Abstract

Outpatient waiting time is one of the national quality indicators. Most patients visiting outpatient clinics in Indonesian hospitals complain about the long waiting time before receiving healthcare services. Over the decades, the persistent issue of long waiting times and unresolved patient dissatisfaction has led hospitals worldwide to adopt Lean Healthcare management systems. These systems focus on improving the efficiency and effectiveness of service operations by eliminating waste. Siti Rahmah Hospital is a private type-C hospital facing outpatient waiting time issues exceeding 60 minutes. This community service aims to enhance nurses' knowledge and reduce service waiting times. This community service was conducted using a quantitative approach with an experimental design, and the primary data collection technique was through questionnaires. The implementation consisted of four main stages: preparation, socialization of lean healthcare methods, identification of waiting times, and mapping types of time waste in services. Nurses' knowledge about lean healthcare management increased significantly by 56.70% after the training, as shown by a pretest score of 36.6% and a post-test score of 93.3%. The knowledge components that improved substantially included understanding the outpatient waiting time standard of less than 60 minutes, identifying time-wasting activities in services, and using electronic medical records as a lean healthcare strategy to reduce service waiting times. The lean healthcare method is effective in reducing patient waiting times in outpatient care.

Abstract

Perawat merupakan tenaga profesional di bidang kesehatan yang memiliki peran sentral dalam penyelenggaraan pelayanan keperawatan secara holistik dan berkesinambungan kepada pasien. Tuntutan pekerjaan yang tinggi serta beban kerja fisik dan psikologis dapat menyebabkan kelelahan kerja. Kondisi ini, apabila tidak ditangani secara efektif dapat menurunkan kinerja perawat dan berdampak negatif terhadap mutu pelayanan keperawatan. Penelitian ini bertujuan untuk mengetahui gambaran tingkat kelelahan kerja pada perawat di ruang rawat inap Rumah Sakit Islam (RSI) Siti Rahmah Padang. Desain penelitian menggunakan pendekatan deskriptif kuantitatif dengan rancangan cross-sectional. Populasi penelitian adalah seluruh perawat di ruang rawat inap RSI Siti Rahmah Padang, dengan jumlah sampel 56 responden yang diambil menggunakan teknik total sampling. Data dikumpulkan menggunakan kuesioner Subjective Self Rating Test (SSRT) yang dikembangkan oleh Industrial Fatigue Research Committee (IFRC) of the Japanese Association of Industrial Health untuk mengukur tingkat kelelahan berdasarkan keluhan subjektif perawat. Analisis data dilakukan secara univariat untuk melihat distribusi frekuensi tingkat kelelahan kerja. Hasil penelitian menunjukkan sebagian besar perawat mengalami kelelahan kerja sedang sebesar 64,3% dan kelelahan ringan sebesar 35,7%. Diperlukan upaya manajemen rumah sakit dalam mengatur beban kerja, menyediakan waktu istirahat yang cukup, dan menciptakan kerja yang nyaman guna menurunkan tingkat kelelahan kerja perawat Kata Kunci: Kelelahan Kerja, Perawat, Ruang Rawat Inap

Abstract

Healthcare facilities must adhere to patient safety principles to reduce the risk of harm. Prof. Dr. M.A. Hanafiah SM Batusangkar Hospital is among the healthcare providers in West Sumatra, necessitating endeavors to enhance and sustain service quality, especially regarding patient safety. The effectiveness of the patient safety culture depends on the 4 S supervision elements, including Structure, Skill, Support, and Sustainability. The 4S supervision model guides and aids nurses in advancing their skills related to patient safety. Survey findings at the hospital revealed that the promotion of 4S supervision was suboptimal, manifesting as nurses' limited grasp of the concept. On July 28-29, 2023, a structured socialization, education, and mentorship program was conducted for nursing executives and managers. The objective was to empower nurses to incorporate multilayered supervision into patient care and oversee the implementation of patient safety in inpatient, outpatient, and emergency room settings, aiming to enhance the patient safety culture.

