Abstract

Introduction: Person-centred care (PCC) is increasingly considered as one of the criteria standards for delivering healthcare in hospitals. Registered nurses are increasingly expected to provide PCC in daily practice. However, although the hospital commits to enhancing and improving PCC, implementing this approach into practice has faced barriers and enablers. As the implementation of PCC is still at an early stage of development, it is well-recognised that the need to identify barriers and enablers is essential. This study aims to explore these barriers and enablers to implementing PCC in Indonesian hospitals. Methods: This study employed a qualitative exploratory study conducted from July 2023 to September 2023 at a public hospital in Pekanbaru, Riau Province. It applied semi-structured interviews to collect data on barriers and enablers to implement PCC from 2 managers and 12 nurses from the medical ward. All participants were selected using a purposive sampling technique. Data collected from interviews were transcribed and then thematically analysed. Results: Three barriers-related themes emerged, namely time constraints and workload, lack of resources, and communication challenges. Furthermore, three enablers themes also emerged, including leadership, commitment, and sufficient monitoring and supervision. Conclusions: The study results lead hospital managers and policymakers to overcome existing barriers using adaptive planning and strategies and to improve enablers with well-trained professionals and strong leadership to deliver a PCC approach into practice. The implications to practice are that healthcare organisations must continue their support to decrease the barriers and optimise professional care in healthcare facilities to improve healthcare services.

Abstract

Background: Cancer patients face various symptoms and problems caused by the disease or cancer treatment. Physical and psychological symptoms worsen the patient’s quality of life (QOL). Objectives: To predict how age, gender, marital status, educational level, working status, type of cancer, stage of cancer, physical factors (pain, fatigue, nausea, lack of appetite, shortness of breath, constipation, drowsiness, well-being) and psychological factors (anxiety and depression) affect to quality of life in patients’ cancer. Methods: The study was quantitative research with a cross-sectional approach with purposive sampling. Ninety-nine cancer patients participated in this study. Data were collected in General Hospital, Pekanbaru, in October - November 2022. The European Organization for Research and Treatment of Cancer Quality of Life Questionnaire (EORTC QLQ-C30) was used to assess patient’s cancer quality of life. The Multivariate analysis uses the multiple linear regression. Results: The EORTC QLQ-C30 mean scores for QoL (65,9725,18), physical, emotional and cognitive functioning were high, at 60 or above. The score of most symptom scales in this study were reportedly good, at 37 or below. However, for pain and fatigue, the score was reportedly worse (range:45, 3227,84 to 64,2725,58). The results of multiple linear regression analysis showed that factors predictive of quality of life were pain (p=0,032), fatigue (p=0,004), shortness of breath (p=0,020), constipation (p=0,002) and depression (p=0,042). These factors together can cause quality of life problems in cancer patients by 55% (adjusted R2 = 0.468). Conclusion: The study finding indicate that QoL score were influenced with several symptoms (pain, fatigue, shortness of breath, constipation and depression) of the ESAS-r score. Healthcare professionals should properly address pain and fatigue, as patients in this patient group consistently report them.

Abstract

Pentingnya self-efficacy pada mahasiswa merujuk pada keyakinan seseorang terhadap kemampuannya untuk beradaptasi dalam mengikuti pelajaran dan menyelesaikan tugas-tugas untuk mencapai hasil yang baik. Penelitian ini bertujuan untuk menggambarkan self-efficacy pada mahasiswa Fakultas Keperawatan tahun pertama dalam proses pembelajaran. Penelitian ini menggunakan desain penelitian deskriptif kuantitatif. Sampel penelitian ini adalah mahasiswa keperawatan tahun pertama dengan total 243 sampel yang dipilih dengan mengunakan teknik total sampling. Self-efficacy diukur dengan menggunakan kuesioner GSES (Generalized Self Efficacy Scale) yang sudah tervalidasi dengan nilai validitas t-value = 16,77 r hitung p =1,96 serta nilai reabilitas 0,75 hingga 0,91. Data terkumpul di analisis dengan menggunakan analisis deskriptif. Hasil penelitian ini juga menunjukkan bahwa mayoritas mahasiswa Fakultas Keperawatan memiliki kategori self-efficacy kategori rendah berjumlah 139 orang (57,2%), memiliki self-efficacy kategori tinggi berjumlah 104 orang (42,8%). Dari hasil penelitian ini memberikan gambaran bahwa mahasiswa pada tahun pertama cenderung memiliki self-efficacy kategori yang rendah. Hal ini menunjukkan bahwa adanya pengalaman yang belum cukup dalam menghadapi tantangan akademik dan sosial sehingga mahasiswa merasa kurang percaya diri.

