Abstract

HIV IV/AIDS merupakan masalah kesehatan masyarakat global yang terus meningkat, termasuk di Indonesia. Di Kota Jambi, kasus HIV/AIDS terus mengalami kenaikan setiap tahun, dengan Puskesmas Simpang Kawat mencatat jumlah tertinggi selama dua tahun berturut-turut sebanyak 222 kasus pada 2022 dan meningkat menjadi 251 kasus pada 2023. Penelitian ini bertujuan untuk mengevaluasi pelaksanaan program penanggulangan HIV/AIDS di wilayah kerja Puskesmas Simpang Kawat menggunakan model evaluasi CIPP (Context, Input, Process, Product). Penelitian menggunakan kualitatif dengan pendekatan studi kasus. Data dikumpulkan melalui wawancara mendalam terhadap 10 informan selama Januari Februari 2025. Analisis data dilakukan dengan software Nvivo 12 menggunakan triangulasi sumber dan metode. Hasil penelitian menunjukkan bahwa pada aspek Context masih terdapat stigma social serta sebagian informan belum memahami tujuan program. Pada aspek Input, SDM belum memadai karena tidak adanya kader HIV di lapangan, pelatihan tim belum merata, dana penyuluhan terbatas, dan belum tersedia fasilitas komputer. Pada aspek Process, perencanaan sudah dilaksanakan dengan baik namun pelaksanaan kegiatan belum optimal dan masih terkendala, monitoring dan evaluasi dilakukan oleh Kepala Puskesmas dan pihak Dinas Kesehatan. Pada aspek Product, capaian layanan mencapai 117% dari target, namun tingginya capaian berasal dari pasien luar wilayah kerja puskesmas Simpang Kawat. Pelaksanaan program penanggulangan HIV/AIDS di wilayah kerja Puskesmas Simpang Kawat sudah terlaksana cukup baik namun masih terdapat kendala pada aspek context, input dan proses pelaksanaanya.

Abstract

This study aims to analyze the effect of management practices on improving service quality through the practice of integrated infrastructure EFQM and Six Sigma Public Health Center (PHC) in Jambi Province. This research is a quantitative research using a cross sectional design approach through two stages of research. The research data was obtained using a questionnaire from a sample of 560 civil servant employees in 24 PHCs that were randomly selected and analyzed by second order using Smart-PLS 3.0. The results of the study found that the implementation of management practices, infrastructure practices had a fairly good average by employees and each had a direct effect on the quality of PHC services. The practice of core infrastructure is able to partially mediate between management practices and the quality of PHC services. However, in practice, not all PHCs have implemented quality management practices.

Abstract

Compliance with independent participants in paying National Health Insurance (JKN) contributions in Jambi City is not optimal. This is because Jambi City is the district/city that has the largest number of independent participants in arrears, namely 103,195 (16.95%) participants. Contributions greatly influence the smooth running of BPJS Health so that there is no deficit. The aim of this research is to determine the factors associated with compliance with paying monthly BPJS health contributions for independent participants in the informal sector in Jambi City. This research uses a cross sectional design, with purposive sampling data collection techniques. The samples taken were 139,091 independent BPJS participants in the informal sector. Analysis was carried out using univariate and bivariate tests with the chi-square test. The results of the study show that there is a relationship between education (p-value=0.039) and income (p-value=0.005) with compliance with paying BPJS Health contributions and there is no relationship between employment (p-value=0.190), contribution rates (p-value =0.129) and access to payments (p-value=0.887) with compliance in paying BPJS Health contributions for independent participants in the informal sector in Jambi City. BPJS Health must strictly enforce sanctions in accordance with applicable regulations if payments are late. People are expected to choose a treatment class that suits their ability to pay JKN contributions or if they feel they cannot afford it, people can apply to become PBI participants.

Abstract

Background: The incidence of chronic diseases in Jambi City is still high from year to year. The Referback Program (PRB) is one of the government's efforts through BPJS Health in collaboration with health facilities to ensure chronic disease sufferers have easier access to obtain the medicines they need at an efficient cost. This research aims to analyze the main problems of quality control and cost control of PRB implementation in terms of man, material-machine, method, market, money and time elements at FKTP in Jambi City. Methods: The research was conducted using qualitative methods using the perspective of Root Cause Analysis (RCA) theory to form research procedures. The informants consisted of the person in charge of PRB, the head of the community health center and PRB participants at FKTP in Jambi City. The research instrument is a structured interview sheet. Data analysis uses thematic analysis techniques according to the RCA flow Results: The research results found that the main cause of the problem was weak coordination between PRB officers and BPJS Health in implementing PRB at FKTP. Conclusion: It is recommended that BPJS Health be more active in providing outreach and special training to PRB officers as well as setting up a clear monitoring and evaluation system regarding the implementation of PRB in FKTP

