Abstract

Stroke is the second most common cause of death worldwide after heart disease. About80% stroke cases are ischemic stroke. One of 4 people with ischemic stroke had cardioembolicstroke. Ninety percent of cardiac emboli obstruct cerebral blood vessels and causedneurological deficit. Cardioembolic stroke may be due to abnormalities in cardiac walls,rhythm, valve, or function. Stroke patients with cardiac abnormalities have poor prognosis. Thisretrospective descriptive study was done in May-June 2017. We collected 103 medical recordsof stroke patients from Department of Neurology, Dr. Hasan Sadikin General Hospital duringJanuary 2015- March 2016. Data regarding sex, age, education level, economic status, andcardiac abnormalities were recorded. Majority of subjects were female (53.4%). The mostfrequent age group was 45-65 years old (49.51%). Most subjects had low educational level(76.7%) and low economic status (77.67%). Among subjects, 41.75% had cardiac rhythmabnormalities, 29.13% had cardiac wall abnormalities, 14.56% had cardiac valveabnormalities, and 14.56% had cardiac function abnormalities. Abstract

Vertigo often occurs from the ages of 18 to 79 years with a prevalence of 7.4%. Central vertigo is less common than peripheral vertigo, but it can worsen disability in stroke patients. This study aims to determine the profile of stroke patients with vertigo in Hasan Sadikin General Hospital Bandung Neurology Ward. This is a retrospective descriptive study, using medical record data from 2013 to 2017, and collected 173 medical record data. The results showed that most stroke patients with vertigo were women (59%), the most age group was 55-64 years (34.1%), the most common type of stroke was ischemic stroke (75.7%), the most disturbance regarding the vertebrobasilar system was (72.8%), the most patients got the first stroke was (52.6%), 60.7% of patients suffered from hypertension, 23.1% of patients suffered from hyperlipidemia 40 (23.1%), and 22% of patients suffered from diabetes mellitus type 2. This study concludes that stroke patients who experience vertigo are more common in women, and in the 55-64 year age group. The most common stroke is ischemic in the vertebrobasilar system. Most symptoms in stroke patients with vertigo are dizziness not affected by position, vomiting and nystagmus. Hypertension, hyperlipidemia and diabetes mellitus play an important role in the incidence of stroke in patients. Keywords: vertigo, risk factor, stroke, vertebrobasilar

Abstract

Background: Stroke is a neurological emergency that requires prompt and precise diagnosis and treatment. However, errors in diagnosis and treatment are still frequently seen. One of the main causes of this problem is the lack of doctors’ knowledge. This study aims to determine the level of knowledge among undergraduate medical students about stroke. The results of this study can be used as basic information to provide educational interventions to medical students to prevent and minimize the occurrence of medical errors. Methods: This study used a quantitative-descriptive method with a cross-sectional approach. The study subjects were third-semester medical undergraduate students at the Medical Faculty of Padjadjaran University during the 2018/2019 academic year. Samples were 43 secondary data collected from ischemic and hemorrhagic stroke Students Objective Oral Case Analysis (SOOCA) exam scores and 168 primary data collected using a 30-item questionnaire regarding stroke knowledge. The data obtained was then analyzed descriptively and the result was categorized into 3 levels of knowledge: Good, moderate, and poor. Results: Each variable demonstrates a varying degree of knowledge. However, based on the total scores and averages collected from both the questionnaire and the SOOCA exam, most of the students had a moderate level of knowledge regarding ischemic and hemorrhagic stroke. Conclusion: Most of the students had a moderate level of knowledge about stroke, so educational interventions and student’s capabilities improvement are required to increase knowledge of stroke.

