Abstract

Latar Belakang dan Tujuan: Disabilitas yang berat pada stroke meningkatkan risiko terjadinya malnutrisi. Malnutrisi pada pasien stroke dapat disebabkan oleh defisit neurologis dan faktor risiko. Malnutrisi pada stroke meningkatkan morbiditas dan mortalitas. Penelitian ini bertujuan untuk mengetahui karakteristik klinis dan status nutrisi pasien stroke.Subjek dan Metode: Penelitian bersifat deskriptif retrospektif pada pasien stroke yang dirawat di RSUP Hasan Sadikin Bandung pada periode 1 Agustus 2020 hingga 30 September 2020. Status nutrisi dinilai menggunakan Indeks Massa Tubuh (IMT) dan skor Subjective Global Assessment (SGA). IMT dihitung menggunakan kg/m2, dikelompokkan menjadi obesitas (IMT 30), overweight (IMT 25,029,9), normal (IMT 18,524,9) dan underweight (IMT 18,5). Skor SGA 2 termasuk nutrisi baik, dan ?2 tergolong malnutrisi.Hasil: Didapatkan 52 orang pasien stroke yang sebagian besar berjenis kelamin laki-laki (57,7%) dengan rentang usia 3886 tahun (rata-rata 57 tahun), 51,9% mengalami malnutrisi berdasarkan skor SGA, IMT normal dan overweight sebanyak 23 (44,2%) orang, diikuti obesitas (5,7%) dan underweight (5,7%). Ditemukan 71,4% usia ?65 tahun mengalami malnutrisi. Kondisi malnutrisi didominasi pasien stroke infark kardioemboli (63,6%), dengan derajat stroke berat (58,3%), disfagia (59,4%), dengan komorbid infeksi (71,4%) dan stress ulcer (55,6%).Simpulan: Berdasarkan penelitian ini, didapatkan bahwa usia tua , derajat stroke sedang-berat, komorbid infeksi dan stress ulcer akan memiliki kecenderungan malnutrisi sehingga penanganan stroke yang baik dapat menurunkan risiko terjadinya malnutrisi. Kejadian stroke berulang tidak menjadi risiko terjadinya malnutrisi pada penelitian ini.Clinical Characteristics and Nutrition Status in Acute Stroke PatientsAbstractBackground and Objective: Severe disability in stroke increase the risk of malnutrition. This study aims to determine the clinical characteristics and nutritional status of stroke patients.Subject and Methods: This study was a retrospective descriptive study of stroke patients who were treated in the Neurology ward of Hasan Sadikin Hospital Bandung from 1 August 2020 to 30 September 2020. Nutritional status was assessed using Body Mass Index (BMI) and Subjective Global Assessment (SGA) scores. BMI was calculated using kg/m2, grouped into obese (BMI 30), overweight (BMI 25.0-29.9), normal (BMI 18.5-24.9) and underweight (BMI 18.5). SGA score 2 is considered as good nutrition, and ?2 is classified as malnutrition.Results: There were 52 stroke patients, most of them were male (57.7%) with an age range of 38-86 years old (mean 57 years), 51.9% were malnourished based on the SGA score, normal BMI and overweight were 23 (44.2%) people, followed by obesity (5.7%) and underweight (5.7%). We also found 71.4% aged ?65 years are malnourished. This condition was dominated by cardioembolic stroke patients (63.6%), with severe stroke (58.3%), dysphagia (59.4%), with comorbid infections (71.4%) and stress ulcers (55.6%).Conclusion: Based on this study, it was found that elderly, moderate-severe stroke, comorbid infections and stress ulcers have a tendency related to malnutrition. Good or better management can/indeed reduce the risk of malnutrition. Stroke history was not a risk for malnutrition in this study.

