Abstract

Background: Cosmetics are one of the most commonly encountered pharmaceutical products used daily by people of all ages, from babies to adults. With the rapid expansion of the beauty industry and e-commerce markets, the circulation of unregistered and hazardous beauty products has risen significantly, posing a serious public health threat. Objective: This community service activity was conducted at SMK Kesehatan Insan Bhakti Husada Lamongan to assess and enhance students’ knowledge regarding the dangers of illegal cosmetics. Vocational health students represent a crucial demographic, as they serve as future health advocates in their communities. Method: The method used in this activity is an interactive lecture method with evaluation through pre-test and post-test questionnaires. Results: The results show an average knowledge increase of 11.99%. The highest average increase was found in the question about types of heavy metals whose contamination levels are regulated by BPOM, with the average score rising from 22.72% in the pre-test to 90.90% in the post-test, demonstrating that targeted educational interventions can effectively eliminate critical knowledge gaps regarding regulatory safety standards. Conclusion: This community service activity successfully heightened students' awareness, proving that early educational outreach is a vital strategy for preventing the health risks associated with unauthorized cosmetic products.

Abstract

The month of Ramadan is a moment that is eagerly awaited by all Muslims in the world. Ramadan fasting is mandatory for every Muslim, but there is relief for sick people who can cancel it. People who experience health problems can affect the pattern of drug use as usual. It is important to pay attention to changes in the medication use schedule so that it does not affect the effect of the therapy being given or currently being undertaken, so education regarding the use of medication during the fasting month is necessary so that it is rational and ensures that the medication that enters the body provides maximum effectiveness without causing danger or bad effects on the body. The community service methods used are face-to-face lectures, discussions and question and answer methods which was attended by 34 posbindu participants. The results of providing this education show that the level of knowledge of respondents before being given education, namely 60%, is in the sufficient category, while the level of knowledge of respondents after being given education is 100% in the good category. Based on the research results, it can be concluded that the level of knowledge of respondents before and after being given education experienced a significant increase in knowledge with a p value <0.05. Community service activities through providing education can increase respondents' knowledge about the use of drugs during fasting.

Abstract

Epilepsi merupakan salah satu penyakit neurologis kronis yang ditandai dengan kejang berulang. Kepatuhan pengobatan berperan penting dalam mengendalikan kejang dan meningkatkan kualitas hidup pasien. Namun, rendahnya kepatuhan sering menjadi kendala dalam terapi, sehingga berpotensi meningkatkan risiko kekambuhan kejang. Mengetahui pengaruh kepatuhan pengobatan terhadap kekambuhan kejang pada pasien epilepsi di Rumah Sakit Muhammadiyah Lamongan. Penelitian ini menggunakan desain cross sectional dengan metode purposive sampling. Sampel pada penelitian ini adalah pasien dengan diagnosa epilepsy usia ≥16 tahun yang menjalani pengobatan epilepsi di poli rawat jalan. Evaluasi kepatuhan menggunakan kuesioner ARMS (Adherence to Refills and Medication Scale), sedangkan kekambuhan kejang dievaluasi selama 1bulan terakhir menggunkan wawancara dan data rekam medis. Sebanyak 85,19% pasien tercatat patuh terhadap pengobatan. Kekambuhan kejang masih terjadi pada 35,19% pasien. Analisis menunjukkan nilai p = 0,002, yang berarti terdapat pengaruh signifikan antara kepatuhan pengobatan dan kekambuhan kejang. Kepatuhan pengobatan berpengaruh signifikan terhadap kekambuhan kejang pada pasien epilepsi. Perlu adanya edukasi berkelanjutan dari tenaga kesehatan untuk meningkatkan kesadaran dan kepatuhan pasien dalam menjalani pengobatan, serta pemantauan rutin guna mencegah kekambuhan kejang.

Abstract

Background: Number of geriatric residents has increased annually. Owing to pathological and physiological conditions, the geriatric population tends to consume more medications, thereby increasing their risk of adverse side effects and drug interactions. Objective: This study was to evaluate the appropriateness of therapy for outpatient geriatric patients with cardiovascular disease at Muhammadiyah Lamongan Hospital from August to October 2023. Methods: American Geriatrics Society (AGS) Beers Criteria are one of the tools used to identify drugs whose potential harm outweighs the expected benefits and should be avoided in the elderly population. This study employed a descriptive method, with retrospective data collection from secondary sources, including medical records and electronic prescriptions. Results: Findings revealed that out of 252 prescriptions for geriatric patients with cardiovascular disease, four types of medications were potentially inappropriate according to the Beers Criteria: nifedipine with 23 prescriptions (9.13%), amiodarone with one prescription (0.40%), digoxin with 13 prescriptions (5.16%), and diltiazem with four prescriptions (1.59%). Conversely, three types of medications were deemed appropriate: aspirin (96 prescriptions, 38.10%), digoxin (30 prescriptions, 11.90%), and diltiazem (11 prescriptions, 4.37%). In conclusion, of the 252 prescriptions reviewed for geriatric patients with cardiovascular disease, three types of drugs are appropriate, while four types increase the risk of potentially inappropriate treatment (PIM) based on the Beers Criteria. Conclusion: These findings underscore the need for careful consideration to mitigate the risk of drug reactions. If the medication cannot be used in geriatric patients, an alternative therapy should be used or a dose adjustment may be necessary.