Abstract

Continuity of care is a consistent and interconnected service provided from time to time according to patient needs. This study aims to determine the relationship between the implementation of continuity of care and the quality of life of patients with cardiovascular problems. This study used a quantitative correlation design with a cross-sectional approach. The sample of this study amounted to 87 respondents with a diagnosis of coronary heart disease 71 respondents and heart failure 16 respondents selected using purposive sampling technique. Continuity of care variables were measured using the Patient Continuity of Care Questionnaire (PCCQ) and quality of life was measured using the World Organization Quality of Life-BREF (WHOQOL-BREF) questionnaire. Data were analyzed using the Chi Square test. Data on respondent characteristics showed that the majority of respondents were aged 56-65 years (46.1%), male gender (79.3%), high school education (66.7%). The results of this study showed that the continuity of care with high significant results (63.2%) and the quality of life of patients with good conditions (52.9%). The results of bivariate analysis showed a substantial relationship between continuity of care and patient quality of life with a coefficient value of p value (0.001). The results of this study indicate that health workers play an important role in ensuring the continuity of health services at all levels. With the continuity of services, it is expected that there will be a decrease in recurrence in patients with cardiovascular problems, especially patients with coronary heart disease and heart failure.

Abstract

Background: Cancer patients undergoing chemotherapy often have physical and psychological impacts. The psychological impact caused is powerlessness. The problem of powerlessness must be handled well because otherwise it can develop into despair and the risk of suicide. The intervention to overcome powerlessness is logotherapy. Logotherapy is a type of psychotherapy that aims to help patients find the meaning of life. Purpose: To determine the effectiveness of logotherapy in cancer patients who experience powerlessness during chemotherapy. Method: A quantitative with a quasi-experimental nonequivalent control design. The total sample was 56 participants and used a nonprobability sampling technique with purposive sampling type. The instrument used was the Powerlessness Assessment Tool for Adult Patients Questionnaire. This research uses statistical tests are the Wilcoxon Sign Rank Test, Mann-Withney U Test, and Independent Sample T Test. Results: Statistical test results showed that there was a significant effect of logotherapy on the powerlessness of cancer patients undergoing chemotherapy with p value (0.000) < alpha (0.05). Conclusion: Logotherapy is effective in overcoming the problem of powerlessness in cancer patients undergoing chemotherapy.

Abstract

Background: Nurses in the Cardiovascular Care Unit (CVCU) have a key role in delivering Person Centered Care (PCC), which focuses on respecting patient preferences and improving care quality. Despite its importance, the implementation of PCC still faces several challenges in clinical practice. Objective: This study aimed to explore the experiences of CVCU nurses in implementing Person Centered Care in a hospital setting. Method: A qualitative research design with a phenomenological approach was used. Six nurses working in the CVCU were selected through purposive sampling. Data were collected using semi-structured interviews and analyzed through thematic analysis based on Colaizzi’s method. Results: The study identified five main themes: (1) Satisfaction with nursing care, (2) Involvement of patients and families, (3) Communication and collaboration between nurses and doctors, (4) Suboptimal communication patterns between nurses and patients, and (5) Time management and workload. Discussion: The findings indicate that PCC contributes to greater satisfaction in nursing care delivery. However, several barriers, such as limited communication and workload issues, need to be addressed. These insights can inform strategies for broader PCC implementation across different hospital wards. Hospital management should support this process by addressing potential obstacles and facilitating a supportive environment for PCC.

Abstract

Discharge planning is a dynamic and systematic process of assessment, preparation, and coordination aimed at facilitating the supervision of health and social services before and after hospital discharge. This study aims to determine the effectiveness of discharge planning implementation on family knowledge in caring for patients with Coronary Heart Disease (CHD). A quasi-experimental design with a one-group pretest-posttest approach was used. The sample consisted of 27 respondents selected through accidental sampling based on inclusion criteria. The intervention involved providing educational material using a leaflet on family knowledge in caring for CHD patients. Knowledge was measured using a 25-item questionnaire. The results showed that most respondents were aged 45–54 years (48.1%), female (59.3%), had a high school education (59.3%), and had cared for CHD patients for less than one year (55.5%). In the bivariate analysis, the pretest scores indicated that 73.4% had moderate knowledge, while posttest scores revealed that 96.74% had good knowledge. These findings demonstrate that discharge planning significantly improves family knowledge in caring for CHD patients, which may help reduce recurrence and hospital readmission rates.

