Abstract

Abstract Background: Anxiety has been going on during the COVID-19 pandemic and can occur to anyone. Including midwives need to give services with direct physical contact for people who are susceptible to infection. As a solution against the pandemic, the COVID-19 vaccines rapidly developed to reduce the risk of COVID-19 transmission. With the presence of the COVID-19 Vaccines, this study aims to examine the anxiety of midwives after COVID-19 vaccination in midwifery services at IMP (Independent Midwifery Practice). Methods: Descriptive quantitative research with the number of respondents 137 midwives based on total sampling technique. The variable in this study is the level of anxiety measured by the 7-item Coronavirus Anxiety Scale. The data analysis with univariate analysis. Results: Based on the results, it shows of the 137 respondents who have been receiving the COVID-19 vaccine has found 46 respondents (33.6%) with normal category, 55 respondents (40.1%) with mild category, then moderate category as many as 23 respondents (16.8%), and severe category as many as 13 respondents (9.5%). Conclusion: most of the anxiety categories levels experienced are normal until mild. The possible causes are from the visitors who do not apply health protocols and the midwives who have families with susceptible group status. Key words : anxiety, midwives, COVID-19 vaccination

Abstract

Utilization of health services by pregnant and postpartum women can save women from the incidence of maternal death and severe complications during pregnancy and postpartum. Several factors that directly contribute to the utilization of health services are decision making and sources of information obtained by pregnant and postpartum women regarding the utilization of health services. This study aims to analyze gender equality in decision making on the use of health services in urban and rural areas of Indonesia. This study used a mixed-method approach from primary data and a literature review. The target in this article was pregnant women and postpartum mothers who were spread out in several health centers in Surabaya City and Bojonegoro Regency with a total of 175 respondents who were calculated using the proportion estimation formula. Data analysis used univariate and bivariate analysis. The results showed that the wife in terms of the number of frequencies dominated in making decisions on the use of health services (92.5%). In addition, when the decision maker is the husband, all postpartum mothers (100%) will take advantage of health services. Related to the source of information that many respondents chose in providing information related to pregnancy health for the use of health services, it was health workers (86.3%) compared to other media. The wife's decision is a determinant in making decisions about the use of health services. On the other hand, husband’s support as a decision maker can also influence his wife to take advantage of health services.

Abstract

Pregnancy is a stressful reproductive process and increases anxiety. The prevalence of anxiety symptoms in Indonesia reaches 22.9%. One alternative therapy applied to reduce anxiety by music therapy. This literature review aims to determine the effect of music therapy on anxiety in pregnant women in Indonesia. Literature searches were carried out on Pubmed, Web of Science, Google Scholar, and Scopus with the last ten years of publication. The results of the analysis of the seven articles obtained related to the effect of music therapy on anxiety in pregnant women in Indonesia, that anxiety often occurs in primigravida pregnant women and before delivery. Anxiety is an indicator of stress and physiologically causes activation of the hypothalamus-pituitary adrenal (HPA) axis and increased activity of the sympathetic nervous system such as heart rate and blood pressure is released. Utilization of various types of music, including classical music, gamelan, and others as therapy has significant results in reducing anxiety, because the rhythm and tempo of music resembles the human heart rate, thus providing a state of rest for the body. Music intervention research found the most effective time duration, which is in the 30-minute relaxation period and is optimal in the morning, and has the best duration at 12 weeks of intervention. It can be concluded that the use of music therapy for anxiety in pregnant women in Indonesia can be used as an effective alternative to reduce anxiety for pregnant women, especially before delivery.

Abstract

Indonesia menempati peringkat kedua dengan jumlah kasus Tuberkulosis (TB) tertinggi di dunia, yang menjadi tantangan besar dalam kesehatan masyarakat. Dampak TB sangat serius pada ibu hamil, membawa risiko ganda baik bagi ibu maupun bayi. Pemberdayaan masyarakat melalui upaya promotif dan preventif menjadi salah satu pendekatan strategis untuk pengendalian TB di Indonesia. Kader kesehatan, sebagai anggota masyarakat yang telah dilatih, memainkan peran penting dalam menjembatani layanan kesehatan dengan masyarakat. Namun, peran kader ibu hamil belum sepenuhnya terintegrasi dengan upaya pencegahan dan deteksi dini TB selama kehamilan. Program pengabdian masyarakat ini bertujuan untuk menjawab kebutuhan tersebut melalui metode tiga tahap: pelatihan kader, tindak lanjut pelatihan, dan evaluasi. Sebanyak tiga puluh kader kesehatan dari Puskesmas Bluto di Jawa Timur berpartisipasi dalam inisiatif ini. Program ini terbukti efektif dan efisien dalam meningkatkan pengetahuan tentang TB paru, baik pada kader maupun ibu hamil, meningkatkan keterampilan kader dalam edukasi kesehatan, serta memperluas cakupan skrining mandiri TB paru dan tindak lanjut hasil skrining. Analisis statistik menggunakan Paired T-Test menunjukkan peningkatan signifikan pada pengetahuan kader sebelum dan setelah pelatihan, serta peningkatan jumlah ibu hamil yang mendapatkan edukasi dan melakukan skrining mandiri TB paru. Umpan balik dari peserta juga menyoroti dampak positif program ini. Inisiatif ini menegaskan potensi optimalisasi peran kader kesehatan sebagai strategi yang dapat diadaptasi untuk meningkatkan upaya pengendalian TB, khususnya pada kelompok rentan seperti ibu hamil.

