Abstract

Background: Functional status refers to the ability of an individual to perform normal daily activity required to meet basic needs, fulfill usual roles, and maintain health and well-being. The objective of this study was to evaluate the level of disability and independency of stroke patients who had undergone rehabilitation therapy as the routine activity using Instrumental Activity of Daily Living (IADLs) and basic Activity of Daily Living (BADLs). Methods: This descriptive study carried out from September to October 2014 at Neurologic outpatient clinic Dr. Hasan Sadikin General Hospital Bandung with a total 33 subjects. Barthel index and IADLs questionnaire was used as an instrumental tool. Barthel index was used to measure the level of disability and IADLs was used to measure the level of independency of an individual.Results: From the 33 patients, 20 patients completed their rehabilitation therapy; 5 patients showed a moderate disability and 15 patients showed a mild disability at the Barthel Index of ADLs. The result of IADLs showed that 7 patients who completed the therapy had moderate level of independency, and 3 patients were at the category of high level of independency.Conclusions: Most of the stroke patients have moderate disability in Barthel Index and had low level of independency in IADLs. Abstract

Background: Stroke is a disease with a high mortality rate and common cause of disability. Nutritional factors are strongly associated with this disease. Malnutrition in hospitalized patients increases the incidence of complications, prolonged the length of stay and also the cost of hospitalization. Furthermore, nutritional status of stroke patients can deteriorate during hospitalization. The prevalence of malnutrition in hospitalized stroke patients is about 6% to 62%. The objective of this study was to identify  the nutritional status of hospitalized stroke patient.Methods: This was a descriptive cross-sectional study. Population of the study was hospitalized stroke patients at Neurology Ward, Kemuning Building Dr. Hasan Sadikin General Hospital Bandung, Indonesia from August until October 2014 who meet the inclusion criteria. Nutritional status was measured objectively using Body Mass Index (BMI) and subjectively using Subjective Global Assessment (SGA) method. The collected data were processed using  frequency tabulation and percentage.Results: Twenty six hospitalized stroke patients were included in this study. The hospitalized patients with normal BMI were about 12 people (46.15%), 8 people were overweight (30.77%), 4 people were undernourished (15.39%) and 2 people (7.69%) were obese. According to SGA measurement, approximately 18 people (69.22%) were moderately malnourished, and as much as 4 people (15.39%) were in good nutrition, whereas 4 people (15.39%) were severely malnourished.Conclusions: Majority of the hospitalized stroke patients has normal BMI and moderately malnourished based on SGA. DOI: 10.15850/amj.v4n2.1090

Abstract

Background: Cognitive impairment in thalassemia patients are prevalent, therefore early detection of cognitive impairment in adult thalassemia patients is crucial for prevention. Montreal Cognitive Assessment (MoCA) is a public domain cognition screening tools that covers all cognitive domains in detecting mild cognitive impairments.Objective: To compare cognitive function between adult thalassemia patients and healty control by using Indonesia version of MoCA test (MoCA-Ina)Methods: This prospective observational analytic with case control study, compared the total scores and scores of each domain of cognition between adult thalassemia patients and healthy subjects at the Medical Hematology Oncology Clinic of Dr. Hasan Sadikin General Hospital, Bandung, Indonesia using MoCA-Ina from August to October 2018.Results: A total of 32 thalassemia subjects and 50 healthy subjects were conducted. A total of 16(50%) subjects in the case group had a value of MoCA-Ina <26, while only 1(2%) healthy control had a value of MoCA-Ina <26. The median total MoCA-Ina score in case and control groups were 25.5 and 27.50 (p <0.001). The median score of memory domains, executive functions and visuospatial of the case and control groups were 3 versus 4 (p <0.001), 3 versus 3.5 (p <0.001) and 3.53 ± 0.671 versus 3.88 ± 0.385 (p <0.003), respectively.Conclusion: Adults thalassemia patients have lower score in total MoCA-Ina, domains of memory, executive function and visuospatial score compared to healthy control.

