Abstract

Patients in intensive care unit (ICU) have a high risk for systemic bacterial infection. Serum procalcitonin (PCT) known as a markerto predict bacterial infection, systemic inflammation responsse (SIRS) or sepsis. Another simple and easy indicator is by using leucocytecount-differential count, absolute neutrophyl count (ANC), and immatur/total (I/T) neutrophyl ratio. The aim of this study was toknow the comparation of the leucocyte count-differential count, ANC, and I/T ratio with procalcitonin serum value in patients suspectto systemic bacterial infection at ICU. A cross sectional study on 20 patients suspected with systemic bacterial infection in ICU. The datawas classified in 3 groups based on PCT serum value: group A (< 0.5 ng/mL), group B (0.5–2 ng/mL) and group C (> 2 ng/mL). Thedata was analysed by one way ANOVA test if normally distributed, and by Kruskall-Wallis test if not normally distributed. Significancywas confirmed at p < 0.05. A post hoc and Mann-Whitney test performed on a significant result. The frequency of group A was 3 (15%),group B = 5 (25%), and group C = 12 (60%). There is no significant difference on leucocyte count in 3 groups (p = 0.953), neutrophylI/T ratio (p = 0.259), ANC (p = 0.91), eosinophyl count (p = 0.287), segment neutrophyl (p = 0.094), and monosit (p = 0.152).There was a difference on lymphocyte count (p = 0.01) between group C with group A and group B and there was a difference on staffneutrophyl count (p = 0.029) and total neutrophyl count (p = 0.003) between group A with group B and C (p = 0.029). In this studywere found differences on lymphocyte, staff neutrophyl and total neutropyl count to the PCT value

Abstract

The distribution of fat tissue is related to the risk of metabolic diseases. Parathyroid Hormone (PTH) is an essential hormone for calcium homeostasis. According to several types of research, body fat affects PTH levels. Currently, Body Mass Index (BMI) is not the only parameter needed to identify the body fat distribution in accordance with chronic disease risks such as Waist Circumference (WC), Waist to Hip Ratio (WHR), and Waist to Height Ratio (WHtR). The study aimed to determine the relationship between body anthropometric measurement and PTH. A cross-sectional study was performed on a healthy population of 75 healthy female volunteers with a BMI ≥ 23 kg/m2. Waist circumference, WHR, WHtR, and BMI measurements were carried out and followed by the PTH fragment 1-84 (PTH1-84) test. Data were analyzed using the Spearman test with a significance of p<005. There was no significant correlation between PTH and WHR (r=0.057; p=0.628). There was weak correlation between PTH and BMI (r=0.268; p=0.020), WC (r=0.287; p=0.012) and WHtR (r=0.238; p=0.04). Body mass index, WC, and WHtR can be used as anthropometric parameters to determine PTH disorders

Abstract

Anak usia sekolah perlu mendapatkan perhatian dalam kesehatan tubuhnya untuk dapat menunjang proses tumbuh kembang yang optimal. Salah satu organ yang penting dalam menunjang proses tumbuh kembang adalah mata. Pemeriksaan kesehatan mata diperlukan untuk menapis dan mendiagnosis penyakit mata secara dini. Anak-anak SDN Gulon 2 mayoritas mempunyai orang tua yang bekerja sebagai petani dan memiliki tingkat pengetahuan tentang kesehatan yang rendah. Anak-anak SDN Gulon 2 tersebut belum pernah mendapatkan pemeriksaan kesehatan mata sebelumnya. Tujuan kegiatan pengabdian masyarakat ini adalah penemuan kasus penyakit mata pada anak SDN Gulon 2 dan pemberian edukasi kepada guru SD mengenai penyakit mata yang ditemukan serta pemeberian saran untuk penatalaksanaan penyakit yang ditemukan. Pemeriksaan kesehatan mata yang dilakukan meliputi pemeriksaan visus atau tajam penglihatan dengan Optotype Snellen, pemeriksaan segmen anterior mata dengan loop dan senter, serta pemeriksaan segmen posterior mata dengan oftalmoskop. Edukasi kepada guru dilakukan setelah pemeriksaan selesai dilakukan dan didapatkan suatu diagnosis penyakit mata. Kegiatan pengabdian masyarakat ini berhasil menemukan kelainan tajam penglihatan sebanyak 19 kasus, kelainan segmen anterior 1 kasus dan 1 kasus kelainan segmen posterior. Kegiatan pengabdian masyarakat seperti ini perlu dilakukan di sekolah-sekolah lain di Desa Gulon, dan tidak hanya pemeriksaan kesehatan mata namun kesehatan tubuh secara menyeluruh.