Abstract

Secondary old primi postpartum mothers are mothers who have just given birth again after a previous birth gap of more than 10 years. During the postpartum period, socio-cultural factors such as family support, community norms, and traditional belief practices play an important role and can affect the health status of mothers and babies. Limited knowledge and beliefs that conflict with health principles are challenges in providing care. This study is a case report with a descriptive method using a case study approach to a postpartum mother who was accompanied by a team. midwife from one of the Community Health Centers in Surabaya through the Continuity of Midwifery Care (CoMC) model. This article aims to describe and analyze in depth one case of a postpartum mother with a complex social, economic, and cultural background. Case Report : A 39 - year-old secondary primi mother ( P2A0) who experienced psychological stress due to traditional care practices culture implemented by family members, such as prohibitions on consuming animal protein, prohibitions on leaving the house, and the obligation to wear a strict corset. This condition has a negative impact on the recovery of surgical wounds and breast milk production. Support from husbands and health workers plays an important role in reducing these negative impacts. Direct assistance or through telehealth services has been shown to help mothers understand postpartum care that is in accordance with health principles. Conclusion: Continuous mentoring through the CoMC model, including the use of telehealth, can be an effective strategy in overcoming socio-cultural barriers during the postpartum period.

Abstract

Mental health problems in pregnant women are one of the health problems that have not been handled optimally in Banyuwangi Regency both during pregnancy and postpartum. Mental health screening of pregnant/postpartum women in antenatal care services is currently not running optimally because not all health workers have the ability to screen. Likewise, cadres as the spearheads in the community who assist pregnant/postpartum women in their area. Therefore, increasing literacy both knowledge and practice of mental health screening is needed. This capacity-building training activity aims to increase knowledge and skills in mental health screening using SRQ-20 to health workers and cadres. The 88 training participants consisted of 48 health workers and 40 cadres representing all sub-districts in Banyuwangi District. The results of this activity show that there are differences in knowledge and skills before and after with a value of p = 0.000 (health workers) and p = 0.000 (cadres). The average score before training also increased from 65.972 to 90.972 (health workers) and 41.833 to 71.667 (cadres) with a maximum score of 100. This activity is expected to improve antenatal care services and the participation of health workers and cadres in conducting mental health screening at puskesmas and their assisted areas.

Abstract

Indonesia menjadi negara tertinggi kedua jumlah kasus Tuberkulosis (TB) terbanyak di dunia. Dampak TB akan lebih besar pada ibu hamil yang dapat mengakibatkan beban ganda baik bagi ibu maupun bayinya. Pemberdayaan masyarakat dalam upaya promotif dan preventif merupakan salah satu upaya strategis penanggulangan TB di Indonesia. Kader kesehatan merupakan anggota masyarakat yang peduli kesehatan dan telah dilatih mengenai tugas khusus dalam bidang kesehatan. Selama ini kegiatan kader ibu hamil belum pernah terintegrasikan dengan upaya pencegahan dan deteksi dini TB Paru dalam kehamilan. Metode kegiatan pengabdian masyarakat ini terdiri dari tiga bagian, yaitu pelatihan kader, tindak lanjut pelatihan, dan evaluasi. Sejumlah tiga puluh kader di wilayah kerja Puskesmas Guluk-Guluk mengikuti program ini. Rangkaian program terbukti efektif dan efisien dalam upaya peningkatan pengetahuan, baik kader maupun ibu hamil mengenai TB Paru, keterampilan kader dalam mengedukasi ibu hamil, dan cakupan skrining mandiri TB Paru oleh ibu hamil serta tindak lanjut hasil skrining oleh kader. Hal tersebut dapat dilihat dari nilai pre dan post-test yang dianalisis menggunakan uji statistik Paired T-test menunjukkan p-value: 0.00 (p-value < α: 0,05) sehingga didapatkan adanya peningkatan pengetahuan kader sebelum dan setelah pelatihan, jumlah ibu hamil yang diberi edukasi oleh kader dan melakukan skrining TB Paru mandiri, serta kesan kader selama mengikuti program ini. Peran kader kesehatan ibu hamil dalam TB Paru diharapkan optimal dan dapat diterapkan di wilayah lain di Indonesia untuk mewujudkan eliminasi TB pada tahun 2030.