Abstract

Latar Belakang dan Tujuan: Status epileptikus merupakan kasus emergensi neurologis dengan mortalitas 57%, 63% merupakan status epileptikus non-konvulsivus (SENK). Diagnosis SENK tidak mudah karena pasien tidak menunjukkan bangkitan yang jelas sehingga diperlukan pemeriksaan elektroensefalografi (EEG). Penyakit serebrovaskular, infeksi susunan saraf pusat (SSP), tumor otak, penyakit autoimmun, dan gangguan metabolik dapat mengakibatkan SENK selain itu dapat memiliki gambaran klinis menyerupai SENK. Tujuan penelitian untuk melihat faktor-faktor yang berperan pada diagnosis SENK.Subjek dan Metode: Penelitian observasional analitik potong lintang retrospektif pada 132 pasien dengan diagnosis klinis SENK di RSUP dr. Hasan Sadikin Bandung selama periode Juli 2017 Juni 2020. Hasil: Dari 132 subjek dengan diagnosis klinis SENK, hanya 100 pasien yang memenuhi kriteria inklusi. Pemeriksaan EEG dilakukan pada semua pasien, sebagian besar dalam waktu 24 jam (82,4 87,9%), hanya 34 pasien yang terkonfirmasi sebagai SENK. Gangguan metabolik secara signifikan berperan pada SENK sebesar 29,4% (p=0,049). Pada pasien yang tidak terkonfirmasi SENK, penurunan kesadaran diakibatkan gangguan metabolik.Smpulan: Gangguan metabolik berperan pada kejadian SENK. Pasien dengan diagnosis klinis SENK memerlukan pemeriksaan EEG segera untuk menghindari diagnosis berlebihan

Abstract

Introduction: The high mortality rate due to stroke causes a burden on health services, so it is essential to optimize stroke services. Mortality is one of the parameters used to assess stroke services. Hasan Sadikin General Hospital, as a stroke service support hospital, should carry out comprehensive service development, one of which is through continuous evaluation regarding the mortality rate of stroke patients being treated. This study aims to analyze the description of mortality in stroke patients at Hasan Sadikin General Bandung, Indonesia. Subject and Methods: The study used a retrospective analytic method for stroke patients in the Neurology ward from January to April 2023. The patients were divided into two groups: those who died and those who returned home with improvement. Data were taken from medical records regarding age, onset, length of stay, NIHSS, initial leukocytes, NLR, and complications. The data were then analyzed using the Mann-Whitney correlation test. Results: From this study showed that 22 patients died, and 70 patients were discharged with improvement. Abstract

Early diagnosis of dysphagia and aspiration risk is very important in minimizing the risk of various medical consequences. This study aimed to establish a scientific database for future sEMG-based dysphagia screening investigations by examining the electrical activity of swallowing muscles in acute stroke cases. A quantitative cross-sectional study was conducted on 61 stroke inpatients admitted to Dr. Hasan Sadikin General Hospital, Bandung, Indonesia, from July to November 2019. The objective of this study was to quantify electrical activities of the suprahyoid (SH) and infrahyoid (IH) muscles during swallowing. Patients participated in both dry swallowing and a 3 mL water swallowing task, with the sEMG used to measure electrical activity parameters (duration, swallowing initiation, time-to-peak, and amplitude) on both paretic and normal sides. The Wilcoxon test was used to compare the electrical activity parameters between the patients paretic and normal sides. Only amplitude showed a significant difference between the paretic and normal side (p=0.023) when performing the 3 mL water swallowing test. Other parameters did not exhibit significant differences in activity between muscle groups during both swallowing tests (p>0.05),  although varying water volumes led to distinct activities in both muscles. Despite the absence of a clear pattern in SH and IH contractions during swallowing, the result showed that the sEMG quantification method might become a promising method for screening dysphagia, complementary to FEES and VFSS. The non-invasive and cost-effective sEMG method can serve as an early screening tool for dysphagia in stroke patients. This study underscores the importance of further investigation on sEMG use, incorporating a larger sample size and diverse cohort results with various swallowing exercises (varying in volume and viscosity) to validate the use of sEMG in dysphagia screening.

Abstract

In facing the challenges of globalization, selecting the right raw material supplier is a key factor in supporting the performance of PT Akrilik Kurnia Kencana. Companies face difficulties in choosing reliable suppliers, which risks affecting product quality and production costs. As a solution, a Decision Support System (DSS) was implemented using the Analytical Hierarchy Process (AHP) method, which allows objective evaluation of various criteria. It is hoped that the implementation of this system can increase efficiency and effectiveness in supplier selection, as well as support the company's strategic decisions. Overall, AHP can help companies make more precise decisions, improve performance, and utilize information technology in decision making.