Abstract

Background: The national health insurance system which implements a tiered referral system requires BPJS participants to visit the community health center as a First Level Health Facility (FKTP) before seeking treatment at an Advanced Level Health Facility (FKTl). Research Objective: To determine the availability and distribution of Jambi City Advanced Level (FKTL) facilities. Research method: Quantitative with an exploratory descriptive design. Research results: Show that all sub-districts in Jambi City have been reached by FKTL, Putri Ayu Community Health Center made the most referrals to FKTL at 5,927 due to being close to several FKTL, while the FKTL with the highest referrals was DKT Jambi Hospital, namely around 65,375. The most common illnesses referred to FKTL are hypertension, diabetes mellitus, coronary heart disease, stroke and kidney failure. Raden Mattaher Regional Hospital is a hospital that receives many referrals for this disease, one of the reasons is because it has the largest number of specialist doctors in Jambi City. Conclusion: The distribution of Jambi City Hospitals is quite even, the number of FKTL referrals can be influenced by the distance or distribution of health centers close to the FKTL and the availability or ability of medical personnel to treat referral diseases.

Abstract

The prevalence of hypertension in Jambi City has always increased in the last 5 years, including in the working area of ​​the Talang Bakung Health Center. On the other hand, the costs of treating and treating hypertension that must be borne by the community are relatively high, especially if complications occur and are not covered by insurance, so there is a risk of catastrophic expenses that trigger the fragility of the household economy due to illness. The purpose of the service activity is to provide education and outreach about hypertension and its impact on health costs. The activity was attended by 30 participants with hypertension in the Talang Bakung Health Center area. The core activities include providing education about hypertension and its impact on health costs, socializing catastrophic hazards and socializing various supporting programs from the Government. The results of the activity showed that 80% of the participants experienced a significant increase in knowledge and there was a difference in the participants' knowledge between before and after the PPM activity (p=0.000). It is recommended to the Government and Puskesmas to increase campaign efforts to mobilize the community to adopt a healthy and proactive lifestyle in various health programs that support saving health costs so as to avoid catastrophic expenses.

Abstract

Kondisi metabolik yang disebut diabetes didefinisikan oleh peningkatan kadar glukosa darah yang lebih tinggi dari biasanya. Ulkus diabetik, juga dikenal sebagai luka diabetik, merupakan akibat dari masalah pembuluh darah dan neuropati yang mempengaruhi ekstremitas bawah. Dengan menjaga luka tetap lembab, perawatan dengan metode moist wound healing yang mempercepat pertumbuhan jaringan dan mempercepat proses penyembuhan. Proyek pengabdian ini bertujuan untuk meningkatkan kesadaran masyarakat akan pentingnya menjaga kesehatan, meningkatkan pengetahuan dan wawasan tentang pengertian merawat luka diabetes (ulkus), dan mengedukasi masyarakat mengenai hal tersebut. Kegiatan yang dilakukan di salah satu rumah warga sekitar tersebut diikuti oleh sepuluh orang. Pengabdian ini meliputi serangkaian kegiatan yang meliputi melakukan pre-test, memantau tekanan darah dan gula, memberikan informasi pendekatan moist wound healing (penyembuhan luka lembab) untuk pengobatan ulkus diabetik, dan diakhiri dengan post-test. Berdasarkan hasil evaluasi, pemahaman warga mengenai cara penanganan luka diabetes (ulkus) meningkat setelah mendapat penyuluhan, dan peserta menyatakan antusias terhadap kegiatan tersebut. Alternatif yang paling efektif untuk mengurangi dampak memperdalam luka diabetes adalah dengan melakukan penyuluhan ini

Abstract

Kecelakaan lalu lintas di jalan raya setiap tahunnya selalu mengalami peningkatan terkait peristiwa terjadinya kecelakaan dan korban jiwa akibat dari kecelakaan. Usia remaja mendominasi korban kecelakaan lalu lintas, dua per tiga kematian berdampak pada usia produktif 18-59 tahun. Tujuan dari kegiatan PPM ini adalah untuk memberikan Pelatihan Keselamatan Berkendara Sepeda Motor Dalam Menekan Angka Kecelakaan Lalu Lintas Bagi Siswa SMK Negeri 4 Kota Jambi. Metode yang digunakan adalah pendekatan kontekstual dan partisipatif, dengan model penyuluhan, pelatihan, dan pemutarab video. Kegiatan pelatihan dan pendampingan siswa pelatihan keselamatan berkendara sepeda motor ini dilaksanakan melalui beberapa tahapan yaitu tahap perencanaan, pelatihan dan evaluasi. Evaluasi kegiatan dilakukan dengan melihat pengetahuan siswi sebelum dan sesudah diberikannya pelatihan didapatkan hasil ada peningkatan nilai rata-rata dan didapatkan nilai p-value 0.0001 < 0.05 artinya terdapat perbedaan yang signifikan antara tingkat pengetahuan sebelum dan sesudah diberikan edukasi. Pengabdian ini terbukti dapat meningkatkan pengetahuan siswi tentang kemanan berkendara sehingga nantinya diharapkan dapat menurunkan angka kecelakaan lalu lintas.

