Abstract

Latar Belakang : Low Back Pain (LBP) adalah gejala ketidaknyamanan pada daerah punggung bagian bawah yang berupa rasa sakit dan gangguan pada sistem muskuloskeletal terkait. Penggunaan tulang belakang yang berlebih menyebabkan kemampuan fungsional penderitanya terganggu. Penderita LBP mekanik sering merasakan nyeri yang berkaitan dengan aktivitas kerja yang berat dan dapat mempengaruhi kualitas hidup penderitanya.Tujuan Penelitian : Mengetahui adanya hubungan tingkat kemampuan fungsional dan derajat nyeri pada pasien LBP mekanik di Instalasi Rehabilitasi Medik RSUP dr Kariadi SemarangMetode Penelitian : Penelitian ini merupakan penelitian deskriptif analitik dengan menggunakan data primer BPFS dan VAS yang diperoleh dari kuesioner yang diisi oleh pasien. Subjek penelitian adalah pasien penderita LBP mekanik di Poliklinik Rehabilitasi Medik RSUP Dr. Kariadi Semarang.Hasil Penelitian : Hasil uji hubungan BPFS dan VAS menggunakan Spearman correlation negatif sangat kuat bermakna (r = -0,920 , p = 0,000). Hal ini menunjukkan bahwa semakin tinggi nilai BPFS akan semakin rendah nilai VAS.Kesimpulan : Rerata pasien LBP mekanik memiliki kemampuan fungsional yang buruk dan derajat nyeri yang sedang hingga buruk.Terdapat hubungan kuat antara kemampuan fungsional dan derajat nyeri pada pasien LBP mekanik di Instalasi Rehabilitasi Medik RSUP dr. Kariadi Semarang.

Abstract

Latar Belakang: Kondisi underweight dapat memengaruhi tumbuh kembang anak. Olahraga dapat menstimulasi sekresi human growth hormone (hGH) dan endorfin. hGH dapat meningkatkan sintesis protein, meningkatkan massa otot, sementara endorfin dapat memperbaiki mood anak, memperbaiki nafsu makan anak, sehingga asupan makanan anak meningkat.Tujuan: Membuktikan perbedaan indeks massa tubuh sebelum dan sesudah latihan Senam Sehat Anak Indonesia pada anak dengan underweight.Metode: Penelitian eksperimen one group pre post test menggunakan 12 subjek penelitian dengan purposive sampling. Instrumen penelitian menggunakan timbangan dan microtoise. Analisis data menggunakan uji t berpasangan.Hasil: Rerata indeks massa tubuh sebelum dan sesudah perlakuan dengan rerata sebelum perlakuan 13,40 ± 0,55 kg/m2 dan 13,9±0,59 kg/m2 setelah perlakuan serta nilai p=0,005.Simpulan: Terdapat perbedaan bermakna indeks massa tubuh sebelum dan sesudah latihan Senam Sehat Anak Indonesia pada anak dengan underweight

Abstract

Latar Belakang: Merokok menyebabkan perubahan struktur, fungsi saluran pernapasan dan jaringan paru. Merokok aktif akan mempercepat penurunan faal paru. Salah satu cara untuk mengetahui fungsi faal paru adalah melalui pemeriksaan arus puncak ekspirasi (APE). Deep breathing dapat meningkatkan compliance paru dan mencegah kolaps sehingga memperbaiki pertukaran gas dan akhirnya memperbaiki nilai APE. Tujuan: Membuktikan perbedaan nilai APE sebelum dan setelah latihan deep breathing tipe akut maupun kronik pada perokok aktif. Metode: Penelitian eksperimen one group pre post test menggunakan 10 subjek penelitian dengan purposive sampling. Instrumen penelitian menggunakan timbangan, microtoise dan peak flow meter. Analisis data menggunakan uji t berpasangan. Hasil: Rerata APE tipe akut sebelum dan sesudah perlakuan adalah 546,0 ± 64,1 dan 553,0±63,9; sedangkan rerata APE tipe kronik sebelum dan sesudah perlakuan adalah 522,5±66,7 dan 553,0±63,9. Terdapat perbedaan bermakna pada uji analisis tipe akut maupun kronik ( tipe akut p=0,029; tipe kronik p=0,002). Simpulan: Terdapat perbedaan bermakna nilai APE sebelum dan setelah latihan deep breathing tipe akut maupun kronik pada perokok aktif.Kata kunci: Latihan deep breathing, arus puncak ekspirasi, perokok aktif.