Abstract

Latar Belakang: Penyakit paru obstruktif kronik (PPOK) adalah salah satu penyebab kematian terbanyak di dunia. Gangguan kebugaran kardiorespirasi penderita PPOK dan meningkatnya keadaan inflamasi pada penderita PPOK akan menyebabkan penurunan kualitas hidup bagi penderita PPOK. Latihan pernafasan berupa Hatha Yoga dan Tai Chi merupakan bentuk latihan pernafasan yang terbukti manfaatnya pada penderita gangguan fungsi paru. Penelitian ini bertujuan untuk menganalisis perbedaan efektivitas Hatha Yoga dan Tai Chi terhadap kebugaran kardiorespirasi dan kondisi inflamasi pada penderita PPOK.Metode: Penelitian berdesain randomized controlled pre and post experimental, dilakukan di Balai Kesehatan Paru Masyarakat (BKPM) Semarang, dengan subyek 11 penderita PPOK di BKPM Semarang yang memenuhi kriteria inklusi. Subyek dibagi menjadi dua kelompok. Kelompok I berjumlah 5 orang diberi intervensi latihan Hatha Yoga 3 kali/ minggu selama 6 minggu, kelompok II berjumlah 6 orang diberi intervensi latihan Tai Chi 3 kali/ minggu selama 6 minggu. Kedua kelompok dilakukan pemeriksaan uji jalan 6 menit/ 6 MWT (6 minute walk test) dan pemeriksaan jumlah leukosit dan neutrofil sebelum dan setelah 6 minggu perlakuan.Hasil: Rerata VO2 max kelompok I dan II adalah 8,22± 1,24 dan 9,12 ± 1,62. Tidak didapatkan perbedaan VO2 max sebelum dan setelah latihan pada kelompok I dan II dengan p=0,33 dan p=0,78. Rerata jumlah leukosit dan neutrofil pada kelompok I dan II adalah 7.700 ± 2.137, 8.400 ± 2.520, 4.561 ± 2.069 dan 6.079 ± 1.823/ mm3. Tidak didapatkan perbedaan jumlah leukosit dan neutrofil sebelum dan setelah latihan pada kelompok I dan II dengan p=0,63 dan p=0,097.Simpulan: Tidak didapatkan perbedaan efektivitas Hatha Yoga dan Tai Chi terhadap kebugaran kardiorespirasi dan kondisi inflamasi pada pasien PPOK. Kata kunci: Hatha Yoga, Leukosit, Neutrofil, PPOK, Tai Chi, Uji jalan 6 menit