Abstract

Background: Preeclampsia during the postpartum period is one of the biggest contributors to maternal morbidity and mortality. The importance of discharge planning and good monitoring during return visits in postpartum cases with preeclampsia will have a positive impact in reducing morbidity and mortality due to postpartum hypertension complications. Objective: The purpose of this study was to examine and describe the implementation of discharge planning and return visits for postpartum cases with preeclampsia in one of the Type B Hospitals in Surabaya and the Surabaya City Community Health Center. Method: This study used a qualitative design with a phenomenological approach. Data collection was carried out through in-depth interviews and observations. The sample size was 18 informants, namely 11 midwives and 7 postpartum mothers with preeclampsia, with purposive sampling. Results: The results of the study found several obstacles in the implementation of discharge planning and return visits for postpartum cases with preeclampsia. The implementation of discharge planning, postpartum services and return visit schedules were not all carried out according to service standards, so there was no two-way communication between the Hospital and the Community Health Center. Conclusion: Implementing appropriate discharge planning and postpartum care serves as a benchmark for service quality and represents continuity of care in midwifery care, particularly for postpartum mothers with preeclampsia. Evaluation of any identified challenges is necessary to ensure optimal service delivery.

Abstract

Latar belakang: Tuberkulosis (TB) anak di Indonesia terutama di Kecamatan Pabean Cantian Surabaya memiliki tingkat kesakitan yang tinggi, meskipun begitu, banyak kasus TB yang tidak terdiagnosa dan tidak dilaporkan. Untuk mengatasi masalah tersebut, Geliat Unair, Unicef, dan Dinas Kesehatan Kota Surabaya bekerjasama dalam program pelatihan untuk meningkatkan kemampuan kader dan guru dalam penyuluhan dan skrining TB anak serta mendukung eliminasi TB anak. Tujuan: Penelitian ini bertujuan untuk menganalisis perbedaan pengetahuan kader dan guru saat sebelum dan sesudah mendapatkan pelatihan TB anak. Metode: Metode penelitian menggunakan kuantitatif observasional. Teknik pengambilan sampel adalah total sampling yakni seluruh peserta pelatihan kader dan guru yang berjumlah 60 orang yang merupakan kader kesehatan dan guru (KB, PAUD, TK, SD, SMP, dan guru pondok pesantren) di wilayah Kecamatan Pabean Cantian Surabaya. Pengukuran pengetahuan dalam kegiatan ini diperoleh dari hasil pre-test dan post-test menggunakan kuesioner. Analisis data dilakukan menggunakan uji statistik Paired T-test. Hasil: Hasil statistik menunjukkan p-values: 0,00 (p < ?) sehingga ada perbedaan pengetahuan sebelum dan sesudah pelatihan. Kesimpulan: Terdapat perbedaan pengetahuan kader dan guru saat sebelum dan setelah diberikan pelatihan TB anak.

Abstract

Abortus merupakan salah satu komplikasi kehamilan yang berkontribusi terhadap meningkatnya morbiditas dan mortalitas ibu, terutama di daerah dengan keterbatasan akses layanan kesehatan reproduksi seperti Kabupaten Sumba Tengah. Rumah Sakit Umum Daerah (RSUD) Waibakul mencatat 68 kasus abortus pada tahun 2023, namun informasi mengenai faktor-faktor yang berhubungan dengan jenis abortus masih terbatas. Penelitian ini bertujuan untuk menganalisis faktor-faktor yang berhubungan dengan jenis abortus pada ibu hamil yang dirawat di RSUD Waibakul. Penelitian ini menggunakan desain observasional analitik dengan pendekatan cross-sectional. Sampel penelitian berjumlah 68 ibu hamil yang mengalami abortus dan dipilih dengan teknik total sampling. Data sekunder diperoleh dari rekam medis. Variabel bebas meliputi usia ibu, paritas, dan jarak kehamilan, sedangkan variabel terikat adalah jenis abortus. Analisis data dilakukan menggunakan uji chi-square dengan tingkat signifikansi p 0,05. Hasil penelitian menunjukkan terdapat hubungan yang bermakna antara usia ibu (p = 0,014) dan jarak kehamilan (p = 0,004) dengan jenis abortus. Sementara itu, paritas tidak menunjukkan hubungan yang signifikan dengan jenis abortus (p = 0,479). Secara deskriptif, abortus inkomplit merupakan jenis abortus yang paling banyak ditemukan (61,8%), diikuti abortus iminens (38,2%). Penelitian ini menunjukkan bahwa usia ibu dan jarak kehamilan berhubungan secara signifikan dengan jenis abortus. Namun, karena menggunakan desain cross-sectional, penelitian ini tidak dapat menjelaskan hubungan sebab-akibat. Diperlukan penguatan upaya deteksi dini, pemantauan kehamilan berisiko, serta strategi promotif dan preventif seperti edukasi kesehatan reproduksi dan skrining prakonsepsi untuk menurunkan kejadian abortus dan mendukung pencapaian Sustainable Development Goals (SDGs) 3, khususnya target 3.1 dan 3.7.