Abstract

The Covid-19 Pandemic, spanning from 2020 to 2022, ushered in unprecedented challenges for the healthcare system, impacting nurses who played a pivotal role in the global and Indonesian responses. Nurses faced emotional exhaustion, diminished job satisfaction, and suboptimal working conditions, compounded by increased responsibilities and monotony in delivering healthcare services. Objective: This study aimed to identify predictive factors influencing the post-pandemic quality of life among nurses in Padang City hospitals. Utilizing purposive sampling, 88 nurses participated, providing data through a demographic questionnaire, Perceived Stress Scale, and Professional Quality of Life Scale (ProQoL-21). Nurses' quality of life (p-value = 0.000). Conversely, age, gender, tenure, experience in caring for Covid-19 patients, and career level exhibited no significant correlations. The findings underscore the importance of addressing stress as a determinant of nurses' well-being. Care managers at Padang City Hospital can leverage these insights to enhance nurses' quality of life, ultimately optimizing healthcare delivery to the community. This study offers valuable guidance for healthcare management, emphasizing the need to prioritize stress management interventions to bolster the overall effectiveness of nursing care.

Abstract

Career ladder systems are an important mechanism in supporting the professional development of nurses and have been implemented in various models in a number of countries. Effective management support is critical to the successful implementation of career ladder systems in Indonesia. This study aims to determine the description of management support in the implementation of career ladder system in Local Government Hospitals. This study is a quantitative study with a cross-sectional design conducted in May-June 2024. The study population was all nurses who applied for competency assessment and credentialing/recredentialing as many as 76 people who also became samples with total sampling technique. Data were analyzed using descriptive statistics including frequency distribution and percentage. These questionnaires have undergone validity and reliability testing, with Cronbach's alpha values of 0.905 for the Management Support Questionnaire and 0.927 for the Nursing Career Path Questionnaire. The results showed that the majority of executive nurses were female (92%) with vocational education (84%) and were in early adulthood (26-35 years old). Most of them had between 8 to 15 years of service and almost half of them were at the Pre PK level (43%). Management support in implementing career ladders was mostly categorized as good (54%). This study concludes that management support in the implementation of the career ladder system in Local Government Hospitals is mostly good, but there are still some aspects that need to be improved. Optimal management support includes the provision of adequate resources, structured training programs, and continuous feedback to ensure the successful implementation of the career ladder system.

Abstract

The low adherence of pregnant women in consuming Fe tablets is regarded to be the main cause of the ineffectiveness of the Fe supplementation program held by the government to overcome anemia issues on pregnancy. One of the factors that influence the adherence of pregnant women is family support. This research was aimed at finding out the relationship between family support and the adherence in consuming Fe tablets among pregnant women in Puskesmas Nanggalo Kecamatan Nanggalo, Padang, West Sumatra in 2010. This research was designed in a correlation study with a cross-sectional approach and 32 respondents. Respondents were those who had got Fe tablets in their last visit from Puskesmas and also had a health history with anemia in pregnancy. The data was collected by using a questionnaire. Univariate and bivariate analyses were done to variable family support as well as the adherence of pregnant women. Bivariate analysis was done by cross-tabulation with a Chi-Square test. The result of the research showed that 54.9% of respondents got low family support and 71.9% of respondents had low adherence in consuming Fe tablets. From the statistics experiment, it can be concluded that there is a relationship between family support and women's adherence in consuming Fe tablets (p < 0.05). Therefore, it is suggested that nurses or other health workers in order to start asking and taking the family members, especially for husbands, to join treatment toward pregnant women with anemia.