Abstract

Latar Belakang: Osteoporosis merupakan salah satu penyakit tidak menular terbanyak di dunia. Prevalensi osteoporosis pada wanita di atas usia 50 tahun lebih tinggi dibandingkan dengan pria. Osteoporosis meningkatkan resiko terjadinya fraktur. Gold standard dalam penegakkan diagnosis osteoporosis adalah pemeriksaan Bone Mineral Density (BMD) dengan alat Dual Energy X-ray Absorptiometry (DXA). Jumlah perangkat DXA di Indonesia masih terbatas, dan untuk periksa butuh biaya yang mahal, maka dibutuhkan alat penapisan osteoporosis yang mudah digunakan. Salah satu alat penapisan yang praktis adalah Osteoporosis Self-assessment Tools for Asians (OSTA).Tujuan: Mengetahui nilai diagnostik OSTA terhadap DXA untuk penapisan osteoporosis pada wanita post menopause di Rumah Sakit Panti Wilasa Dr.Cipto Semarang.Metode: Penelitian ini merupakan uji diagnostik. Dilakukan pemeriksaan OSTA dan hasilnya dibandingkan dengan hasil pemeriksaan DXA pada 97 catatan medik (CM) wanita pasca menopause. Hasil yang diperoleh di uji dengan tabel 2x2 untuk memperoleh hasil sensitivitas, spesifisitas, positive predictive value (PPV), dan negative predictive value (NPV).Hasil: Hasil pemeriksaan OSTA yang dibandingkan dengan DXA memperoleh hasil sensitivitas 92,5%, spesifisitas 42,1%, PPV 52,9%, dan NPV 88,9%.Simpulan: OSTA merupakan alat penapisan yang efektif untuk osteoporosis pada wanita post menopause di RS Panti Wilasa Dr. Cipto Semarang.

Abstract

BACKGROUND: Serum levels of 25-hydroxyvitamin D (25(OH)D), prostaglandin E2 (PGE2), and cyclooxygenase 2 (COX2) expression differ between breast cancer stages. Since, previous studies showed mixed results, in this study, we aimed to analyze vitamin D levels related to breast cancer stages and serum levels of COX2 and PGE2 in Indonesia.METHODS: This was a cross sectional study involving 75 breast cancer patients. Subjects were divided into 3 groups, namely operable early stage (K1), locally advanced stage (K2), and advanced stage (K3). Venous blood samples were taken from each subject, then were analyzed for the 25(OH)D, COX2, and PGE2 serum levels by enzyme-linked immunosorbent assay (ELISA) method.RESULTS: There were significant differences in 25(OH)D among groups (p=0.012); between K1 and K2 (p=0.009) and between K1 and K3 (p=0.023). However, there was no significant difference in serum COX2 level (p=0.328). There were significant differences of PGE2 among groups (p=0.002); between K1 and K2 (p=0.036) and between K1 and K3 (p=0.001). Correlation test showed that there were differences between 25(OH)D serum levels and PGE2 serum level (r=0.306, p=0.008) and also between 25(OH)D serum level and breast cancer stage (r=-0.229; p=0.048).CONCLUSION: There were differences in serum Vitamin D and PGE2 levels at various stages of breast cancer. Serum 25(OH)D levels had weak correlation with breast cancer stage and PGE2 serum level. Serum vitamin D level in advanced breast cancer were lower than early stage breast cancer and indicate a poor prognosis.KEYWORDS: breast cancer, 25-hydroxyvitamin D, cyclooxygenase 2, prostaglandin E2