Abstract

Latar belakang: Anemia merupakan manifestasi klinis yang sering dijumpai pada pasien Penyakit Ginjal Kronik (PGK) dengan hemodialisis. Penyebab utama anemia pada PGK adalah defisiensi dari eritropoetin (EPO) yang diproduksi di ginjal. Penurunan kadar EPO menyebabkan gangguan eritropoiesis, sehingga produksi eritrosit menurun. Kondisi kronis pada PGK sering dihubungkan dengan penyakit komorbid pada beberapa organ, diantaranya disfungsi tiroid. Disfungsi hormon tiroid juga dapat mempengaruhi parameter eritrosit yang meliputi MCV, MCH, MCHC, dan RDW. Penelitian ini bertujuan untuk mengetahui hubungan TSH dan parameter eritrosit pada penyakit ginjal kronik dengan hemodialisis.Metode: Penelitian belah lintang pada 30 pasien PGK dengan hemodialisis di RSUP Dr. Kariadi Semarang selama Agustus – September 2017. Pemeriksaan Parameter eritrosit (MCV, MCH, dan MCHC dan RDW) dengan hematology analyzer. TSH diperiksa dengan metoda ELISA. Uji analisis dengan Korelasi Spearman.Hasil: Terdapat peningkatan kadar TSH pada 6 (20%) pasien PGK dengan hemodialisis. Tidak terdapat hubungan antara TSH dengan MCV (p=0,927), MCH (p=0,958), MCHC (p=0,990) dan RDW (p=0,645). Variasi pada penelitian ini mungkin disebabkan oleh terapi, diet pasien sebelum pemeriksaan.Simpulan: Tidak terdapat hubungan antara TSH dengan parameter Eritrosit dan RDW pada penelitian ini. Diperlukan penelitian lebih lanjut dengan mempertimbangkan diet dan terapi pasien.

Abstract

Latar belakang: Talasemia merupakan penyakit genetik dimana terjadi kelainan pembentukan rantai globin. Penderita talasemia dapat terjadi gangguan sistem endokrin, salah satunya disfungsi tiroid. Prevalensi dan derajat keparahan pada disfungsi tiroid ini belum banyak diteliti. Kadar Thyroid Stimulating Hormone (TSH) menggambarkan keadaan hipotalamus–hipofisis–tiroid dan digunakan untuk menentukan fungsi tiroid. Pasien talasemia dengan transfusi terjadi inflamasi. Feritin, salah satu parameter cadangan besi juga merupakan protein fase akut yang meningkat pada keadaan inflamasi. Kadar feritin pada penderita talasemia menunjukkan inflamasi dan jumlah cadangan besi pada penderita tersebut. Penelitian ini bertujuan menganalisis hubungan feritin dan jumlah leukosit sebagai petanda inflamasi dengan kadar TSH pada pasien talasemia dengan transfusi.Metode: Penelitian belah lintang terhadap 40 pasien talasemia dengan transfusi rutin di PMI Kota Semarang selama periode Juni–Juli2016. Uji normalitas data menggunakan Shapiro–Wilk dan analisa hubungan menggunakan uji Spearman. Pemeriksaan leukosit dengan menggunakan metode flow cytometri pada alat hematologi analyzer, sedangkan untuk pemeriksaan kadar TSH dan kadar Feritin menggunakan metode Enzyme–linked immunosorbent assay (ELISA).Hasil: Analisis korelasi terdapat hubungan positif lemah bermakna antara jumlah leukosit dengan kadar TSH (r=0,326; p=0,004) dan tidak terdapat hubungan bermakna antara kadar feritin dengan kadar TSH (r=0,084; p=0,605)Simpulan: Terdapat hubungan bermakna antara jumlah leukosit dengan kadar TSH dan tidak terdapat hubungan bermakna antara kadar feritin dengan kadar TSH. Penelitian lebih lanjut diperlukan untuk menganalisis faktor-faktor yang berpengaruh pada disfungsi tiroid dan diperlukan pemeriksaan tambahan untuk fungsi tiroid lain, yaitu free thyroxine (FT4). Pemeriksaan ulang fungsi tiroid dalam jangka waktu tertentu diperlukan pada nilai TSH dengan batas atas tinggi dari nilai normal.

Abstract

Background: Systemic inflammatory response syndrome (SIRS) is a state of systemic inflammatory activation by various causes. SIRS have a high mortality rate. Prolactin is known to regulate cellular function of immune system.  Neutrophil-lymphocyte ratio (NLR) is simple, cost effective and easy parameter that currently used as inflammation marker.Objective: The aims of this study is to determine the correlation between prolactin serum with NLR in SIRS patients.Methods: A cross sectional study was conducted on 50 clinically SIRS patients. Prolactin serum was measured by enzyme linked immunosorbent assay (ELISA) and NLR was calculated manually from absolute neutrophil and lymphocyte count measured by automatic hematology analyzer. Non-parametric Spearman test was used to analyze the correlation between prolactin with NLR.Results: Median value of serum prolactin level was 11.32 ng/mL (2.76-194.81), whereas the mean value NLR was 16.36 ± 11.58. The correlation between prolactin levels with NLR was r = 0.345, p = 0.014.Conclusion: There is a weak positive significant correlation between prolactin with neutrophil lymphocyte ratio in SIRS