Abstract

Prescribing medications that do not comply with the official information stated on the drug label and marketing authorization, such as dose, age, route of administration, contraindications, and indications, is known as off-label drug use. Elderly patients may experience undesirable adverse effects due to off-label medication use. This study aimed to evaluate off-label drug use among elderly individuals with degenerative diseases at Sugio Primary Health Center. This study collected data retrospectively from the medical records of elderly patients with degenerative disorders at Sugio Primary Health Center using a descriptive non-experimental design. The sample was selected using a purposive sampling method. The inclusion criteria were geriatric patients aged ≥60 years diagnosed with degenerative diseases who received outpatient care during the period of January–August 2024. Data were collected using an observation sheet and analyzed using univariate analysis. The findings from 150 patients showed that 69 (46%) were categorized as off-label dose and 58 (38.7%) as off-label age. These findings indicate that off-label drug use still occurs among elderly patients, highlighting the need to evaluate the rationality of prescribing practices.

Abstract

Penggunaan obat off-label pada pasien anak diperlukan monitoring yang ketat untuk menghindari efek yang tidak dikehendaki. Efek samping dan risikonya mungkin lebih besar daripada potensi manfaatnya. Penelitian ini bertujuan untuk mengetahui penggunaan obat off-label pada pasien anak berdasarkan dosis, usia/ berat badan dan indikasi. Penelitian ini merupakan jenis penelitian deskriptif, pengumpulan data secara retrospektif, menggunakan data resep pasien anak di Puskesmas Deket Lamongan. Penelitian ini juga dilakukan analisis data untuk mengetahui obat off-label kategori usia/berat badan, dosis, dan indikasi obat. Penelitian ini dilakukan pada bulan Januari-Agustus 2021 menggunakan total sampling. Dari 471 resep ditemukan off-label dosis sebanyak 68(65%), Off-label usia sebanyak 20 (19%), Off-label indikasi sebanyak 17(16%). Dilihat dari kategori off-label diketahui bahwa off-label dosis ditemukan lebih banyak daripada off- label usia dan off-label indikasi. Off-label dosis banyak ditemukan karena kurangnya informasi dosis pediatrik, kurangnya formulasi pediatrik yang tepat, kurangnya data uji klinis. Penelitian peresepan obat off-label tinggi sehingga perlu pemilihan obat yang tepat serta monitoring dalam penggunaan obat pada anak.

Abstract

Background: The SARS-CoV-2 Delta variant surge was marked by high transmissibility and clinical severity, leading to substantial increases in hospitalizations and mortality. N-acetylcysteine (NAC) has been proposed as an adjunctive therapy for its mucolytic, antioxidant, and anti-inflammatory properties, particularly its ability to inhibit pro-inflammatory cytokines and restore glutathione levels. However, clinical evidence on its impact on hospital length of stay during the Delta variant surge remains limited. Objective: This study aimed to investigate the association between NAC therapy and length of hospital stay (LOS) among patients hospitalized with the SARS-CoV-2 Delta variant. Methods: A retrospective, cross-sectional observational study was conducted at Muhammadiyah Lamongan Hospital in Indonesia. Using purposive sampling, data were extracted from the medical records of 444 adult patients (≥18 years) with PCR-confirmed SARS-CoV-2 infection who were hospitalized between June and August 2021. All subjects received NAC therapy (≥1200 mg/day) as part of their clinical management. LOS was categorized as short (<10 days) or long (≥10 days) based on established clinical criteria. Results: The study population was predominantly male (50.5%), with the largest age group being 56–65 years (32.2%). The most prevalent comorbidities were pneumonia (60.4%), diabetes mellitus (21.0%), and hypertension (6.8%). Regarding clinical outcomes, 81.98% (n=364) of patients receiving NAC therapy were hospitalized for less than 10 days, while 18.02% (n=80) were hospitalized for 10 days or longer. Conclusion: Hospitalized patients with the SARS-CoV-2 Delta variant receiving NAC therapy predominantly had shorter hospital stays (≤10 days). These findings provide preliminary evidence of NAC's potential as a beneficial adjunctive treatment to expedite recovery.

Abstract

Background: Inappropriate use of antibiotics and analgesics among adolescents remains a public health concern, mainly due to self-medication practices and limited understanding of proper medication use principles, including DAGUSIBU (Get, Use, Store, Dispose). Low medication literacy may increase the risk of adverse effects and antimicrobial resistance. Objective: This community service activity aimed to improve students’ knowledge regarding the safe and appropriate use of antibiotics and analgesics at SMK NU Sekaran. Methods: The activity was conducted through interactive socialization and educational sessions supported by PowerPoint presentations and educational leaflets. The partner involved was SMK NU Sekaran. The improvement in knowledge was evaluated using a questionnaire consisting of 10 questions. Results: The mean pretest score was 7.62% and the posttest score was 8.76%, showing a statistically significant difference (p 0.001). The results indicated an increase in knowledge before and after the educational intervention.

Abstract

The COVID-19 pandemic has necessitated the exploration of various therapeutic strategies, including the repurposing of existing antiviral drugs. This narrative review examines the use of anti-retroviral agents in the treatment of COVID-19, focusing on their mechanisms of action, efficacy, and safety profiles. Key antiretrovirals discussed include Remdesivir, Lopinavir/Ritonavir, Ribavirin, Oseltamivir, Favipiravir, and Sofosbuvir. These agents primarily act by inhibiting RNA-dependent RNA polymerase (RdRp), a critical enzyme in the replication cycle of SARS-CoV-2. Clinical trials and in vitro studies have provided mixed results regarding their effectiveness, with some agents showing promise in reducing mortality and improving recovery times, while others have demonstrated limited efficacy. The review highlights the urgent need for further clinical research to optimize antiviral regimens and improve patient outcomes in the ongoing battle against COVID-19.
