Abstract

AbstractIntroduction: Wounds are a breakdown of tissue integrity / continuity that can lead to infection. Virgin Coconut Oil is a processed coconut product made by processing fresh coconut flesh at low temperature and has a high nutritional content. Ozone therapy is an alternative therapy that has disinfectant properties and can induce strong oxidative stress. Methods: This study was conducted on 50 Sprague-Dawley rats that had injuries. We performed full thickness wounds and administered ozonated doses of VCO to mice. We looked at shrinkage of wounds and TGF Beta levels in mice. Assessment was carried out on day 7 and day 14 to assess the wound and TGF-beta immunohistochemically. Results: There was a significant difference in the wound shrinkage variable between the negative control group and the group that received VCO offerings both on day 7 and day 14. We also found a significant difference in the TGF-beta variable between the negative control group and the group that received VCO offerings both on day 7 and day 14. Conclusion: The administration of ozonated virgin coconut oil was effective in increasing the expression of TGF-? in the full thickness defect of Sprague Dawley rats and was effective in increasing the reduction in wound size in the full thickness defect of Sprague Dawley rats.

Abstract

Abstract Introduction : Skin grafts are now one of treatment option in wound healing process that is always developing. TGF-bexpression and the number of neutrophils have an important role in healing skin graft wounds. Ozone (O3) has disinfecting properties that are effective in wound healing. Objective : Proving the effectiveness of Ozonated VCO for Full Thickness Skin Graft wound healing using parameter of TGF-b expression and neutrophil number. Method : This study is an experimental study with a post-test only design group of 40 Sprague Dawley rats performed autologous skin graft at the same time. Samples were divided randomly into 8 groups (K1 = without Ozonated VCO), (A1 = Ozonated VCO 50 mg / ml), (B1 = Ozonated VCO 100 mg / ml), (C1 = Ozonated VCO 200 mg / ml), ( K2 = without Ozonated VCO) (A2 = Ozonated VCO 50 mg / ml), (B2 = Ozonated VCO 100 mg / ml), (C2 = Ozonated VCO 200 mg / ml). Assessment of TGF-b expression and neutrophil number of tissue was performed by staining hematoxylin & eosin and immunohistochemistry on days 6 and 12 after skin graft. Results : There were significant differences (p <0.05) TGF-b expression and neutrophils number of tissue between the control group and the administration of Ozonated VCO doses of 50 mg / ml, 100 mg / ml and 200 mg / ml on days 6 and 12 post skin graft. Conclusion : The administration of Ozonated VCO effectively improve Full Thickness Skin Graft wound healing seen from macroscopic wounds, increase TGF-b expression and decrease the number of neutrophils.

Abstract

Background: Diabetic ulcer is one of the most feared chronic infections due to Diabetes Mellitus because it can lead to amputation and death. Aim: To prove the effectiveness of sucralfate and platelet-rich plasma (PRP) combination for Vascular Endothelial Growth Factor (VEGF) expression in diabetic ulcer healing. Methods: This research is an experimental study of Phase I Clinical Trial with post-test only group design. There were 20 patients with diabetic ulcers divided into two groups, namely the treated group that was given sucralfate and PRP therapy and the control group was given standard therapy of normal saline drainage and gauze covered. Parameters were VEGF expression levels, wound area after being given therapy, and side effect from the treatment. Data on VEGF expression levels were obtained by means of examination with the Quantikine Human VEGF-ELISA Quantikine, R&D System, Inc, Minneapolis. The measurement of the wound area was assessed based on several criteria, namely grade 0 (no change), grade 1 (wound size reduced to less than of the previous wound), grade 2 (wound size was reduced to less than of the previous wound, but granulation was visible), and grade 3 (wound has closed completely). Result: In unpaired t-test, the mean VEGF expression was 98.18+10.96 in the treatment group and 66.69+23.79 in the control group which showed significant difference in VEGF expression levels (p = 0.003). In Mann-Whitney test, the mean wound area was 0.68+0.40 in the treatment group and 0.77+0.67 in the control group which showed that there was not any significant difference in wound area (p = 0.152). There were no side effects in both study group. Conclusions: The combination of sucralfate and PRP can increase VEGF levels significantly in diabetic ulcer patients but does not show a different effect in reducing wound area compared to standard treatment. The combination did not cause any side effects in the study subjects, as well as those using standard treatment.

