Abstract

Abstract

Introduction: Paediatric Systemic Lupus Erythematosus (pSLE) is a chronic autoimmune disease that affects the quality of life of patients. Anxiety is a mental health problem that is rarely addressed in Indonesia. There are still many instances of children and adolescents with anxiety due to the duration of therapy, the severity of the disease, psychological conditions, and support from the environment and family. Therefore, the detection of anxiety disorders in children and adolescents really needs to be improved. This community service activity aims to provide counselling on anxiety detection in children with SLE. Methods: This community service has been carried out in June 2024 at Polyclinic of Dr Saiful Anwar Hospital Malang. The counselling about SLE, mental health, and the relationship between the two was done directly to the paediatric and adolescent patients with SLE and the evaluation was done through questionnaires using the Screen for Child Anxiety and Related Emotional Disorders (SCARED). Results: After the delivery of content related to SLE and mental health as well as the completion of questionnaires, from this activity there were 12.5% of children and adolescents from 32 patients with systemic lupus erythematosus disease who experienced anxiety disorders so that further evaluation and intervention were needed. Conclusion: Although the number of patients who did not experience anxiety was more than patients who experienced anxiety in this community service activity, psychological support to maintain optimal mental health is needed besides primary treatment for paediatric and adolescent patients with chronic illnesses.

Abstract

Introduction: Juvenile Idiopathic Arthritis (JIA) is the most common chronic rheumatic disease in children, characterized by persistent joint inflammation. However, early diagnosis is often challenging due to heterogeneous clinical manifestations. Among its subtypes, oligoarthritis is the most common, involving up to four joints within six months of symptom onset. This case is distinctive due to its very early onset at the age of 3 years, prolonged diagnostic delay despite multiple healthcare visits, and diagnostic challenges related to nonspecific initial symptoms. Delayed diagnosis can result in joint deformity, growth retardation, and long-term disability, highlighting the importance of early recognition and timely management. Case Presentation: A 3-year-old male child presented with progressive joint pain, morning stiffness, and recurrent joint swelling that was initially associated with a history of trauma. Symptoms had been present since the age of 1.5 years, leading to a significant delay in definitive diagnosis. Based on clinical evaluation, imaging findings, and laboratory investigations, a diagnosis of oligoarticular Juvenile Idiopathic Arthritis was established according to the International League of Associations for Rheumatology (ILAR) criteria. The diagnostic process was further complicated by trauma-associated symptom onset and involvement of the temporomandibular joint (TMJ), which underscores the need for heightened clinical suspicion. The patient received multidisciplinary management, including pharmacological therapy, physical rehabilitation, and nutritional support. Conclusion: This case illustrates the complexity of diagnosing and managing oligoarticular JIA in pediatric patients, particularly in cases with very early onset, delayed diagnosis, and TMJ involvement. A comprehensive multidisciplinary approach, along with increased clinical awareness and early diagnosis, is essential to control inflammation, preserve joint function, and prevent long-term complications in children with JIA.

Abstract

Background: rapid physical and psychological growth, especially fine motor development, characterizes the preschool period. Delays in fine motor skills can affect children's self-confidence and learning in the future. Purpose: this study aimed to examine the effect of puzzle-based educational stimulation on fine motor outcomes among preschool children using the Denver Developmental Screening Test II (DDST II). Methods: a quasi-experimental pretest–posttest control group design was employed, involving 46 children from TK Dharma Wanita 1 Keniten, Kediri, divided equally into intervention and control groups. Fine motor outcomes were assessed based on DDST II fine motor adaptive items, including tasks such as building towers, copying shapes, and drawing. Results: the fine motor development status of preschool aged children in the intervention group increased from 60.9% at pretest to 100% at posttest, whereas the control group also showed a smaller improvement from 78.3% to 95.7%. The Wilcoxon test indicated a significant improvement within the intervention group (p < 0.005). However, the Mann Whitney U test showed no significant difference between the intervention and control groups in the posttest scores Conclusion: puzzle-based stimulation effectively enhances children’s fine motor development, highlighting its potential as an engaging educational strategy in early childhood programs