Abstract

ASSOCIATION BETWEEN STROKE SEVERITY AND STROKE-ASSOCIATED PNEUMONIAABSTRACTIntroduction: Stroke-associated pneumonia (SAP) occurs in 5-26% and decreases the quality of life and clinical outcomes of stroke patients. One of the factors that affect the incidence of SAP is the stroke severity.Aims: To determine the association between the stroke severity and the incidence of stroke-associated pneumonia in the neurological ward of Dr. Hasan Sadikin Hospital, Bandung.Methods: This is an analytic retrospective (historical) cohort design study. The study population was stroke patients who were treated in the neurological ward of Dr. Hasan Sadikin Hospital, Bandung between 2014-2016. Exclusion criteria were patient with pneumonia other than SAP and using mechanical ventilation. Stroke severity was assessed using NIHSS (National Institute of Health Stroke Scale). Chi-square was used to determine inter-variable association.Results: Among 81 subjects, 24 were assessed as SAP (29.6%). The SAP prevalence were mostly male (58.3%), age group between 65-74 year old (41.7%) with hypertension risk (87.5%), stroke onset <48 hours, lesion location on left hemisphere, onset of SAP≥48 hours, and those with consciousness impairment. Those with high stroke severity tend to have higher risk of pneumonia 3.063 times compare to patients with low stroke severity.Discussion: There was a significant association between the severity of stroke and the incidence of SAP in the neu- rological ward of Dr. Hasan Sadikin Hospital, Bandung.Keywords: NIHSS, stroke-associated pneumonia, stroke severityABSTRAKPendahuluan: Stroke-associated pneumonia (SAP) cukup sering terjadi (5-26%) pada pasien stroke, sehingga menurunkan angka kualitas hidup dan luaran klinis. Salah satu faktor yang memengaruhi kejadian SAP adalah derajat keparahan stroke.Tujuan: Mengetahui hubungan antara derajat keparahan stroke dengan kejadian SAP di Ruang Rawat NeurologiRSUP Dr. Hasan Sadikin, Bandung.Metode: Penelitian analitik dengan desain kohort retrospektif (historikal) terhadap pasien stroke iskemik yang di rawat di Ruang Rawat Neurologi RSUP Dr. Hasan Sadikin, Bandung pada tahun 2014-2016. Kriteria eksklusi adalah pasien dengan pneumonia selain SAP dan menggunakan alat ventilasi mekanik. Derajat keparahan stroke dinilai menggu- nakan skor NIHSS (National Institutes of Health Stroke Scale). Uji korelasi Chi-square digunakan untuk melihat hubungan antar-variabel.Hasil: Didapatkan 81 subjek yang 24 orang di antaranya  mengalami SAP (29,6%). Prevalensi SAP tertinggi pada laki-laki (58,3%) kelompok usia 65-74 tahun (41,7%) dengan faktor risiko hipertensi (87,5%), onset stroke<48 jam, lokasi lesi di hemisfer kiri, onset SAP≥48 jam, serta pada subjek dengan penurunan kesadaran. Abstract

Introduction: Global pandemic of Coronavirus disease 2019 (COVID-19) have been developed with a respiratory infection as main symptom and vaccines had been proven against SARS-CoV-2 infection. We would like to present acute transverse myelitis with hypercoagulable condition case, after receiving the inactive SARS-CoV-2 vaccine (SinoVac). Case Presentation: This is a case from a 20-year-old male with acute onset of flaccid paraplegia, hyporeflexia, loss of sensation below 5th thoracic level, loss of autonomic function, and a positive Babinski sign bilaterally on 7 days after receiving SARS-CoV-2 vaccine. From lumbar puncture examination, we found elevated liquor cerebrospinal protein with mononuclear cell predominantly and elevated d-dimer and also decreased fibrinogen levels from laboratory examination. From Magnetic Resonance Imaging (MRI) showed hyperintensity at the 5th until 9th thoracal level. He improved motoric strength of the lower limb (walking without assistance), numbness and pain, urine and fecal retention after receiving methylprednisolone for 3 days. Conclusion: Acute transverse myelitis with hypercoagulable state was a rare case associated with COVID-19 vaccination, but clinician should be aware for early detection and giving proper treatment.