Abstract

ABSTRACT The management of Health Operational Assistance funds at Puskesmas is crucial so that budget planning for each program can be carried out as needed. Based on information obtained from the DJPK of the Ministry of Finance (2020), there is a difference between the allocation and realization of the BOK budget in all provinces, with some areas experiencing budget overruns. Puskesmas with the lowest realization of BOK funds in Jambi City are Puskesmas Kebun Kopi and Puskesmas Talang Bakung with the realization of the use of BOK funds only reaching 57%. This study aims to determine how the management of BOK funds at the Kebun Kopi Health Center. This research is a qualitative research with a descriptive approach. The location of this research is at the Kebun Kopi Health Center with a total of 7 informants. In the context aspect, the objectives of the BOK Fund have been aligned with the basic service needs of the community. However, in terms of input, the human resources who manage the funds are not competent. In the process, the main obstacles lie in the delay of technical guidelines, cross-sector coordination, and double workload. In terms of products, the realization of funds is still not optimal. Conclusion: The management of BOK funds at Puskesmas Kebun Kopi still faces challenges in the aspects of human resources, planning, and reporting. Training and strengthening the capacity of managers are needed so that BOK funds can be used more efficiently and on target. Keywords: BOK Fund, Management, Puskesmas, CIPP, Qualitative  ABSTRAK Pengelolaan dana Bantuan Operasional Kesehatan di Puskesmas menjadi krusial agar perencanaan anggaran untuk tiap program dapat terlaksana sesuai kebutuhan. Berdasarkan informasi yang diperoleh dari DJPK Kemenkeu (2020), tercatat adanya perbedaan antara alokasi dan realisasi anggaran BOK di seluruh provinsi, dengan beberapa daerah mengalami kelebihan anggaran. Puskesmas dengan realisasi Dana BOK paling rendah di Kota Jambi adalah Puskesmas Kebun Kopi dan Puskesmas Talang Bakung dengan realisasi penggunaan Dana BOK yang hanya mencapai 57%. Penelitian ini bertujuan untuk mengetahui bagaimana pengelolaan dana BOK di Puskesmas Kebun Kopi. Penelitian ini merupakan penelitian kualitatif dengan pendekatan deskriptif. Lokasi penelitian ini di Puskesmas Kebun Kopi dengan jumlah informan sebanyak 7 orang. Pada aspek konteks, tujuan Dana BOK telah selaras dengan kebutuhan pelayanan dasar masyarakat. Namun, dari sisi input, SDM yang mengelola dana tidak sesuai kompetensi. Pada proses, kendala utama terletak pada keterlambatan juknis, koordinasi lintas sektor, serta beban kerja ganda. Dari sisi produk, realisasi dana masih belum optimal. Pengelolaan Dana BOK di Puskesmas Kebun Kopi masih menghadapi tantangan pada aspek SDM, perencanaan, dan pelaporan. Diperlukan pelatihan dan penguatan kapasitas pengelola agar dana BOK dapat digunakan lebih efisien dan tepat sasaran. Kata Kunci: Dana BOK, Pengelolaan, Puskesmas, CIPP, Kualitatif

Abstract

The Public Health Development Index (IPKM) is composite indicator that describe the progress of health development in a district. Enviromental health is one of the indicator contained in the IPKM. There are 2 sub-indicator for environmental health, access to clean water and access to sanitasion. This research was conducted in Tanjung Jabung Timur regency with the aim of knowing the description of IPKM achivements on environmental health indicator in 2016-2018. Methodology this was quantitative research. This study was conducted 5 months starting from May to October 2021. Data analyzed used public health development index formula that has been determined by calculating weights for each indicator and then used minimum and maximum standars values. This study found that the sub-index value of the group from 2016-2018 continued to increase. In 2016 amounted to 0.111411387, in 2017 amounted to 0.363059484, and in 2018 amounted to 0,696495001. The results of this study indicate that for 3 consecutive years the environmental health conditions have continued to improve towards a better direction, both in terms of access to clean water and access to sanitation. However, it still to improvements access to clean water and access to healthy sanitation so that it evenly distributed for all communities in Tanjung Jabung Timur Regency.
