 weeks, followed by oral administration of sim/yoghurt during the last 4 weeks. Mice were sacrificed and the liver was collected. SOD and MDA expression were analyzed by flow cytometry. Histopathology analysis was done by evaluating hematoxylin-eosin (HE) staining of the liver.Result: The percentage of necrotic cells were 34.55, 34.31, and 21.95%, when treated with 2.5, 5, and 10 g/kg BW with GTY-administered HFFD, respectively, these were lower than the ones in the HFFD group (69.49%). The percentage of MDA expression were 15.55, 18.69, and 22.42%, respectively, these were lower than the ones in the HFFD group as well. The percentage of SOD expression were 9.49, 7.85, and 11.11%, respectively, these were higher than the ones in the HFFD group (3.44%). Conclusion: GTY-LpE1 could decrease the number of necrotic cells in the HFFD mice livers and improve the hepatocyte damage by reducing MDA expression and enhancing SOD expression. GTY-LpE1 can be used as an alternative food to control obesity.Keywords: alginate, chitosan, encapsulation, green tea, probiotic