Abstract

Stroke iskemik merupakan salah satu penyebab stroke tersering, disebabkan oleh oklusi pembuluh darah serebral dan penyebab kematian ketiga. Saat awitan stroke iskemik terjadi, area otak yang diperdarahi oleh pembuluh darah akan kekurangan oksigen dan nutrisi sehingga sel otak terutama neuron berada dalam risiko, neuron ini masih dapat berfungsi yang dikenal sebagai penumbra. Hipoksik, salah satu karakteristik penumbra merupakan stimulus utama regulasi protein HIF-1α. Hipoksik sendiri merupakan stimulus utama prekondisi iskemik. Prekondisi iskemik akan menghasilkan fenotipe tahan hipoksia yakni protein hypoxia inducible factor (HIF)-1α. HIF-1α merupakan satu-satunya zat yang dikeluarkan oleh jaringan yang mengalami hipoksia. HIF-1α bertindak sebagai protein sinyal yang dapat meregulasi gen protein lain. Efektor HIF-1α antara lain eritropoitin dan vascular endothelial growth factor (VEGF). Pertumbuhan, diferensiasi dan ketahanan sel endotel diregulasi oleh VEGF yang distimulasi dari HIF-1α. Selama iskemik serebral, jaringan yang rusak mencoba untuk meningkatkan pengiriman oksigen melalui induksi angiogenesis melalui produksi VEGF. Hal ini ditandai dengan adanya peningkatan jumlah pembuluh-pembuluh darah mikro di area infark. VEGF dan reseptornya diregulasi oleh HIF-1α dalam hari pertama iskemik.Hypoxia Inducible Factor (HIF) 1-Α and Vascular Endothelial Growth Factor (VEGF) in Acute Ischemic StrokeAbstractIschemic stroke is one of the most common causes of stroke, caused by cerebral vascular occlusion and the third cause of death. When the onset of an ischemic stroke occurs, the area of the brain bleeding by blood vessels will lack oxygen and nutrients so that brain cells, especially neurons, are at risk, these neurons can still function known as penumbra. Hypoxic, one of the characteristics of penumbra is the main stimulus for regulation of HIF-1α protein. Hypoxia itself is the main stimulus of ischemic precondition. The ischemic precondition will produce a hypoxic-resistant phenotype namely protein hypoxia inducible factor (HIF) -1α. HIF-1αis the only substance released by tissue that experiences hypoxia. HIF-1α acts as a signaling protein that can regulate other protein genes. Effectors of HIF-1αinclude erythropoitin and vascular endothelial growth factor (VEGF). Growth, differentiation and endurance of endothelial cells are regulated by VEGF stimulated from HIF-1α. During cerebral ischemia, damaged tissue tries to increase oxygen delivery through induction of angiogenesis through VEGF production. This is characterized by an increase in the number of micro blood vessels in the infarct area. VEGF and its receptors are regulated by HIF-1α in the first day of ischemia.

Abstract

Hampir 10% stroke terjadi pada usia relatif muda (kurang dari 45 tahun). Angka kejadian stroke pada usia di bawah 45 tahun diperkirakan 7 – 15 kasus/100.000 penduduk/tahun dan lebih jarang pada kelompok anak-anak, yaitu 1 – 8 kasus/100.000/tahun. Faktor risiko terjadinya stroke pada usia muda berbeda-beda, antara lain: (1) Migren, (2) Sickle-cell disease, (3) Penyakit jantung, (4) Abnormalitas pembuluh darah serebral, (5) Focal cerebral arteriopathy of childhood (FCA), (6) Ensefalopati pasca-varisela, (7) Mitochondrial encephalopathy with lactic acidosis and stroke-like episodes (MELAS), (8) Diseksi arteri, (9) Penyalahgunaan obat, (10) Vaskulitis serebral seperti arteritis temporal, arteritis Takayasu, polyarteritis nodosa (PAN), Wegener’s granulomatosis, (11) Systemic lupus erythematosus (SLE), (12) Cerebral autosomal dominant arteropathy with subcortical infarcts
and leucoencephalopathy (CADASIL), (13) Fabry’s disease, dan (14) Trombofilia. Khusus pada wanita usia muda, beberapa faktor risiko lainnya antara lain: (1) Penggunaan kontrasepsi oral terutama pada wanita perokok, migren dengan aura, dan hipertensi; (2) Kehamilan, terutama jika terdapat preeklampsi, eklampsi, dan sindrom HELLP (hemolysis, elevated liver enzymes, and low platelets). Stroke pada usia muda memiliki karakteristik tersendiri, sehingga membutuhkan pendekatan yang berbeda. Faktor risiko perlu diidentifikasi untuk penatalaksanaan dan pencegahan terjadinya stroke berulang. Penelitian berbasis populasi dengan metodologi yang baik penting untuk lebih mengetahui besaran masalah stroke pada usia muda.Nearly 10 % strokes occur at a relatively young age (less than 45 years old). Abstract

