Abstract

Noncommunicable diseases (NCDs) are the leading cause of death worldwide. There are around 36 million deaths every year. An example of NCD, a non-communicable disease, is hypertension. Excessive consumption of sodium chloride (NaCl) is the main factor that can cause hypertension and organ fibrosis. A natural ingredient that has the potential to act as an antifibrosis agent against fibrosis is quercetin by increasing PPAR-γ expression. Based on research, one of the plants that contains quercetin is the herb of Patikan Kebo (Euphorbia hirta Linn). Collagen is a protein biomarker of fibrosis synthesized via the PPAR-γ signaling pathway. The aim of this research was to determine the organ protective effect of administration of Patikan Kebo herbal extract against renal fibrosis and heart in Wistar rats (Rattus norvegicus) induced with 1.5% NaCl. The method involved 15 male Wistar rats aged 2.5 months with a body weight of 100-150 grams, which were divided into three groups (KI: control; KII: induced by 1.5% NaCl; KIII: induced by 1.5% NaCl + Patikan Kebo extract 200 mg/kg body weight. The induction was carried out for 8 consecutive weeks. On day 56, the kidneys and heart were removed in groups I and II, while in group III the treatment with the Patikan-Kebo extract from Patikan-Kebo at a dose of 200 mg/kg body weight was continued for 28 days in Group III. Specifically, for group III, kidneys and heart were collected on day 84 after treatment with Patikan kebo extract on day 84. Collagen was stained with Giemsa stain. Data were analyzed using a Kruskal-Wallis test. The difference is considered significant if the probability value is p0.05. The result showed that there were significant differences between the control and treatment groups. Patikan Kebo extract can reduce collagen in rats with induced fibrosis with 1.5% NaCl.

Abstract

Thyroid cancer is the most common cancer of the endocrine glands with a prevalence of approximately 95% of all malignancies. Its incidence has increased more rapidly since the 1990s than any other cancer. Thyroid cancer is divided into 4 types based on its histopathology. The nature of this cancer varies, ranging from slow progressive to aggressive. Like other cancers, thyroid cancer if left untreated will invade other organs of thebody. We collect several reading journals that discuss epidemiology, risk factors, management for thyroid gland cancer and present them descriptively. The purpose of writing this article is to discuss the latest epidemiology and risk factors that influence the incidence of thyroid cancer as well as the old and newest treatments in treating thyroid cancer. The results of this article found several risk factors thought to influence prevalence, namely radiation exposure, genetic mutation, heredity, iodine deficiency, increased TSH, autoimmune, thyroid nodules, environment, lifestyle, and estrogen. Treatments that need to be carried out are surgery, RAI therapy, thyroid hormone therapy, external radiation, systemic chemotherapy, and combination therapy. Through this discussion, it is hoped that it can become material for analysis and reference in the treatment of thyroid gland cancer. Abstract

Thyroiditis is an inflammation that occurs in the thyroid gland and can affect the functioning of the thyroid gland. Based on its etiology, thyroiditis is distinguished into 2, namely thyroiditis which in its clinical manifestations causes pain and does not cause pain in the neck. De Quervain's thyroiditis is the most frequent cause of neck pain. Based on research, this disease is more common in women, which is about 3-4 times more than men. The clinical manifestations that arise vary, ranging from neck pain, symptoms such as hyperthyroidism, to symptoms of thyrotoxicosis. The diagnosis of De Quervain’s thyroiditis can be clinically established based on the results of physical examination and laboratory findings. The findings of laboratory examination results generally occurred an increase in LEDs, CRP, FT3, FT4, and a decrease in TSH levels. The management of this disease is divided into 3, namely supportive therapy, pharmacological, and monitoring.

Abstract

Thyroid carcinoma holds the 11th place in the incidence of new cancers, with a prevalence of 586,202 cases worldwide. The incidence of thyroid cancer in Indonesia reached 13,114 cases with 2,224 mortality in 2020. This literature review aims to provide information on thyroid cancer's clinical manifestations, diagnosis, and management. We conducted a literature searching from books, literature reviews, case studies, and guidelines from professional organizations such as the AJCC (American Joint Committee on Cancer) and European Society for Medical Oncology (ESMO) guideline. The initially noticed clinical manifestation of thyroid cancer is usually a painless lump or enlargement at the front-lower part of the neck. Diagnosis of thyroid cancer is done step-by-step from history taking, physical examination, laboratory testing of thyroid stimulating hormone and thyroglobulin, thyroid ultrasonography, and fine needle aspiration biopsy (FNAB). Management of thyroid cancer include pharmacological management, surgery, or/and radiotherapy according to the type and stage of the thyroid cancer. Early diagnosis and therapy of thyroid cancer are expected to improve the prognosis of thyroid cancer.

Abstract

Background: Malaria is an infectious disease that continues to pose a global health threat. Malaria cases will reach 247 million in 2021, with malaria accounting for an estimated 619,000 fatalities worldwide. Indonesia is the second largest provider of malaria cases in the WHO South-East Asia Region, trailing only India. In 2020, Indonesia contributed 254,050 positive cases of malaria. In 2021, the number of cases has risen to 304,607. Objective: The goal of this research is to determine the factors of the physical condition of the house (including the use of wire mesh for ventilation, the presence of ceilings, and the density of walls) and the environment around the house (including the presence of bushes, puddles of water, and livestock pens) that influence the incidence of malaria. Method: This research is a literature review that use the Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA) technique. The literature search was carried out on the internet using Google Scholar. Out of the search results, 11 articles were chosen for review. Results: The results of this study indicate that the use of wire mesh for ventilation, the presence of ceilings, the density of walls, the presence of bushes, the presence of standing water, and the presence of livestock pens in the environment surrounding the house are risk factors that influence the incidence of malaria. Conclusion: The physical condition of the house and the quality of the surrounding environment are both important factors to consider in malaria prevention measures.

Abstract

Abstract: Hyperthyroidism is an increase in the amount of thyroid hormone synthesis and secretion by the thyroid gland. Graves Disease (GD) is the most common cause of hyperthyroidism. Graves Disease (GD) is more common in women than men and in patients with a family history of GD. Graves Disease can also be triggered by environmental factors such as stress, smoking, infection, and exposure to iodine. Graves Disease manifests in the form of the Merseburger triad consisting of thyrotoxicosis, diffuse goiter, and ophthalmopathy (orbitopathy) with symptoms of agitation, palpitations, and muscle weakness. In making the diagnosis of GD, 2 tests are carried out, namely a thyroid function test which consists of examinations to diagnose hypothyroidism, namely Wayne index scoring, TSH test, and FT4 as well as tests to distinguish GD from other causes of hyperthyroidism, namely TRab, radioactive iodine uptake scan with I-123 or I-131, thyroid ultrasound with doppler, T3/T4 ratio values, and CT or MRI. The goal of treatment for GD is to control and improve the condition based on the pathophysiology of GD, namely the antigen-antibody reaction in the thyroid gland. Modalities for the treatment of GD consist of anti-thyroid drugs, surgery, and radioactive iodine (RAI) treatment with sodium-131(131I). The choice of treatment is based on the severity of thyrotoxicosis, age, thyroid size, availability of modalities, response to treatment, and other comorbidities.