Abstract

BACKGROUND: Inflammation occurs in chronic diseases such as type 2 diabetes mellitus (DM). Insulin resistance and inflammation in type 2 DM with obesity can increase interleukin (IL)-6, causing an increase in hepcidin synthesis in the liver. Increased inflammation can exacerbate the course of type 2 DM. This study aims to prove that there are differences in the levels of hepcidin and IL-6 between obese and non-obese type 2 DM.METHODS: This cross-sectional study was conducted on 61 patients with type 2 DM, consist of 22 male and 39 female with an age of more than 40 years. Type 2 DM subjects were obtained from a doctor's diagnosis and were divided into obese and non-obese groups based on body mass index (BMI). Hepcidin and IL-6 levels were examined using the Enzyme-Linked Immunosorbent Assay (ELISA) principle. The data were analyzed using an independent t-test and Mann-Whitney test.RESULTS: The mean level of hepcidin in the obese with type 2 DM group was 25.32±11.54 ng/mL, and non-obese was 11.94±5.31 ng/mL. The median level of IL-6 in the obese with type 2 DM group was 11.9 (5-61) pg/mL, and non-obese 4.8 (1.5-9.8) pg/mL. There was a significant difference in hepcidin and IL-6 levels between the obese and non-obese groups (p=0.000).CONCLUSION: Hepcidin and IL-6 levels in the obese group with type 2 DM were higher than non-obese group.KEYWORDS: type 2 diabetes mellitus, obesity, hepcidin, interleukin-6

Abstract

BACKGROUND: Lack of appetite in young children leads to growing incidences of physical and mental growth disorders. Supplementation of certain micronutrients can increase appetite and improve nutritional status. This study aims to analyze the effects of zinc and iron supplementation on appetite, nutritional status and intelligence quotient (IQ) in young children.METHODS: An experimental study withrandomized control group pre/post-test design was conducted in Semarang, Indonesia. A total of 68 children were divided into four groups. The first group was the control group, which was given a placebo; the second group was given a zinc supplement at 10 mg/day; the third group was given an iron supplement at 7.5 mg/day; andthe fourth group was given zinc and iron for three months. Appetite was assessed based on eating frequency and energy intake. Nutritional status was assessed by weight per age (W/A) and height per age (H/A) z score. IQ score was assessed based on Wechsler Preschool and Primary Scale of Intelligence (WPPSI).RESULTS: Before intervention, low zinc intake was observed in 27.7% of the subjects and low iron intake was observed in 58.5% of them. After intervention, appetite in the second and fourth groups increased. W/A z score increased in the second and third groups. IQ score increased in the third group. No significant effect on H/A z score was observed in all groups.CONCLUSION: Supplementation of zinc and iron for three months had a positive effect on appetite, body weight and IQ score but no significant effect on body height.KEYWORDS: appetite, zinc, iron, growth

Abstract

ABSTRACT Background: expression of VEGF-C and CA 15-3 may be useful to differentiate between malignant and benign breast tumour because VEGF-C plays a role in promoting angiogenesis and lymphangiogenesis in malignant processes and CA 15-3 is the soluble form of transmembrane protein MUC1, a tumour marker which shows higher expression in breast cancer.Objective: to determine the diagnostic value of VEGF-C and CA 15-3 as tumour markers in patients with breast cancer.Methods: this diagnostic study recruited 76 patients that underwent surgical biopsy procedures at Dr. Kariadi and Pantiwilasa Citarum Hospitals Semarang. The VEGF-C and CA 15-3 levels in blood specimens taken before surgical biopsy procedure was determined using ELISA method. An ROC curve and AUC were used to establish the cut-off points and diagnostic value. Pathology examination results from the biopsy specimens were used as the gold standard.Results: the cut-off value for VEGF-C and CA 15-3 were 989.50 pg/mL and 74.00 U/mL. Sensitivity for VEGF-C, CA 15-3 and VEGF-C+CA 15-3 were 76.6%, 64.1% and 89.1%. Specificity for VEGF-C, CA 15-3 and VEGF-C+CA 15-3 were 75.0%, 75.0% and 50.0%. The AUC for VEGF-C, CA 15-3 and VEGF-C+CA 15-3 was 0.831 (95% CI = 0.727-0.934), 0.742 (95% CI = 0.628-0.856) and 0.840 (95% CI = 0.742-0.938).Conclusion: VEGF-C in combination with CA 15-3 is the best diagnostic parameter for breast cancer and has the best accuracy as a tumour marker for breast cancer.