Abstract

Hampir 10% stroke terjadi pada usia relatif muda (kurang dari 45 tahun). Angka kejadian stroke pada usia di bawah 45 tahun diperkirakan 7 – 15 kasus/100.000 penduduk/tahun dan lebih jarang pada kelompok anak-anak, yaitu 1 – 8 kasus/100.000/tahun. Faktor risiko terjadinya stroke pada usia muda berbeda-beda, antara lain: (1) Migren, (2) Sickle-cell disease, (3) Penyakit jantung, (4) Abnormalitas pembuluh darah serebral, (5) Focal cerebral arteriopathy of childhood (FCA), (6) Ensefalopati pasca-varisela, (7) Mitochondrial encephalopathy with lactic acidosis and stroke-like episodes (MELAS), (8) Diseksi arteri, (9) Penyalahgunaan obat, (10) Vaskulitis serebral seperti arteritis temporal, arteritis Takayasu, polyarteritis nodosa (PAN), Wegener’s granulomatosis, (11) Systemic lupus erythematosus (SLE), (12) Cerebral autosomal dominant arteropathy with subcortical infarcts
and leucoencephalopathy (CADASIL), (13) Fabry’s disease, dan (14) Trombofilia. Khusus pada wanita usia muda, beberapa faktor risiko lainnya antara lain: (1) Penggunaan kontrasepsi oral terutama pada wanita perokok, migren dengan aura, dan hipertensi; (2) Kehamilan, terutama jika terdapat preeklampsi, eklampsi, dan sindrom HELLP (hemolysis, elevated liver enzymes, and low platelets). Stroke pada usia muda memiliki karakteristik tersendiri, sehingga membutuhkan pendekatan yang berbeda. Faktor risiko perlu diidentifikasi untuk penatalaksanaan dan pencegahan terjadinya stroke berulang. Penelitian berbasis populasi dengan metodologi yang baik penting untuk lebih mengetahui besaran masalah stroke pada usia muda.Nearly 10 % strokes occur at a relatively young age (less than 45 years old). The incidence of stroke in under 45 year-old is between 7-15 cases/100,000 persons/year and 1-8 cases /100,000/year in children. Abstract

Background: Pneumonia is a common complication of ischemic stroke that may worsens brain injury, leading to cognitive impairment as well as patient outcome. The aim of this study was to compare cognitive function in ischemic stroke patients with and without pneumonia.Methods: This study was a comparative numerical analytic retrospective cross-sectional study that was conducted from September 2020 to February 2021. This study used data from ischemic stroke patients treated at Dr. Hasan Sadikin General Hospital in 2019 with the total sampling method. Data were stratified into patients with pneumonia and without pneumonia. The mini mental state examination (MMSE) was used to measure cognitive function in both groups and  scores were compared using the Mann-Whitney Test.Result: Of the 164 data, 25 were patients with pneumonia and 139 without pneumonia. Male patients were predominantly prevalent in the pneumonia and non-pneumonia groups. Hemiparesis was the most common neurological deficit. The MMSE score was significantly lower in the pneumonia group (22.6 ± 5.4 vs. 24.7 ± 4.8, respectively; p-value=0.032). Conclusion: Cognitive function is more prevalent in ischemic stroke patient with pneumonia compared to non-pneumonia. However, other factors such as recurrent stroke and possible vascular risk factors should be considered for future studies to better identify the relationship between ischemic stroke and pneumonia.