Abstract

Stroke is the second most common cause of death worldwide after heart disease. About80% stroke cases are ischemic stroke. One of 4 people with ischemic stroke had cardioembolicstroke. Ninety percent of cardiac emboli obstruct cerebral blood vessels and causedneurological deficit. Cardioembolic stroke may be due to abnormalities in cardiac walls,rhythm, valve, or function. Stroke patients with cardiac abnormalities have poor prognosis. Thisretrospective descriptive study was done in May-June 2017. We collected 103 medical recordsof stroke patients from Department of Neurology, Dr. Hasan Sadikin General Hospital duringJanuary 2015- March 2016. Data regarding sex, age, education level, economic status, andcardiac abnormalities were recorded. Majority of subjects were female (53.4%). The mostfrequent age group was 45-65 years old (49.51%). Most subjects had low educational level(76.7%) and low economic status (77.67%). Among subjects, 41.75% had cardiac rhythmabnormalities, 29.13% had cardiac wall abnormalities, 14.56% had cardiac valveabnormalities, and 14.56% had cardiac function abnormalities. Abstract

Vertigo often occurs from the ages of 18 to 79 years with a prevalence of 7.4%. Central vertigo is less common than peripheral vertigo, but it can worsen disability in stroke patients. This study aims to determine the profile of stroke patients with vertigo in Hasan Sadikin General Hospital Bandung Neurology Ward. This is a retrospective descriptive study, using medical record data from 2013 to 2017, and collected 173 medical record data. The results showed that most stroke patients with vertigo were women (59%), the most age group was 55-64 years (34.1%), the most common type of stroke was ischemic stroke (75.7%), the most disturbance regarding the vertebrobasilar system was (72.8%), the most patients got the first stroke was (52.6%), 60.7% of patients suffered from hypertension, 23.1% of patients suffered from hyperlipidemia 40 (23.1%), and 22% of patients suffered from diabetes mellitus type 2. This study concludes that stroke patients who experience vertigo are more common in women, and in the 55-64 year age group. The most common stroke is ischemic in the vertebrobasilar system. Most symptoms in stroke patients with vertigo are dizziness not affected by position, vomiting and nystagmus. Hypertension, hyperlipidemia and diabetes mellitus play an important role in the incidence of stroke in patients. Keywords: vertigo, risk factor, stroke, vertebrobasilar

Abstract

Objective: To investigate prolonged neurological impacts of COVID-19 and establish a connection between initial COVID-19 symptom severity and chronic fatigue syndrome (CFS) development, poor sleep quality (PSQ), and cognitive impairment (CI) in individuals recovered from COVID-19.Methods:  This cross-sectional study recruited COVID-19 survivors at Dr. Hasan Sadikin General Hospital Bandung, Indonesia, between June and December 2021. All participants gave informed consent and underwent interviews on demography, clinical features, long-COVID questionnaire, and neurological examination. Participants underwent cognitive examination (MOCA-INA), Chalder Fatigue Scale and Pittsburgh Sleep Quality Index (PSQI) to assess CI, CFS, and PSQ variables. Chi-Square analysis was performed to determine the probability of neurological long COVID-19 syndrome manifestations using SPSS 24.0.Results: Of the 127 participants recruited, 67.7% were women, median (IQR) age of 33 (21-65) years, and time from hospitalization to examination of nine months (1-13). The most common neurological Long COVID symptoms were PSQ (59.8. %), CFS (51.2%), and CI (33.9%). Participants with more than five acute phase COVID-19 symptoms had a higher probability of CFS and CI (OR 2.38 (1, 16-4.9, CI 95%); OR 2.20 (1.01-4.79, CI 95%)) than those with less than five symptoms. The study did not find a significant correlation between sleep quality and number of acute-phase COVID-19 symptoms (OR 1.56 (0.76-3.20, CI 95%)).Conclusion: Almost two-thirds of the COVID-19 survivors experienced PSQ, more than half had CFS, and almost one-third had CI. The study revealed an increasing likelihood of CFS and CI in COVID-19 survivors as the number of acute COVID-19 symptoms increases.
