Abstract

ABSTRAK Demam berdarah dengue (DBD) merupakan suatu penyakit endemik yang tidak asing bagi masyarakat Indonesia bahkan menjadi masalah kesehatan dunia. DBD disebabkan oleh virus RNA rantai tunggal, yaitu dengue virus1 yang penyebarannya mengalami pola perubahan dari siklik menjadi tidak teratur sehingga berdampak pada peningkatan kasus DBD itu sendiri2. Pemerintah berupaya mencegah penyebaran kasus DBD dengan memutus rantai perkembangbiakan nyamuk aedes aegypti dengan menggunakan insektisida sintetik atau kimia. Namun, pencegahan menggunakan metode ini memberikan dampak buruk bagi lingkungan maupun kesehatan manusia. Oleh karena itu perlu dilakukan penelitian untuk mengkaji potensi ekstrak daun sambiloto sebagai larvasida nabati terhadap larva nyamuk aedes aegypti. Penelitian ini merupakan studi eksperimental sesungguhnya dengan rancangan randomized posttest only control group design. Populasi dalam penelitian ini adalah seluruh larva aedes aegyptiyang dikembangbiakan di Laboratorium Parasitologi Fakultas Kedokteran Universitas Udayana dengan besar sampel sebanyak 600 sampel larva yang terbagi menjadi 400 sampel kelompok uji (P1, P2, P3, dan P4) serta 200 sampel kelompok kontrol (P0 dan P5). Hasil analisis ANOVA dan chi square menunjukkan bahwa ekstrak etanol daun Sambiloto (Andrographis paniculata) aktif sebagai larvasida pada konsentrasi efektif 1,390% dengan waktu kontak 24 jam. Daun Sambiloto aktif sebagai larvasida karena mengandung senyawa golongan fenolat, alkaloid, flavonoid, dan terpenoid. Kata kunci: Demam Berdarah Dengue, Ekstrak Sambiloto, Konsentrasi aktif

Abstract

The rise in dengue fever in recent decades combined with the emergence of COVID-19 at the end of 2019, has created new challenges in the healthcare sector. This research is a descriptive study with a cross-sectional research design and using medical record data at Udayana University Hospital in 2020–2021. According to the study, 1.22% cases of misdiagnosis out of a total of 2365 suspected cases of COVID-19 were found at Udayana University. The majority of cases of misdiagnosis involved people older than 60 years, namely 7 people (24.1%) and were dominated by men, namely 17 people (58.6%). The most common symptoms found are fever, cough, shortness of breath, headache, and malaise, According to laboratory results, dominant patients have thrombocytopenia, followed by high alanine transaminase (ALT), high aspartate transaminase (AST), and leukopenia. The appearance of thrombocytopenia in cases of COVID-19 with dengue fever is the result of suppressed platelet synthesis due to virus induction which causes bone marrow suppression and platelet clearance. Leukopenia and leukocytosis may coexist with lymphopenia as an indicator of disease severity. The similarity of symptoms and laboratory results between COVID-19 and dengue fever allows for misdiagnosis that will affect the patient's management. Therefore, the aim of this study is to determine the misdiagnosis rate of COVID-19 with dengue fever at Udayana University Hospital in 2020–2021, so that it can reduce misdiagnosis of the disease.

Abstract

BACKGROUNDMalaria remains one of the major causes of death worldwide. Holothuria scabra has a high nutritional content and many biological effects. The development of alternative anti-malarial drugs is necessary, considering that resistance to the newest anti-malarial drugs has been reported. This research study aimed to determine the anti-malarial effects of Holothuria scabra in mice infected with Plasmodium berghei. METHODSThis was a post-test only control group study conducted on 24 Balb/c mice. Peter’s four-day suppressive test was employed to ascertain the claimed anti-plasmodial effect of the simplicia. Following inoculation with P. berghei, all 24 mice were infected and randomized into 4 groups, namely 3 treatment groups and 1 control group. The control group was given carboxymethyl cellulose, two of the treatment groups were given doses of 10 and 100 mg/kg BW Holothuria scabra, respectively, using Peter’s four-day suppressive test, while the remaining treatment group received a dose of 100 mg/kgBW using the prophylactic method. Data were analyzed using One way ANOVA. RESULTSThe results showed by using both the four-day suppressive test and the prophylactic method, that Holothuria scabra has antimalarial activity. Holothuria scabra at a dose of 100 mg/kg BW was significantly effective in decreasing the percentage of parasitemia (p=0.000) and tended to inhibit the growth of Plasmodium berghei in mice (p=0.054). CONCLUSIONThis study demonstrated that Holothuria scabra possesses anti-plasmodial activity in mice. Hence, the sea cucumber could serve as a potential source of a newer antimalarial agent.

Abstract

Background: Male infertility is a significant global health issue, with rising prevalence linked to environmental factors such as secondhand smoke, which impairs sperm quality. This study aimed to examine how light physical activity and vitamin D supplementation improved sperm quality in rats exposed to cigarette smoke. Methods: This study used a randomized experimental design with 38 rats, divided into two groups. The control group was exposed to cigarette smoke and received vitamin D at 18 IU/kg body weight (BW) daily. The treatment group was exposed to cigarette smoke, received the same vitamin D dose, and underwent light physical activity. The 35-day research period involved the analysis of evaluated parameters. Differences between these parameters were assessed using an independent t-test, with statistical significance set at a P-value < 0.05. Results: Compared to the control group (p-values< 0.001), the treatment group, receiving light physical activity and vitamin D supplementation, exhibited significantly lower malondialdehyde (MDA) levels (0.96 ± 0.14 nmol/mL), higher vitamin D receptor expression (2.44 ± 0.58 cells/HPF), and increased testosterone levels (4.33 ± 0.76 ng/dL). Sperm quality was also significantly improved in the treatment group, showing higher sperm count (2.24 ± 0.81 × 10⁶/mL), motility (4.20 ± 0.23%), viability (3.07 ± 0.39%), and normal morphology (5.34 ± 0.17%) (p-values< 0.001). Conclusion: Light physical activity and vitamin D supplementation improved sperm quality in cigarette smoke-exposed male rats, correlating with reduced MDA and elevated vitamin D receptor expression and testosterone. Further clinical trials are warranted to explore human applications.