Abstract

Background: The SARS-CoV-2 rapid diagnostic test antibody (RDT-Ab) was most often used as an early detection tool for COVID-19 at the beginning of pandemic. Whereas the antibody response was formed in the second week after the onset of symptoms.Objective: To evaluate the diagnostic value of the SARS-CoV-2 RDT-Ab, including sensitivity (Se), specificity (Sp), positive predictive value (PPV), negative predictive value (NPV), positive likelihood ratio (PLR), and negative likelihood ratio (NLR), in patients at Diponegoro National Hospital, Semarang, Indonesia.Methods: Data subjects have been selected retrospectively using purposive sampling based on inclusion criteria (patients who had shortness of breath, pneumonia, suspected, possible, or confirmed COVID-19, and data on the results of the SARS-CoV-2 RDT-Ab IgM and/or IgG (Leccurate® SARS-CoV-2 Antibody Rapid Test Kit) with a valid RT-PCR as gold standard) and exclusion criteria (patients who only had one of either SARS-CoV-2 RDT-Ab or RT-PCR). Researchers analyzed the diagnostic value of SARS-CoV-2 RDT-Ab with RT-PCR which gave the possibility of true-positive, false-positive, true-negative, and false-negative results arranged in a 2x2 table. According to WHO, the diagnostic value is said to be good at least having a sensitivity value of 80% and specificity of 97%.Results: The diagnostic value of SARS-CoV-2 RDT-Ab with RT-PCR, which was evaluated from 1142 patients retrospectively, included IgM (Se 65.25%, Sp 89.51%, PPV 46.70%, NPV 94.81%, PLR 6.22, NLR 0.39), IgG (Se 58.16%, Sp 93.01%, PPV 53.95%, NPV 94.04%, PLR 8.32, NLR 0.45), IgM and IgG (Se 53.90%, Sp 94.21%, PPV 56.72%, NPV 93.55%, PLR 9.30, NLR 0.49), IgM and/or IgG (Se 69.50%, Sp 88.31%, PPV 45.58%, NPV 95.36%, PLR 5.95, NLR 0.35).Conclusion: SARS-CoV-2 RDT-Ab (Leccurate® SARS-CoV-2 Antibody Rapid Test Kit) is not ideal to be used as a rapid diagnostic test for COVID-19.Keywords: COVID-19, Rapid diagnostic test, RT-PCR, SARS-CoV-2 antibody

Abstract

Weight loss is more of a worldwide health problem and a risk factor for chronic disease. The decrease in serum vitamin D levels in body weight leads to more low grade inflammation. This study aims to determine the relationship between the platelet index, namely MPV, P-LCR as a marker of inflammation and vitamin D with body mass index (BMI). This study was a cross-sectional study of 78 women at the Diponegoro National Hospital with overweight (BMI> 23) in July - September 2020. Data analysis was done using Pearson and Spearman tests and significant if p <0.05. Of the 78 research samples, there was a significant relationship between BMI and MPV (r = 0.404; p = 0.000); P-LCR (r = 0.425; p = 0.000) and 25 (OH) D (r = -0.231; p = 0.04). Low vitamin D levels in obesity and low-grade inflammation increase pro-inflammatory cytokines. Increased leptin levels in obesity contribute to platelet hyperreactivity. Increased platelet activity compensates for the platelets so that the MPV and P-CLR values increase. There is a moderate significant positive relationship between MPV, P-CLR and BMI and a weak significant negative relationship between 25 (OH) D and BMI.