Abstract

Latar Belakang: Reaksi Kusta adalah suatu episode hipersensitivitas akut yang dapat terjadi sebelum, saat, atau setelah pengobatan. Beberapa kasus, reaksi kusta yang parah dapat mengancam jiwa pasien. Interleukin-6 (IL-6) adalah sitokin multifungsi terlibat dalam regulasi respon imun, peradangan, reaksi fase akut, dan pertumbuhan hematopoietik. Pada reaksi kusta IL-6 menginduksi respon inflamasi akut saat disintesis di lokasi infeksi oleh sel-sel inflamasi. Moringa oleifera diketahui memiliki senyawa bioaktif yang me-miliki fungsi biologis salah satunya anti-inflamasi. Penelitian ini bertujuan mengetahui efek dari ekstrak Moringa oleifera sebagai terapi Adjuvan dalam menurunkan kadar IL-6 pada pasien dengan Reaksi Kusta Tipe 1Metode: Penelitian ini menggunakan studi eksperimental dengan desain pre and post randomized single blinded controlled trial. Sampel pada penelitian ini adalah pasien kusta dengan reaksi tipe 1 yang berkunjung ke Poliklinik Kulit atau dirawat di RSUD dr. Rehatta Provinsi Jawa Tengah pada bulan Desember 2023-April 2024 dengan jumlah 22 subjek yang dibagi kedalam dua kelompok yaitu kontrol dan perlakuan. Subjek pada kelompok perlakuan mendapatkan terapi Prednison dan ekstrak Moringa oleifera 2gr/hari. Sedang-kan pada kelompok kontrol hanya mendapatkan terapi Prednison yang dilakukan selama 1 bulan. Kadar IL-6 pretest dan postest dilakukan meng-gunakan metode sandwich Enzyme-linked immunosorbent assay (ELISA).Hasil: Analisis perbedaan kadar serum IL-6 pretest dan postest dengan uji Wilcoxon pada kelompok perlakuan didapatkan nilai p=0,155 dan kontrol p=0,110. Sedangkan perbedaan antar kelompok perlakuan dan kontrol dengan uji Mann Whitney menunjukkan nilai p=0,412 pada postest. Sehingga dapat dilihat bahwa tidak terdapat perbedaan yang bermakna (p>0,05) antara pretest dan postest serta perbandingan antar kelompok.Kesimpulan: Pemberian terapi adjuvan ekstrak Moringa oleifera 2gr/hr tidak berpengaruh terhadap penurunan kadar IL-6 pada pasien reaksi kusta tipe 1.

Abstract

Background: Ulcers are one form of chronic wounds as one form of complication experienced by leprosy sufferers. In the process of healing wounds such as ulcers, TGF-β has a broad role. Several studies have stated that in chronic wound conditions there is a decrease in TGF-β levels so that the wound is increasingly difficult to heal. This study aims to determine the relationship between TGF-β serum levels and the degree of ulcer wound healing. Methods: This study is an observational analysis study with a cross-sectional design and involved 33 leprosy ulcer patients undergoing treatment at the dr. Rehatta Jepara Hospital in the period March-April 2024. Serum TGF-β levels were measured using the Enzyme-linked immunosorbent assay (ELISA) method while the degree of wound healing was assessed by scoring the Pressure Ulcer Scale for Healing (PUSH). Bivariate analysis using the Spearman Correlation Rank Test. Results: The correlation test between serum TGF-β levels and the degree of wound healing (p = 0.807) and duration of leprosy as covariate (p = 0.96) showed insignificant results. Conclusion: There is no relationship between serum TGF-β levels and the degree of wound healing and duration of leprosy.