Abstract

Early detection of Sensory Processing Disorder (SPD) is important for deciding on appropriate interventions for children at risk. However, there is no valid screening tool available at this moment. The purpose of this study was to validate the deGangi, Sensorimotor History Questionnaire for Preschooler (SHQP), and neurological soft sign of Cambridge Neurological Inventory (NSS CNI) against intelligent quotient (IQ) to establish a reliable cut-off for SPD screening tools for children aged 4–6 years. Sixty-four parent-child were recruited randomly from 3 kindergartens in Bandung, Indonesia. Eligible parents were asked to fill out the SHQP, while their child was assessed using theWechsler Preschool and Primary Scale of Intelligent (WPPSI) and NSS CNI. The deGangi SHQP score and NSS CNI showed a weak and moderate correlation with Full-scale IQ (FIQ). The total score of deGangi SHQP’s sensitivity was 50% with a specificity of 53.4% and a negative predictive value (NPV) of 91.2% agaisnt the FIQ score. The total sensitivity for the NSS CNI was 66.7%, while the total specificity was 58.6% and the NPV was 94.4%. Meanwhile, the sensitivity for the sensory integration subscale score of the NSS CNI was 83.3%, with a specificity of 60.3% and an NPV of 97.2%. The deGangi SHQP can be considered to be an SPD screening tool with the cut-off scores for each subscale of self-regulation, sensory processing of touch, sensory processing of movement, emotional maturity, and motor maturity of 3, 3, 3, and 2, respectively, while the NSS CNI can be used for identifying SPD in children aged 4–6 years with a cut-off scores for the motor coordination, sensory integration, and disinhibition subscales of 7, 10, and 3, respectively.

Abstract

Cerebral venous sinus thrombosis (CVST) is a cerebrovascular disease in the form of occlusion due to thrombus in the venous and cerebral sinuses. It rarely occurs and has varied clinical symptoms and radiological features and challenging to diagnose. D-dimer used as a diagnostic marker for cases of venous thromboembolism, with a sensitivity of around 90–92%. However, the specificity is not too high (70–73%) because it can also increase in other conditions. Digital subtraction angiography (DSA) is a gold standard examination to establish the diagnosis of CVST. The purpose of this study was to determine the relationship between the D-dimer level and CVST using DSA at Dr. Hasan Sadikin General Hospital in Bandung. This study used an observational analytic method with a case-control study design using retrospective data from medical records at Dr. Hasan Sadikin General Hospital in January 2017–August 2019. The research subjects divided into two groups, namely the high D-dimer levels and the normal/low D-dimer level. Forty people meet the inclusion criteria, ages averaging from 44.77±14.40 years, and consists of 9 male patients (22%) and 31 women patients (78%). For normal/low D-dimer levels 20 patients (50%) and high D-dimer levels 20 patients (50%). Statistical test results measuring D-dimer and CVST levels found a significant relationship (p<0.05). In conclusion, there is a relationship between D-dimer levels with CVST events that have been done by DSA. The higher the D-dimer level, the higher the suspicion of CVST. KADAR D-DIMER DENGAN KEJADIAN CEREBRAL VENOUS SINUS THROMBOSIS (CVST) MENGGUNAKAN DIGITAL SUBTRACTION ANGIOGRAPHY (DSA)Penyakit cerebral venous sinus thrombosis (CVST) merupakan penyakit serebrovaskular berupa oklusi akibat trombus di saluran vena dan sinus serebral yang jarang terjadi dengan gejala klinis dan gambaran radiologis yang bervariasi, serta sangat sulit untuk didiagnosis. D-dimer dapat dijadikan sebagai penanda diagnostik bagi kasus-kasus tromboembolisme vena dengan sensitivitas 90–92%, namun spesifisitasnya tidak terlalu tinggi (70–73%) karena dapat juga meningkat pada kondisi lain. Digital subtraction angiography (DSA) merupakan pemeriksaan baku emas untuk menegakkan diagnosis CVST. Tujuan penelitian ini mengetahui hubungan antara kadar D-dimer dan CVST menggunakan DSA di RSUP Dr. Hasan Sadikin Bandung. Penelitian ini merupakan observasional analitik dengan rancangan kasus kontrol menggunakan data retrospektif dari rekam medis di RSUP Dr. Hasan Sadikin Bandung pada bulan Januari 2017–Agustus 2019. Subjek penelitian dibagi menjadi 2 kelompok, yaitu kelompok D-dimer tinggi dan kelompok D-dimer normal/rendah. Hasil penelitian didapat 40 orang yang memenuhi kriteria inklusi, usia rerata 44,77±14,40 tahun yang terdiri atas pasien laki-laki 9 orang (22%) dan perempuan 31 orang (78%). Untuk kadar D-dimer kategori normal/rendah 20 orang (50%) dan tinggi 20 orang (50%). Hasil uji statistik mengukur kadar D-dimer dan CVST didapatkan hubungan yang bermakna (p<0.05). Simpulan, terdapat hubungan antara kadar D-dimer dan kejadian CVST yang telah dilakukan DSA. Semakin tinggi kadar D-dimer, semakin tinggi kecurigaan kejadian CVST.