Abstract

Discharge planning is a series of decisions and activities in providing continuous and coordinated nursing care. Discharge planning must be known by the patient before the patient returns home. Discharge planning is related to nurse performance, if the nurse is good, the impact will be felt by the patient. Previous research stated that discharge planning was very influential on motivation. The purpose of this study was to determine the relationship between motivation with the documentation of Discharge Planning at RSI Siti Rahmah Padang 2018. A quantitative study with cross-sectional design was carried out in this study. The proportional stratified random sampling method has been used with 74 respondents as the sample. The data were collected using questionnaires containing documentation items discharge planning and motivation. The univariate analysis is presented by frequency distribution and bivariate tables using the chi-square test with 95% confidence degree (α=0.05). The results showed a significant relationship between motivation with the documentation of Discharge Planning at RSI Siti Rahmah Padang 2018 (p=0.000). It is recommended for nurses to maintain a good motivation.

Abstract

Ketidaktepatan dalam pengelolaan stres dapat memunculkan perilaku emotional eating, sebuah kecenderungan makan berlebihan sebagai salah satu bentuk mekanisme koping maladaptif terhadap emosi negatif seperti stres. Tujuan penelitian ini untuk mengetahui gambaran stres dan perilaku emotional eating pada mahasiswa S1 Keperawatan. Penelitian ini merupakan riset kuantitatif menggunakan pendekatan cross sectional. Dilakukan di Fakultas Keperawatan Universitas Andalas, Padang, Sumatera Barat pada bulan Februari s.d Agustus 2022. Sampel penelitian berjumlah 82 orang mahasiswa yang diambil menggunakan teknik Proportional random sampling. Data dikumpulkan menggunakan kuisioner menggunakan instrumen Eating and appraisal due to Emotions and Stres (EADES) dan Student-Life Stres Inventory (SSI). Hasil penelitian menunjukkan hampir separuh (48,8%) mahasiswa memiliki perilaku emotional eating, dan perilaku ini ditemukan pada setengah (50%) mahasiswa dengan tingkat stres berat, pada lebih dari separuh (56,1%) dengan stres sedang, dan pada sebagaian kecil (20%) mahasiswa dengan stres ringan. Diharapkan agar pihak kampus dapat mendampingi mahasiswa dalam mengelola stres selama menjalani pendidikan, agar dapat mengembangkan koping yang lebih adaptif dalam menghadapi stres, dan mencegah/mengatasi perilaku emotional eating yang muncul pada mahasiswa saat stres.

Abstract

Kanker payudara merupakan kanker dengan prevalensi tertinggi pada perempuan dan berdampak pada aspek psikososial. Selain beban fisik akibat pengobatan, pasien juga menghadapi stigma yang memengaruhi citra diri, hubungan sosial, dan pemulihan.Penelitian ini bertujuan untuk menggambarkan tingkat stigma yang dialami oleh pasien kanker payudara di RSUP Dr. M. Djamil Padang. Desain penelitian yang digunakan adalah deskriptif kuantitatif dengan teknik purposive sampling. Sebanyak 280 responden yang memenuhi kriteria inklusi berpartisipasi dalam penelitian ini, yaitu perempuan berusia ≥18 tahun yang telah didiagnosis kanker payudara dan sedang atau telah menjalani pengobatan. Instrumen yang digunakan adalah Breast Cancer Stigma Assessment Scale (BCSAS) yang terdiri atas 28 item dengan tujuh domain. Analisis data dilakukan secara deskriptif menggunakan SPSS versi 26. Hasil penelitian menunjukkan bahwa sebagian besar pasien mengalami tingkat stigma tinggi (39,6%), sedangkan 25,4% dengan tingkat sedang dan 35% pada tingkat rendah. Domain dengan skor tertinggi adalah dapat disembunyikan dan perubahan citra diri, sedangkan skor terendah terdapat pada gangguan keluarga dan asal penyakit. Hasil ini menunjukkan bahwa stigma masih menjadi beban psikososial signifikan bagi pasien kanker payudara. Oleh karena itu, diperlukan intervensi psikososial yang berkelanjutan, seperti pembentukan peer support group, untuk membantu pasien saling berbagi pengalaman, meningkatkan penerimaan diri, dan mengurangi dampak stigma terhadap kesejahteraan emosional.