Abstract

Background: Leprosy is an infectious disease caused by Mycobacterium leprae bacteria which is still the focus of a global problem and causes disability in patients. The degree of disability in leprosy is assessed as mild and severe as a result of the inflammatory process. This study aims to determine the relationship between the degree of disability in leprosy and IL-6 serum levels as an inflammatory biomarker in leprosy patients. Methods: A Cross-sectional observational analytical study on leprosy patients aged 20-60 years, not pregnant, and undergoing treatment at Donorojo Jepara Hospital from March to April 2024. Serum IL-6 levels were analyzed using the ELISA method and tested for correlation with the degree of disability leprosy. Results: Statistical analysis of Spearman's correlation rank test shows a p value = 0.279 and the results of the line pattern on the Scatter dot graph are not linear. Conclusion: There is no correlation between serum IL-6 levels and the degree of disability in leprosy patients at Donorojo Jepara Hospital.Keywords: Degree of Disability in Leprosy, Leprosy, Serum IL-6

Abstract

Moringa oleifera Lam. atau tanaman kelor dikenal dengan berbagai khasiat medisnya. Moringa oleifera telah terbukti memiliki banyak manfaat kesehatan, termasuk efek antitumor, antiinflamasi, antioksidan, dan antimikroba. Penelitian ini bertujuan untuk menganalisis efek ekstrak daun Moringa oleifera sebagai terapi adjuvan terhadap kadar faktor pertumbuhan dalam penyembuhan luka, yaitu TGF-β, pada pasien ulkus kusta. Penelitian eksperimental ini menggunakan desain pre- and post-randomized single-blinded controlled trial. Penelitian ini membandingkan kelompok intervensi yang menerima terapi adjuvan ekstrak daun Moringa oleifera (2 gram/hari) dengan kelompok kontrol yang hanya menerima terapi standar. Penelitian ini melibatkan pasien kusta dengan ulkus di Rumah Sakit Donorejo, Jepara. Analisis fitokimia ekstrak Moringa oleifera menunjukkan adanya flavonoid, tanin, dan saponin. Kadar serum TGF-β diukur sebelum dan sesudah intervensi. Analisis statistik menggunakan uji Wilcoxon menunjukkan tidak ada perbedaan signifikan dalam kadar serum TGF-β sebelum dan sesudah pemberian ekstrak Moringa oleifera (p>0,05). Demikian pula, uji Mann-Whitney menunjukkan tidak ada perbedaan signifikan dalam kadar serum TGF-β antara kelompok perlakuan dan kelompok kontrol (p>0,05). Aplikasi secara topikal ekstrak Moringa oleifera disarankan untuk mengeksplorasi potensi manfaatnya dalam penyembuhan luka. Moringa oleifera Lam., known for its various medicinal properties. This study aims to analyze the effect of Moringa oleifera leaf extract as an adjuvant therapy on TGF-β levels in leprosy ulcer patients. This experimental study employed a pre- and post-randomized single-blinded controlled trial design. The study compared an intervention group, which received adjuvant therapy with Moringa oleifera leaf extract (2 grams/day), with a control group that received standard therapy only. The study included leprosy patients with ulcers at Donorejo Hospital, Jepara. Phytochemical analysis of Moringa oleifera extract indicated the presence of flavonoids, tannins, and saponins. The TGF-β serum levels were measured before and after the intervention. Statistical analysis using the Wilcoxon test showed no significant difference in TGF-β serum levels before and after administration of Moringa oleifera extract (p>0.05). Similarly, the Mann-Whitney test revealed no significant difference in TGF-β serum levels between the treatment and control groups (p>0.05). Topical application of Moringa oleifera extract is suggested to explore its potential benefits in wound healing.