Abstract

Background: Smoking which is a lifestyle trend continues to increase and take its toll each year. Cigarettes have chemical compounds. Which has toxic and carcinogenic effects that can cause respiratory, cardiovascular, and other irritants. This can affect the distribution of oxygen to the body tissues. Forward leaning and PLB exercises can help maximize breathing thereby improving tissue oxygen distribution. Objective: To determine the differences in oxygen saturation of adult smokers before and after practice forward leaning and pursed lips breathing.Methods: This study used a quasi-experimental method with two group pre-post test design. The research subjects were 20 male respondents, the place of implementation at the Faculty of Medicine, Diponegoro University, Semarang. Research procedures included CKD and PLB exercises in the respondent group.Results: Analysis using the Wilcoxon test obtained significant results, with p values <0.05 on acute and chronic effects after CKD and PLB exercises were performed on 20 respondents Conclusion: A significant result in the oxygen saturation value of the group with PLB and CKD treatment with the PLB group before and after exercise on acute and chronic effects in adult smokers

Abstract

Thalassemia major patients with routine transfusions are at risk of experiencing iron overload and bone turn over disorders which will increase the risk of osteoporosis. Community training activities with BMD examinations and osteoporosis exercise training in thalassemia major patients are expected to identify the prevalence of bone fragility in thalassemia major patients, increase the awareness of thalassemia major patients and their families to the risk of osteoporosis in thalassemia major patients and osteoporosis exercise training as an effort to prevent osteroporosis. The results of BMD on the spine showed 57.1% osteoporosis, 28.6% normal and 14.3% osteopenia. The results of BMD examination on the cervical spine showed 71.4% osteopenia, 21.4% normal and 7.2% osteoporosis. The results of BMD examination on the femur found 50% normal, 42.8% osteopenia and 7.2% osteoporosis. The bone location with the highest percentage of osteoporosis was the spine (57%), and the bone location with the highest percentage of osteopenia was the neck bone (71.4%). The high percentage of osteopenia locations in the cervical spine of thalassemia major patients with repeated transfusions should be become a concern. Based on these findings, we held education and counselling on the risk of osteoporosis in thalassemia patients with repeated transfusions, osteoporosis exercise training and distribution of osteoporosis exercise compact disk (CD) for at home osteoporosis exercise training.

Abstract

Background: Obesity has been associated with the risk of various diseases. The correlation between anthropometry and bone health is complex. Visceral adipocyte index (VAI) is one of the compositions of body fat compositions that mostly associated with proinflammatory cytokine, which can stimulate C- reactive protein (CRP) that affects bone density.Objectives: This study aimed to prove the correlation between VAI, CRP and BMD in Asian reproductive obese womenMaterials and Methods: A cross sectional study was conducted in November 2020 - February 2021 and a total of 134 women of reproductive age with obesity participated in this study. Weight, height, body mass index (BMI) and rate of visceral fat were measured by body composition analyzer. Waist (WC) and hip circumferences (HC) measured by using tape; lipid profile and CRP level were examined using a clinical chemistry automatic analyzer, and the examination of bone mineral density (BMD) using dual-energy x-ray absorptiometry. Waist to hip ratio (WHR)and VAI calculated manually. Pearson and Spearman test were used for statistical analysis.Results: There were positive weak correlations between weight, height, BMI, WC, HC, rate of visceral fat, WHR and VAI. There were weak positive correlations between HC, WHR and CRP. There were positive moderate correlations between weight, height, BMI, waist, visceral fat, WHR and CRP. Conclusion: Although there were correlations between anthropometric parameters and VAI and also CRP, but we did not found correlation between VAI and CRP to all BMD parameters.