Background: As life expectancy continues to increase, the elderly population in Indonesia has significantly doubled in the last five decades, making Indonesia a home to 4th largest population of elderly people in Asia. Unlike other age group, there are more challenges in handling older persons due to their increased mortality, morbidity, and disability risks. Objective: We performed this study to understand the disease pattern in elderly patients admitted in the neurology ward of Hasan Sadikin General Hospital. Methods: This was a descriptive cross-sectional study using data from medical resume. We included medical resume of elderly patients admitted in the neurology ward of Hasan Sadikin General Hospital from January 2015 to December 2019. All incomplete, lost, or inaccessible medical resumes were excluded. Results: Among a total of 400 subjects, 210 (52.5%) were males and 190 (47.5%) were females. Young old (59,5%) presented to be the most observed age group. The most frequent diseases of elderly patients seen in this study were ischemic stroke (55.3%), followed by hemorrhagic stroke (19.5%), subarachnoid hemorrhage (5.3%), and tetanus (4.5%). Atherothrombotic subtype (50,6%) of ischemic stroke was seen to be the most frequent. Conclusion: Noncommunicable diseases were the majority of all cases in the elderly with cerebrovascular diseases, such as stroke and subarachnoid haemorrhage, as the most common conditions. It is important to develop efficient hospital-based management strategies particularly for these diseases and intensify early detection and management of risk factors.

Abstract

Background: Stroke is the second leading cause of death and the third cause of disability globally. Clinical outcomes caused by stroke are varied and influenced by various factors. Objective: This study aims to describe the clinical outcome of stroke patients based on neurological deficit using NIHSS at Dr. Hasan Sadikin General Hospital, Bandung. Methods: This study is a retrospective descriptive, cross-sectional study using secondary data from medical records recruited between January 2019 – December 2019 at the Department of Neurology Dr. Hasan Sadikin General Hospital Bandung with the total sampling method. Variables include demographic data, risk factors, and clinical characteristics. Neurological deficit was assessed using NIHSS. Data were analyzed and presented in the form of tables. Results: There were 202 subjects eligible for this study, including ischemic (71.8%) and intracerebral hemorrhage (28.2%) stroke patients. Most of the subjects were male (53%), aged ≥60 years (50,5%), presenting with onset >6 hours (72.3%), and had hypertension (92,1%). Most of the subjects have a moderate stroke (n=102;50.5%) based on the NIHSS score at admission and mild stroke (n=117;57.9%) based on the NIHSS score at discharge, with a motor deficit as the most common neurological deficit found. Conclusion: The majority of stroke patients presented with a moderate stroke on admission and had a clinical improvement during hospital treatment, with most of the patients experiencing a mild stroke on discharge based on NIHSS score. The motor deficit is the most common neurological deficit that affects clinical outcomes.