Abstract

Penuaan menyebabkan perubahan fisiologis dan biomekanik, termasuk penurunan otot, berkurangnya panjang langkah, kecepatan berjalan lebih lambat, dan peningkatan risiko jatuh. Studi ini membandingkan efek latihan inti dan keseimbangan pada kecepatan berjalan dan tingkat kelemahan pada orang tua. Ini menggunakan Desain Kelompok Kontrol Pra dan Pasca Tes dengan 40 responden yang dibagi secara acak menjadi dua kelompok. Satu kelompok melakukan latihan inti, sementara yang lain berlatih latihan strategi keseimbangan, keduanya dilakukan tiga kali seminggu selama empat minggu. Kecepatan berjalan diukur menggunakan tes kecepatan berjalan 4 meter, dan kelemahan dinilai dengan Indeks Kelemahan. Hasil penelitian menunjukkan bahwa pada kelompok inti, kecepatan berjalan pasca-tes rata-rata 1,27 m/s, meningkat dari 0,98 m/s (p=0,00), sedangkan pada kelompok keseimbangan, meningkat dari 1,03 m/s menjadi 1,14 m/s (p=0,004). Skor Indeks Kelemahan kelompok inti menurun dari 1,55 menjadi 0,6 (p=0,00), sedangkan skor kelompok keseimbangan menurun dari 1,6 menjadi 1,4 (p=0,046). Tidak ditemukan perbedaan signifikan antara kelompok dalam peningkatan kecepatan berjalan (p=0,051), tetapi latihan inti secara signifikan mengurangi kelemahan lebih banyak daripada latihan keseimbangan (p=0,00). Sebagai kesimpulan, kedua jenis latihan tersebut meningkatkan kecepatan berjalan dan mengurangi kelemahan pada lansia. Namun, meskipun latihan inti dan latihan keseimbangan memiliki efek yang sama pada kecepatan berjalan, latihan inti secara signifikan lebih efektif dalam menurunkan tingkat kelemahan.

Abstract

Leprosy (Morbus Hansen) is a chronic infectious disease caused by Mycobacterium leprae, primarily affecting the peripheral nerves and leading to sensory loss and the formation of chronic ulcers. This study investigates the efficacy of Curcuma longa rhizome and Moringa oleifera leaf extracts, known for their antibacterial and anti-inflammatory properties, in promoting wound healing in leprosy-related ulcers. Utilizing a pre-post, randomized, single-blinded controlled trial design, the study was conducted at Kelet Regional Hospital, Jepara, from January to April 2024. Participants included leprosy ulcer patients with a mean disease duration of 101.5 months, predominantly male (63.6%), and largely in the 51–60 age group (39.4%). Results indicated significant improvements in PUSH (Pressure Ulcer Scale for Healing) scores from pre-test to post-test within both the intervention and control groups (p<0.001). However, intergroup analysis showed no statistically significant difference in PUSH scores between groups (pre-test p=0.669; post-test p=0.198). These findings suggest that while C. longa and M. oleifera extracts have promising therapeutic potential in enhancing ulcer healing, their clinical efficacy may be limited by factors such as oral bioavailability. This research contributes to the limited literature on the use of plant-based adjunctive therapies in leprosy ulcer management and underscores the need for further studies with larger samples and improved delivery mechanisms to optimize clinical outcomes.

Abstract

Tuberculosis (TB) ranks among the top 10 causes of mortality worldwide. Indonesia stands third in Southeast Asia for the highest prevalence of TB cases. One of the adverse effects of Anti-Tuberculosis Drugs (ATD) is hepatotoxicity. Approximately 20% of patients treated with Isoniazid show elevated Aspartate Aminotransferase (AST) and Alanine Aminotransferase (ALT) levels in their plasma. Despite the direct efficacy of ATD in managing TB, hepatotoxicity necessitates discontinuation of drug administration, leading to complications such as Multidrug-Resistant Tuberculosis (MDR-TB). Pegagan leaves (Centella asiatica) contain alkaloids with antibacterial properties. Extracts from Pegagan leaves have been shown to enhance expression of Cluster of Differentiation (CD)4, CD8, and macrophages. Furthermore, this extract can increase cytokine production, including Tumor Necrosis Factor-alpha (TNF-α) and Interferon-gamma (IFN-γ). Based on these premises, this study aims to investigate whether administration of Pegagan leaves extract significantly impacts levels of Hypoxia-Inducible Factor 1-alpha (HIF-1α), TNF-α, IFN-γ, Serum Glutamic Oxaloacetic Transaminase (SGOT), and Serum Glutamic Pyruvic Transaminase (SGPT) in teenage TB patients.