Abstract

Abstract Despite the increasing emphasis on patient satisfaction as a key indicator of healthcare quality, there remains a lack of studies specifically examining satisfaction levels among cardiac catheterization patients, particularly across different procedural stages. This study aims to determine patient satisfaction with cardiac catheterization services. This study used an analytic descriptive design. Samples involved in this study were patients who undertook catheterization from May to June 2024 with a total of 44 respondents selected using total sampling. The variable of this study was patient satisfaction with cardiac catheterization services measured using a questionnaire developed by the researcher. It was validated with an R-value of 0.039 and a Cronbach value of 0.940. Data were analyzed using descriptive analysis. The result of the study showed that most of the respondents' age were late elderly (43.2%), male (72.7%), and high school education (52.3%). The majority of respondents were satisfied with the pre-catheterization stage (81.8%), the intra-catheterization stage (90.9%), and the post-catheterization stage (81.1%). The majority of respondents (86.4%) were satisfied with the cardiac catheterization services that had been carried out. Moreover, based on the characteristics of the respondents, elderly respondents were satisfied (94.7%), more male respondents stated satisfied (87.5%), and respondents with high school education (91.3%). The level of satisfaction was based on the stages of action in each dimension of satisfaction, and in general, the respondents were satisfied with the service. Patient satisfaction has a vital role in the outcome of health services, and it is an indicator of the service and management of health institutions.

Abstract

Pendahuluan: Stres merupakan fenomena umum di tempat kerja saat ini. Banyak survei dan studi membuktikan bahwa tekanan akibat pekerjaan adalah sumber utama terjadinya stres. Stres kerja dapat menimbulkan dampak, dampak stres kerja seperti banyaknya ketidakhadiran, produktivitas kerja yang berkurang. Produktivitas kerja bukan semata-mata ditujukan untuk mendapatkan hasil kerja sebanyak-banyaknya, melainkan kualitas kinerja yang perlu diperhatikan. Penelitian ini memiliki tujuan untuk menganalisis hubungan stres kerja dengan produktifitas kerja perawat puskesmas di kota Pekanbaru selama masa pandemi COVID-19. Metode: Penelitian ini merupakan penelitian kuantitatif menggunakan desain penelitian korelasi dan pendekatan Retrospektif dengan sampel peneltian sebanyak 45 orang dengan menggunakan total sampling. Alat ukur yang digunakan kuesioner stres kerja dan produktifitas kerja . analisis menggunakan univariat dan bivariat menggunakan Chi Square. Hasil: Hasil penelitian menunjukkan bahwa perawat puskesmas mengalami stres sedang (46,7%), dan produktifitas kerja kurang baik (64,4%). Hasil uji Chi Square menunjukkan adanya hubungan antara stres kerja dengan prossuktifitas kerja perawat puskesmas di kota pekanbaru selama masa pandemi COVID-19 dengan p value (0,005) < alpha (0,04). Kesimpulan: Hasil penelitian ini menunjukkan bahwa stres kerja yang dirasakan perawat puskesmas selama masa pandemi COVID-19 mempengaruhi produktifitas kerjanya. Saran: Berdasarkan hasil penelitian ini diharapkan kepada kepala puskesmas untuk tetap memantau perawat agar tidak mengalami stres kerja yang berpengaruh untuk produktifitas kerjanya.

Abstract

During the Covid-19 pandemic in Indonesia, the use of herbal and non-herbal supplements in preventive and curative efforts increased significantly. Using supplements prevents Covid-19 from complementing vitamin deficiencies in certain conditions. This study aims to identify the behavior of herbal and non-herbal supplements during the Covid-19 pandemic.  This study is retrospective. The number of samples used in this study was 99 respondents were selected using the quota sampling technique. Data was collected using a questionnaire to identify behavioral variables, and validity and reliability tests of this questionnaire were carried out. Data were analyzed using descriptive analysis.  The results of this study indicate that the majority of respondents' age is in the range of 17-25 years (54.4%), most of the respondents were female 64 people (64.6%), the behavior of using herbal and non-herbal supplements seen from the indicator of being aware of side effects is 74.7%, from the indicator the way to store supplements is 58.6%, from the indicator the way to dispose of expired supplements is 48.5% and from indicator types of herbal supplements 71.7%. The results of this study provide implications for the community to apply appropriate behavior in using health supplements.