Abstract

Background: Stroke is the second most common cause of death globally and the third most common cause of disability. The prevalence of stroke is increasing despite managing various stroke risk factors that have improved considerably. Ischemic stroke is the most prevalent type, and about 90% of stroke events are attributable to modifiable risk factors. This study aimed to explore the characteristics and risk factors of patients with acute ischemic stroke in Dr. Hasan Sadikin General Hospital.Methods: A descriptive cross-sectional study was conducted retrospectively in October 2020, using a total sampling method on medical records of acute ischemic stroke patients in Dr. Hasan Sadikin General Hospital year 2019. Medical records with incomplete and missing data were excluded. Demographic and known risk factors data were collected and presented in the table.Results: Of the 355 subjects, male patients (n=69, 76.7%) were predominant, with the age range of 55–64 years old (36.1%). The majority of the subjects had an ischemic stroke of large-artery atherosclerosis (68.2%) with carotid system involvement (93.2%). As much as 305 subjects (85.9%) had hypertension as the risk factor, followed by dyslipidemia (47.3%), heart disease (39.4%), previous stroke history (33.8%), smoking (25.6%), diabetes mellitus (24.5%), hyperuricemia (12.1%), and blood disorders (2.3%).Conclusions: Males in the older age group have a higher risk of getting acute ischemic stroke with ischemic stroke of large-artery atherosclerosis and carotid system involvement, and hypertension has the highest prevalence.

Abstract

Background: Tetanus is a disease characterized by painful muscle spasms all over the body, which still becomes an important health issue worldwide, particularly in developing countries due to the high mortality rate. The clinical features also affect the mortality rate of adult tetanus patients. Tetanus severity score (TSS) is a prognostic scoring system developed in Vietnam that uses clinical features to predict the mortality rate in adult tetanus patients. The aim of this study was to describe the clinical features and outcomes of adult tetanus patients using TSS.Methods: A descriptive, cross-sectional study with a total sampling method was conducted between 2015 and 2019. A total of 71 medical records of adult generalized tetanus patients in the Department of Neurology Dr. Hasan Sadikin General Hospital Bandung were collected to analyze the clinical features and outcomes using TSS. Statistical analysis was performed using SPSS software ver. 25.0.Results: Among seventy-one tetanus patients, 62 (87.3%) were male, and the average age was 55.2±11.2 years. More than half of the patients (71%) were having grade III or worse and experienced dysautonomia complications (57.7%). The mortality rate was 38% and those who survived at discharge, 88.64% had TSS score <8, whereas 37.04% who died had TSS score ≥8.Conclusions: Tetanus in Bandung has a high mortality rate. Those who have died have high TSS scores, while the survivors have low TSS scores, suggesting that the TSS is a prospective scoring to predict the mortality rate in patients with tetanus in Indonesia.

Abstract

Background: Cardioembolic stroke is a category of ischemic stroke that manifests more severe, is prone to recurrence, and is related to a higher mortality rate than the other categories of ischemic stroke. Anticoagulant therapy is recommended in cardioembolic stroke, both for primary and secondary prevention. Objective: The objective of this study is to understand the pattern of anticoagulant therapy in cardioembolic stroke patients admitted at Hasan Sadikin General Hospital Bandung. Methods: The methodology of this study uses retrospective descriptive cross-sectional and use data from medical records collected during January 2015 to December 2020 at Neurology Department of Hasan Sadikin General Hospital with total sampling method. The inclusion criteria were cardioembolic stroke patients that received anticoagulant therapy. Anticoagulant therapy was stratified into Warfarin and New Oral Anticoagulants (NOACs) which consist of dabigatran, rivaroxaban, and edoxaban. To assess stroke severity outcome between warfarin and NOACs we use the National Institutes of Health Stroke Scale (NIHSS) Score. Results: From the 162 cardioembolic stroke patients, the most common oral anticoagulant used was warfarin (90.12%), followed by dabigatran (4.94%), rivaroxaban (3.70%), and edoxaban (1.23%). Only 1 (0.62%) patient who experienced complications from anticoagulant therapy, the complication was non-major bleeding and occurred in warfarin users. Both warfarin and NOACs mainly had NIHSS on discharge in a moderate category with the mean of NIHSS 7.615 and 7.276 for warfarin and NOACs respectively. Conclusion: Warfarin is the most common oral anticoagulant used for cardioembolic stroke based on this study. Both warfarin and NOACs groups have similar stroke severity outcomes measured by NIHSS on discharge and NOACs have lower incidence of bleeding compared to warfarin group.