Abstract

Patients with transfusion-dependent thalassemia (TDT) may experience an increase in ferritin due to shorter erythrocyte lifespan and lysis, as well as side effects of transfusion. Increasing ferritin can cause various complications, including pain, which can develop into chronic pain and interfere with life quality. This study aims to determine the relationship between pain and serum ferritin levels in adults with TDT. This study was an analytical observational study using a cross-sectional design on adult TDT patients with pain who came to the Hemato-Oncology Clinic of Dr. Hasan Sadikin General Hospital Bandung. This research was conducted from March to June 2021. All subjects were assisted to fill out the Indonesian version of the Brief Pain Inventory Short Form (BPI-SF) questionnaire before transfusion. Ferritin levels in the last three months were obtained from medical records. If more than three months, serum ferritin levels were examined. Ferritin levels and BPI-SF scores were then correlated using the Pearson test. The study was conducted on 51 adult TDT patients with pain, and the average value of ferritin levels in research subjects is 5081±2929 g/L. There was a relationship between pain (the dimensions of pain interfere with life on the BPI-SF score) and an increase in ferritin levels (p=0.042, r=0.29). The results showed there is a relationship between pain and serum ferritin levels. Regular consumption of iron chelation tablets can reduce ferritin levels and improve the quality of life for adults with TDT. HUBUNGAN NYERI DENGAN KADAR FERITIN SERUM PADA PENYANDANG TRANSFUSION-DEPENDENT THALASSEMIA DEWASAPenyandang transfusion-dependent thalassemia (TDT) dapat mengalami peningkatan feritin akibat umur eritrosit yang lebih pendek dan mudah lisis, serta efek samping terhadap pemberian transfusi. Peningkatan feritin tersebut dapat menyebabkan berbagai komplikasi di antaranya nyeri yang dapat berkembang menjadi nyeri kronik dan mengganggu kualitas hidup. Penelitian ini bertujuan mengetahui hubungan nyeri dengan kadar feritin serum pada penyandang TDT dewasa. Penelitian dengan observasional analitik menggunakan rancangan potong lintang pada penyandang TDT dewasa dengan nyeri yang datang ke Klinik Hemato-Onkologi RSUP Dr. Hasan Sadikin Bandung. Penelitian dilakukan dari Maret hingga Juni 2021. Seluruh subjek dilakukan pendampingan untuk mengisi kuesioner Brief Pain Inventory Short Form (BPI-SF) versi Indonesia sebelum transfusi. Kadar feritin dalam tiga bulan terakhir didapatkan dari rekam medis dan bila lebih dari tiga bulan dilakukan pemeriksaan kadar feritin serum. Kadar feritin dan skor BPI-SF kemudian dikorelasikan menggunakan Uji Pearson. Penelitian dilakukan kepada 51 penyandang TDT dewasa dengan gejala nyeri dan didapatkan nilai rerata kadar feritin pada subjek penelitian adalah 5081±2929 μg/L. Hasil penelitian memperlihatkan terdapat hubungan antara nyeri (dimensi efek nyeri terhadap kehidupan) dan peningkatan kadar feritin (p=0,042; r=0,29). Hasil penelitian menunjukkan terdapat hubungan antara nyeri dan kadar feritin serum. Konsumsi tablet kelasi besi secara rutin dapat menurunkan kadar feritin dan memperbaiki kualitas hidup penyandang TDT dewasa.
